From closure to catastrophe: Pneumonectomy following missed aortic pseudoaneurysm after transcatheter closure of patent ductus arteriosus.

We describe the case of a large pseudoaneurysm developing after percutaneous closure of patent ductus arteriosus in a 2-year-old girl, which went undiagnosed in time, leading to destruction of the left lung necessitating left pneumonectomy along with resection of the pseudoaneurysm.

Procedural outcome and safety of percutaneous patent ductus arteriosus occlusion in adults.

BACKGROUND: Percutaneous device closure of patent ductus arteriosus (PDA) in infants and children is a frequently performed procedure. However, transcatheter closure is rare in adults, especially in developed countries, as most ducts are typically closed in childhood. PDA in adults poses unique challenges due to age-related comorbidities and some morphological changes in PDA that occur with advanced age. This study analyzes the safety and procedural success of adult PDA transcatheter closure. MATERIALS AND METHODS: A retrospective single-center study analyzed 184 adult PDA device closures bet

Regional oxygen saturation as a non-invasive indicator for neonatal PDA: a systematic review and meta-analysis.

BACKGROUND: The high prevalence of patent ductus arteriosus (PDA) in preterm neonates, particularly those born before 32 weeks of gestation, highlights the need for early diagnosis, where altered oxygen saturation levels are considered a potential indicator. This review evaluates the diagnostic value of multisite oxygen saturation indices for neonatal PDA. METHODS: Relevant studies were sourced from PubMed, Embase, Web of Science, and Cochrane Library up to June 2023, using both medical subject headings and keywords. The quality of studies was assessed via American Agency for Healthcare Resear

Evaluating the Effectiveness of Ibuprofen Versus Acetaminophen in Closing Patent Ductus Arteriosus in Preterm Neonates.

Background/Objective: Patent Ductus Arteriosus (PDA) is a common congenital heart defect causing high morbidity and mortality in preterm neonates. IV ibuprofen is the standard treatment, with acetaminophen as a potential alternative when ibuprofen is contraindicated. However, evidence for acetaminophen's effectiveness is inconclusive. This study aimed to compare the efficacy and safety of IV acetaminophen versus IV ibuprofen as the initial treatment for PDA closure in preterm neonates. Methods: A retrospective cohort study was conducted at a tertiary Saudi hospital. This study included preterm

Liver Regional Oxygen Saturation in Preterm Infants with Patent Ductus Arteriosus Status.

Background/Objectives: Near-infrared spectroscopy measures regional oxygen saturation (RSO2) in target tissues. Cerebral and renal RSO2 are associated with hemodynamically significant patent ductus arteriosus (PDA) in preterm infants. The aim of this study was to determine whether liver RSO2 is associated with PDA status. Methods: Preterm infants born before 32 weeks of gestation and/or weighing <1.5 kg were enrolled. Cerebral, renal, and liver RSO2 values were recorded on day 2 (D2; 48-72 h), day 7 (D7; 7 ± 2 days), and day 14 (D14; 14 ± 3 days), along with the corresponding point-of-care ech

Linking maternal and neonatal circulation in preeclampsia.

Hypertensive disorders of pregnancy, particularly early-onset preeclampsia (EOPE), are major causes of perinatal morbidity and mortality. Although impaired placentation has long been recognized as a key mechanism, increasing evidence highlights the contribution of maternal cardiovascular dysfunction. However, how maternal hemodynamics influences neonatal circulatory transition remains poorly understood. In this prospective study, mother-infant pairs from pregnancies complicated by EOPE were enrolled if the mother had undergone echocardiographic assessment within 3 wk before delivery and the ne

Comparative Analysis of Reported Adverse Events with Nirsevimab and Palivizumab: A FAERS Observational Study.

INTRODUCTION: Respiratory syncytial virus (RSV) is a leading cause of hospitalization among infants worldwide. Passive immunization remains central to prevention. Palivizumab has been used for over two decades, whereas nirsevimab, approved in 2023, provides extended protection with a single dose. However, comparative post-marketing safety data between these agents remain limited. Authors compared post-marketing adverse event profiles of nirsevimab and palivizumab using the U.S. Food and Drug Administration Adverse Event Reporting System (FAERS). METHODS: FAERS reports from July 2023 to March

Surgical repair of isolated right pulmonary artery using pulmonary artery flap and preserved PDA stent.

Isolated right pulmonary artery (RPA) from the ductus arteriosus is rare and often managed in stages to promote vessel growth. We present a 12-year-old girl with ductal-origin RPA who had undergone patent ductus arteriosus (PDA) stenting at age three and serial balloon dilations. Through median sternotomy, a posterior wall flap was created from the main pulmonary artery (MPA) and rotated toward the RPA, incorporating the preserved stented PDA segment as the posterior wall. The anterior wall was reconstructed with a decellularized pulmonary homograft patch, creating a 9 mm lumen. The patient re

Umbilical Artery Access for PDA Stenting: Feasibility in Select Anatomy.

Patent ductus arteriosus (PDA) stenting is a less invasive alternative to surgical shunts in ductal-dependent congenital heart disease. Umbilical artery access for PDA stenting offers femoral-like retrograde advantages while avoiding associated risks, however is underreported in this context. This study evaluates umbilical artery access feasibility and outcomes for PDA stenting. A case series included seven neonates undergoing PDA stenting via umbilical artery access between 01/17/2020 and 10/30/2024 at Rady Children's Hospital, San Diego. Patient demographics, PDA morphology, and procedural d

Navigating the Unique Challenge of Pulmonary Hypertension From Left-to-Right PDA Shunts in Two Cases of Congenital Diaphragmatic Hernia.

Pulmonary hypertension (PH) in congenital diaphragmatic hernia (CDH) is nearly universal yet incredibly complex. A patent ductus arteriosus (PDA) is increasingly utilized to both monitor echocardiographically and manage CDH-PH. However, as pulmonary pressures are driven by both vascular resistance and blood flow, there is a risk that as the former improves, PH may in fact worsen from pathologic left-to-right (LtR) PDA shunting. Herein, we describe two cases of neonatal CDH whose PH was complicated by pathologic LtR PDA shunting and pulmonary overcirculation and improved following resolution of

Surgical repair of a child with tetralogy of Fallot with absent left pulmonary artery: a case report.

INTRODUCTION AND IMPORTANCE: Tetralogy of Fallot (TOF) with unilateral absence of a pulmonary artery is a rare anomaly with challenging surgical management. Key risks include postoperative pulmonary hypertension due to a reduced pulmonary vascular bed. This report presents a case of TOF with absent left pulmonary artery (ALPA) that resulted in a fatal postoperative complication, highlighting a potentially underreported mechanism. CASE PRESENTATION: A 5-year-old female with Tetralogy of Fallot with absent left pulmonary artery (TOF-ALPA) underwent successful surgical repair, including ventricu

Hemodynamically significant PDA impacts adverse outcomes in infants with BPD: a multicenter study.

BACKGROUND: Bronchopulmonary dysplasia (BPD) and significant hemodynamic patent ductus arteriosus (hsPDA) are both common and important clinical issues in extremely preterm infants. The potential impact on prognosis when these conditions coexist is a major focus of clinical concern. This study examined the relationship between hsPDA and adverse outcomes in BPD infants. METHODS: A retrospective analysis of 781 preterm infants (<32 weeks) from three hospitals (2018-2023). Based on echocardiographic assessment, infants were categorized as the non-PDA group, the non-hsPDA group, or the hsPDA grou
