Transesophageal echocardiography diagnosis of dextrocardia during cardiac arrest guided optimal chest compression location: A case report.

The incorporation of transesophageal echocardiography (TEE) into cardiac arrest management overcomes several limitations inherent to transthoracic echocardiography, while producing higher-quality images. Among other advantages, TEE can evaluate chest compression quality and location during ongoing compressions. AHA/ACLS guidelines recommend hand placement for compressions over the lower half of the sternum in order to compress the left ventricle (LV). However, literature demonstrates that chest compressions do not align with the LV over half the time, but rather lie over the left ventricular o

Ultrasonographic diagnosis of secondary cardiac position alterations led by fetal thoraco-abdominal anomalies: a report of three cases.

BACKGROUND: Congenital diaphragmatic hernia (CDH) and congenital diaphragmatic eventration (CDE) are rare congenital anomalies. They occur in approximately one in 2,000-3,000 and 2,500-5,000 children respectively. It is difficult to distinguish between CDE and right diaphragmatic hernia in fetal period, which can cause changes in the position of the heart and lead to dysplasia of the right lung; absence of the right lung can lead to secondary dextrocardia. These are common in dextrocardia and lung agenesis, which are the main obstacles for diagnosis. CASE DESCRIPTION: This paper reports on th

Incidental diagnosis of a complex cyanotic congenital heart defect in a 32-year-old with orbital abscess.

This case describes a 32-year-old adult male who was incidentally diagnosed with isolated dextrocardia, Double inlet left ventricle (l-looped), l-transposed great arteries with subpulmonary stenosis during evaluation for an orbital abscess. This case highlights protective factors that enabled this patient's survival into adulthood without cardiac surgeries or medications despite single ventricle physiology, namely his "self-banded" pulmonary flow.

Prenatal Diagnosis of Joubert Syndrome 23 With Left Isomerism: A Novel Phenotype Associated With Pathogenic KIAA0586 Variant.

Joubert syndrome is a rare autosomal recessive ciliopathy defined by the "molar tooth" sign caused by cerebellar vermis hypoplasia and abnormal superior cerebellar peduncles. Over 40 genes are known to cause the disorder, including KIAA0586, which encodes the centrosomal protein TALPID3, essential for ciliogenesis and Hedgehog signaling. Although KIAA0586-related Joubert syndrome is primarily neurodevelopmental, cilia also play a key role in left-right axis formation, however, laterality defects have not been reported in this context. We describe a prenatal case with suspected Joubert syndrome

Dextrocardia With Relapsing Cough in Newly Diagnosed Tuberculosis: A Case Report.

Dextrocardia is one of the rarest cardiopulmonary conditions worldwide and has physical health complications such as a risk of repeated upper respiratory tract infections. Our study highlights the incidental diagnosis of pulmonary tuberculosis in a patient not previously known to have dextrocardia and highlights the need for testing for tuberculosis in a case of chronic cough, especially in low- and middle-income countries. Prompt testing also contributes to adequate antimicrobial stewardship and saves lives.

A Rare Case of Appendicectomy in Situs Inversus: Challenges and Surgical Approaches.

Left-sided acute appendicitis (LSAA) is an extremely rare cause of acute abdominal pain, and even the most experienced surgeons may encounter it only a few times in their careers. While acute appendicitis remains the most common indication for emergency gastrointestinal (GI) surgery, its diagnosis is typically straightforward. However, in cases of situs inversus totalis (SIT), the atypical anatomical positioning can obscure the clinical picture and lead to diagnostic uncertainty, often mimicking other left-sided pathologies and potentially resulting in delayed treatment and complications. This

Complex Fetal Anomalies: Kluth Type Xc2 Variant Tracheoesophageal Fistula, Srikanth Group III Bronchopulmonary Foregut Malformation, Mirror-Image Dextrocardia with L-Looped Ventricles, Anomalous Coronary Origin from the Pulmonary Artery, and Cardiac-Type Total Anomalous Pulmonary Venous Connection.

We report a fetus at 20 + 5 weeks' gestation with an exceptionally rare constellation of congenital anomalies. Prenatal imaging revealed left congenital diaphragmatic hernia, rightward cardiac displacement, possible tracheoesophageal fistula (TEF), transposition of the great vessels, and a large ventricular septal defect. Autopsy confirmed complete absence of tracheoesophageal separation from the larynx to the carina, consistent with Kluth type Xc2, incorporating extralobar pulmonary sequestration with esophageal communication. The findings met criteria for a communicating bronchopulmonary for

Duodenal Intussusception in an Adult With Situs Inversus Totalis.

Situs inversus totalis (SIT) is a rare congenital abnormality in which there is a mirror image of all thoracic and abdominal organs. Most individuals have no specific health issues. Their unique anatomy can pose challenges when they present to the emergency department with vague symptoms. The condition has been known to be associated with multiple anomalies, including Kartagener syndrome, congenital heart defects, asplenia, gastroschisis, and duodenal atresia, to name a few. However, it is rarely associated with intestinal obstruction. Duodenal intussusception occurs when a portion of the inte

Lethal acantholytic epidermolysis bullosa- a report on the prenatal phenotype of two cases and a review of antenatal sonographic signs of congenital denuding skin diseases.

BACKGROUND: The DSP gene encodes the desmosomal protein desmoplakin and is located on chromosome 6. Pathogenic variants in this gene have been linked to different phenotypes that may include the skin, hair, nails, teeth, and the heart. Lethal acantholytic epidermolysis bullosa (LAEB, OMIM # 609638) is a severe and lethal form of epidermolysis bullosa, caused by biallelic pathogenic variants in the DSP gene. CASE PRESENTATION: This report describes two fetuses from the same family, both affected by LAEB. The parents were second degree cousins. Both fetuses showed multiple sonographic abnormali

Radiofrequency ablation of persistent atrial fibrillation in a patient with dextrocardia and interruption of inferior vena cava in the fluoroless catheterization laboratory: a case report.

BACKGROUND: Catheter ablation is an established therapy for atrial fibrillation, yet its application in patients with complex congenital cardiovascular anomalies remains challenging. Dextrocardia (1-2/20,000 incidence) and interruption of the inferior vena cava are rare structural abnormalities that complicate vascular access and catheter navigation during catheter ablation. CASE PRESENTATION: We report the case of a 39-year-old Chinese woman with persistent atrial fibrillation, dextrocardia, interruption of the inferior vena cava (with azygos continuation to the superior vena cava), severe m
