Transvenous occlusion of a patent ductus arteriosus in a cat with the Amplatzer™ Vascular Plug II.

A three-month-old male intact domestic shorthair cat was presented to Michigan State University for evaluation of a heart murmur and was diagnosed with a left-to-right shunting patent ductus arteriosus (PDA) via transthoracic echocardiography. The patient returned at eight months of age, and the PDA was successfully occluded with the Amplatzer™ Vascular Plug II using a transjugular approach. There are limited reports regarding non-surgical closure of a PDA in cats, including a single report describing closure of a PDA in a cat with this specific device. This case report highlights a feasible t

Hemodynamic evaluation following ductal closure and association with IVH in premature infants born at 22-23weeks gestation.

BACKGROUND: Intraventricular hemorrhage (IVH) remains a major complication in extremely preterm infants, but the immediate hemodynamic changes associated with hemorrhage after ductal closure are not well defined. METHODS: This was a two-center retrospective cohort study conducted at two tertiary neonatal intensive care units. Hemodynamic parameters, including left ventricular diastolic diameter, mean velocity of circumferential fiber shortening, end-systolic wall stress, mean arterial pressure, heart rate, and internal cerebral vein waveform grade, were assessed serially from birth to 72 h of

Diagnostic accuracy of electrocardiographic parameters to detect left atrial enlargement in children with post-tricuspid left-to-right shunt.

BACKGROUND: Left atrial enlargement is a surrogate marker for disease progression in congenital post-tricuspid left-to-right shunt lesions. Despite advanced imaging, a need persists for a more accessible modality like electrocardiography for early detection of left atrial enlargement. This study evaluates the diagnostic accuracy of established electrocardiography standards and a novel electrocardiography criterion (with altered position of chest leads) in identifying left atrial enlargement in children with post-tricuspid shunt lesions, enabling routine monitoring and timely referrals. METHOD

Two-Year Outcomes of a Randomized Controlled Trial of Nonintervention Versus Oral Ibuprofen for Patent Ductus Arteriosus in Premature Infants.

BACKGROUND: Optimal management of patent ductus arteriosus (PDA) in premature infants remains debated, and the long-term impact of different strategies has not been well studied. We previously conducted a randomized clinical trial comparing nonintervention (NI) and oral ibuprofen (IBU) for hemodynamically significant PDA in preterm infants, which showed no significant difference in bronchopulmonary dysplasia or death during hospitalization. In this study, we aimed to present a prespecified longitudinal follow-up investigation to assess the outcomes of persistently open PDA at discharge, as wel

Ureaplasma positivity and respiratory outcomes in very low birth weight infants: a cohort study with a nested double-blind randomized pilot trial of azithromycin.

BACKGROUND: Ureaplasma spp. colonization/infection has been implicated in the inflammatory pathogenesis of bronchopulmonary dysplasia (BPD) in very low birth weight (VLBW) infants, but the benefit of targeted macrolide therapy remains uncertain. We evaluated respiratory outcomes associated with early Ureaplasma positivity in VLBW infants and assessed feasibility and preliminary efficacy of a standardized intravenous azithromycin regimen in a nested pilot randomized trial. METHODS: This single-center study comprised (1) a retrospective cohort of inborn VLBW infants (< 1,500 g) born between Jul

Hemodynamic phenotyping of bronchopulmonary dysplasia: from transitional circulation to precision cardiopulmonary care.

OBJECTIVES: Bronchopulmonary dysplasia (BPD) remains one of the most important complications of extreme prematurity and a leading cause of long-term respiratory, cardiovascular, and neurodevelopmental morbidity. Increasing evidence suggests that BPD should be viewed not only as a parenchymal lung disorder but as a complex cardiopulmonary syndrome involving disrupted vascular development, abnormal transitional circulation, and ventricular dysfunction. This review aimed to summarize current evidence on the hemodynamic mechanisms underlying BPD, emphasizing pulmonary vascular disease (PVD), bronc

Immediate Zero-Contrast Ductal Closure Following Emergent Valvotomy in Pulmonary Atresia With Intact Ventricular Septum.

BACKGROUND: The timing of patent ductus arteriosus (PDA) closure after valvotomy in pulmonary atresia with intact ventricular septum (PA-IVS) remains controversial. CASE SUMMARY: A 23-day-old neonate with PA-IVS and tripartite right ventricle underwent valvotomy. Immediate PDA closure was subsequently performed because systemic diastolic pressure remained low despite adequate antegrade pulmonary flow, suggesting clinically significant ductal runoff with risk of systemic hypoperfusion. Given the patient's impaired renal function, the PDA closure was conducted using zero-contrast technique unde

Late-onset hypercyanotic spells in duct-dominant Tetralogy of Fallot with pulmonary atresia from juxtaductal PDA-LPA stenosis: CT angiography for preoperative mapping.

Tetralogy of Fallot with pulmonary atresia (ToF-PA) requires precise delineation of extracardiac pulmonary blood supply to guide optimal palliation, especially in duct-dependent physiology. We report an 8-month-old infant with late-onset central cyanosis and recurrent cry-triggered hyper cyanotic spells. Computed tomography (CT) thoracic angiography showed classic ToF with short-segment pulmonary atresia, a malaligned perimembranous ventricular septal defect, and hypoplastic yet confluent branch pulmonary arteries. A long, tortuous patent ductus arteriosus (PDA) provided dominant pulmonary flo

Clinical presentation and genotype-phenotype correlation of a novel pure duplication of 6p25.2-p22.3: a case report and literature review.

Duplication of 6p is a rare genetic syndrome of which about 25% have one or more congenital cardiac defects including cardiac septal defects, pulmonary artery hypoplasia and patent ductus arteriosus. We present the first case of a fetus with functional single ventricle and persistent truncus arteriosus during prenatal diagnosis whose genomic analysis revealed a novel pure duplication of 6p25.2-p22.3. The duplication fragment was confirmed to be associated with intrachromosomal insertion from mother via chromosome karyotype. The presentation of this case aims to expand the existing knowledge re

Extrauterine growth restriction definitions and associated factors among Colombian preterm infants: a case control study.

BACKGROUND: Extrauterine growth restriction (EUGR) is common in preterm infants and may impair neurodevelopment. The absence of a standardized definition limits early identification. We evaluated agreement and factors associated with EUGR using cross-sectional and longitudinal definitions. METHODS: Matched case-control study in three kangaroo mother care programs. EUGR was defined cross-sectionally and longitudinally. Cases were matched by gestational age and city (1:2 cross-sectional; 1:1 longitudinal). Conditional logistic regression assessed associations with maternal, perinatal, and nutri

Patent ductus arteriosus in Qatar: a retrospective study on epidemiology and neonatal outcomes following transcatheter closure.

INTRODUCTION: Patent ductus arteriosus (PDA) is a congenital heart defect resulting from failure of the ductus arteriosus to close after birth. It is particularly common in preterm infants and may become hemodynamically significant, requiring intervention. Transcatheter device closure offers a minimally invasive treatment option, though it can be technically challenging in this population. METHODS: This retrospective study included 80 infants diagnosed with PDA at Sidra Medicine, Qatar, between 2018 and 2024. Data were obtained from electronic health records. Patients who underwent medical tr

Assessment of Kidney Function and Acute Kidney Injury following Transcatheter Patent Ductus Arteriosus Closure in Preterm Infants.

OBJECTIVE: Transcatheter patent ductus arteriosus closure (TCPC) has been increasingly used in preterm infants. Hemodynamically significant patent ductus arteriosus (hsPDA) owing to ductal steal and changes in kidney perfusion is associated with the risk of acute kidney injury (AKI) and changes in kidney function. There is limited evidence describing changes in kidney function following TCPC and factors affecting the same. We conducted this study with aim of reviewing the prevalence of AKI and changes in kidney function following TCPC in preterm infants. STUDY DESIGN: A single-center retrospe

Factors associated with procedural difficulty and major adverse events during transcatheter closure of the patent ductus arteriosus.

Transcatheter closure of the patent ductus arteriosus (PDA) is widely accepted as an alternative to surgical ligation. However, major complications are uncommon. Therefore, procedural difficulty, including device repositioning or change, may provide additional insight into technical challenges encountered during PDA closure. We aimed to identify the clinical and anatomical risk factors associated with procedural difficulty or major adverse events during transcatheter PDA closure and to document the prevalence and types of complications encountered in a single tertiary center. This retrospectiv

Current Status of Patent Ductus Arteriosus Stenting: An International Practice Based Study.

Patent ductus arteriosus (PDA) stenting has become an accepted alternative to the modified Blalock–Taussig–Thomas shunt (mBTTS) for duct-dependent pulmonary blood flow (DDPBF). However, several key aspects of practice, including prostaglandin (PGE) titration, antiplatelet therapy, access strategy, back-up planning, and hypoplastic left heart syndrome (HLHS) palliation, remain highly variable. We conducted an international web-based survey of interventional pediatric cardiologists. Domains included PGE management, antiplatelet therapy, platelet function testing, stent-length selection, ECMO and
