Comparison of early and late surgical ligation of patent ductus arteriosus in preterm infants.

Management of the patent ductus arteriosus (PDA) in the preterm infants has been highly controversial. Although the rate of surgical ligation has decreased, surgical ligation remains a necessary treatment option when the patient has a large hemodynamically significant PDA unresponsive with medical treatment or contraindicated of medical treatment. However, optimal timing of surgical ligation is still controversial and has no guideline. We compared clinical characteristics of early and late surgical ligation of PDA in preterm infants to understand the timing of surgical ligation. This retrospec

CD71+ erythroid cells as a potential early biomarker for hemodynamic significant patent ductus arteriosus in preterm infants.

INTRODUCTION: Hemodynamically significant patent ductus arteriosus (hsPDA) in preterm infants is associated with increased mortality and morbidity. Early and accurate identification of infants at risk for PDA is critical for improving outcome. Perinatal inflammation is a known risk factor of PDA. CD71+ erythroid cells (CECs) are immature red blood cells with immunomodulatory roles and show variable abundance in preterm infants. We aimed to examine the proportion of CECs in preterm infants and investigate association with PDA. METHODS: In this prospective study, we evaluated the association be

Development of a web-based preoperative planner for right vertical infra-axillary thoracotomy.

Right Vertical Infra-Axillary Thoracotomy (RVIAT) offers superior cosmetic outcomes but presents challenges due to restricted access. In pediatric patients, the 'crowded thorax' necessitates simultaneous visualization of intracardiac defects and central cannulation sites. Therefore, the choice of the intercostal incision can significantly affect the surgical field. To address this, we propose a patient-specific, geometry-driven framework to objectively optimize surgical corridors. A web-based surgical planner was developed to simulate incision strategies using patient-specific computed tomogra

The Ductus Dilemma: To Close or Not to Close in the Fight Against Endocarditis/Endarteritis.

BACKGROUND: Patent ductus arteriosus (PDA) is a common congenital heart defect. While closure of hemodynamically significant PDAs is well established, closure of small, hemodynamically insignificant PDAs for prevention of infective endocarditis or endarteritis (IEE) remains controversial and is supported only by low-level evidence. METHODS: A systematic PubMed search was performed in May 2025 to identify published case reports of PDA-associated IEE. Data on PDA characteristics, audibility, vegetation location, and causative pathogens were extracted. In addition, the annual national number of

PDA in Prematurity: Rethinking a Decades-Old Debate in 2026.

The management of patent ductus arteriosus (PDA) in premature infants remains a significant debate in neonatology. Interventions aimed at accelerating ductal closure, often using nonsteroidal anti-inflammatory drugs (NSAIDs) or acetaminophen, are common practice. However, recent evidence increasingly challenges this approach. Pharmacological agents for PDA closure demonstrate limited efficacy and carry significant risks of systemic toxicity, affecting renal, gastrointestinal, vascular, and pulmonary systems. Multiple recent randomized controlled trials (RCTs) and meta-analyses have largely fai

Premature foramen ovale restriction/closure in pregnancy: echocardiographic features and overlap with aortic coarctation.

OBJECTIVES: Evaluation of the foramen ovale (FO) is essential for understanding right heart predominance in the fetal circulation during the third trimester. Rather than considering the FO as a simple orifice, it should be assessed as a complex anatomical and functional system. Premature FO restriction or closure represents a clinically relevant but diagnostically challenging condition in fetal echocardiography. METHODS: We conducted a monocentric retrospective study including 600 uncomplicated pregnancies between 22 and 38 weeks of gestation that underwent fetal echocardiography at the Cardi

Multimodality imaging of patent ductus arteriosus with severe pulmonary hypertension following 3 pregnancies.

Patent ductus arteriosus (PDA) typically presents in early childhood. Undiagnosed cases can progress to pulmonary vascular disease with significant morbidity and mortality. We present the case of a 46-year-old woman who presented with exertional chest discomfort and was subsequently diagnosed with severe pulmonary hypertension secondary to a large, previously unrecognized PDA. Multimodal imaging demonstrated right ventricular dilation and dysfunction, with pulmonary arterial pressures at systemic levels. Right heart and left heart catheterization revealed systemic level pulmonary vascular resi

Technical Success and Adverse Events Associated With Percutaneous Carotid Artery Access for Transcatheter Procedures in Infants.

BACKGROUND: Demand for transcatheter interventions in small neonates continues to increase. Percutaneous carotid artery access can facilitate intervention by providing a more direct wire course and/or potentially reduce morbidity associated with femoral artery access in very small patients. To our knowledge, there remains limited information on the technical success and adverse events associated with carotid access in this population. METHODS: A single-center retrospective case series was performed of infants undergoing carotid access between January 6, 2020, and September 30, 2025, for cathe

Closure of patent ductus arteriosus and respiratory function changes in ventilated preterm infants.

BACKGROUND: Active treatment of a haemodynamically significant patent ductus arteriosus (PDA) can be undertaken either pharmacologically or surgically. The effect of PDA treatment on respiratory function and ventilation parameters has not been adequately described. We aimed to report respiratory function changes following PDA treatment using composite respiratory physiological indices. METHODS: Retrospective cohort study of ventilated infants born before 30 weeks of gestational age, over two years at the tertiary Neonatal Unit of King's College Hospital, London, UK. Liver herniation in congenital diaphragmatic hernia is associated with delayed resolution of pulmonary hypertension.

BACKGROUND: Severe and persistent pulmonary hypertension (PH) predicts mortality and short-term pulmonary morbidity in infants with congenital diaphragmatic hernia (CDH). Intrathoracic liver herniation (liver-up) is a predictor of survival and increased need for extracorporeal membrane oxygenation. There is a paucity of data on the timeline of PH resolution in infants with and without liver herniation. We aim to determine the time to PH resolution in infants with liver-up and liver-down CDH, and to describe the resource utilization among these infants. METHODS: We conducted a retrospective st

Challenges in neonatal ductal stenting: When to avoid tortuous ducts and pulmonary artery stenosis.

Ductal stenting (DS) is the preferred palliative approach to the Blalock-Thomas-Taussig shunt (BTTS) for neonates with duct-dependent pulmonary circulation. However, appropriate case selection remains a critical challenge. This report explores the limitations of DS through illustrative examples and defines the subset in which neonatal DS is better avoided. Tortuous ducts increase the risk of procedural failure, ductal spasm, and pulmonary artery (PA) jailing. These can generally be managed with wire techniques and stiff wires. If extreme curvature persists despite these measures, it is best to
