Uncommon but Critical Cardiac Abnormalities on Routine Chest Computed Tomography.

Uncommon but critical cardiac abnormalities may be missed on routine chest computed tomography (CT) due to misinterpretation as motion artifact. The examples include paradoxical embolism through the patent foramen ovale, right ventricular lead perforation, patent ductus arteriosus with endarteritis, infective endocarditis, acute mitral regurgitation secondary to chordal rupture, acute myocardial infarction, left atrial or ventricular thrombus, and paracardiac fat necrosis. Such overlooked diagnoses may lead to an unfavorable outcome. This review provides important tips to discriminate true abn

Flow versus resistance: hemodynamic phenotyping of pulmonary hypertension in infants with shunts and concurrent lung disease.

Neonatal pulmonary hypertension (PH) can be categorized into three distinct physiologies: resistance-driven precapillary pulmonary arterial hypertension (PAH), flow-driven precapillary PH, and postcapillary PH. We present eight patients, divided into three groups: premature infants with BPD, infants with chronic respiratory failure secondary to CDH or genetic syndrome, and an infant with a multilevel shunt. In each case, echocardiography was an important, non-invasive tool for screening risk of PAH. However, many of the common echocardiographic parameters used to assess PAH are confounded by v

Coarctoplasty in a case of ventricular septal defect, patent ductus arteriosus with coarctation of aorta and Eisenmenger's syndrome: Is it safe?

Endovascular Coarctoplasty (CoAP) with stenting is the treatment of choice in Coarctation of Aorta (CoA). Literature on the harms or benefits of CoAP in Eisenmenger's syndrome (ES) is scarce. There is the hypothetical possibility of an increase in right-to-left intracardiac shunting once the coarctation is relieved. We report a 17-year-old male adolescent who presented with secondary hypertension, cyanosis, and lower limb claudication. He was diagnosed as a case of ventricular septal defect (VSD), patent ductus arteriosus (PDA), and CoA with associated ES. He underwent successful balloon CoAP

Development and Validation of a Nomogram for Predicting Oral Ibuprofen Treatment Failure in Preterm Infants With Hemodynamically Significant Patent Ductus Arteriosus.

BACKGROUND: Hemodynamically significant patent ductus arteriosus (hsPDA) is common in extremely and very preterm infants and has been associated with pulmonary overcirculation, systemic hypoperfusion, and adverse neonatal outcomes. However, the optimal timing and selection of pharmacologic treatment remain controversial, highlighting the need for tools that can help identify infants who are unlikely to respond to oral ibuprofen. OBJECTIVE: Hemodynamically significant patent ductus arteriosus remains a frequent and critical complication in preterm infants. Due to the lack of clinical consensus

PDA Closure Rates and Safety after Repeated Courses of Cyclooxygenase Inhibitors in Preterm Infants: A Systematic Review and Meta-Analysis.

BACKGROUND: Cyclooxygenase (COX) inhibitors have been used to treat hemodynamically significant patent ductus arteriosus (PDA) in preterm infants. If the PDA remains hemodynamically significant after an initial course, additional courses of COX inhibitors or surgical intervention are considered. This systematic review evaluates PDA closure rates and safety after repeated courses of COX inhibitors in preterm infants. METHODS: Embase, Medline, and two other databases were searched through April 2026 (pre-registered in PROSPERO [CRD42023454003]). Studies reporting PDA closure rates after two or

Bilateral Anophthalmia Associated With Ventricular Septal Defect, Atrial Septal Defect, and Patent Ductus Arteriosus: A Rare Neonatal Case Report.

BACKGROUND: Anophthalmia is a rare congenital ocular malformation characterized by the complete absence of the eye and its associated ocular structures. Bilateral anophthalmia is particularly uncommon and is frequently associated with multisystem congenital anomalies. Congenital heart disease represents one of the most commonly reported systemic associations; therefore, all infants diagnosed with bilateral anophthalmia should undergo comprehensive systemic evaluation, including detailed cardiovascular assessment. However, the coexistence of bilateral anophthalmia with multiple congenital cardi

Transcatheter Patent Ductus Arteriosus Closure in Extremely Premature Infants: Transition to the Bedside.

BACKGROUND: Extremely premature infants (<28 weeks' gestation) with hemodynamically significant patent ductus arteriosus (PDA) may require transcatheter PDA closure (TCPC) when medical therapy fails. Although TCPC is associated with improved respiratory outcomes over surgical ligation, its availability is limited because of the need to transport these critically ill infants to cardiac catheterization laboratories, delaying care and increasing risk. OBJECTIVES: The authors describe the successful transition of TCPC from the catheterization laboratory to the neonatal intensive care unit (NICU)

A Rare Complete Vascular Ring: Right Aortic Arch, Large Left Sided Patent Ductus Arteriosus, and Isolated Left Subclavian and Vertebral Arteries Mimicking a Double Aortic Arch.

We present the case of a symptomatic 6 month old infant with a very rare form of complete vascular ring caused by a right aortic arch with abnormal branching pattern and a large left-sided patent ductus arteriosus (PDA). The large left sided PDA connected between the main pulmonary artery and the descending thoracic aorta. This configuration in combination with the right aortic arch caused tracheoesophageal compression. In addition, the left subclavian artery (LSA) and left vertebral artery (LVA) were originating from the PDA (Isolated left subclavian and left vertebral artery). This anomaly c

Periodontal Status and Oral Health-Related Quality of Life among Postmenopausal Women Visiting Tertiary Government Hospital.

The aim of this study was to assess the differences in the mortality and in-hospital outcomes of preterm infants with < 28 weeks of gestation who received ibuprofen treatment according to the presence of clinical symptoms (any of oliguria, hypotension, or moderate to severe respiratory difficulty) attributable to hemodynamically-significant patent ductus arteriosus (hsPDA) at the time of first ibuprofen treatment. In total, 91 infants born from April 2010 to March 2015 were included. Fourteen infants (15.4%) received ibuprofen treatment when there were clinical symptoms due to hsPDA (clinical

Comparative neuroanatomy of emotion expression: from flies to humans.

Total anomalous pulmonary venous return is a congenital heart malformation characterized by anomalous pulmonary venous inflow to the right atrium. Surgical repair typically occurs during the first month of life, and survival beyond that age in untreated patients is unlikely. We report an extreme case of supracardiac total anomalous pulmonary venous return in an infant who survived 7 months despite atypical anomalous inflow without atrial-level communication and with right-to-left shunting only through a patent ductus arteriosus. We stabilized the patient's left-sided heart function before surg

Cardiovascular phenotype analysis in large and abnormal offspring calves in relation to assisted reproductive technology†.

Large offspring syndrome/Abnormal Offspring syndrome (LOS/AOS) occurs at an increased frequency in calves derived from assisted reproductive techniques (ARTs). Specific criteria for LOS diagnosis in calves are lacking. Cardiac and vascular anomalies are common in ART derived neonates but have not been described in calves. Calves derived from in vitro produced (IVP) embryos and referred to Cornell University for prolonged gestation were prospectively enrolled. Calves underwent phenotypic evaluation and echocardiography. Two-dimensional, M-mode, and color flow mapping echocardiography was perfor

Bilateral lung transplantation with simultaneous aortic replacement using donor aorta in a patient with pulmonary arterial hypertension and patent ductus arteriosus.

BACKGROUND: A patent ductus arteriosus (PDA) can result in pulmonary arterial hypertension (PAH) due to a left-to-right shunt. Lung transplantation (LTx) is indicated when PAH becomes refractory to medical management. We report a case of bilateral LTx (BLTx) with simultaneous aortic replacement using a donor aorta in an adult patient with PAH complicated by PDA. CASE PRESENTATION: A 27-year-old woman was referred for an LTx evaluation. At 1-year-old, she was diagnosed with a PDA. At the time of diagnosis, PDA closure was not indicated due to severe PAH, with a pulmonary vascular resistance of

Cardiac function in 6-year-old children born extremely preterm and associations to prolonged patent ductus arteriosus shunting.

Follow-up studies of extremely preterm born (EPT) individuals have shown reduced cardiac dimensions and altered biventricular function compared with term born controls. A hemodynamically significant patent ductus arteriosus (hsPDA) increases cardiac load during the neonatal period but potential long-term risks associated with prolonged hsPDA shunting are still largely unknown. We aimed to describe cardiac dimensions and functions in EPT children at 6.5 years of age and to investigate if a prolonged neonatal hsPDA shunt would have a negative cardiac impact. In this follow-up study, EPT children

Patent ductus arteriosus: emerging trial evidence firmly supports conservative management in preterm infants.

PURPOSE OF REVIEW: To summarize recent evidence from randomized clinical trials (RCT) and meta-analysis of patent ductus arteriosus (PDA) closure therapies versus conservative/expectant management on key outcomes such as bronchopulmonary dysplasia (BPD) or death in very preterm (<32 weeks' gestation) infants. RECENT FINDINGS: Of 10 completed RCTs in the past decade of medical therapies before 2 weeks of age to close a PDA, none demonstrated benefit in reducing moderate-severe BPD/death by 36 weeks postmenstrual age as compared to placebo treatment. When combined, the meta-analysis of the 10 R
