Hydroxychloroquine in systemic lupus erythematosus, anti-SSA/SSB, and antiphospholipid antibody-positive pregnancies.

Pregnancies in patients with systemic lupus erythematosus (SLE) and those positive for anti-SSA/SSB or antiphospholipid antibodies carry a heightened risk of adverse pregnancy outcomes (APOs), including preeclampsia, preterm birth, and congenital heart block. Among available therapies, hydroxychloroquine (HCQ) plays a pivotal role due to its immunomodulatory and antithrombotic properties, which may help improve pregnancy outcomes. Emerging evidence supports HCQ's role in reducing SLE flares, as well as lowering the recurrence risk of congenital heart block in anti-SSA/SSB-positive pregnancies.

Maternal autoantibodies to the sodium potassium pump α1 subunit AT1A1 and fetal autoimmune congenital heart block: a case-control study.

BACKGROUND: Fetal autoimmune congenital heart block is a rare but life-threatening condition that is difficult to predict. This study sought to identify a serological biomarker predictive of autoimmune congenital heart block in pregnancies at risk due to maternal systemic lupus erythematosus, Sjögren's disease, undifferentiated connective tissue disease, or a history of congenital heart block offspring. METHODS: This case-control study analysed maternal blood samples from pregnancies affected and unaffected by autoimmune congenital heart block from two centres (The Hospital for Sick Children,

Effects of maternal anti-Ro/La antibodies on fetal atrioventricular conduction evaluated with echocardiography: a state-of-the-art review.

Anti-Ro/SSA and La/SSB antibodies can cross the placenta, affecting fetal cardiac development and leading to congenital heart block (CHB), a condition characterized by rapid progression, high mortality, and poor prognosis, emphasizing the importance of timely diagnosis and treatment. Fetal echocardiography is a critical diagnostic tool for CHB. With advancements in medicine and shifting health perspectives, the incidence, diagnostic approaches, and treatment methods for CHB are evolving. This review examines the impact of maternal antibodies on offspring, including their pathogenic mechanisms

Autoimmune antibodies in arrhythmia: a narrative review of potential therapeutic targets to prevent overtreatment.

BACKGROUND AND OBJECTIVE: The global prevalence of autoimmune diseases (ADs) has increased significantly in recent decades, with cardiovascular complications such as arrhythmia being a major cause of mortality. Traditional mechanistic explanations cannot account for all cases, and autoimmune antibodies have emerged as a novel pathogenic factor. This review summarizes the associations between autoimmune antibodies and arrhythmias, outlining the molecular mechanisms by which these antibodies interfere with cardiac ion channels, receptors, and cellular structures, and providing new insights into

Flail Tricuspid and Mitral Valve in Neonatal Lupus.

BACKGROUND: Neonatal lupus erythematosus (NLE) typically presents as congenital heart block; flail atrioventricular valve leaflets is especially rare. CASE SUMMARY: A neonate with flail tricuspid and mitral valve resulted in severe atrioventricular valve insufficiency. There were high anti-Ro60 antibody titers consistent with NLE. Steroids and immunoglobulins were administered. Mitral and tricuspid valve repairs were undertaken. An inability to wean from extracorporeal membrane oxygenation led to the mitral valve being replaced with a modified Melody valve and then mechanical mitral valve. D

Expert Perspective on a Clinical Challenge: Lupus and Pregnancy.

Systemic lupus erythematosus (SLE), a multiorgan systemic inflammatory disorder, predominantly affects women during their reproductive years. In this review, we summarize the state of knowledge about preconception planning and management of SLE during pregnancy. Achieving remission or low disease activity for several months on medications compatible with pregnancy prior to conception is essential to decreasing the risk of disease flare and improving pregnancy outcomes, including pre-eclampsia, preterm birth, and intrauterine growth restriction. With close management and well-controlled disease

Late-Onset Dilated Cardiomyopathy in Auto Immune-Mediated Complete Congenital Heart Block: A Case Report.

Complete congenital heart block (CHB), a rare and fatal bradyarrhythmia observed in children, carries significant mortality and morbidity. When congenital heart block occurs in isolation with a structurally normal heart, it prompts suspicion of an autoimmune etiology, wherein maternal antibodies are transmitted transplacentally, impacting the fetal conducting system. The manifestation of congenital complete atrioventricular block (CCAVB) can lead to complications such as dilated cardiomyopathies, arrhythmias, and fibroelastosis in certain cases. Notably, dilated cardiomyopathy is a significant

Complex Presentation of Congenital Heart Block and Coexisting Congenital Heart Disease in a One-Year-Old Girl: A Case Report.

Congenital complete heart block (CCHB) is a rare and potentially life-threatening condition, often associated with maternal autoantibodies. We present the case of a one-year-old girl with recurrent respiratory symptoms, ultimately diagnosed with CCHB and congenital heart disease. She exhibited bradycardia and signs of congestive heart failure. A diagnostic workup revealed significant cardiac abnormalities, including dilated chambers, ventricular septal defect, and patent ductus arteriosus. Serological tests for maternal autoantibodies were negative. The child's parents opted for discharge with

Congenital Complete Heart Block-To Stimulate (When?) or Not to Stimulate?

This article presents the case of a 27-year-old female patient with idiopathic congenital complete heart block who does not consent to the implantation of a cardiac pacemaker but was referred by her primary care physician for cardiological evaluation. The conduction disturbance was recognized at the age of 6 and was asymptomatic. The professional disqualification from pacemaker implantation included a detailed history of a patient's symptoms, an echocardiographic assessment of the heart, exercise testing and ECG Holter monitoring. The aid of salbutamol administered orally was also useful.

Complete Congenital Heart Block in the Offspring of an Asymptomatic Woman with Isolated High Titer Anti-Ro Antibody.

Among pregnant women, 1-2% are anti-Ro positive and while half of them have symptoms of connective tissue disease, the rest are asymptomatic. The presence of anti-Ro is of concern because of the risk of congenital heart block in the child. We report the case of an asymptomatic 27-year-old G2P1(1001) woman, who presented with persistent fetal bradycardia in her 21st week of gestation (AOG) and was found to have elevated titers for anti-Ro (>320 U/ml). Hydroxychloroquine 200 mg/day and prednisone 10 mg/day were given from the 33rd week of gestation up until the delivery. At 37 weeks AOG, she del

Sjogren's Antibodies and Neonatal Lupus: A Scoping Review.

Sjogren's syndrome (SS) is an autoimmune disease characterized by inflammation of exocrine glands. The disorder predominantly affects middle-aged women. Autoantibodies, including anti-SS-A/Ro and anti-SS-B/La antibodies, are present in most cases of SS. These antibodies can cross the placenta and likely play a role in pregnancy complications as well as the development of neonatal lupus, resulting in congenital heart block (CHB). It is essential to monitor the fetus for CHB during pregnancy. In particular, screening with echocardiography and monitoring heart rate at home are recommended practic

Dual Chamber Aveir Retrievable Leadless Pacemaker Implant via the Right Internal Jugular Vein in a 13-Year-Old With Congenital Complete Heart Block.

INTRODUCTION: Congenital complete heart block is a condition where there is a risk of Stokes Adam's attacks and sudden death may occur. Once the escape rate is too low, or other high-risk factors occur, these patients ultimately need pacemakers placed. Epicardial or transvenous pacemakers have typically been in employed dependent on size of the patient and other circumstances. We describe the first case of an implant via internal jugular vein (right) of a dual chamber leadless pacemaker implant in a symptomatic pediatric patient with congenital complete heart block. METHODS: The study was app

Myocardial perfusion imaging in a case of cardiac strangulation: A novel approach to detect pacemaker-induced ischemia.

Cardiac strangulation is a rare phenomenon in children following epicardial pacemaker implantation, caused by compression of the heart and great vessels by the epicardial pacemaker wires. We report a rare case of cardiac strangulation presenting after 8 years of epicardial pacemaker implantation. On routine follow-up, computed tomography angiography showed significant extrinsic compression of the mid-left anterior descending (LAD) artery by the epicardial pacing wire. She was referred to our department for myocardial perfusion imaging (MPI), which showed significant inducible ischemia in the L

Newborns with congenital complete heart block: Advice for perinatal care providers.

Congenital complete heart block (CCHB) is a rare, but a potentially life-threatening manifestation of autoimmune diseases in neonates. Bradycardia in CCHB can be misdiagnosed as foetal distress in utero and thus precipitating a Caesarean section. We report a case series of three neonates with bradycardia without any electrolyte abnormalities and structurally normal hearts with favourable outcomes.

Fetal and Maternal Outcomes in a Cohort of Patients With Primary Sjogren's Syndrome: An Observational Study.

Background Pregnant women with primary Sjogren's syndrome (PSS) have a high incidence of maternal and fetal complications due to immunological variations caused by maternal antibodies (anti-Sjogren's-syndrome-related antigen A (SSA) and anti-anti-Sjogren's-syndrome-related antigen B (SSB) crossing the placenta from the 12th week of gestation, mediating the tissue damage. A multidisciplinary approach is required in the management of such patients. Data regarding the effects of PSS on pregnancy are deficient in the Indian context. Methods This was a retrospective observational study on the mater
