Ibuprofen for hemodynamically significant patent ductus arteriosus in extremely preterm infants: a target trial emulation.

BACKGROUND: The optimal management of patent ductus arteriosus (PDA), particularly hemodynamically significant PDA (hsPDA), in extremely preterm infants remains highly controversial. This study aimed to evaluate whether early ibuprofen for hsPDA was associated with clinically meaningful outcomes in extremely preterm infants and whether this association was modified by clinical tolerance to PDA-related hemodynamic changes. METHODS: This prospective cohort study used data from the Chinese Multicenter Collaboration Platform for PDA in Extremely Preterm Infants and included infants with a gestati

Improving prediction of critical coarctation of the aorta in neonates: Key parameters and a novel scoring system.

BACKGROUND: Coarctation of the aorta (COA) is a congenital heart defect that is difficult to diagnose prenatally due to high false-positive and false-negative rates. Postnatal assessment is further complicated by a patent ductus arteriosus (PDA), which can mask COA. This study evaluated outcomes and developed a predictive model based on early postnatal echocardiography. METHODS: We retrospectively analyzed neonates with prenatal suspicion of COA admitted from 2019 to 2022, excluding complex heart defects. Echocardiography was performed within 24 h after birth. Intervention was defined as the

Cardiac Interventions in Neonates: State of the Art.

Percutaneous cardiac interventions in neonates and young infants have fundamentally altered the management paradigm of critical congenital heart disease, offering minimally invasive alternatives to surgery as definitive therapies or as bridges to later repair. Advances in catheter technology, device miniaturization, and imaging have enabled increasingly complex interventions to be performed safely in this highly vulnerable population. This review provides a comprehensive overview of commonly performed neonatal cardiac interventions, including balloon atrial septostomy, balloon dilatation of th

Caring for the smallest hearts: cardiovascular phenotypes and assessment in tiny babies.

With improving survival, periviable neonates (≤25 weeks' gestation) represent a dynamic, but under-studied population in neonatal care, with persistently high cardiopulmonary and vascular vulnerability. Immature cardiovascular structure and function as a consequence of immature myocardial architecture, altered calcium handling, relative adrenal insufficiency, and persistent fetal shunts contribute to complex and dynamic cardiovascular physiology in this population. This may be present clinically in the form of hypotension and low end-organ perfusion. Traditional paradigms of blood pressure-bas

Photon-counting CT angiography for anatomical assessment of patent ductus arteriosus in a child.

Patent ductus arteriosus requires accurate delineation when non-invasive anatomical definition is needed. We describe a 10-year-old girl in whom photon-counting CT angiography demonstrated a ductus connecting the descending thoracic aorta and proximal left pulmonary artery. Multiplanar and three-dimensional images defined the diameter, length, and spatial relationships. Photon-counting CT may complement echocardiography when detailed anatomical assessment is required.

Anomalous origin of the right pulmonary artery from the ascending aorta: a case report and review of the literature.

INTRODUCTION: Anomalous aortic origin of a pulmonary artery (AORPA), or hemitruncus arteriosus, is a rare congenital anomaly in which one pulmonary artery arises directly from the ascending aorta. Without timely repair, exposure to systemic pressure leads to irreversible pulmonary vascular disease and high early mortality. CASE PRESENTATION: A 4-month-old female infant presented with failure to thrive, tachypnea, and feeding difficulties. Transthoracic echocardiography and CT angiography confirmed an anomalous right pulmonary artery (RPA) originating from the posterior ascending aorta at the

Transcatheter closure of PDA: how the pathway changed from classical fluoroscopy approach.

Patent ductus arteriosus (PDA) is a common congenital cardiac defect traditionally managed by surgical and transcatheter closure under fluoroscopic guidance. Over recent years, significant advances in imaging modalities, device technology, and procedural planning have transformed the pathway for PDA closure, reducing reliance on classical fluoroscopy-based techniques. This review outlines the evolution of transcatheter PDA closure from conventional fluoroscopic approaches to contemporary strategies incorporating echocardiographic guidance, hybrid imaging, and radiation-sparing protocols. These

Pharmacologic Therapies for Patent Ductus Arteriosus in Extremely Preterm Infants.

IMPORTANCE: Wide variation exists in patent ductus arteriosus (PDA) treatment practices in extremely preterm infants (defined in this study as born before 29 weeks' gestation). Commonly used treatment strategies include use of standard-dose and adjustable-dose ibuprofen, indomethacin, and acetaminophen as well as nonpharmacological conservative management. OBJECTIVES: To compare the relative effectiveness of different PDA pharmacotherapeutic regimens in extremely preterm infants, and to compare the clinical outcomes between infants who were treated with pharmacotherapy and those who received

Radiographic Findings in Dogs With Patent Ductus Arteriosus: Left-to-Right, Right-to-Left, and Bidirectional Shunt.

Patent ductus arteriosus (PDA) is a common congenital heart defect in dogs, typically presenting as a left-to-right (L-R) shunt. This multicenter retrospective study aimed to characterize thoracic radiographic changes in dogs with L-R, right-to-left (R-L), and bidirectional (BID) PDA. All dogs had an echocardiographic diagnosis of PDA and underwent thoracic radiography. Qualitative radiographic evaluations assessed left heart enlargement (LHE), right heart enlargement (RHE), main pulmonary artery dilation (MPAD), aortic dilation (AD), and lung patterns. Quantitative radiographic assessments in

Postnatal pulmonary artery development from transcript to tissue.

Many congenital conditions and surgical interventions perturb haemodynamics in the proximal pulmonary arteries during postnatal development, thereby altering gene expression and associated changes in vascular structure and function. Among these, pathological conditions include patent ductus arteriosus, pulmonary atresia and stenosis and hypoxemia-induced pulmonary hypertension while surgical interventions include the placement of a Blalock-Thomas-Taussig shunt and the Glenn and Fontan procedures. Despite the significant morbidity associated with these diverse conditions, little attention has b

Radiation Reduction Through the Use of a Novel Ultra-Low Dose Fluoroscopy Imaging Protocol in Congenital Cardiac Catheterization.

Increased radiation exposure during congenital heart disease procedures may have a detrimental effect on patients, especially in the pediatric population. Advances in imaging systems and adherence to the ALARA (As Low As Reasonably Achievable) principle are paramount in minimizing radiation exposure. This is a single center, retrospective study designed to understand the degree of radiation reduction achievable with a novel ultra-low dose fluoroscopy protocol (ULDFP). Patients were included in the study if they underwent any one of the following procedures between 12/1/2021 and 12/31/2023: atr

Bifurcating Patent Ductus Arteriosus: A Rare Anomaly Associated with a Congenital Portosystemic Shunt.

Malformation of the ductus arteriosus (DA) is uncommon in otherwise healthy newborns. We report an extremely rare aneurysmal DA with bifurcation, in which the superior branch connects at the typical location on the roof of the main pulmonary artery, while the inferior branch connects to the left aspect of the left pulmonary artery origin. This unusual DA configuration is associated with an intrahepatic congenital portosystemic shunt.
