Norwood procedure with left coronary artery reimplantation for hypoplastic left heart syndrome combined with Anomalous left coronary artery from the right pulmonary artery: a case report.

BACKGROUND: Anomalous origin of the left coronary artery from the pulmonary artery is a rare congenital heart defect associated with high mortality during infancy if not surgically corrected. Its occurrence combined with hypoplastic left heart syndrome is highly uncommon, presenting considerable diagnostic and technical challenges. Identifying such an anomaly before surgery can be challenging, particularly in patients with complex cardiac anatomy. Intraoperative recognition requires immediate adjustments of the surgical approach, especially regarding myocardial protection and coronary artery r

Serial changes of mitral and tricuspid valve diameter after the Norwood procedure in patients with aortic atresia.

BACKGROUND: In hypoplastic left heart syndrome with aortic atresia, the non-functional left ventricle may influence right ventricular performance. We evaluated serial changes in tricuspid and mitral valve diameters after the Norwood procedure and their relationship to right ventricular function. METHODS: Of 150 patients who underwent the Norwood procedure (2001-2020), 47 died before total cavopulmonary connection. Of the 103 survivors, 76 with sufficient serial echocardiographic data were analysed (1,399 examinations). Tricuspid and mitral valve diameters were measured, and right ventricular

Circulating Factors in Single Ventricle Heart Disease Impair Cardiomyocyte Bioenergetics, With Partial Rescue by PDE5 Inhibition.

BACKGROUND: Although operative and perioperative care continues to improve for patients with single ventricle (SV) congenital heart disease, long-term morbidities and mortality remain high. Importantly, phosphodiesterase-5 inhibitor (PDE5i) therapies are increasingly used, but their direct myocardial effects in SV congenital heart disease remain poorly understood. We previously demonstrated that the failing SV myocardium is characterized by increased PDE5 (phosphodiesterase-5) activity and impaired mitochondrial bioenergetics. Here we sought to determine whether systemic circulating factors co

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Isolated foetal left ventricular apical hypoplasia: would you know how to spot it?

Isolated left ventricular apical hypoplasia is a rare cardiac malformation. This entity is easily diagnosed if its key features are recognised, but will puzzle the clinician who is not aware of its existence. It is characterised by echocardiographic findings of a spherical left ventricle, an elongated right ventricle wrapping around the deficient left ventricular apex, and abnormal papillary muscles' origin.

Proposal: Bold New Indications for Transcatheter Pulmonary Flow Restrictors.

This proposal presents a proof of concept for the use of pulmonary flow restrictors (PFRs) based on MVP™-devices, drawing from clinical experience, and explores their potential role in the management of newborns with hypoplastic left heart syndrome (HLHS), other complex left heart lesions, and infants with end-stage dilated cardiomyopathy (DCM). At this early stage of age, manually adjusted PFRs can be tailored to patient's size and hemodynamic needs. Although currently used off-label, PFRs have substantial potential to improve outcomes in these vulnerable patient populations. When integrated

Mitotic block and epigenetic repression underlie neurodevelopmental defects and neurobehavioral deficits in congenital heart disease.

Hypoplastic left heart syndrome (HLHS) is a severe congenital heart disease associated with microcephaly and poor neurodevelopmental outcomes. Here we show that the Ohia HLHS mouse model, with mutations in Sap130, a chromatin modifier, and Pcdha9, a cell adhesion protein, also exhibits microcephaly associated with mitotic block and increased apoptosis leading to impaired cortical neurogenesis. Transcriptome profiling, DNA methylation, and Sap130 ChIPseq analyses all demonstrate dysregulation of genes associated with autism and cognitive impairment. This includes perturbation of REST transcript

Extracorporeal cardiopulmonary resuscitation outcomes in pre-Glenn single ventricle infants: Analysis of a ten-year dataset.

BACKGROUND: While several studies have reported on outcomes of extracorporeal membrane oxygenation (ECMO) in patients with single ventricle physiology, few studies have described outcomes of extracorporeal cardiopulmonary resuscitation (ECPR) in this unique population. The objective of this study was to determine survival and risk factors for mortality after ECPR in single ventricle patients prior to superior cavopulmonary anastomosis, using a large sample from the Extracorporeal Life Support Organization (ELSO) Registry. METHODS: We included single ventricle patients who underwent ECPR for i

Neo-aorta reconstruction with retained stented patch in Norwood procedure after hybrid palliation of high-risk hypoplastic left heart syndrome: a case report.

Hybrid procedure of hypoplastic left heart syndrome, comprising ductus arteriosus stenting and bilateral pulmonary artery banding, is a good surgical option for initial palliative procedure for high-risk patients for Norwood procedure. However, ductal stenting may cause retrograde aortic blood flow obstruction. Furthermore, complete removal of stent while performing the Norwood procedure make the operation more difficult. We report a case that overcame these problems using a novel surgical technique.

A Controversial Clinical Case of a Child With Hypoplastic Left Heart Syndrome.

We present the case of a child born with hypoplastic left heart syndrome. After developing multiple complications following the first surgical stage (Norwood procedure), her parents decide not to proceed with the second stage (Glenn operation). Cardiac surgeons, pediatric intensivists, a psychologist, and a bioethicist analyze whether further surgical intervention is ethically obligatory. A cardiac surgeon believes that after getting beyond the difficulties of the first stage, the patient seems to be a reasonable candidate for the next procedure. He considers the intervention to be ethically p

First report of hypoplastic left heart syndrome in 3p- syndrome and review of candidate genes.

OBJECTIVE: 3p deletion syndrome is a rare monosomal disease that encompasses deletions throughout the short arm of chromosome 3. It is often in the distal region (3p25-pter), but variations in breakpoints and a complex clinical manifestation exist, with congenital heart defects being considered rare. We present the first case of hypoplastic left heart syndrome and minor dysmorphic features associated with 3p- syndrome. Furthermore, we aim to establish a gene-phenotype association. CASE DESCRIPTION: The diagnosis was made by karyotyping, followed by a literature investigation and in silico bio

Volume-Outcome Relationship of Norwood Procedures: Insights from the National Pediatric Cardiology-Quality Improvement Collaborative Database.

BACKGROUND: Prior investigations of the center-specific case volume on outcomes in hypoplastic left heart syndrome have conflicting results. This study utilized the National Pediatric Cardiology-Quality Improvement Collaborative registry to investigate the center volume-outcome relationship in patients after the Norwood procedure with consideration of preoperative high-risk features. METHODS: Between 2016 and 2023, centers were categorized by Norwood procedure volume into low- (≤5 cases/y), medium- (6 to 10 cases/y), and high-volume centers (>10 cases/y). We compared preoperative high-risk fe

Impact of Lesion Severity in Fetal Cardiac Counseling.

Approximately 1% of all live births in the USA are affected by congenital heart disease (CHD), the leading cause of congenital defect-related illness and infant death. Although technological innovations have improved CHD diagnosis in utero, variation among fetal cardiac counseling practices persists. Our study aims to evaluate physician counseling content based on cardiac defect complexity. We conducted an anonymous survey of providers who perform fetal cardiac counseling. Participants stated their counseling practices for 21 CHD lesions, choosing from postnatal surgical intervention, terminat

Noninvasive assessment of myocardial perfusion using ultrafast ultrasound: clinical study for congenital heart disease.

AIMS: Myocardial perfusion impacts cardiac function following surgical repair of critical congenital heart disease (CCHD). Temporal variation assessment of myocardial blood volume throughout the cardiac cycle can be a surrogate for perfusion. Ultrafast power Doppler (UPD) is an ultrasound imaging technique capable of noninvasively quantifying myocardial blood volume changes. The objective of this study is to demonstrate the feasibility of perioperative transthoracic UPD assessment and to determine if UPD reflects physiologic changes in myocardial perfusion. METHODS AND RESULTS: Five neonatal
