Dextrocardia with situs solitus poses significant surgical challenges for left ventricular assist device (LVAD) implantation, particularly in patients with prior sternotomy and complex valvular disease. We report on a 60 year old man with dextrocardia, advanced heart failure, prior mechanical mitral valve replacement, and bicuspid aortic valve disease who underwent successful Corheart 6 LVAD implantation during redo sternotomy with concomitant bioprosthetic aortic and mitral valve replacement. Intraoperative findings included extensive pannus/thrombus on the mechanical mitral prosthesis (causi

This educational review describes the historical development of pediatric congenital cardiac anesthesia and its expanding role in the care of patients with congenital heart disease (CHD). Over the past eight decades, the specialty has evolved from an era in which few congenital heart defects were surgically treatable and perioperative mortality was high to one in which survival into adulthood is expected for most children with CHD. This transformation has been driven by parallel advances in surgical techniques, cardiopulmonary bypass technology, myocardial protection, monitoring, intensive car

PURPOSE: Neonatal hemostatic immaturity complicates anticoagulation during cardiopulmonary bypass (CPB). Current practice largely follows the strategy of 400 U/kg from adult protocols, as neonatal-specific guidelines and high-quality evidence remain lacking, which frequently fails to achieve adequate anticoagulation. This single-center randomized controlled trial evaluated the initial heparin dose of 600 U/kg to refine neonatal CPB anticoagulation.
PATIENTS AND METHODS: In this prospective, randomized, controlled, single-blind trial, 76 neonates undergoing CPB were assigned to receive an init

Left ventricular dysfunction and thrombosis in unrepaired tetralogy of Fallot are rarely reported. We report the case of a 9-year-old boy with unrepaired tetralogy of Fallot, severe biventricular dysfunction, large left ventricular thrombus, and diffuse subendocardial fibroelastosis. After surgical repair, recovery of ventricular function and thrombus resolution were observed. This case highlights the interplay between cyanosis, myocardial remodelling, and hypercoagulability and emphasises the role of multimodality imaging.

BACKGROUND AND OBJECTIVES: Univentricular heart is a complex congenital heart disease with rare survival into adulthood. It could be part of a heterotaxy syndrome when, apart from the cardiac malformations, there are associated abnormal arrangements of extra-cardiac organs like the lungs, spleen and pancreas. The syndrome is said to be rarer than the univentricular heart and to have a worse clinical outcome.
METHODS: After clinical assessment, the patient was investigated with Chest X-ray, Electrocardiography, Echocardiography, and contrast-enhanced CT scan of the chest and abdomen to make th

Background/Objectives: Noonan syndrome (NS) is characterized by marked phenotypic variability. Although the classical phenotype includes short stature, craniofacial dysmorphism, skeletal abnormalities, and congenital heart defects, some patients present with subtle or atypical manifestations, making clinical recognition challenging. Methods: This retrospective case series included six pediatric patients with molecularly confirmed NS managed at the Department of Endocrinology and Metabolic Diseases, Polish Mother's Memorial Hospital-Research Institute, Lodz, Poland. Medical records were reviewe

Duplications of distal 20q are uncommon and the connection to oculo-auriculo-vertebral spectrum (OAVS) is still not clear. In this paper we describe a family-based cytogenetic case of a geographically isolated community in Northern Italy. Clinical data, pedigree information, and cytogenetic report from the proband were collected. Relatives were examined for OAVS-related features. Subtelomeric FISH analysis was performed with probes for 20qter and D22S1726 using peripheral blood lymphocytes to assess the rearrangement in detail. Descriptive statistics with 95% confidence intervals were computed

Innovation in paediatric and adult congenital cardiology increasingly depends on collaboration among academia, industry, and professional communities. From this perspective, the author argues that clinical prediction represents a natural convergence point for these stakeholders, aligning safe, personalised care with economic incentives. The author discusses emerging evidence highlighting the promise of artificial intelligence-driven prediction across various cardiovascular domains, while highlighting current limitations related to narrow scope, static design, and weak integration into clinical

We report a case of foetal-onset biventricular noncompaction with an MYH7 variant. Prenatal fetal hydrops and right heart failure resolved after birth, but progressive left heart failure developed at 5 months. This transition reflects how postnatal hemodynamic changes influence the biventricular noncompaction phenotype, necessitating vigilant monitoring for left heart failure in foetal-onset biventricular noncompaction cases with MYH7 variants.

BACKGROUND: Adults with repaired tetralogy of Fallot represent a growing clinical population at risk for long-term cardiovascular complications. Characterising their postoperative course and late outcomes is essential to guide surveillance and management.
OBJECTIVES: To describe the clinical characteristics and long-term outcomes of adult patients (≥18 years) with repaired tetralogy of Fallot who received care at a quaternary-care institution between January 2000 and December 2024.
METHODS: We performed a retrospective cohort study. Adult patients with a diagnosis of repaired tetralogy of Fa

Interventional therapy has, as a minimally invasive approach, been widely adopted for treating cardiovascular diseases, particularly structural heart disease, where occluders play a pivotal role in transcatheter closure. This paper comprehensively reviews the evolutionary trajectory and emerging frontiers of cardiovascular occluders, beginning with their interventional applications in congenital heart defects (atrial septal defect, ventricular septal defect, patent foramen ovale, and patent ductus arteriosus) and left atrial appendage closure. The focus then shifts to occluder materials and de

BACKGROUND: Acute type A aortic dissection during pregnancy is a rare but life-threatening condition, with an estimated incidence of approximately 4 cases per 1 million women aged 15-45 years. Hormonal and hemodynamic changes during pregnancy are factors that may predispose to aortic dissection. These changes begin in the first and second trimesters but are most pronounced in the third trimester and the early postpartum period. Genetic syndromes, such as Marfan syndrome, significantly increase the risk of aortic dissection.
CASE PRESENTATION: A 41-year-old pregnant woman at 31 weeks of gestat

Systemic atrioventricular valve (SAVV) regurgitation contributes importantly to systemic right ventricular deterioration in adults with congenitally corrected transposition of the great arteries (ccTGA) and dextro-transposition of the great arteries (d-TGA) after atrial-switch repair. Transcatheter edge-to-edge repair (TEER) has emerged as an option for selected patients at high or prohibitive surgical risk, and prior congenital series have established the feasibility of transseptal, transbaffle, and alternative access strategies. However, procedural execution remains highly anatomy-dependent

Congenital heart defects (CHD) are characterized by pathological intracardiac blood flow, exposing the endocardium to disturbed shear stress. This contributes to the development of endocardial fibroelastosis (EFE), a restrictive fibrotic remodeling process associated with diastolic dysfunction. Despite the pathomechanistic role of the endocardium in CHD, EFE-derived endocardial endothelial cells (EdEECs) remain largely understudied. No standardized protocol exists for isolating and purifying them from human pediatric tissue. Tissue from N = 15 pediatric patients with (1) hypoplastic left heart

BACKGROUND: Most atrial septal defects (ASDs) occur sporadically, but familial cases can occur and carry a 10% risk of transmission to offspring.
CASE SUMMARY: We describe a 3-generation family with autosomal dominant secundum ASD and conduction system disease related to a novel NKX2-5 variant (c.300del; p.Asp101Thrfs∗75). The index patient, a 58-year-old woman with prior ASD repair and mitral valve replacement, presented with chronotropic incompetence, atrial fibrillation, left bundle branch block, and prolonged pauses, leading to pacemaker implantation. Her mother had an ASD, atrial fibrill

BACKGROUND: Marfan syndrome (MS) is associated with mutations in the FBN1 gene, which encodes fibrillin-1, an essential protein in elastic fibers. Cardiovascular damage in MS has serious complications such as aneurysms and aortic dissection (AD), which increase during pregnancy.
OBJECTIVE: To evaluate the cardiovascular and clinical status of women with Marfan syndrome and pregnancy.
MATERIALS AND METHODS: Retrospective study in women with MS or other connective tissue diseases (CTD) classified according to the 2010 Ghent criteria.
RESULTS: One hundred women were included, 35 were admitted

Recent advances in high-resolution genomic technologies have revealed that several syndromes are associated with submicroscopic chromosomal deletions and, less frequently, duplications, thereby substantially contributing to the functional annotation of the human genome. Among these structural variants, recurrent microdeletions or microduplications often disrupt dosage-sensitive genes, leading to shared phenotypic outcomes in affected individuals. While features such as congenital heart defects, neurodevelopmental delays, and intellectual disability (ID) have been extensively studied, phenotype
