Different Dosing Strategies of Total Parenteral Nutrition in Very Low Birth Weight Infants: A Network Meta-Analysis of Randomized Controlled Trials.

INTRODUCTION: Total parenteral nutrition (TPN) is essential for growth in very-low-birthweight (VLBW) infants. The worldwide variation in TPN dosing strategies warrants investigation. This study compared clinical outcomes of aggressive, rapid-increase, and standard TPN dosing strategies in VLBW infants. METHODS: A systematic review and network meta-analysis were conducted following the PRISMA NMA guideline. Searches were performed in PubMed, Scopus, Web of Science, CINAHL, CENTRAL, and ProQuest. Dosing strategies were classified as aggressive (higher starting dose), rapid-increase (standard s

Preterm PDA Device closure using the KONAR MFO device in a resource-limited centre of North-East India: a Case series.

BACKGROUND: Percutaneous device closure is the preferred method for closing patent ductus arteriosus (PDA), even in preterm infants. We report our experience using the new KONAR-MFO™ ventricular septal defect occluder for transcatheter closure of preterm patent ductus arteriosus as an alternative device in resource-limited centres. METHODOLOGY: Case 1: A preterm baby with Down's syndrome and tracheobronchomalacia was born at 29 weeks, weighing 1.68 kg with multiple cardiac defects, including a 4 mm PDA and a 6 mm mid-muscular ventricular septal defect (VSD), stuck on a ventilator. Case 2: Ano

Patent ductus arteriosus status and treatment response alters myocardial adaptation in preterm infants.

INTRODUCTION: This study investigates the impact of patent ductus arteriosus (PDA) status and treatment response on myocardial adaptation in preterm infants by comparing serial echocardiographic trajectories across three groups: high-risk infants with treatment success, high-risk infants with treatment failure, and low-risk infants not requiring treatment. METHODS: In this prospective cohort study, preterm infants born < 29 weeks' gestation were stratified using the EL-Khuffash PDA Severity Score and subsequent response to medical therapy. Echocardiographic assessments were performed at three

Risk factors for hemodynamically significant patent ductus arteriosus and ibuprofen treatment failure in premature twins: a retrospective case-control study.

BACKGROUND: Premature twins face a dual challenge: a high incidence of hemodynamically significant patent ductus arteriosus (hsPDA) and a poor response to ibuprofen. Current management, derived from singleton data, fails to address this population's unique risks. Our study specifically investigates risk factors for hsPDA development and ibuprofen treatment failure in preterm twins to enable personalized prevention and therapy. Thus, the potential biomarkers for the occurrence of hsPDA and ibuprofen closure failure were examined. METHODS: A single-center, retrospective case-control study. 736

Late Aortic Obstruction Post Patent Ductus Arteriosus Closure Managed with Renata Minima Stent.

Descending aorta obstruction is a known though uncommon event that can occur post patent ductus arteriosus device closure in premature infants. This adverse event has generally been managed with surgical repair or stent placement if the child is large enough for the vascular access needed. We present two cases of late aortic obstruction associated with device closure of a patent ductus arteriosus managed with the Renata Minima stent. Key Words: PDA, Coarctation, Renata Minima Stent.

Prolonged patent ductus arteriosus exposure and risk for late acute kidney injury in extremely preterm infants.

OBJECTIVE: To study duration of hemodynamically significant patent ductus arteriosus (HPDA) exposure increases the risk of late acute kidney injury (AKI) and severity of AKI. STUDY DESIGN: This was a single-center retrospective cohort study. Included infants born between 22 and 28.6 weeks' gestation with >1 echocardiographic finding for HPDA were stratified by HPDA duration: 4-7 weeks, 8-11 weeks, and greater than 12 weeks. AKI was determined utilizing KDIGO AKI criteria. Logistic regression analysis was used to evaluate odds ratios of each HPDA exposure groups for any AKI and severe (stage 2

Early Maturation of Heart Rate Variability in Very Preterm Infants Depends on Neonatal Factors and Is Associated With Neurodevelopmental Risk.

AIM: This study investigated central autonomic network maturation deviations using a previously defined machine learning model set to estimate a functional maturation age (FMA) from heart rate variability (HRV) analysis. We investigated whether these deviations were associated with maternal, fetal, perinatal, or postnatal complications and with neurodevelopment at 2 years of age. METHODS: Differences (ΔHRV) between post-menstrual age (PMA) and FMA were measured in a multicenter prospective cohort of 132 preterm infants born < 30 weeks gestational age (GA). The relationship between ΔHRV and cl

Neonatal Kommerell diverticulum with mid-ductal aneurysmal patent ductus arteriosus: serial imaging documentation of early natural history.

We report an exceptionally early diagnosis of a Kommerell diverticulum on the first day of life, distinguished by unusual mid-ductal aneurysmal dilatation of the patent ductus arteriosus. Serial echocardiography documented the serial evolution of spontaneous ductal closure and thrombosis formation. CT confirmed a right aortic arch with a retroesophageal Kommerell diverticulum. The infant remains asymptomatic under close surveillance. This case represents one of the youngest reported cases with serial imaging documentation, providing unique insights into the early natural history and highlighti

Contemporary Management Strategies for Patent Ductus Arteriosus.

Patent ductus arteriosus (PDA) is one of the most prevalent congenital cardiac abnormalities. It is more common in preterm newborns and is inversely correlated with gestational age. Whether to treat PDA remains a subject of considerable debate partly attributable to the limited evidence demonstrating long-term benefits of the interventions studied. In the past, surgical ligations or cyclooxygenase inhibitor medications were used to close the PDA. Transcatheter PDA closure is one alternative treatment for this population because comorbidities linked to the PDA have increased with improved survi

Artificial intelligence for patent ductus arteriosus-a systematic review.

INTRODUCTION: Optimal management of patent ductus arteriosus (PDA) remains controversial. Complexity in severity appraisal, high-dimensional data, and the need for longitudinal, individualized assessment make PDA a compelling candidate for Artificial Intelligence (AI)-driven approaches. This systematic review evaluates AI research in the context of PDA, identifying strengths, limitations, and future directions. METHODS: Following PRISMA 2020, databases were searched for peer-reviewed articles from January 1, 2010, to May 31, 2025. Eleven studies met inclusion criteria. Data on design, populat

Shadowed Horizons: The Congenital Crisis of Aortic Arch Discontinuity.

We report a rare case of dual ductal-dependent systemic perfusion in type C interrupted aortic arch. Despite imaging confirmation, ductal closure led to cardiopulmonary failure, and surgery was declined. This case underscores the critical need for prenatal diagnosis and coordinated perinatal management to enable timely intervention.

Significant Congenital Cardiovascular Malformations and Falling Birth Rates: The Nevada Experience.

We investigated significant congenital cardiovascular malformations (CCVM) in Nevada, accompanied by declining birth rates. We identified those diagnosed pre- and postnatally in Nevada with significant CCVM between January 2012 and December 2024. We defined significant CCVM as cases that underwent or were likely to undergo a surgical or interventional cardiac catheterization procedure between 0 and 24 months of age. We excluded those with a secundum atrial septal defect or patent ductus arteriosus. We identified 1573 cases. Of the 1573, 1427 were live born. Of the 1573, 1486 (94%) had prenatal

Spontaneous thrombotic occlusion of a patent ductus arteriosus in an adult dog.

A 7-year-old Pomeranian with a history of patent ductus arteriosus (PDA) was presented with acute hindlimb paralysis. Hindlimb aortic thrombosis (ATh) was suspected, and spontaneous PDA thrombus was incidentally identified on echocardiography and computed tomography (CT). Concurrent protein-losing nephropathy was identified as the underlying condition contributing to the hypercoagulable state. Low-dose antithrombotic therapy was initiated to minimize the risk of abrupt PDA recanalization, which could result in cardiovascular compromise. Despite treatment, distal ATh progressed, resulting in re

Immediate aortic obstruction in young infants undergoing patent arterial duct closure with the duct occluder.

We report three young infants (118, 45, and 80 days; 3.5, 4.1, and 3.8 kg) with patent ductus arteriosus (PDA) who had a uniform pattern of immediate device displacement into the aorta after an apparently secure deployment of a duct occluder. All had aortic obstruction requiring urgent surgical relief. During the study period, 18 infants (<6 months) underwent PDA closure with the same device size (6-4 Lifetech duct occluder). The aortic diameter and PDA length were similar in cases versus the 15 controls with uneventful procedures. The percentage shortening screw to pin distance ratio, which i
