Autologous umbilical cord blood mononuclear cell therapy for hypoplastic left heart syndrome: a nonrandomized control trial of the efficacy and safety of intramyocardial injections.

BACKGROUND: Preliminary phase I clinical trial results revealed that autologous umbilical cord blood-derived mononuclear cells (UCB-MNCs) preserved right ventricular cardiac function. To establish the efficacy of intramyocardial injections of an autologous UCB-MNC product at the time of stage II palliation surgery in patients with hypoplastic left heart syndrome (HLHS). METHODS: A phase IIb, multicenter, open-label, nonrandomized study was conducted. Ninety-five children (fifty treated and forty-five controls) with HLHS and its variants, a history of stage I palliation surgery, and planned st

The ERBB2 c.1795C>T, p.Arg599Cys variant is associated with left ventricular outflow tract obstruction defects in humans.

Non-syndromic congenital heart defects (CHDs) are occasionally familial and left ventricular outflow tract obstruction (LVOTO) defects are among the subtypes with the highest hereditability. The aim of this study was to evaluate the pathogenicity of a heterozygous ERBB2 variant c.1795C>T, p.Arg599Cys identified in three families with LVOTO defects. Variant detection was done with exome sequencing. Western blotting, digital PCR, mass spectrometry (MS), MS microscopy, and flow cytometry were used to study the function of the ERBB2 variant c.1795C>T. Cardiac structure and function were studied in

Don't Give up on the Left Ventricle! Surgical Strategies for Recruitment of the Borderline Left Heart.

Patients with hypoplastic left heart syndrome often undergo a definitive decision in infancy regarding lifetime circulatory management. However, the decision between single ventricle palliation or primary biventricular repair is less obvious for patients with 'borderline left heart' structures. An alternative strategy for the patient with borderline left heart is staged left ventricular recruitment (SLVR). SLVR refers to the strategy in which initial stage I palliation is pursued, and interstage procedures are then performed to promote flow through the left heart and induce growth of left hear

Hypoplastic left heart syndrome-a scoping review.

BACKGROUND: An estimated 3% of all newborns with congenital heart disease develop hypoplastic left heart syndrome (HLHS), making it a prominent cause of mortality in this group if surgical procedures or a heart transplant are not implemented. While compelling evidence supports a genetic element, identifying a particular genetic cause is limited to a subgroup of patients, indicating a complex and multifaceted origin for this condition. The objective of this scientific contribution was to identify, synthesize, and analyze the scientific knowledge produced regarding the implications of researchin

Respiratory Function of Fontan Pediatric Patients with Hypoplastic Left Heart Syndrome and Other Morphologies of Functionally Single Ventricle-A Pilot Study.

Background/Objectives: Management of complex congenital heart defects with functionally single ventricle remains one of the greatest challenges of pediatric cardiology. The multistage surgical treatment completed with Fontan procedure is related to multiple complications. Due to non-pulsatile continuous pulmonary flow and chronic hypoxia, Fontan circulation may induce pulmonary endothelial dysfunction. However, the impact of Fontan physiology on respiratory system function is not well studied. The aim of the research was to assess respiratory function in Fontan pediatric patients with hypoplas

Thrombotic Complications of Low-Dose Versus High-Dose Four-Factor Prothrombin Complex Concentrate for Massive Bleeding in Pediatric Cardiovascular Surgery.

INTRODUCTION: Major bleeding is a life-threatening complication in pediatric cardiovascular surgery, particularly in neonates and infants with immature coagulation systems and congenital heart defects (CHD). Cardiopulmonary bypass (CPB) exacerbates these risks by depleting coagulation factors and platelets. Four-factor prothrombin complex concentrates (4F-PCC) provide targeted hemostatic correction but carry potential thrombotic risks. METHODS: Among 1296 pediatric patients who underwent cardiovascular surgery between January 2018 and April 2024, a retrospective cohort study analyzed 104 pati

Early Neurodevelopmental Outcomes in Patients Who Undergo Hybrid Stage I Palliation Compared to Norwood Stage I Palliation for Hypoplastic Left Heart Syndrome.

Children with congenital heart disease are at increased risk of neurodevelopmental impairment and those with hypoplastic left heart syndrome (HLHS) are among the highest risk group. The first stage of palliation for HLHS, typically performed in the newborn period, is either a Norwood stage I procedure (NS1P) or hybrid stage 1 procedure (HS1P). Our study sought to evaluate the neurodevelopmental outcomes of patients who undergo HS1P compared to NS1P using multicenter registry data. The National Pediatric Cardiology Quality Improvement Collaborative (NPC-QIC) registry was used to identify infant

Cellular and Transcriptional Landscape of Human Hypoplastic Left Heart Syndrome.

Hypoplastic left heart syndrome (HLHS) is a congenital heart defect characterized by impaired development of the left ventricle, often managed through surgical palliation creating a single ventricle (SV). Failure of the anatomical right ventricle (RV) represents a common complication with high mortality. We used single-nucleus RNA sequencing to generate a map of the pediatric non-failing (NF) and failing (SysHF) SV. Fibroblasts and endocardial cells displayed the greatest transcriptional shifts between NF and SysHF. Notably, activated fibroblasts expanded in SysHF, and endocardial cells in NF

Case report: a thrombus to remember: a case of warfarin non-compliance in a total cavopulmonary circulation patient.

BACKGROUND: The lack of pulsatile nature of the pulmonary blood flow through total cavopulmonary circulation (TCPC) has shown to prompt endothelial dysfunction thus creating a hypercoagulable state. Patients with a Fontan-type circulation are, therefore, at an increased risk of venous and arterial thromboembolism. In the setting of hypoplastic left heart syndrome following staged surgical repair, subaortic thrombosis is relatively common, but thrombosis of the hypoplastic aortic root is less commonly described. CASE SUMMARY: Here, we present the case of a 25-year-old male with a background of
