The Hidden Regulators: MicroRNAs in Pediatric Heart Development and Disease.

The development and function of the heart are governed by a highly coordinated network of regulatory mechanisms, among which miRNAs play a central role. These small, non-coding molecules modulate gene expression predominantly through mRNA degradation. This narrative review aims to summarize current knowledge about biogenesis, its impact on heart development and function, and its clinical implications in pediatric cardiology. We discuss how specific miRNAs contribute to shaping the normal heart and influencing the pathogenesis of congenital malformations. Furthermore, we review disease-specific

The changing landscape of multimodality imaging in congenital heart disease: white paper.

The congenital heart disease (CHD) population has seen substantial growth due to advancements in paediatric cardiac care, cardiothoracic surgery, anaesthesia, and intensive care. Although improved surgical interventions have enhanced survival rates for even the most severe defects, such as hypoplastic left heart syndrome, most CHD patients remain palliated rather than cured, requiring lifelong expert care. With increasing survival, new challenges emerge in managing the ageing CHD population, particularly regarding changing medical, psychosocial needs, and the evolving epidemiology of CHD. Imag

Challenges to overcome obstacles for pediatric donor heart availability.

Although surgical techniques for congenital heart disease (CHD), including hypoplastic left heart syndrome, have progressively advanced, pediatric heart transplantation remains the most effective surgical option for complex CHD and cardiomyopathy with severe heart failure. However, donor heart availability for children continues to be limited. This article reviews past and current challenges in overcoming obstacles to pediatric donor heart availability.

Genome Sequencing Identifies a Heterozygous Deletion of RBFOX2 in a Family With Congenital Heart Disease: A Case Report.

We report a 15-year-old female with hypoplastic left heart syndrome and a strong family history of congenital heart defects. Quad genome sequencing was performed following negative chromosomal microarray and congenital structural heart disease gene panel testing, revealing a heterozygous 144 kb deletion encompassing exons 4-14 of RBFOX2, the entirety of the APOL5 gene, and exon 3 of APOL6 in all affected family members. Although RBFOX2 has been implicated in causing hypoplastic left heart syndrome in animal models, and variants in this gene are enriched in patients with congenital heart diseas

Textbook Outcome for the Fontan Operation: A Holistic Quality Metric in Congenital Heart Surgery.

OBJECTIVE: To develop a holistic measure of congenital heart center performance, we created a composite "textbook outcome" (TO) for the Fontan operation using postoperative endpoints. We hypothesized that achieving the TO would have a positive prognostic and financial impact. METHODS: This was a single-center study of primary Fontan operations from 2005 to 2022. TO was defined as freedom from operative mortality, reintervention, 30-day readmission, extracorporeal membrane oxygenation, major thrombotic complication, chylothorax, >75th percentile length of stay, and >75th percentile mechanical

Use and Outcomes of the Medical Hybrid Procedure For Stage 1 Palliation in Infants With Hypoplastic Left Heart Syndrome and Variants.

BACKGROUND: Staged palliation of hypoplastic left heart syndrome and variants begins with the Norwood or hybrid procedure. Hybrid palliation is used in a minority of cases and often reserved for high-risk patients. Stented hybrid (SH) comprises bilateral pulmonary artery bands and ductal stenting, and medical hybrid (MH) comprises bilateral pulmonary artery bands and prostaglandins. MH use and outcomes have not been well described. We sought to compare MH, SH, and surgical stage 1 (SS1) using a national database. METHODS: Patients from the National Pediatric Cardiology Quality Improvement Col

Fetal Diagnosis of Hypoplastic Left Heart Syndrome With Restrictive Atrial Septum-Atrial Septal Morphology, Associated Lung Disease and Outcomes.

OBJECTIVE: Fetuses with hypoplastic left heart syndrome (HLHS) and restrictive/intact atrial septum (RAS) have high mortality, partly due to pulmonary lymphangiectasia (PL). This study aimed to characterize atrial septal morphology in fetuses with HLHS and RAS and evaluate the impact of fetal intervention on PL and outcomes. METHOD: We retrospectively reviewed clinical data, fetal echocardiograms, and MRIs from all fetuses with HLHS/HLHS-variants and RAS over 20 years. RAS was defined as pulmonary venous forward-to-reverse VTI ratio ≤ 5:1. RESULTS: We identified 54 fetuses (gestational age o

A Patient-Specific Computational Model for Neonates and Infants with Borderline Left Ventricles.

PURPOSE: Borderline left ventricle (BLV) presents a dilemma between pursuing a biventricular repair (BiVR) and a Stage 1 palliation (S1P) because a discordant pursuit of BiVR increases mortality risk. We aim to develop and validate a personalized computational model to assist surgical decision-making by predicting virtual surgery hemodynamics in BLV patients. METHODS: We developed a novel multi-block lumped parameter network (LPN) model of a BLV circulatory system. Patient-specific model parameters were estimated using a semi-automatic tuning framework to fit clinical data in ten retrospectiv

Histologic changes in systemically loaded right ventricles at time of transplantation.

INTRODUCTION: In a subset of complex congenital heart disease patients, the right ventricle (RV) is connected to the higher-resistance systemic circulation, often leading to RV dysfunction. This study characterizes the systemic RV histology at time of heart transplantation in two groups - patients with hypoplastic left heart syndrome with Fontan palliation (HLHS-F) and those with dextro-transposition of the great arteries post atrial switch operation (d-TGA-AS). We sought to better understand histological differences between the systemic RV in the single ventricle vs. biventricular circulation

Hybrid procedures in the management of hypoplastic left heart syndrome: a systematic review.

BACKGROUND/OBJECTIVES: Hypoplastic left heart syndrome is considered one of the most severe CHDs and occurs in approximately 2-3% of all CHD cases. Hybrid procedures have been introduced as an alternative to traditional surgical techniques, such as the Norwood procedure, particularly for neonates at high risk. Although hybrid approaches claim to reduce surgical risks and improve recovery, little is known regarding their comparative effectiveness and safety. This review aims to describe the contribution of hybrid procedures in hypoplastic left heart syndrome management regarding survival rates,

Fetal magnetic resonance imaging diagnosis of pulmonary lymphangiectasia in hypoplastic left heart syndrome: Association with fetal echocardiography and postnatal outcome.

BACKGROUND: Secondary pulmonary lymphangiectasia (PL) is a recognised complication of hypoplastic left heart syndrome (HLHS) with an intact or restrictive atrial septum, associated with poor postnatal outcomes. Fetal MRI has been increasingly used to assess pulmonary abnormalities in HLHS, but the prognostic significance of subtle PL-like changes remains unclear. In this study, we evaluate the relationship between fetal MRI lung findings, echocardiographic markers of pulmonary venous obstruction, and postnatal outcomes. METHODS: A retrospective analysis of all fetuses with HLHS who underwent

The rudimentary left ventricle does not impact on right ventricular size and function in hypoplastic left heart syndrome during serial follow up after Fontan completion.

BACKGROUND: Previous studies in patients with hypoplastic left heart syndrome (HLHS) suggested that a larger left ventricle (LV) might negatively impact right ventricular (RV) function. This study aimed to analyse the impact of the presence of an LV remnant on RV size and function during serial follow up. METHODS: Serial cardiovascular magnetic resonance (CMR) examinations after completion of the total cavopulmonary connection were retrospectively analysed. Patients were divided into those with and those without a rudimentary LV. RV and LV end diastolic and end systolic volumes as well as str

Management of cardiopulmonary bypass in children with hypoplastic left heart syndrome undergoing multiple sternotomy procedures: a case report.

BACKGROUND: Hypoplastic left heart syndrome (HLHS) is a severe and complex congenital heart defect characterized by the underdevelopment of the left-sided cardiac structures. Management of HLHS typically involves multiple staged surgical interventions, often requiring repeated use of cardiopulmonary bypass (CPB). Optimizing CPB strategies is crucial for improving outcomes in these patients. CASE DESCRIPTION: We present the case of a pediatric patient with HLHS who underwent eight surgical procedures, six of which involved the use of CPB. The case highlights the evolution of CPB management str
