A Multi-Institutional Case Series of Neonatal Lupus Erythematosus Without Cardiac Involvement From Vietnam.

Neonatal lupus erythematosus (NLE) is a rare autoimmune disease in newborns and infants, caused by the transplacental transmission of anti-Sjögren's syndrome-related antigen A (anti-SSA), anti-Sjögren's syndrome-related antigen B (anti-SSB), and anti-ribonucleoprotein (anti-RNP) maternal antibodies to fetal tissues during pregnancy, with manifestations in the skin, heart, liver, and hematologic systems. While cardiac abnormalities in NLE, primarily congenital heart block, may be detected prenatally through fetal echocardiography, noncardiac manifestations become evident after birth and can be

Maternal Primary Sjögren's Syndrome Complicated by Irreversible Fetal Third-Degree Congenital Heart Block: A Case Report From Nepal.

In pregnancies of anti-Ro/SSA- and anti-La/SSB-positive women with primary Sjögren's syndrome, third-degree fetal congenital heart block is usually irreversible despite corticosteroids. Preconception counseling, hydroxychloroquine prophylaxis, and serial fetal echocardiography from 16 weeks are essential to enable timely counseling, including discussion of termination in poor prognosis cases.

Case Report: The case of atypical OAPS complicated by positive anti-SSA and anti-SSB antibodies.

Antiphospholipid syndrome (APS) is a systemic autoimmune disease. When pathological pregnancy manifestations-such as recurrent miscarriage, stillbirth, preeclampsia, and preterm birth-predominate, the condition is called obstetric APS (OAPS). OAPS can be typical or atypical. Pregnant women positive for anti-Ro/SSA and SSB antibodies may develop fetal congenital heart block (CHB). Meanwhile, antiphospholipid antibodies (aPL) can cause preeclampsia, fetal growth restriction, intrauterine fetal death, and other adverse outcomes through mechanisms including disruption of trophoblast function. Treatment options for preventing autoimmune-mediated congenital heart block: a systematic review.

OBJECTIVE: Complete congenital heart block (CHB) due to anti-Ro/La antibodies in pregnancy can lead to adverse fetal outcomes and neonatal death. National and international guidelines do not recommend specific treatment for women identified with anti-Ro antibodies while pregnant. DESIGN: Systematic review of studies of pregnant women with anti-Ro/La antibodies undertaking treatment(s) to prevent CHB. Studies solely focused on treating developing or established CHB were excluded. MAIN OUTCOME MEASURE: CHB incidence. RESULTS: Of 1049 articles, 20 studies were eligible, of which 13 were retros

Dual- vs Single-Chamber Ventricular Pacing in Isolated Congenital Complete Atrioventricular Block in Infancy.

BACKGROUND: The optimal pacemaker programming strategy for infants with isolated congenital complete atrioventricular block (CCAVB) remains unresolved. Dual-chamber pacing maintains atrioventricular synchrony and physiological heart rate variability but increases the burden of ventricular pacing on a myocardium that may be inherently prone to left ventricular (LV) dysfunction. OBJECTIVES: This study sought to compare clinical outcomes of dual (DDD)- vs single (VVI)- chamber pacing in infants with CCAVB (DAVINCHI). METHODS: A multicenter retrospective study (2006-2023) identified infants with

Congenital Complete Heart Block Requiring Temporary Pacemaker Placement at Birth: A Case Report.

Congenital heart block (CHB) is a rare congenital cardiac disease where disruption of the atrioventricular (AV) node results in cardiac conduction abnormalities. The majority of CHB cases are associated with neonatal lupus antibodies that are associated with autoimmune destruction and fibrosis of the AV node, most commonly resulting in a congenital complete heart block (CCHB). We report the case of a 23-year-old mother who was found to have a fetus with significant bradycardia at a 21-week anatomy ultrasound. Subsequent fetal echocardiograms demonstrated a third-degree heart block, and laborat

Safety and Feasibility of Implanting Dual-Chamber Permanent Pacemakers in Patients Under 3 kg.

Purpose: Permanent pacing in the young pediatric population can be challenging, especially in small neonates. We describe our institutional experience with the surgical placement of dual-chamber permanent pacemaker (PPM) systems in neonates weighing <3 kg at the time of implantation. Methods: The Cincinnati Children's Hospital database was used to identify patients who had an epicardial dual-chamber PPM and weighed <3 kg at implantation from 2000 to 2022. Descriptive analyses focused on surgical techniques, postimplant complications, and survival. Results: Twenty patients underwent dual-chambe

Heartbeat of hope: newborn's journey with a pacemaker.

Congenital heart block is a rare disorder of neonates. It is generally diagnosed in the antenatal period or at birth and is commonly associated with transplacental passage of anti-Ro and/or anti-La antibodies to the fetus. It has a high mortality rate if left untreated. A pacemaker is considered as the treatment of choice, but the expertise for pacemaker implantation is available at only a few centres in India. We present a case of a late preterm neonate with antenatally diagnosed heart block due to anti-Ro antibody in the mother. The baby required delivery room resuscitation and mechanical ve

Congenital Heart Block.

Congenital heart block (CHB) is a rare fetal arrhythmia resulting from disruptions in the heart's electrical conduction system, often linked to maternal autoantibodies or congenital heart defects. This article examines the clinical presentation, pathophysiology, and diagnostic strategies of CHB, emphasizing advances in fetal echocardiography, maternal hydroxychloroquine prophylaxis, and multimodal therapies for refractory cases. Management options, including corticosteroids, intravenous immunoglobulin, and pacemaker implantation, are discussed alongside emerging technologies and pacing systems

Hydroxychloroquine After Complete Heart Block Diagnosis and Relationship to Patient Outcomes: A Descriptive Cohort Study.

OBJECTIVE: Congenital complete heart block (CHB) carries significant morbidity and mortality. Maternal Hydroxychloroquine may prevent CHB in subsequent pregnancies. No studies have explored the benefit of Hydroxychloroquine initiated after the diagnosis of CHB. We hypothesized fetuses exposed to Hydroxychloroquine, even after CHB diagnosis, will have improved outcomes using time to pacemaker placement as the primary outcome. METHODS: A single-center retrospective study was conducted on patients with CHB secondary to maternal anti-SSA antibodies approximately 2005-2024. RESULTS: All patients

Spontaneous conversion of fetal heart block to sinus rhythm post externalized pacemaker placement in an undiagnosed mother with lupus antibodies.

The incidence for congenital heart block is estimated as high as 1 in 15,000 live births. Up to 90% of cases of congenital heart block, in which there is no anatomical abnormalities, are attributed to maternal systemic lupus erythematous or Sjögren's disease. 50% of these mothers are asymptomatic at time of diagnosis. The post-natal cardiac manifestations have been felt to be irreversible. In this article we present, to our knowledge, the first case of spontaneous conversion of immune mediate 3rd degree heart block to sinus rhythm postnatally.

When should a neonatologist consult a rheumatologist?

UNLABELLED: Pediatric rheumatologic diseases are complex conditions that can present with various clinical manifestations, including fever, rash, joint involvement, and diarrhea, impacting more than one organ system and affecting all pediatric age groups from 0 to 18 years. This review focuses on rheumatologic diseases in neonates, encompassing both primary neonatal-onset conditions and those influenced by maternal autoimmune diseases and treatments during pregnancy. Diagnosing rheumatologic diseases in neonates is challenging due to their nonspecific symptoms, which can overlap with other con
