Perinatal management and delivery-room stabilization in congenital complete heart block: an 11-year experience at a tertiary center.

Congenital complete heart block (CCHB) is a rare condition associated with significant perinatal hemodynamic instability. Although advances in prenatal diagnosis and pacing have improved survival, evidence guiding delivery-room stabilization remains limited. To describe our institutional experience in the perinatal management of CCHB, focusing on delivery-room stabilization and neonatal outcomes. We conducted a retrospective study at a tertiary center including newborns with CCHB over an 11-year period (2009-2020). Fetal characteristics, delivery-room interventions, and neonatal outcomes were

Immune profiling of anti-SSA-positive at-risk individuals reveals increased type I IFN activity preceding lymphocyte abnormalities in Sjögren's disease.

OBJECTIVES: Identifying immune features that distinguish asymptomatic autoantibody-positive at-risk individuals from patients with established autoimmune disease is key to understanding the evolution from autoreactivity to established autoimmune disease. This study investigated immune characteristics of anti-SSA-positive at-risk individuals, including pregnant women, compared with patients with Sjögren's disease (SjD) and healthy donors (HDs). METHODS: Peripheral blood samples were obtained from asymptomatic anti-SSA-positive women (at-risk individuals; n = 7, including two ongoing pregnancie

Unique coexistence of tetralogy of Fallot and congenital heart block linked to maternal autoantibodies in a Down syndrome fetus: a case report.

We report a rare fetal presentation of tetralogy of Fallot and complete atrioventricular block linked to maternal anti-Ro antibodies. However, trisomy 21 was confirmed postnatally. Despite pacemaker implantation, the neonate died on day 55 due to pulmonary complications. This complex case highlights diagnostic and management challenges involving tetralogy of Fallot, Down syndrome, and complete heart block associated with maternal autoantibodies.

Severe pulmonary valve stenosis associated with advanced atrioventricular conduction disease: A rare case report.

Congenital pulmonary valve stenosis is a common cause of right ventricular (RV) outflow tract obstruction. Advanced atrioventricular (AV) block in congenital heart disease is usually associated with abnormalities of AV septation or conduction system malposition. An association between isolated pulmonary valve stenosis and advanced AV block has not previously been reported. A 16-year-old male presented with exertional dyspnea and recurrent presyncope. Electrocardiography (ECG) revealed 2:1 AV block. Transthoracic echocardiography demonstrated severe valvular pulmonary stenosis with a peak gradi

Diagnostic Yield of Serial PR Interval Screening for Fetal Heart Block in Anti-SSA/SSB Pregnancies.

METHOD: We conducted a retrospective cohort study of singleton pregnancies with anti-SSA and/or anti-SSB antibodies who underwent serial fetal echocardiography between 2010 and 2022. Surveillance was weekly from 18 to 28 weeks' gestation and monthly thereafter, with mechanical PR intervals obtained by pulsed Doppler. Prolonged PR interval was defined as ≥ 140 ms on three consecutive measurements. RESULTS: A total of 124 pregnancies met the inclusion criteria. Screening began at a median of 19.5 week gestation and continued until 34 weeks, with a median of 14 ultrasounds and 10 PR interval mea

Dual-chamber epicardial pacing in preterm and low birth weight infants with congenital complete atrioventricular block: a staged approach.

BACKGROUND: Congenital complete atrioventricular block (CCAVB) in preterm low birth weight neonates poses significant technical challenges for permanent pacemaker implantation. Epicardial pacing is generally preferred in this population; however, definitive implantation in infants weighing < 2500 g may be technically demanding. We evaluated the feasibility and safety of dual-chamber epicardial pacing in preterm low birth weight neonates with autoimmune-associated CCAVB. METHODS: Eight preterm neonates weighing < 2500 g underwent epicardial pacing between April 2021 and May 2022. All patients

Impact of hydroxychloroquine on pregnancy outcomes in systemic lupus erythematosus: A 25 years retrospective cohort study from Asia.

BackgroundSystemic lupus erythematosus (SLE) is an autoimmune disorder that affects various organs and follows a pattern of remission and relapse. Hydroxychloroquine (HCQ), an antimalarial drug, has recently become widely used in pregnant women with SLE due to its multi-level immune anti-inflammatory mechanisms and potential endothelial protective and thromboprophylaxis properties. While several studies have evaluated the impact of HCQ on SLE activity and the occurrence of neonatal lupus, its effects on improving early delivery, preeclampsia, and intrauterine growth restriction (IUGR) remain c

Association between maternal anti-Ro and anti-La antibody levels and congenital heart block: a 20-year cohort study.

OBJECTIVE: To evaluate the association between maternal anti-Ro and anti-La antibody levels and the occurrence of congenital heart block (CHB) in fetuses and newborns. METHODS: This retrospective cohort study included 182 pregnant women with positive anti-Ro and/or anti-La antibodies who received prenatal care at our tertiary center between 2002 and 2022. Maternal clinical, laboratory, and obstetric variables were analyzed. RESULTS: Thirteen fetuses (7.1%) were diagnosed with CHB. Mothers of affected fetuses had significantly higher anti-Ro (median 240 vs. 42; p < 0.001) and anti-La (median

Systemic Manifestations of Neonatal Lupus in an Infant Born to an Asymptomatic Mother: A Case Report of Anemia and Transaminitis.

BACKGROUND Neonatal lupus erythematosus (NLE) is a rare autoimmune disorder caused by the transplacental transfer of maternal autoantibodies, primarily anti-Ro/SSA and anti-La/SSB. Common clinical manifestations include cutaneous lesions and congenital heart block; however, systemic involvement such as anemia and transaminitis remains infrequently reported, particularly in Southeast Asian populations. Diagnosis is often missed, especially in infants born to asymptomatic mothers. CASE REPORT A 1-month-old full-term female infant born via cesarean section presented with annular erythematous skin

Role of Hydroxychloroquine in Prevention of Congenital Heart Block in Pregnant Women With Positive Anti-Ro/SSA and/or Anti-La/SSB.

Neonatal lupus, a condition primarily affecting infants born to mothers with anti-Ro/SSA and/or anti-La/SSB antibodies, is linked to congenital heart block (CHB), a potentially life-threatening complication. Hydroxychloroquine (HCQ), an antimalarial with immunomodulatory properties, has been shown to reduce CHB recurrence. This review examines current evidence on HCQ's role in preventing CHB in pregnancies affected by those autoantibodies, focusing on overall, primary and secondary prevention. Evidence supports HCQ in overall and secondary prevention, but data on its effectiveness for primary

Systemic lupus erythematosus in pregnancy: Maternal and fetal outcomes and management strategies (Review).

Systemic lupus erythematosus (SLE) is a chronic, multisystem, autoimmune disease that predominantly affects women during their reproductive years. Pregnancy in women with SLE is no longer considered uniformly prohibitive; however, it remains a high-risk clinical scenario due to maternal disease activity, lupus nephritis, antiphospholipid antibodies, chronic organ damage, and treatment-related factors that can adversely affect both maternal and fetal outcomes. Current evidence indicates that the risk of flare is highest in women who conceive with an active disease, discontinue effective therapy

Skin-Limited Neonatal Lupus in an Infant Conceived by In Vitro Fertilization and Surrogate Gestation.

Neonatal lupus erythematosus (NLE) is a rare autoimmune condition caused by the transplacental passage of maternal anti-Ro/SSA and anti-La/SSB antibodies, typically presenting with annular erythema or congenital heart block. We report a 3-month-old infant conceived by in vitro fertilization and carried by a surrogate mother who developed annular erythematous plaques in typical and atypical locations at 1 month of age. Skin biopsy and serologic testing confirmed NLE despite the absence of a known maternal autoimmune history; maternal anti-Ro/SSA and anti-La/SSB positivity was identified retrosp

Pacemakers in Pediatric Patients.

Pacemaker insertion in children presents unique challenges because of their small size and developing anatomy. Since pacemaker insertion is relatively uncommon in pediatric patients, and as a result, specialized technologies for this population remain limited and under-researched. The most common indication for pacemaker insertion in children is congenital or acquired heart block. Congenital heart block is often associated with maternal autoimmune disease, in which autoantibodies damage the developing cardiac conduction system. It may also occur in association with congenital heart disease. Ac
