Lung ultrasound evolution after interventional closure of the ductus in preterm babies.

Interventional closure of patent ductus arteriosus (PDA) in preterm infants may result in significant hemodynamic and pulmonary changes. Lung ultrasound (LU) is increasingly used to assess pulmonary aeration; however, data on early respiratory changes following PDA closure remain limited. A prospective observational study was conducted including preterm infants born at ≤ 32 weeks' gestation with hemodynamically significant PDA undergoing surgical or catheter-based closure between May 2022 and November 2025. LU was performed the day before intervention and at 1-2 h, 6-12 h, 24 h, and 2-3 days a

Bedside patent ductus arteriosus ligation via anterior mini-thoracotomy in preterm infants weighing ≤ 1500 g: a retrospective cohort study.

BACKGROUND: Hemodynamically significant patent ductus arteriosus (hsPDA) remains a major clinical challenge in preterm infants. Surgical ligation remains necessary in selected patients when medical treatment fails or is contraindicated, or when transcatheter closure is not feasible. Left anterior mini-thoracotomy (LAMT) has been proposed as a less invasive alternative, and bedside ligation in the neonatal intensive care unit (NICU) may avoid transport-related risks in critically ill neonates. METHODS: This retrospective cohort study included preterm infants weighing ≤ 1500 g who underwent sur

Acetaminophen versus Ibuprofen for the management of hemodynamically significant patent ductus arteriosus in very low birth weight infants: randomized trial.

OBJECTIVE: The objective was to evaluate the efficacy and safety of Acetaminophen versus Ibuprofen in the treatment of hemodynamically significant patent ductus arteriosus. METHOD: A randomized, partially blinded controlled study was conducted in very low birth weight infants. Diagnostic criteria included a ductal diameter of 1.5 millimeters or greater, left cavities dilation or a left atrium to aortic root ratio greater than 1.4, and retrograde flow. Preterm infants were randomized into intravenous Acetaminophen or Ibuprofen treatment groups. Ductal closure was evaluated at the end of the th

Clinical trajectory and determinants of pulmonary hemorrhage in neonates: a 10-year case-control study.

BACKGROUND: Pulmonary hemorrhage remains a life-threatening condition in neonates. This study aimed to identify factors associated with pulmonary hemorrhage, characterize the clinical trajectory during the 24 h preceding the event, and evaluate factors associated with mortality. METHODS: In this 10-year single-center case-control study with frequency-matched control selection, neonates with pulmonary hemorrhage were compared with controls matched at the sampling stage according to gestational age, birth weight, and admission period. Perinatal and postnatal variables were analyzed together wit

Unroofed coronary sinus and persistent left superior vena cava: a rare neonatal venous anomaly.

Approach to abnormal cardiovascular signs in the early neonatal period is challenging due to delayed fetal adaptation, closure of the patent ductus arteriosus and physiological immaturity. We present a rare case of unroofed coronary sinus (UCS) with persistent left superior vena cava (PLSVC) in a term neonate who presented with recurrent desaturation episodes and persistent oxygen requirements. Transthoracic echocardiography (TTE) revealed this rare venous anomaly, which is mainly reported in the older population and primarily as an incidental finding. TTE with an agitated saline study was the

Ductal Stenting in Double Aortic Arch with Tetralogy of Fallot and Pulmonary Atresia: Challenges and Imaging Aspects.

Patent ductus arteriosus (PDA) stenting is a safer alternative to palliative cardiac surgery for newborns with duct-dependent or diminished pulmonary circulation. However, its application in patients with aortic arch abnormalities poses unique challenges. The rare combination of tetralogy of Fallot (TOF) with pulmonary atresia and a double aortic arch (DAA) has been infrequently reported. This case report insights a 1-month-old male infant with a chromosome 22q11 deletion who presented acutely with severe cyanosis with an initial diagnosis of TOF with pulmonary atresia and a right-sided aortic

Optimal timing for transcatheter device occlusion of a hemodynamically significant patent ductus arteriosus in preterm infants: evidence, uncertainty, and future directions.

Transcatheter device occlusion is the primary method for definitive closure of hemodynamically significant patent ductus arteriosus (hsPDA) in preterm infants. While debate persists over expectant versus active medical management, a major knowledge gap remains regarding the optimal timing for transcatheter device occlusion when definitive closure is indicated. Pediatric and cardiovascular society statements recommend procedural intervention for high-volume, hsPDAs that persist beyond 10-14 days after failed conservative or pharmacologic therapy, or when pharmacotherapy is contraindicated. Universal practice based management of patent ductus arteriosus in premature neonates in 2025: BRONX PDA protocol and BRONX clinical scoring system (BCSS).

BACKGROUND: Management of hemodynamically significant patent ductus arteriosus (hsPDA) in neonates remains controversial, with wide variation in practice among neonatologists and pediatric cardiologists. METHODS: An IRB-approved anonymous 12-item survey assessing hsPDA screening, treatment preferences, and perceived risks and benefits was distributed to over 5,000 U.S. providers. Responses were statistically analyzed using standard parameters. RESULTS: A total of 462 completed surveys were analyzed (35% cardiologists, 64% neonatologists). Most providers-initiated PDA screening based on clini

Antibiotic Prophylaxis for Congenital Cardiac Catheterizations: Lessons Learned From the Pediatric Health Information System.

BACKGROUND: There is significant antibiotic prophylaxis (ABP) variability before congenital cardiac catheterization procedures. The true incidence of procedure-related infective endocarditis (IE) remains unclear. OBJECTIVE: This study aimed to evaluate national periprocedural ABP utilization patterns and determine whether ABP dosing strategies are associated with early postprocedural IE. METHODS: A retrospective cohort study was conducted using data from the Pediatric Health Information System from 39 pediatric centers. Data were collected on the following procedures: patent ductus arteriosu

SMAD5 as a novel susceptibility gene for congenital patent ductus arteriosus.

Congenital heart disease (CHD), which represents the most common type of human birth defect affecting approximately 1% of all live births globally, is a leading cause of substantial infant mortality and morbidity worldwide. Although aggregating evidence has convincingly suggested a strong genetic basis underpinning CHD, the inherited components underlying CHD in most cases remain indefinite. Hence, the current investigation aimed to identify and characterize novel genetic variations underlying CHD. A five-generation pedigree with patent ductus arteriosus (PDA) and another group of 174 index pa

Multimodal imaging of infantile scimitar syndrome with Eisenmenger physiology: A case report of complex intracardiac and extracardiac shunts.

Scimitar syndrome is a rare congenital vascular malformation in which the right pulmonary veins drain abnormally into the inferior vena cava, right atrium, or portal vein instead of the left atrium. It occurs in about 1-3 per 100,000 live births and has 2 forms: an infantile form, usually more severe and associated with multiple anomalies and poorer prognosis, and a child/adult form, which is often milder or asymptomatic. This report describes an 8-year-old boy diagnosed with complex Scimitar syndrome complicated by Eisenmenger syndrome. Echocardiography, chest X-ray, and computed tomography a

Early cardiac rehabilitation in pediatric fenestrated Fontan completion: A case report despite postoperative complication.

Early rehabilitation after Fontan surgery in children is poorly described, especially in complicated postoperative courses. We report the clinical evolution and rehabilitation of an 8-year-old girl with double-inlet left ventricle and patent ductus arteriosus who underwent fenestrated Fontan completion. The postoperative period was complicated by bilateral pleural effusions, pulmonary hypertension, hematemesis, psychomotor alteration, and mechanical ventilation with inhaled nitric oxide. A structured bedside rehabilitation program was started early and adjusted according to clinical stability.

Validation of a new Japanese classification for predicting severe bronchopulmonary dysplasia in preterm infants.

BACKGROUND: Bronchopulmonary dysplasia (BPD) is the most prevalent chronic lung disease in very preterm infants; however, conventional classifications have limited ability to predict severity before 36 weeks' postmenstrual age (PMA). A new Japanese classification, based on small for gestational age (SGA), bubbly/cystic chest radiographic findings, and chorioamnionitis (CAM), was proposed to enable earlier risk stratification. However, its validation in homogeneous cohorts is warranted. PURPOSE: This study aimed to examine the association between this new Japanese classification and severe BPD

Echocardiographic hemodynamics during early postnatal period and 3-year neurodevelopment outcomes in preterm infants.

BACKGROUND: The relationship between early echocardiographic markers of patent ductus arteriosus (PDA)-related hemodynamics and long-term neurodevelopmental outcomes in preterm infants remains unclear. METHODS: We analyzed data from a multicenter registry of infants born at 23-29 weeks. Echocardiographic data obtained within the first 14 days were used to assess hemodynamic burden, including the PLASE (PDA and Left Atrial Size Evaluation) score. The primary outcome was an adverse neurodevelopmental outcome at 3 years. Associations were evaluated overall and by gestational-age strata. RESULTS
