Thoracıc duct reroutıng for Fontan faılure-assocıated lymphatıc dısease: a case report.

Fontan-associated lymphatic failure may result in severe complications such as plastic bronchitis. We report a 6-year-old Fontan patient with refractory plastic bronchitis despite catheter-based interventions. Surgical thoracic duct decompression via innominate vein rerouting to the right atrium resulted in marked symptomatic improvement and resolution of bronchial cast expectoration, highlighting its role in refractory Fontan-related lymphatic disease.

Awareness and practice of physical activity in Omani children with CHDs: a cross-sectional study.

BACKGROUND: CHDs, affecting 1.1% of newborns, are the most prevalent congenital anomalies. Improved survival rates expose children with CHD to long-term risks such as metabolic and acquired cardiovascular disorders. Despite physical activity's benefits, participation is often limited by real and perceived safety concerns. This study evaluates awareness and practice of physical activity among Omani children with CHD. AIM: To quantify physical activity levels and identify influencing factors in Omani children with CHD. METHODS: This cross-sectional study, conducted at major Omani paediatric ca

"To save them we have to hurt so many other parts of them": clinician perceptions of stress in infants undergoing cardiac surgery.

Stressful encounters within the neonatal or early infant period are harmful both acutely and longitudinally. Prior research on stress exposure in hospitalised infants excludes infants with CHD, limiting our understanding of stress exposure in this uniquely vulnerable population. This study aimed to identify and describe sources of stress, stress cues, stress responses, and clinical implications in neonates and infants undergoing cardiac surgery. Conducted at tertiary cardiac centres, 17 expert clinicians in nursing, anaesthesia, surgery, and intensive care medicine were included. Participants

A 10-year single-centre report on Fontan attrition and non-Fontan candidacy.

BACKGROUND: As patients progress through single-ventricle palliation, changing haemodynamics and patient conditions can prevent progression to Fontan. We sought to determine the incidence of Fontan completion at our institution and to investigate the reasons for non-Fontan candidacy. METHODS: Patients who underwent superior cavopulmonary anastomosis from 2010 to 2020 at a single institution were included. Pre-Fontan testing was reviewed for all patients, and the primary reason for non-candidacy was determined based on a review of the electronic medical records. RESULTS: Of the 427 patients i

Infective endocarditis in patients with and without CHD: a meta-analysis of age-specific outcomes.

BACKGROUND: Infective endocarditis remains a serious condition. Patients with CHD are particularly susceptible due to structural abnormalities and repeated interventions. However, comparative data on infective endocarditis outcomes in patients with and without CHD stratified by age group remain limited. METHODS: We searched PubMed, Embase, and Cochrane for cohort studies comparing infective endocarditis outcomes in CHD versus non-CHD. Risk ratios with 95% confidence intervals were pooled using random-effects models. RESULTS: We included six observational cohort studies encompassing 180,194 p

"Hope is so fragile": parent perspectives on the care of children with single-ventricle heart disease.

INTRODUCTION: Children born with single-ventricle heart disease face a myriad of medical comorbidities, psychological risks, and quality of life challenges as they age. Parent perspectives on their care from diagnosis through development are critical to understand best ways and times to intervene to promote child and family adjustment to illness. MATERIALS AND METHOD: Parents of children with status post-Fontan procedure were recruited to complete an electronic qualitative survey exploring parent perspectives of care. Grounded theory was utilised to analyse the data and identify themes and su

Prospective observational study comparing the outcomes of fenestrated and non-fenestrated extracardiac Fontan procedure.

BACKGROUND: Fenestration in the Fontan procedure was introduced to improve early postoperative outcomes by reducing systemic venous pressure and augmenting preload to the systemic ventricle. It is believed to decrease pleural drainage, shorten hospital stay, and reduce complications. However, fenestration may result in systemic desaturation and carries a potential risk of paradoxical thromboembolism. The benefits of routine fenestration remain controversial. This study aimed to evaluate whether adding a fenestration influences early postoperative outcomes. PATIENTS AND METHODS: Between March

Double trouble: double aortic arch with bilateral interruption and bilateral arterial ducts.

Double aortic arch with bilateral interruptions is a rare abnormality. Indeed, to the best of our knowledge, it has not previously been reported before. Rare variants, such as a double arch with ligamentous atresia of the left component and double arch with bilateral coarctation, have received previous attention. These rare examples can pose significant diagnostic challenges for those relying on transthoracic echocardiography. Use of cardiac CT or MRI is now critical for prompt surgical planning and correction.

Optimal dosing of enoxaparin in children with CHD less than 1 year of age.

INTRODUCTION: Venous thromboembolism prevalence amongst paediatric patients with CHD has increased over the years. Enoxaparin's favourable pharmacokinetic and pharmacodynamic properties, paired with less intensive monitoring parameters, make it a desirable treatment option. Currently, reported enoxaparin dosing strategies and their correlation to therapeutic anti-Xa levels are variable for infants aged 2 to 12 months. METHODS: This retrospective chart review evaluated the percentage of patients who achieved initial target anti-Xa levels on therapeutic enoxaparin. Patients were divided into st

Frequency of non-cardiac surgical procedures in neonates undergoing cardiac surgery: potential use of setting family and care team expectations.

INTRODUCTION: Most children with CHD who undergo cardiac surgery in infancy require anaesthesia for additional non-cardiac procedures early in childhood. The purpose of our study was to characterise the use of non-cardiac procedures during the first year of life among neonates requiring cardiac surgery in order to set family and care team expectations. MATERIALS AND METHODS: We conducted a retrospective descriptive study comprising data collection from the medical record since programme inception from September 1, 2018, to November 3, 2025. Impact of impaired foetal maternal environment on neonates with CHD: are we ready for the mother-baby dyad?

INTRODUCTION: Emerging evidence suggests that an impaired foetal environment-defined as maternal factors such as hypertensive disorders and diabetes-might contribute to outcomes in neonates with CHD. With this multicentre study, we prospectively collected data regarding impaired foetal environment to assess the impact on mortality in two ventricle and single ventricle neonates with CHD. MATERIALS AND METHODS: A module of prospectively collected maternal-foetal environment data was linked to established Pediatric Cardiac Critical Care Consortium data from June 2019 to July 2020. Successful thrombectomy for acute ischaemic stroke in an infant supported with a single ventricle assist device.

Acute ischaemic stroke is rare in infancy and presents unique diagnostic and therapeutic challenges, particularly in patients supported with mechanical circulatory devices. We describe an infant with CHD supported with a single ventricle assist device who developed an acute ischaemic stroke and underwent successful endovascular thrombectomy. This case highlights the feasibility of thrombectomy in select paediatric patients on mechanical circulatory support and underscores the importance of multidisciplinary decision-making.

Cardiac CT for paediatric patients on veno-arterial extracorporeal membranous oxygenator: learning points from a single centre experience.

Cross-sectional imaging in children on an extracorporeal membranous oxygenator can be challenging due to logistics of transportation and technical aspects of image acquisition. Nevertheless, cardiac CT can be of value in answering important clinical questions in these patients. We share our experience while establishing a successful clinical workflow to perform cardiac CT in children with CHD on extracorporeal membranous oxygenator, which has not been previously described.

Tailored hybrid treatment of adult aortic re-coarctation using a custom thoracic endograft with balloon-expandable stent reinforcement.

We report a 40-year-old man (167 cm, 69 kg) with a complex surgical history, including repaired coarctation of the aorta and a Bentall procedure with mechanical valve replacement, who presented with recurrent, haemodynamically significant coarctation of the aorta, severe arterial hypertension, and a challenging 17 mm diameter mismatch between the aortic arch and descending aorta. As surgery was declined, a tailored hybrid solution was adopted: deployment of a custom thoracic endograft to bridge the calibre transition, reinforced with a balloon-expandable stent to optimise luminal gain. The congenital heart early-career exchange programme: collaboration to advance faculty development.

INTRODUCTION: Developing the next generation of leaders in congenital cardiology is critical to advancing the field. Increasingly, it is recognised that earlier opportunities to establish professional networks, gain unique skills, and disseminate scientific work can be highly beneficial. While cross-institutional collaboration has been successful in supporting quality improvement, it has not been widely used in other areas. We describe our initial experience with a multi-centre congenital heart early-career exchange programme. METHODS: Faculty (<10 years from training) among five participatin

Standardised tube weaning in children with CHD: telemedical Netcoaching approach is as affective as on-site treatment.

BACKGROUND: Feeding difficulties and prolonged reliance on enteral nutrition are common in children with CHD, often leading to tube dependency. Despite its clinical relevance, evidence-based recommendations for tube weaning remain limited. METHODS: This study prospectively evaluated outcomes of the "Graz Model of Tube Weaning" in 70 tube-dependent children with CHD (median age 1.67 years) treated between 2016 and 2024. Children participated either in a two-week on-site programme ("Eating School") or a telemedical home-based programme ("Netcoaching"). Outcomes included weaning success, changes

Opioid exposure in neonates and infants following cardiac surgery: a comparative analysis.

INTRODUCTION: Both untreated pain and opioid exposure impact neurodevelopment in critically ill infants. Mitigating both remains a challenge in the management of infants undergoing cardiac surgery. Our aim was to describe and compare opioid exposure in neonates (≤ 30 days of age), and infants' (>30 days of age) post-cardiac surgery. METHODS: We conducted a secondary analysis of a single center retrospective cohort of all CICU surgical patients < 1 year of age to quantify opioid exposure within 5 days post-surgery. Patient demographics, surgical data, pain medications and pain/sedation scores
