Regional Right Ventricular Remodeling in Hypoplastic Left Heart Syndrome Across Staged Surgical Palliation and Its Association With Global Right Ventricular Function and Clinical Outcomes.

BACKGROUND: Right ventricular (RV) remodeling and (mal)adaptation contribute to high morbidity and mortality in children with hypoplastic left heart syndrome (HLHS). The mechanisms are incompletely understood. The authors hypothesized that apical hypertrophy leads to a loss of RV volume, necessitating basal functional compensation, which determines RV function and outcomes. Consequently, the aim of this study was to examine regional patterns of RV hypertrophic remodeling and their relationships to RV function in HLHS. METHODS: Longitudinal clinical and echocardiographic parameters in 111 chil

Long-Term Impact of Tricuspid Valve Intervention on Survival, Tricuspid Valve, and Ventricular Function in Hypoplastic Left Heart Syndrome.

BACKGROUND: Tricuspid regurgitation (TR) is a significant survival risk in hypoplastic left heart syndrome. METHODS: This is a retrospective single-institution study of 485 patients with hypoplastic left heart syndrome who underwent Fontan from 1985 through 2017. Interstage death or failure and tricuspid valve replacement without attempted repair were excluded. The primary outcome was survival free of transplant or Fontan takedown. Secondary outcomes were conditional cumulative incidence of tricuspid valve failure (≥ moderate TR or surgical tricuspid valve intervention [TVI]) and ventricular

Exploring Serum Biomarker Levels in Tetralogy of Fallot, Hypoplastic Left Heart Syndrome, and Healthy Children.

Serum biomarkers have emerged as tools for diagnosis and management in adult heart disease but are less investigated in the pediatric population. This exploratory study reports biomarker profiles in unrepaired tetralogy of Fallot (TOF), repaired TOF (rTOF), hypoplastic left heart syndrome (HLHS) following Fontan surgery, and healthy controls. We compared circulating biomarker patterns between TOF, rTOF, HLHS, and control groups, aiming to characterize potential disease-specific profiles and generate hypotheses for future research. We prospectively enrolled subjects and collected single-time bl

Surgical pathways and risk categories in the management of hypoplastic left heart syndrome and variants.

OBJECTIVE: The distribution of risk factors in neonates with hypoplastic left heart syndrome is heterogeneous, and their impact remains unclear. We describe the risk categories, surgical strategies, and outcomes in this population. METHODS: Retrospective chart review was performed for 233 consecutive neonates undergoing surgery for hypoplastic left heart syndrome and variants at a single center in 2006-2023. The criteria for our risk categories were determined by expert consensus, refined based on Cox regression for mortality, and applied retrospectively to a historical cohort. RESULTS: Of t

The Impact of Fontan Circulatory Failure on Heart Transplant Survival: A 20-Center Retrospective Cohort Study.

BACKGROUND: Fontan circulatory failure (FCF) is a chronic state in palliated single ventricle heart disease with high morbidity and mortality rates, including heart failure, multisystem end-organ disease, and need for heart transplant. Specific FCF morbidities have not been rigorously defined, limiting study of how FCF morbidities affect pre-heart transplant and post-heart transplant outcomes. We hypothesized that FCF-related morbidities affect survival from heart transplant waitlisting through 1 year after heart transplant. METHODS: This 20-center, retrospective cohort study collected demogr

Pulsed Field Ablation for Atrial Fibrillation in Fontan Palliation.

INTRODUCTION: Pulsed-field ablation (PFA) is a novel and effective modality for ablation in patients with atrial fibrillation (AF). Available case reports and small series showed promising results in patients with congenital heart disease. However, PFA ablation in Fontan patients has not been reported yet. METHODS AND RESULTS: We present a 35-year-old patient with a history of Fontan palliation for hypoplastic left heart who underwent successful ablation for AF with PFA. We describe procedure characteristics and illustrate the potential benefits of PFA in this patient population. CONCLUSION:

A novel modification for the Sano conduit during stage I Norwood palliation for hypoplastic left heart syndrome: towards a more anatomical and physiologic connection.

We present a 3.7-kg neonate with a foetal diagnosis of hypoplastic left heart syndrome who underwent stage I Norwood palliation using two technical modifications. We reconstructed the aortic arch and the aorto-pulmonary connection by following the Birmingham technique. The source of pulmonary blood flow was provided via a right ventricular-to-pulmonary artery shunt (Sano). A novel modification to the Sano conduit is demonstrated that we believe is more anatomical and physiologic. The composite Sano conduit was constructed using a 5-mm externally reinforced polytetrafluoroethylene graft connect

Increased Global Post-Systolic Contraction During 1st Interstage Predicts Medium-Term Death or Transplantation in Children With Hypoplastic Left Heart Syndrome.

BACKGROUND: Echocardiography parameters of right ventricular (RV) dysfunction, fractional area change (RVFAC), and global longitudinal strain during the first interstage have been shown to be associated with death or transplantation in patients with HLHS. However, both parameters lack adequate discriminatory characteristics. This study sought to examine global post-systolic contraction (PSC), a marker of myocardial inefficiency, as a predictor of death or transplantation (Tx) in patients with classic hypoplastic left heart syndrome (HLHS). METHODS: We performed a post-hoc analysis of 62 prosp

Cardiac Transplantation After HYBRID+VAD Support in Patients With HLHS: Technical Considerations and Outcomes.

BACKGROUND: High-risk patients with hypoplastic left heart syndrome can be supported with combined Hybrid Stage 1 (arterial duct stenting, bilateral pulmonary artery banding, and atrial septectomy if needed) and ventricular assist device (VAD) insertion (HYBRID+VAD) as a bridge to transplantation. Concerns have been raised about the challenges and complexity of the subsequent cardiac transplantation. The purpose of this analysis was to review the investigators' experience with 7 consecutive infants with functionally univentricular ductal-dependent systemic circulation who underwent cardiac tra

Fetal Cerebrovascular Response to Maternal Hyperoxia Testing and Association With Brain Growth and Postnatal Brain Injury in Congenital Heart Disease.

BACKGROUND: Neurodevelopmental outcomes are impaired in significant congenital heart disease (CHD) with prenatal origins. The cerebrovascular response to maternal hyperoxia (MH) varies in fetuses with CHD, which may reflect brain health in utero. We investigated the association between lack of cerebrovascular reactivity with MH and adverse neurologic outcomes in CHD measured as brain growth and risk of postnatal white matter injury. METHODS: This is a prospective cohort study of pregnant participants whose fetuses had CHD requiring a neonatal operation. We performed fetal echocardiograms with

Prioritising gaps in care in the real-life journey of single ventricle CHD.

BACKGROUND: Single ventricle CHD requires lifelong care, yet its broader impact on patients and families remains unclear. Engaging patients in care improvement can strengthen relationships and outcomes. OBJECTIVES: This study evaluates how individuals with single ventricle CHD prioritise gaps in care based on personal and family impact. METHODS: Using Mery et al.'s identified care gaps, a survey was distributed to parents of children with single ventricle CHD and adults with single ventricle CHD in English or Spanish. Participants rated each gap from 1(not important) to 10(extremely importan

Type and Frequency of Arrhythmias Status Post Hybrid Procedure at 6 Years of Age.

The hybrid pathway is an alternative surgical route for patients with hypoplastic left heart syndrome (HLHS) and patients with single-ventricle systemic obstructive physiology. Limited data exist on post-Fontan arrhythmias in these patients. This study aimed to assess the type and frequency of arrhythmias post-Fontan in hybrid pathway patients and compare them to the Single Ventricle Reconstruction Trial (SVRT). A retrospective chart review was performed on all patients with single-ventricle anatomy who underwent the hybrid procedure and subsequently completed the Fontan surgery. Exclusion cri

Mechanical circulatory support in failing single-ventricle physiology.

The use of ventricular assist device (VAD) therapy for advanced heart failure in infants and children with single-ventricle (SV) congenital heart disease (CHD) has expanded rapidly. However, issues pertaining to patient size, developmental hemostasis, device availability, complicated anatomy, and complex modes of hemodynamic/circulatory failure associated with each stage of SV palliation make VAD support particularly challenging in this population. Over the last 20 years, there has been significant improvement in support outcomes for this complex population; however, survival and rates of adve

Hypoplastic Left Heart Syndrome in U.S. Children: A 20-Year Look at Lives Lost and Gaps That Remain.

Hypoplastic left heart syndrome (HLHS) is a complex congenital heart defect, characterized by hypoplasia of the left ventricle, the ascending aorta, and the aortic arch often with accompanying aortic and mitral valvular anomalies. If left untreated, HLHS is uniformly fatal in infancy, yet mortality trends remain unexplored. We aim to assess trends in HLHS-related mortality among the U.S. pediatric population. Death certificate data was analyzed from the CDC WONDER database for HLHS-related mortality from 1999 to 2020. Data were stratified by year, age, sex, race, urbanization status, state and
