Neurological involvement and pregnancy outcomes in autoimmune disease: mechanisms, clinical implications, and research gaps.

Pregnancy in systemic autoimmune diseases is associated with increased maternal and fetal morbidity, yet the specific impact of neurological involvement remains poorly characterized. This review synthesizes pregnancy outcomes in systemic lupus erythematosus (SLE), Sjögren's syndrome, sarcoidosis, and Behçet's disease, focusing on neurological manifestations. A narrative review (2010-2025) was conducted via PubMed/MEDLINE. Observational cohorts, registries, and systematic reviews reporting maternal, fetal, and neonatal outcomes were included. Across autoimmune conditions, pregnancy risks are si

Single Cell RNA Sequencing Reveals THBS1+CD14+ Monocyte Modulates Inflammatory Activation via NRLP3-Inflammasome in Congenital Heart Block.

Isolated congenital heart block (iCHB) is defined as atrioventricular block without structural cardiac defects, characterized by irreversible fibrosis of the cardiac conduction system. Maternal autoantibodies may elicit systemic exaggerated immune responses involving type I interferon (IFN) signalling cascade, yet peripheral circulating immunity in CHB pathogenesis remains poorly understood. To investigate this, we performed single-cell RNA sequencing (scRNA-seq), followed by differential expression gene (DEG) analysis, SCENIC analysis, pseudotime analysis and cell communication analysis, to c

Maternal and fetal outcomes in pregnancies affected by mixed connective tissue disease.

INTRODUCTION: Mixed connective tissue disease (MCTD) is an autoimmune condition with overlapping features of lupus, systemic sclerosis, and polymyositis. Maternal-fetal outcomes specific to MCTD remain poorly characterized; a scoping review was conducted. METHODS: Five databases were queried. Extracted data included demographics, antibody profiles, symptoms, treatments, and maternal-fetal outcomes. Descriptive analysis was performed. RESULTS: Of 269 articles, 33 met inclusion criteria, including 375 pregnancies. Common symptoms and disease manifestations included Raynaud's phenomenon, arthra

Children exposed to hydroxychloroquine in utero show no signs of retinal toxicity at age 5.

OBJECTIVE: Limited examinations from prior studies support the absence of retinal toxicity in infants exposed to maternal hydroxychloroquine (HCQ). However, the infant retina does not have adult morphology that could show typical signs of HCQ retinopathy. Using the Preventive Approach To Congenital Heart Block with Hydroxychloroquine (PATCH) study, children at age 5 (when adult morphology has been reached) were evaluated using modern imaging techniques. METHODS: Ocular coherence tomography (OCT) was performed (CIRRUS HD-OCT, Carl Zeiss Meditec) at age 5 on 10 PATCH children exposed to materna

[Neonatal lupus and congenital heart block].

Neonatal lupus (NL) is a rare syndrome resulting from the transplacental transfer of maternal anti-Ro/SSA autoantibodies, with or without anti-La/SSB antibodies, and is clinically characterized by cutaneous, hematological, hepatic, and especially cardiac manifestations. The most severe manifestation is congenital heart block (CHB), which occurs in structurally normal fetal hearts, is most often complete and irreversible, and is associated with substantial morbidity and mortality. Although anti-Ro/SSA antibodies are common in women with systemic lupus erythematosus and Sjögren disease, most cas

Maternal systemic lupus erythematosus and the risk of congenital cardiovascular abnormalities in offspring: a systematic review and meta-analysis.

Maternal systemic lupus erythematosus (SLE) has been associated with adverse fetal outcomes; however, the risk of structural cardiovascular malformations and congenital heart block (CHB) remains incompletely characterized. This systematic review and meta-analysis quantified the incidence of these two distinct outcomes in offspring of mothers with SLE and explored possible correlates. Electronic databases were searched through January 2026. The pooled prevalence was estimated using random-effects models with the Freeman-Tukey double arcsine transformation. Pooled odds ratios (ORs) were calculat

Discordant Manifestation of Congenital Heart Block in a Twin Pregnancy: A Case Report.

Background: Congenital heart block (CHB) is a severe disorder, with an estimated incidence of 1 in 20,000 live births. In most cases, the underlying pathophysiology involves transplacental transfer of maternal anti-Ro/SSA and/or anti-La/SSB antibodies, which are thought to bind to L-type calcium channels on fetal cardiac conduction cells, triggering apoptosis and initiating an inflammatory response that ultimately leads to destruction of the conduction system. Case presentation: We report a case of a dichorionic diamniotic twin pregnancy in which one fetus exhibited complete heart block in the

Heart Block in Pregnancy From Parvovirus B19.

BACKGROUND: Complete heart block is a rare and possibly life-threatening condition in pregnancy, requiring timely recognition and multispecialty management. CASE SUMMARY: This case report describes a previously healthy pregnant woman who developed high-grade atrioventricular block due to a parvovirus B19 infection. Before her presentation, she had a upper respiratory infection with a "slapped cheek" rash, classic for parvovirus B19. Other cardiac, infectious, and rheumatologic etiologies were excluded. She had subsequent recovery of atrioventricular conduction with conservative management. D

Case Report: A multidisciplinary, protocol-driven pathway from recurrent pregnancy loss to live birth in an anti-Ro/SSA-positive primary Sjögren's syndrome pregnancy with literature review.

BACKGROUND: Maternal anti-Ro/SSA (Sjögren's syndrome-related antigen A) [± anti-La/SSB (Sjögren's syndrome type B antigen)] antibodies can lead to neonatal lupus, which may present most severely as autoimmune congenital atrioventricular block (CAVB). Although CAVB is uncommon (~ 2% of anti-Ro/SSA-positive pregnancies), once a complete block develops, spontaneous reversal is rare, and many affected infants require permanent pacemaker implantation. Consequently, prevention and early detection are critical components of management, particularly in pregnancies following previous antibody-mediated

Foetal echocardiographic surveillance in anti-SSA/Ro-SSB/La-positive SLE pregnancies: risk stratification of congenital heart block.

OBJECTIVE: To evaluate the clinical utility of serial foetal echocardiographic surveillance, specifically atrioventricular interval (AVI) monitoring, in managing anti-SSA/Ro-SSB/La-Positive SLE pregnancies. METHODS: SLE pregnancies with positive anti-SSA/Ro-SSB/La antibodies were retrospectively included (n = 50). Pregnancies were stratified into two groups based on whether optimal pre-gestational conditions were met. All pregnancies underwent serial foetal echocardiography, including structural assessment and precise AVI measurement. RESULTS: The overall incidence of foetal cardiac conducti

Prenatal Diagnosis of Immune-Mediated Fetal Complete Heart Block Revealing Maternal Systemic Lupus Erythematosus.

Immune-mediated congenital heart block (CHB) is a rare but severe manifestation of neonatal lupus, resulting from transplacental transfer of maternal anti-SSA/Ro and anti-SSB/La antibodies. It is associated with significant fetal and neonatal morbidity and mortality and may reveal previously undiagnosed maternal autoimmune disease. We report the case of a 34-year-old multigravida woman with no known systemic disease, referred at 29 weeks of gestation for suspected fetal arrhythmia. Fetal echocardiography revealed complete atrioventricular block associated with myocardial hypertrophy and perica

Maternal Sjögren's Disease and Its Long Shadow: Adult-Onset Seronegative Lupus Nephritis Following Congenital Heart Block.

Congenital complete atrioventricular block (CCAVB) is a well-recognized manifestation of neonatal lupus erythematosus (NLE), caused by transplacental transfer of maternal anti-Ro/Sjögren's Syndrome Type A (SSA) and anti-La/Sjögren's Syndrome Type B (SSB) antibodies. Although non-cardiac manifestations of NLE typically resolve within the first year of life, the long-term autoimmune risk in affected offspring remains unclear. We report a 24-year-old woman with CCAVB secondary to maternal Sjögren's disease who presented with acute heart failure, nephrotic-range proteinuria, bicytopenia, pericardi

Adverse Pregnancy Outcomes in Sjogren's Disease Compared to Controls: An Interdisciplinary Approach with Maternal-Fetal Medicine.

OBJECTIVES: Outside of the association of SS-A antibody with congenital heart block, little is known about adverse maternal and neonatal outcomes, in patients with Sjogren's disease (SjD). Our study involved collaboration with maternal-fetal medicine (MFM). METHODS: A retrospective cohort study of pregnant patients: SjD patients were matched 1:3 with non-SjD controls. SjD patients were included by meeting the 2016 ACR/EULAR Criteria or by a rheumatologist diagnosis. Exclusion criteria were concurrent autoimmune disease or related antibodies. A composite of grouped outcomes was utilized and ve

Assessment of fetal left ventricular function in pregnancies with Anti-Ro/SSA or Anti-La/SSB antibodies using velocity vector imaging.

BACKGROUND: Intrauterine exposure to anti SSA/Ro and anti SSB/La antibodies has been associated with the development of fetal congenital heart block and, in severe cases, fetal demise. Therefore, early and accurate assessment of fetal cardiac function is of critical importance. Velocity vector imaging (VVI) is an emerging technique capable of providing a detailed assessment of cardiac function. This study aimed to assess and compare left ventricular myocardial function in fetuses of pregnant women with and without anti-Ro/SSA or anti-La/SSB antibodies, and to explore the correlation between gl
