Interstage mortality remains high after introduction of right ventricle (RV) to pulmonary artery conduits to stage I palliation for hypoplastic left heart syndrome (HLHS). Prior studies have not shown long-term survival benefit with conduits, likely due to multiple factors including regurgitation contributing to decreased RV function. Valved conduits (VC) were introduced to address these limitations. The study aimed to assess differences in RV size and function using echocardiography during the interstage period between patients undergoing stage I palliation with VC versus non-valved conduits

BACKGROUND: Duct-dependent congenital heart diseases (CHDs) require prompt postnatal intervention to prevent life-threatening deterioration. While prenatal diagnosis may improve outcomes, the impact of route of admission on preoperative mortality remains unclear in Japan.
METHODS: This cross-sectional study examines the impact of the route of admission on preoperative mortality in 2704 admissions of patients with duct-dependent CHD between 2014 and 2019, at hospitals employing the Diagnostic Procedure Combination payment system. The route of admission was categorized as either born in hospita

BACKGROUND: Hypoplastic left heart syndrome (HLHS) is a severe congenital heart defect characterised by underdevelopment of left-sided cardiac structures. While genetic predisposition contributes to HLHS, the relevance of environmental stressors is increasingly recognised, yet the cellular mechanisms linking genetic susceptibility to environmental vulnerability remain unclear. We aimed to identify molecular and functional differences between cardiomyocytes derived from HLHS patients and healthy controls to uncover potential susceptibilities contributing to the HLHS phenotype.

Neonates with single ventricle congenital heart disease (SV CHD) may have noncardiac comorbidities, including congenital airway anomalies (CAA). Using a national administrative database, we analyzed neonatal admissions with ICD-10 codes for SV CHD with and without CAA from October 2020 to December 2024, excluding admissions with genetic syndromes to reduce confounding by the potential contribution of syndromes to CAA. Among 5144 neonatal admissions with SV CHD, 535 (10.4%) had CAA, compared to 23,837 (0.4%) of 5,942,621 neonatal admissions without CHD (odds ratio [OR] [95% confidence interval]

BACKGROUND: In utero stenting of the atrial septum has been introduced for fetuses with hypoplastic left heart syndrome (HLHS) and intact atrial septum (IAS) to prevent secondary pulmonary vascular damage caused by left atrial hypertension. Previous reports revealed a high risk of significant or complete in-stent obstruction after successful intervention due to endothelial proliferation.
CASE SUMMARY: We report a fetus with HLHS and IAS complicated by nutmeg lung who underwent successful fetal atrial septal stenting using an everolimus-eluting stent (Xience Sierra; Abbott). The procedure resu

Fontan-associated liver disease (FALD) is a disorder arising from hemodynamic changes and venous congestion in the liver. This disease is prominent in patients with hypoplastic left heart syndrome (HLHS). Although HLHS patients typically survive into adulthood, they have reduced cardiac output due to their univentricular physiology (i.e., a Fontan circuit). As a result, they have insufficient blood delivery to the liver. In comparison, patients with double outlet right ventricle (DORV), also having a univentricular circuit, have a lower incidence of FALD. In this study, we use a patient-specif

Infants with hypoplastic left heart syndrome undergoing single ventricle (SV) palliation may need early heart transplant, but optimal referral timing is unknown, and referrals may be inequitable. We performed a retrospective study of infants undergoing stage 1 SV palliation (S1P) from 2016-2022 enrolled in the National Pediatric Cardiology-Quality Improvement Collaborative (NPC-QIC). Competing risk methods were used to evaluate predictors of Transplant Referral (TxR) and death without TxR. Neighborhood opportunity was determined through use of Child Opportunity Index (COI). CASE SUMMARY: An 8-day-old boy with univentricular physiology presented with respiratory failure and persistent hypoxemia. Cardiac computed tomography angiography showed a dominant right ventricle with mitral atresia and an intact atrial septum, double-outlet right ventricle, and persistent ductus arteriosus. Pulmonary veins connected to the left atrium but were markedly dilated, with anomalous dual drainage through a levoatrial cardinal vein into the innominate and azygos veins. Despite emergent atrial septectomy, pulmonary artery banding, and ductal ligation, the outcome was unfavorable.

BACKGROUND: Neonates with congenital heart disease may require delivery room interventions beyond standard care. Because existing guidelines and educational programs rarely address these needs, we developed and evaluated targeted educational modules.
METHODS: Teams were randomized to online eLearning modules or directed readings. Individual pre-/post-knowledge scores were compared. Team performance was tested in simulated delivery scenarios including abruption, hypoplastic left heart syndrome, d-transposition of great arteries and congenital complete heart block.
RESULTS: 32 teams from 9 cen

OBJECTIVE: In the modified Norwood procedure for hypoplastic left heart syndrome, right ventricle-to-pulmonary artery (RV-PA) conduit laterality is largely surgeon dependent. We evaluated whether conduit laterality influences early outcomes and subsequent bidirectional Glenn (BDG) surgery.
METHODS: We retrospectively reviewed consecutive patients with hypoplastic left heart syndrome or hypoplastic left heart complex undergoing Norwood with an RV-PA conduit between September 2023 and March 2026. Patients were stratified according to whether the RV-PA conduit was positioned to the right (R-PA)

The valved Sano (VS) right ventricle-to-pulmonary artery (RV-PA) shunt was developed to eliminate conduit regurgitation and thereby decrease volume load on the single right ventricle (RV) after the Norwood procedure. By reducing diastolic backflow, VS may limit tricuspid regurgitation and preserve RV function. Additional theoretical advantages include facilitated postoperative recovery and improved pulmonary artery (PA) growth. Despite compelling physiology, available evidence remains limited and largely retrospective. We describe the postoperative course of a low-weight neonate with hypoplast

UNLABELLED: Hypoplastic left heart syndrome (HLHS) is a critical congenital heart that requires complex surgical and medical management. Despite recent clinical advances, population-level data on infant mortality trends related to HLHS remain limited. This study examines temporal patterns in HLHS-related infant mortality rates (HLHS-IMR), timing of death, interstate variation, and demographic disparities, along with updated prevalence estimates. We analyzed U.S national data from 2007 to 2021 using CDC-WONDER linked births/deaths database. HLHS-related infant mortality rates (HLHS-IMR), identi

BACKGROUND: Neonates with congenital univentricular conditions require staged palliation culminating in a Fontan circulation to support survival. The first-stage Norwood procedure carries substantial risk of myocardial dysfunction, frequently necessitating extracorporeal membrane oxygenation (ECMO) support. The aim of this systematic review was to examine the incidence, risk factors, and outcomes of post-Norwood ECMO use during the interstage period.
METHODS: We systematically searched MEDLINE, Embase, Cochrane Library, and ClinicalTrials.gov for studies published between January 1990 and Mar
