Pregnancy-Associated Spontaneous Coronary Artery Dissection: A Report of the iSCAD Registry.

IMPORTANCE: Spontaneous coronary artery dissection (SCAD) is an increasingly recognized cause of myocardial infarction predominantly affecting women. There is a paucity of reproductive health data surrounding pregnancy-associated SCAD (P-SCAD). OBJECTIVE: To examine detailed reproductive variables as well as demographics, psychosocial, and SCAD-event factors among women with a history of at least 1 prior pregnancy in a large, multicenter registry (iSCAD Registry). DESIGN, SETTING, AND PARTICIPANTS: This cohort study compared data of women with P-SCAD vs women with non-pregnancy-associated SC

Surgical unroofing of long and deep myocardial bridges in children: A 2-case report.

INTRODUCTION: Myocardial bridge (MB) is a congenital coronary artery malformation, generally considered benign. However, we reported 2 cases of long and deep coronary MB in children and their surgical treatment. PATIENT CONCERNS AND DIAGNOSIS: Two children with coronary MB presented with syncope as the initial symptom. Case 1: female, 11 years old, body weight 32.5 kg. Cardiac enzymes were elevated. Electrocardiograms showed ST-T changes. Echocardiography: No abnormalities were found in cardiac function and ventricular wall motion. Coronary computed tomography angiography (CCTA) showed that t

Challenges of Managing Acute Coronary Syndromes in Patients With Congenital Single Coronary Artery.

Single coronary artery (SCA) is a rare congenital disorder representing 0.024%-0.066% of the population. Patients with SCA presenting with acute coronary syndromes (ACS) pose diagnostic and treatment challenges. This case series demonstrates the clinical diversity and management challenges of three patients with SCA. The first patient is a 58-year-old gentleman who presented with non-ST-elevation myocardial infarction (NSTEMI) and was found to have an anomalous right coronary artery (RCA) originating from the left coronary artery, he had percutaneous coronary intervention (PCI) to a severe les

Surgical Outcomes of anomalous aortic origin of coronary arteries: early and mid-term results from a single-center experience.

BACKGROUND: Anomalous aortic origin of a coronary artery (AAOCA) is the second leading cause of sudden cardiac death in young athletes. While anomalous aortic origin of the right coronary artery (AAORCA) predominates, anomalous aortic origin of the left coronary artery (AAOLCA) carries higher mortality. Surgical correction requires anatomical precision due to heterogeneous anatomy. METHODS: A retrospective analysis (June 2016–June 2023) included 17 AAOCA patients (mean age: 11.9 ± 3.8 years). Surgical techniques included unroofing (94.1%), coronary reimplantation (11.8%), pulmonary translocati

A Rare Anomalous Origin of the Right Coronary Artery From Mid-Left Anterior Descending Artery: Do Not Rush Into Action.

BACKGROUND: Coronary artery anomalies represent the second leading cause of sudden cardiac death among young athletes, with limited evidence for the management of symptomatic individuals. CASE SUMMARY: We present a case of a single coronary artery anomaly; the right coronary artery (RCA) originating from the mid-left anterior descending artery (LAD). Further evaluation with stress imaging showed no evidence of stress-induced ischemia. DISCUSSION: This represents a rare case of an anomalous origin and course of the RCA from the mid-LAD with a malignant interarterial course. The initial diagno

A Case of Sinus of Valsalva Aneurysm Associated With a Single Coronary Artery Successfully Treated by Bentall Surgery.

We report a case of a sinus of Valsalva aneurysm associated with a single coronary artery, incidentally detected in a 73-year-old woman. Imaging revealed a Lipton type L2-A single coronary artery and a sinus of Valsalva aneurysm. The patient underwent successful Bentall surgery with careful coronary reimplantation and combined antegrade-retrograde myocardial protection. This case highlights surgical considerations in managing this uncommon combination of anomalies. Clinical registration number: Because this was a single-patient case report and written informed consent for participation in medi

Predicting coronary artery abnormalities in Kawasaki disease: Model development and validation.

Routine echocardiography at one month after diagnosis is the current standard for screening coronary artery abnormalities (CAA), a major complication of Kawasaki disease. The study aimed to develop and validate models to predict CAA and assess whether routine echocardiography could be safely reduced in low-risk patients. Two prospective multicenter Japanese registries were utilized: PEACOCK (development/internal validation) and Post-RAISE (external validation). Variables obtained within one week of diagnosis were used to predict CAA at one month after diagnosis, defined as a maximum coronary a

Clinical characteristics and outcomes in Takotsubo syndrome vs. spontaneous coronary artery dissection: a systematic review and meta-analysis.

BACKGROUND: Although a large body of literature describes Takotsubo syndrome (TTS) and spontaneous coronary artery dissection (SCAD) as having overlapping clinical features and benign outcome measures, they differ significantly in their pathophysiological mechanisms, demo-graphic profile, and natural history. Herein, we sought to investigate differences in clinical profile and outcomes between patients of these two conditions. METHODS: Following PRISMA guidelines, we compared TTS and SCAD in adult patients regarding epidemiological, clinical, and prognostic features. A systematic search of Pu

Double myocardial bridge causing acute myocardial infarction in a premenopausal woman.

Myocardial bridging is a congenital abnormality which is characterized by an intramuscular course of a segment of the coronary artery. The presence of myocardial bridging influences coronary vasculature due to mechanical compression and systolic milking. The symptoms and complications of myocardial bridging depend on the number and depth of the tunneled artery segments, the grade of systolic narrowing and the heart rate, and include myocardial ischemia, coronary spasm or acute coronary syndrome. Dual myocardial bridges are rare. We present a case of an acute myocardial infarction in a premenop

Arterial switch operation with autologous sinus tube for transposition of great vessels with single coronary.

This video tutorial demonstrates an arterial switch operation utilizing an autologous coronary sinus tube in a 6-day-old female born with dextro-transposition of the great arteries, ventricular and atrial septal defects, and complex coronary anatomy, with a single coronary artery origin from left-facing running anterior to the aortic root. A standard button transfer was deliberately avoided to minimize the risk of distortion of the coronary arteries. Thus, native aortic sinus tissue was harvested as a contiguous flap and fashioned into a tubular conduit. The constructed tube was implanted into

A Rare Cardiac Triple Siege in Youth: Right Atrial Lipoma, Coronary Stenosis, and Myocardial Bridge.

Right atrial lipoma combined with severe coronary stenosis and myocardial bridge is an extremely rare triad in young adults. We report a 34-year-old male presenting with acute palpitations and chest tightness. Multimodal imaging diagnosed a right atrial lipoma, severe left anterior descending artery stenosis, and myocardial bridge. One-stage surgical resection and coronary bypass were successfully performed, with histopathology confirming benign lipoma. At 1-year follow-up, there was no tumor recurrence and cardiac function was normal. This case demonstrates the value of multimodal imaging and

A case of triple trouble: malignant inter-arterial course of RCA with its high transcommissural origin and deep intramyocardial bridging presenting as chest pain.

Coronary artery anomalies are uncommon in routine clinical practice, but some variants can have important clinical implications when they compromise coronary blood flow. One such pattern is an anomalous origin of the right coronary artery (RCA) with an inter-arterial course with deep intramyocardial bridging and transcommissural origin, which is considered a potentially high-risk configuration. We report a 57-year-old man who presented with exertional chest pain and was diagnosed with non-ST elevation myocardial infarction. To better delineate the anatomy, ECG-gated cardiac computed tomography

The heart's hidden blueprint: a rare variant of coronary artery origin from the right coronary cusp.

INTRODUCTION: Single coronary artery (SCA) is a rare congenital anomaly in which a solitary coronary ostium supplies the entire myocardium. While often benign, certain anatomical variations can predispose patients to ischemia or sudden cardiac death. CASE PRESENTATION: We report the case of a 51-year-old male with chest pain and normal ECG findings. Coronary computed tomography angiography (CCTA) revealed a rare variant of SCA in which the LAD, LCX, and RCA each arose separately from the right coronary sinus, without a common trunk. The LAD followed a pre- pulmonic course, while the LCX course

Proximal ligation in coronary artery bypass grafting for anomalous origin of right coronary artery: A case series with transit-time flow measurement.

Anomalous aortic origin of the right coronary artery (AAoRCA) is a rare anomaly associated with sudden cardiac death, especially with an interarterial course. Coronary artery bypass grafting (CABG) is a treatment option, but graft occlusion from competitive native flow is a concern. In our series, transit-time flow measurement often showed systolic reverse flow, which disappeared after proximal right coronary artery ligation. Coronary artery bypass grafting achieved excellent long-term outcomes. These findings suggest CABG is a suitable treatment for AAoRCA, particularly when proximal ligation

Spontaneous Coronary Artery Dissection: Advances in Understanding and Management in Women.

PURPOSE OF REVIEW: Spontaneous coronary artery dissection (SCAD) is an increasingly recognized cause of acute myocardial infarction (MI), particularly among young to middle-aged women without traditional cardiovascular risk factors. This review summarizes recent advances in the understanding, diagnosis, and management of SCAD. RECENT FINDINGS: SCAD results from an intramural hematoma (IMH) or intimal tear causing separation of the coronary arterial wall and compression of the true lumen. Fibromuscular dysplasia is the most consistently associated arteriopathy, supporting a systemic vascular v

Coronary artery anomalies: contemporary approaches to risk stratification and management.

The widespread adoption of coronary computed tomography angiography (CCTA) has led to a growing detection of coronary artery anomalies in contemporary clinical practice, often as incidental findings during the evaluation of suspected or known coronary artery disease. Among these, anomalous aortic origin of a coronary artery (AAOCA) represents the most clinically debated entity, historically associated with myocardial ischaemia and sudden cardiac death, yet increasingly identified in asymptomatic or minimally symptomatic adults. Accumulating evidence indicates that most AAOCA diagnosed in adult
