An updated systematic review of the impact of volume of surgery and specialization in Norwood procedure.

BACKGROUND: Volume-outcome relationship proposed to exist for high-risk, low-volume procedures, such as the Norwood procedure. A systematic review published in 2014 examined impacts of hospital and surgeon volume, specialization, regionalization and teaching status on the patient-related outcomes of the Norwood procedure. The aim of this systematic review was to update the 2014 work by synthesizing current evidence alongside the original review. METHODS: We searched PubMed, Embase (Elsevier), and the Cochrane Library for peer-reviewed comparative studies published from 1 March 2013 to 30 Dece

Association of Digoxin Use at Norwood Discharge with Fontan Completion: A Study from the Pediatric Heart Network Public Dataset.

BACKGROUND: Digoxin use after the Norwood procedure has been associated with improved interstage survival in hypoplastic left heart syndrome and related conditions. Whether this benefit translates into improved longer-term outcomes through staged palliation remains unknown. We aimed to determine the association of digoxin use at Norwood discharge with transplant-free survival and Fontan completion. METHODS: We conducted a retrospective cohort study using the Pediatric Heart Network (PHN) Single Ventricle Reconstruction trial public dataset, including 549 infants enrolled at 15 North American

Outcomes of the Rapid Two-Stage Norwood Procedure in Hypoplastic Left Heart Syndrome: A Retrospective Cohort Study Stratified by Birth Weight.

BACKGROUND: We evaluated the surgical outcomes of the rapid two-stage Norwood procedure for hypoplastic left heart syndrome (HLHS) and its variants, focusing on low birth weight. METHOD: From October 2010 to September 2023, 67 consecutive children with HLHS or HLHS variants underwent bilateral pulmonary artery banding (bil-PAB). They were categorised into Group S (birth weight <2,500 g) and Group L (birth weight ≥2,500 g). Clinical perioperative data were analysed. RESULTS: There were no inter-stage deaths or patients requiring comfort care in either group. The in-hospital mortality rate for

Hybrid vs. Norwood: An Analysis of the NPC-QIC Database to Determine Optimal Approach for High-Risk Single Ventricle Patients.

We sought to identify the optimal management strategy of infants with high-risk hypoplastic left heart syndrome and variants (HR-SV) and identify factors associated with futility. Patients with birth weight ≤ 2.5 kg or gestation ≤ 35 weeks, and age ≤ 30 days at admission were included (N = 398). Norwood (n = 225), hybrid (pulmonary artery band (PAB) + ductal stent, n = 76), and PAB with prostaglandin (PAB/PGE, n = 77) were compared from the National Pediatric Cardiology Quality Improvement Collaborative database. Transplantation referral (n = 1) and comfort care (n = 19) occurred. Baseline fac

Generation of the maternal iPSC line NCHi030-A from an unaffected heterozygous NOTCH1 variant carrier in a family with hypoplastic left heart syndrome.

Mutations in the NOTCH1 gene are associated with a higher incidence of hypoplastic left heart syndrome (HLHS) in newborns. NCHi030-A is an iPSC line derived from peripheral blood mononuclear cells of the unaffected mother of a patient with hypoplastic left heart syndrome. Both individuals harbor the same heterozygous NOTCH1 missense variant (c.2058G > A); however, only the child is affected, suggesting incomplete penetrance. NCHi030-A serves as a genetically matched, unaffected control for comparative studies with NCHi009-A, an iPSC line derived from the subject's symptomatic HLHS daughter, to

Protein-losing enteropathy in a child with hypoplastic left heart syndrome after Fontan palliation: a case report.

UNLABELLED: Protein-losing enteropathy (PLE) is a rare but serious complication of the Fontan procedure in patients with single-ventricle physiology. It results from excessive loss of plasma proteins into the gastrointestinal tract, leading to hypoalbuminemia, edema, and ascites. We report the case of a 10-year-old girl with hypoplastic left heart syndrome who underwent staged surgical palliation including the Norwood procedure, bidirectional Glenn shunt, and extracardiac Fontan completion. She presented with progressive abdominal distension, peripheral edema, and severe hypoalbuminemia (album

Necrotizing Enterocolitis in Infants with Hybrid Stage 1 Palliation is Common and Difficult to Predict.

Hybrid stage 1 palliation (HS1P) is performed at many centers for patients with single ventricle heart disease or complex biventricular heart disease and high-risk features. Necrotizing enterocolitis (NEC) is common in these infants, although specific risk factors have not been identified to predict those who will develop NEC. A single-center retrospective chart review of 105 infants who underwent HS1P at < 30 days of age was performed. Clinical, procedural, and echocardiographic features at HS1P and prior to NEC episodes were collected. Hypoplastic left heart syndrome accounted for 54.3% of p

Surgical strategies to avoid Glenn physiology during staged left heart recruitment.

OBJECTIVES: Staged left ventricular recruitment can promote left ventricular growth through controlled volume loading. The optimal pulmonary blood flow source and the need for the bidirectional Glenn during staged left ventricular recruitment remain unclear. This study investigates staged left ventricular recruitment outcomes in patients who underwent staged left ventricular recruitment with or without the bidirectional Glenn. METHODS: This is a single-institution retrospective review of children undergoing staged left ventricular recruitment (2014-2024). Patients were stratified by pulmonary

Sex-stratified trends in hypoplastic left heart syndrome-related mortality among children and young adults in the United States, 1999-2024: a pre- and post-COVID-19 analysis.

BACKGROUND: Hypoplastic left heart syndrome (HLHS) is a congenital heart disease that accounts for approximately 3% of congenital heart defects, making it a leading cause of mortality in this population. Timely staged palliation, such as Norwood, Glenn, and Fontan, are required to reduce the incidence of adverse outcomes. Data over the long term suggest that only about 31% of patients with HLHS are alive at age 35 years after Fontan completion. However, national mortality trend data stratified by sex, extending through 2024 and spanning the COVID-19 era, have not been characterised. METHODS:

Multicenter comparison of hybrid and Norwood procedures in patients with Fontan circulation.

OBJECTIVE: The hybrid stage 1 palliation (HS1P) procedure has been proposed as an alternative to the Norwood operation. We utilized a multicenter database to compare both strategies. METHODS: The Fontan Outcomes Registry using Clinical Examinations was queried for patients who underwent HS1P or Norwood. Propensity score matching was performed. Composite outcome (death, transplant listing, protein losing enteropathy, plastic bronchitis, atrial/ventricular tachyarrhythmia, or pulmonary artery reintervention) and cardiac magnetic resonance variables were compared. Secondary analyses compared bet

Outcomes of Delayed Norwood Operation in High-Risk Patients-Time to Reconsider the Dogma of a Primary Norwood Operation?

OBJECTIVES: The delayed Norwood (DN) approach, preceded by bilateral pulmonary artery banding (bPAB), has emerged as a strategy to improve outcomes in high-risk infants by addressing neonatal haemodynamic instability and organ immaturity. METHODS: This single-centre, retrospective study analysed 20 consecutive patients with hypoplastic left heart syndrome/variants and multiple risk factors who underwent a DN operation following bPAB between 2019 and 2024. RESULTS: Bilateral pulmonary artery banding was performed at a median age of 2.5 days (interquartile range [IQR]: 2-4). Preoperative risk

Transcatheter pulmonary flow restriction in congenital heart disease: evolving concepts and future perspectives.

Pulmonary artery banding (PAB) remains an important palliative strategy in congenital heart disease (CHD), particularly for temporary modulation of pulmonary blood flow and pressure during staged management. Over the past two decades, catheter-based pulmonary flow restrictors (PFRs) have emerged as less invasive alternatives to surgery, expanding treatment options for fragile infants and selected older patients. This review traces the evolution of transcatheter PFR, from early experimental concepts to off-label use of manually fenestrated Medtronic microvascular plugs (MVPs). It examines devic

Unveiling the Healthcare Journey of Patients with Hypoplastic Left Heart Syndrome: From Birth to Adulthood.

ObjectiveWhile many studies view patients with hypoplastic left heart syndrome (HLHS) through the lens of financial burden, an overall sense of the medical burden of these patients has rarely been reported. We sought to introduce a granular analysis of the medical burden of patients with HLHS to capture a view of their journey to adulthood.MethodsChildren with HLHS (≤18 years) born from 2004 to 2008 with 18 years of follow-up were identified in the Pediatric Health Information System (PHIS). Children who did not have 18 years of complete follow-up were excluded.ResultsBy 1 year of age, a child
