Sudden death following a single coronary ostium located below an aortic valve leaflet: A rare autopsy case and review of the literature.

Congenital coronary anomalies are rare but can cause serious cardiovascular events, including sudden death, in both young and old. A forensic autopsy of a 67-year-old woman found dead at her home revealed a single low coronary ostium located under an aortic valve leaflet, with an unobstructed coronary pathway. Myocardial scarring from a previous infarction was identified in the interventricular septum. This case illustrates the importance of recognising congenital coronary anomalies, even in elderly people, and evaluating their possible role in a sudden and unexplained deaths. A review of the

Anesthetic Challenges in Interarterial Right Coronary Artery Anomaly from the Left Sinus of Valsalva: A Case Report.

Coronary artery anomalies are rare but clinically significant malformations, particularly when involving an interarterial course, which predisposes to ischemia and sudden cardiac death. We report the case of a previously healthy 22-year-old man who suffered out-of-hospital cardiac arrest associated with recreational inhalant use. Imaging revealed an anomalous right coronary artery originating from the left sinus of Valsalva with interarterial trajectory. Surgical reimplantation was performed under tailored anesthetic management emphasizing hemodynamic stability. The patient recovered uneventfu

Myocardial bridging: a practical guide for clinicians.

Myocardial bridging is the most common congenital coronary anomaly, characterized by an intramyocardial course of a segment of an epicardial coronary artery, with considerable variability in its depth and length. In most cases, MB is a benign anatomical variant without clinical significance, often asymptomatic and not requiring treatment. However, with the increasing use of coronary computed tomography angiography for coronary assessment, myocardial bridging is now more frequently identified during non-invasive imaging. It can also be diagnosed via invasive coronary angiography, where it appea

Pathology of Genetic Variants in Spontaneous Coronary Artery Dissection.

BACKGROUND: Spontaneous coronary artery dissection (SCAD) is characterized by a separation of the coronary artery wall, causing myocardial infarction and sudden death. This study is one of the first to clarify the impact of pathological genetic variants in candidate SCAD-related genes on the histopathological and morphological properties of human SCAD lesions. METHODS: A total of 28 SCAD cases were selected from the CVPath Autopsy Registry. Histological differences in collagen and smooth muscle cells were compared among 3 groups: culprit SCAD (C-SCAD), defined as the coronary segment containi

Is Carbon Monoxide Poisoning a Potential Trigger for Spontaneous Coronary Artery Dissection? A Case Report and Review of the Literature.

Spontaneous coronary artery dissection (SCAD) is a rare but potentially fatal cause of acute coronary syndrome (ACS), often associated with younger individuals and women without traditional cardiovascular risk factors. Carbon monoxide (CO) poisoning, a known cause of systemic hypoxia, has been implicated in various cardiovascular complications; however, its role in triggering SCAD remains scarcely reported. We present the case of a 55-year-old woman who developed SCAD following acute CO poisoning caused by a domestic gas leak. The patient presented with altered mental status and respiratory fa

Impact of Adjustable Pulmonary Artery Band on Outcomes of Anomalous Left Coronary Artery From the Pulmonary Artery (ALCAPA) Repair.

Objective: Anomalous left coronary artery from the pulmonary artery (ALCAPA) is a rare congenital anomaly resulting in myocardial ischemia, global left ventricular (LV) dysfunction, and mitral regurgitation (MR). Surgical placement of a pulmonary artery band (PAB) at ALCAPA repair may assist with recovery of patients with severe LV dysfunction. We assessed the outcomes of patients with ALCAPA and severe dysfunction treated with, and without a PAB. Methods: We performed a retrospective analysis of cases presented to our center between January 2007 and December 2023. Baseline and follow-up data

PCI for in-stent restenosis of an anomalous right coronary artery.

Coronary angioplasty for congenital anomalies of the coronary arteries (ANOCOR) remains a technically demanding procedure due to the difficulties associated with selective cannulation of the coronary ostium and achieving adequate support for the guide catheter. We report a case of in-stent restenosis in a proximal connection anomaly of the right coronary artery arising from the left coronary sinus, which was successfully treated using an innovative telescopic technique. A 63-year-old female patient, presenting with multiple cardiovascular risk factors and having previously undergone angioplast

Prenatal Diagnosis of an Isolated Persistent Left Superior Vena Cava With Absent Right Superior Vena Cava: A Case Report.

Persistent left superior vena cava (PLSVC) with absent right superior vena cava (RSVC) is a rare congenital venous anomaly resulting from the failure of the left anterior cardinal vein to regress. Although often asymptomatic in isolated cases, prenatal diagnosis has clinical benefits, including the exclusion of coexisting congenital heart disease (CHD) and awareness of possible complications during central venous access or pacemaker implantation. We report the case of a 34-year-old G2P1 woman who underwent a fetal echocardiography at 34 weeks of gestation. Examination of the four-chamber view

Managing the Critically Ill Postpartum Patient in the Emergency Department.

This review article prepares emergency physicians to recognize and manage critical illness in postpartum patients, a population at heightened risk of preventable morbidity and mortality in North America amid persistent racial/geographic disparities and shrinking obstetric access. It highlights key physiologic changes after delivery that complicate patient management. The article focuses on 3 immediate postpartum emergencies (postpartum hemorrhage, postpartum preeclampsia, and amniotic fluid embolism) and 3 subacute (pulmonary embolism, peripartum cardiomyopathy, and spontaneous coronary artery

From Nash Equilibrium to Social Optimum and Back: A Mean Field Perspective.

Mean field games (MFGs) and mean field control (MFC) have been introduced to study large populations of strategic players. They correspond respectively to non-cooperative or cooperative scenarios, where the aim is to find the Nash equilibrium and social optimum. They approximate finite player problems and have many applications in economics, biology, machine learning. This paper studies how players can pass from a non-cooperative to a cooperative regime, and vice-versa. The first direction is reminiscent of mechanism design, in which the game's definition is modified so that non-cooperative pl

A Simple and Reproducible ImageJ Workflow for Measuring Areas of Irregularly Shaped Necrotic Lesions on Plant Leaves.

Accurately quantifying the areas of necrotic lesions on plant leaves is essential for evaluating plant-pathogen interactions and disease resistance. Although digital image analysis methods using ImageJ are widely employed, they often require case-specific optimization and may not be readily applicable across different experimental conditions. Furthermore, many studies have used ImageJ for lesion measurement without providing methodological details, which limits reproducibility. Here, we present a simple, step-by-step ImageJ workflow for measuring irregular necrotic lesions using a standard per

A contemporary algorithm to guide percutaneous coronary intervention in high-risk spontaneous coronary artery dissection.

Spontaneous coronary artery dissection (SCAD) is increasingly recognized as a cause of acute myocardial infarction, especially in women and young patients. The majority of SCAD cases can be managed conservatively, but failure to revascularize in the setting of reduced coronary flow, ongoing ischemia, or hemodynamic instability can result in extensive infarction, heart failure, or death. Higher complication rates have historically been reported with percutaneous coronary intervention in SCAD because of iatrogenic dissection, non-luminal wiring, and hematoma propagation. The authors propose an a
