The role of cytokines in Plasmodium vivax malaria.

The cytokine tumor necrosis factor and other as yet unidentified factor(s) which together mediate the killing of intraerythrocytic malaria parasites are transiently elevated in sera during paroxysms in human Plasmodium vivax infections in non-immunes. These factors which included TNF and parasite killing factor(s) are associated with the clinical disease in malaria to the extent that their transient presence in infection sera coincided with paroxysms, the the most pronounced clinical disturbances of P. vivax malaria and secondly because their levels were markedly lower in paroxysm sera of semi

Imported malaria in pregnancy: report of four cases and review of management.

Malaria is a common infection worldwide. Increased travel by pregnant women makes it likely that physicians in the United States will see cases of malaria in this population. We observed four cases of malaria during pregnancy over an 8-month period at a general hospital in the United States. These cases illustrate the association between pregnancy and severe malaria in the mother and congenital infection in the newborn. We also noted delays in diagnosis because malaria was mistaken for other common illnesses. Therapy was complicated by concerns about the safety of antimalarial agents for the f

Ovalocytosis protects against severe malaria parasitemia in the Malayan aborigines.

The malaria parasite rates and densities were compared in 79 ovalocytic-normocytic pairs of Malayan Aborigines matched for age, sex, proximity of residence to each other, and use of bed nets when sleeping in their jungle settlement in central Peninsular Malaysia. Malaria infection was determined from thick and thin Giemsa-stained blood films collected monthly for a period of six months. Blood films from ovalocytic individuals were found to be positive for malaria less often than in persons with normal red blood cells (P less than 0.05). Malaria infections per 100 person-months at risk were 9.7

Imported malaria in the Bronx: review of 51 cases recorded from 1986 to 1991.

The cases of 51 patients with malaria seen at the Albert Einstein College of Medicine hospitals from January 1986 to June 1991 are reviewed. Thirty-five patients acquired infection on journeys to their country of origin. Of these 35 patients, 83% of whom had lived in the United States for > or = 2 years, only 17% received antimalarial prophylaxis. Ten of the 51 patients were born and raised in the United States, and 70% received prophylaxis (P < .01). Six of the 51 patients were visitors to the United States from areas endemic for malaria. Overall, 64% of patients acquired malaria in West Afri

[Malaria in intravenous drug users associated with HIV seropositivity].

Cases of induced malaria have been notified in S. Paulo State, Brazil, in recent years. At the same time the number of cases imported from endemic regions of Brazil has been increasing. One case of induced malaria by Plasmodium vivax was registered in Presidente Prudente, located in the west of the State, in 1988 and a further eleven cases in 1989. This city is considered to be one of the main transit ports for people who come into the State from the Amazonian region. The patients declared that they had not been to any possible transmission area of malaria. All of them had, however, taken coca

[Possibilities and limitations of malaria therapy in general practice].

Malaria infections by Plasmodium (P.) vivax. P. ovale or P. malariae follow mostly a benign course, and ambulatory treatment is safe, provided there is no other significant morbidity. Ambulatory treatment for falciparum malaria must be considered only if a number of conditions are met, making potential complications most unlikely. Furthermore, it is imperative that the patient can be reliably supervised at home. Taking into account the increase of P. falciparum strains resistant to chloroquine, cases meeting the criteria for ambulatory care can mostly be treated with mefloquine, pyrimethamine/

Treatment of malaria--1990.

Malaria has become an increasingly common health problem in the 1970s and 1980s, both in areas where infection is endemic and in travellers returning to non-endemic areas. The severity of infection varies widely, depending on the plasmodial species involved, and there is an extensive chemotherapeutic armamentarium currently available to combat malarial infection. Drug chemistry, pharmacokinetics, mechanism of drug action and resistance, and toxicities are outlined for the cinchona alkaloids (quinine and quinidine), chloroquine, amodiaquine, pyrimethamine, the sulphonamides, pyrimethamine/sulfa

Patterns of erythropoiesis and anaemia in malaria.

Of 138 patients with malaria, 90 were found to be having Plasmodium falciparum in their peripheral blood smears. Megaloblastosis alone or in combination with the other patterns of erythropoiesis was observed in 82.1 percent cases of chronic P. falciparum malaria as compared to 36.3 and 26.5 per cent cases of acute P. falciparum and P. vivax malaria respectively. Iron deficiency was observed in 15.5 percent cases of chronic P. falciparum, 18.2 per cent cases of acute P. falciparum and 13.3 per cent patients of P. vivax infection. Of patients with chronic falciparum malaria, 33.3 percent reveale

Treatment of severe malaria in the United States with a continuous infusion of quinidine gluconate and exchange transfusion.

During the past decade the incidence of Plasmodium falciparum malaria in the United States has increased 10-fold. Treatment may be delayed because the therapy recommended for severe or complicated disease, intravenous quinine dihydrochloride, is available only from the Centers for Disease Control. We studied 17 patients who were treated for severe or complicated P. falciparum malaria in the United States between 1985 and 1987. Five patients were treated with a continuous infusion of quinidine gluconate, 10 with an exchange transfusion in addition to the continuous infusion of quinidine glucona

Review of 40 children with imported malaria.

Forty cases of imported malaria (1978 to 1988) are reviewed and management principles are discussed. All 15 cases of Plasmodium falciparum malaria were acquired in Africa, 5 of which were probably chloroquine-resistant. Most cases of Plasmodium vivax (80%) were acquired on the Indian subcontinent, including 2 cases of congenital malaria. Six children developed P. falciparum malaria despite chemoprophylaxis. All children had a history of fever, usually with other influenza-like symptoms. Two-thirds had splenomegaly, and one-third were afebrile on admission. Thrombocytopenia (70%) and anemia (70

[Malaria: medical and surgical complications].

The resurgence of malaria in Venezuela as an epidemic has caused an increase in the number of cases of this disease. 898 cases were registered in 1962 in contrast to 46,279 cases in 1988. This reality implies that all hospitals will have to face the increase of patients with this disease. Ten cases of malaria have been described and analyzed. These have been chosen for their medical and surgical complications, for which they were admitted to the "Hospital Universitario de Caracas" and "Hospital de Clínicas Caracas". Of these ten cases, eight were acquired in a natural way and two in an induced

Hyperreactive malarial splenomegaly in Venezuela.

A cross-sectional seroepidemiological survey seeking hyperreactive malarial splenomegaly was carried out in isolated Yanomami hamlets in Amazonas Territory in Venezuela. All 110 inhabitants greater than 1 year of age were evaluated clinically and 98 were studied immunologically. The spleen index for individuals greater than 10 years of age was 44%. Only 3 patients had Plasmodium spp. on thick blood smears. All had serological evidence of infection with Plasmodium falciparum and P. vivax. Twenty-three patients were considered to show hyperreactive malarial splenomegaly. Clinical manifestations

Malaria. A city hospital experience.

We reviewed the charts of 24 patients with malaria seen at the Queens Hospital Center, Jamaica, NY, over the past five years. Twenty-three patients were foreign citizens. Eighteen patients were infected with Plasmodium vivax and six with Plasmodium falciparum. Malaria was suspected on admission in 19 of the 23 hospitalized patients. Five patients were admitted with unrelated diagnoses, and four of these experienced diagnostic delay. All diagnoses were confirmed with thin blood smears. Twenty-one patients were febrile, and 18 patients had prominent gastrointestinal tract symptoms. Serum glucose

Neonatal exchange transfusions complicated by transfusion-induced malaria.

Two neonates who became infected with Plasmodium vivax subsequent to exchange transfusion are described. This complication of neonatal exchange transfusion is unusual in that only two other similar cases have been reported to the Centers for Disease Control. Although malaria is an unusual complication, it should be kept in mind when considering the differential diagnoses of fever following neonatal exchange transfusion. The risk of transfusion-induced malaria may be increasing due to the influx of immigrants from areas endemic for malaria. Careful blood donor selection and blood donor educatio

Malaria 1984. Part I. Malaria prophylaxis.

Guidelines are presented to assist the prescriber in arriving at a logical and up-to-date approach to the chemoprophylaxis of malaria, particularly for residents of and visitors to Africa. Areas of controversy are highlighted, with a critical but simplified practical approach being taken to a complex subject. The importance of a history of recent travel in a patient presenting with an unexplained fever is stressed. A series of tables listing geographical areas where chloroquine- and other drug-resistant Plasmodium falciparum is found is provided as an aid to the logical selection of prophylact

Courses of infections with Plasmodium falciparum in own monkeys displaying a microfilaremia.

Infections with the Uganda Palo Alto, Malayan Camp-CH/Q, Vietnam Oak Knoll, and Vietnam Smith strains of Plasmodium falciparum in owl monkeys (Aotus trivirgatus griseimembra) with concomitant microfilaremias usually, but not always, followed a more benign course than infections with the same strains in monkeys free of filarial infections. Four distinct microfilariae were identified in systematic examinations of 26 monkeys, 5 with self-limited infections with P. falciparum, 9 with normally benign self-limited infections with P. vivax, and 12 without previous malaria. The microfilariae found inc

[Epidemiology and prophylaxis of malaria in France (author's transl)].

A study of 100 cases of malaria in France, 98 of which were imported, shows the strong predominance (83%) of Plasmodium falciparum and of contamination in Africa (86%). The reasons why malaria is rare among tourists returning from Asia are given. The risk of re-introduction of the disease into France seems to be slight, especially for P. falciparum. Chemoprophylaxis is easy when aminoquinolines can be recommended, but much more complicated otherwise, the reasons for failure being the lack of effect of other drugs on pre-erythrocytic forms, resistance to chemotherapy and lack of information or

A case of congenital malaria in Malaysia with IgM malaria antibodies.

Congenital malaria from Malaysia is reported here for the first time. It occurred in a baby boy born to a 16-year-old primigravida who contracted Plasmodium falciparum infection during pregnancy. She suffered malaria during the later stages of pregnancy and at parturition. The placenta was heavily infested with various asexual stages of P. falciparum. Gametocytes were not seen. Extensive search did not show other species. Cord blood showed very light infection with young trophozoites of P. falciparum. Serological studies using IFA technique showed specific IgG and IgM antibodies to P. falcipar

Malaria and pregnancy.

A review of current literature on the effects of malaria on pregnant women and the foetus is made and its effective control is discussed. The role of placenta as an exchange organ and as the first defence line for foetus is also described. Reduced resistance to malaria during pregnancy may be due to immunological and hormonal changes. In malarious areas, the effective control of malaria results in reduced maternal anaemia and in increased birth-weights. Prophylaxis, treatment and other antimalarial measures are more beneficial to the primiparous than multiparous women.
