BACKGROUND: Transcatheter stenting has become the preferred treatment for native and recurrent coarctation of aorta (CoA), but complications such as stent migration occur in approximately 5% of cases. Proximal stent migration is particularly challenging and often requires surgical intervention. This report highlights the successful transcatheter management of proximal stent migration during CoA stenting in a high-risk patient.
CASE PRESENTATION: A 22-year-old woman with Turner syndrome and chronic idiopathic thrombocytopenia purpura (ITP) presented with severe native CoA and refractory hypert

INTRODUCTION: Infantile hemangiomas are common benign vascular tumors in the pediatric population, but they rarely present as unilateral facial swelling in infants, making such cases atypical and warranting further investigation. This case report presents a one-month-old male with unilateral facial swelling, raising suspicion for an atypical infantile hemangioma and possible PHACE syndrome, which includes posterior fossa malformations, hemangioma, arterial anomalies, coarctation of aorta or cardiac defects, and eye abnormalities.
CASE PRESENTATION: A one-month-old male presented with right-si

Coarctation of aorta (CoA) in adults is often diagnosed incidentally during the work-up for secondary or resistant hypertension causes, making it one of the treatable causes of hypertension. Treatment options range from surgery to transcatheter interventions. We present 2 cases of complex coarctation anatomy managed successfully with a transcatheter approach, facilitated by meticulous planning using advanced imaging techniques. The first case had long-segment CoA, which required deployment of a custom-made 120-mm-long Cheatham platinum stent. The second case had coarctation associated with a h

Objective. To summarize the characteristics and reliability of clinical practice guidelines (CPG) for diagnosing and managing common congenital heart diseases (CHD). Methods. We conducted a scoping review of CPG for CHD (ventricular septal defects, atrial septal defects, patent ductus arteriosus, and coarctation of aorta) published or updated from 2018 to April 2023. We searched in general search engines, CPG repositories, and webpages of organizations that elaborate CPG. To assess the reliability of CPG, we used the Appraisal of Guidelines Research and Evaluation II (AGREE-II) tool, with a sc

BACKGROUND: Right ventricular (RV) systolic dysfunction and pulmonary hypertension is present in 20 % of adults with repaired coarctation of aorta (COA). However, the prognostic value of RV to pulmonary artery (RV-PA) coupling in this population is unknown. The purpose of this study was to assess the relationship between RV-PA coupling and clinical outcomes (heart failure hospitalization and all-cause mortality) in this population.
METHODS: Retrospective cohort study of adults with repaired COA and normal RV systolic function defined as RV free wall strain ≥ -24 %. RV-PA coupling was assessed

BACKGROUND: Coarctation of the aorta (CoA) is a relatively rare congenital narrowing of the distal aortic arch to proximal descending thoracic aorta, usually diagnosed in infancy or early childhood with upper extremity hypertension as a typical finding. Late detection in asymptomatic, physically active older adults is rare and can be clinically significant.
CASE PRESENTATION: We present the case of a 65-year-old lifelong endurance athlete found to have transient severe left ventricular (LV) systolic dysfunction after completing a marathon. Stress testing revealed a hypertensive response, prom

BACKGROUND: The treatment of vascular diseases has evolved significantly over the years, with recent advancements in interventional strategies, particularly in the management of acute limb ischaemia (ALI). Endovascular and catheter-based interventions, including aspiration mechanical thrombectomy, are now commonly employed and well-documented in the adult population. However, their use in paediatric patients remains rare and underreported. This case report presents our experience managing ALI in a paediatric patient using an endovascular approach-a relatively novel technique in this population

BACKGROUND: Coarctation of the aorta is often associated with aortic dilatation, presenting a complex challenge for stent-based interventions. To address this, we have developed a novel combination of stent treatments to manage this dual pathology effectively.
CASE SUMMARY: A 29-year-old hypertensive patient with a coarctation of the aorta and a descending thoracic aortic aneurysm was evaluated for treatment. Contrast-enhanced computed tomography angiography (CTA) revealed significant narrowing at the aortic arch (Zone 2, proximal to the left subclavian artery) with a diameter of 5.04 mm. Add

BACKGROUND: Left atrial (LA) stiffness is linked to pulmonary congestion and exercise intolerance in patients with acquired form of heart failure but has not been studied in adults with coarctation of aorta (COA). We hypothesized that adults with COA had increased LA stiffness, and in turn, worse cardiac reserve, pulmonary congestion, and impaired aerobic capacity compared to controls, and that LA stiffness index was associated with the presence of these abnormalities in the COA group.
METHOD: In this prospective study, 46 adults with repaired COA and 46 controls underwent exercise echocardio

BACKGROUND: Pulmonary hypertension is associated with cardiovascular events, but when assessed at rest, it has limited sensitivity. Pulmonary vascular reserve can be assessed noninvasively using exercise echocardiography, but this has not been studied in adults with coarctation of aorta (COA). We hypothesized that adults with COA had worse pulmonary vascular reserve compared to controls and that impaired pulmonary vascular reserve was associated with clinical indices of disease severity independent of right ventricular (RV) indices at rest.
METHOD: We assessed pulmonary vascular reserve using

OBJECTIVE: Contemporary management of aortic coarctation in adults is primarily by percutaneous intervention; however, this is not always possible. Intrapericardial ascending-to-descending aortic grafting is an alternative approach in adults with complex aortic coarctation. It can be performed off-pump and allows for a simultaneous treatment of concomitant cardiac lesions. Our aim was to examine the indications and the clinical long-term outcomes of intrapericardial ascending-to-descending aortic grafting in adults with aortic coarctation.
METHODS: We retrospectively reviewed demographic, cli

BackgroundCoarctation of the aorta (CoA) is a congenital condition that often requires intervention, especially when associated with other intracardiac abnormalities. Surgical approaches to treat complex CoA and associated defects vary, with no universally accepted standard approach. This study evaluates the outcomes of a single-stage extra-anatomic bypass from the ascending to the descending thoracic aorta, along with simultaneous correction of associated intracardiac abnormalities in adult patients.MethodsWe conducted a retrospective analysis of 17 adult patients who underwent a single-stage

Combination of bicuspid aortic valve and coarctation of aorta (CoA) is common. With the widespread use of transcatheter aortic valve replacement (TAVR), cases associated with either untreated or treated CoA forms an important subset and pose specific challenges. We discuss a case of a patient with previously stented coarctation presenting with degenerated aortic bioprosthesis considered for TAVR due to porcelain aorta. The CoA stent was significantly underexpanded, which was balloon dilated, followed by TAVR with a self-expandable valve. We discuss the factors determining sequence of managemen

BACKGROUND: Infective aortitis is a rare complication following coarctoplasty. Unrecognized concomitant congenital defects such as patent ductus arteriosus (PDA) could be devastating.
CASE SUMMARY: We describe a young woman presenting with constitutional symptoms, elevated inflammatory markers, and a history of coarctoplasty with stent a decade earlier. Imaging revealed a large previously overlooked PDA and an aortic pseudoaneurysm distal to the stent covered by vegetations. Positron emission tomography/computed tomography demonstrated metabolically active lesions within the pulmonary artery

BACKGROUND: Coarctation of aorta (COA) results in chronic left ventricular (LV) pressure overload and LV hypertrophy, which is associated with cardiovascular mortality. Therapies that decrease LV pressure overload result in regression of LV hypertrophy and improved survival. The goal of COA intervention is to relieve LV pressure overload, and the guidelines recommend the use of several COA severity indices as indications for intervention. These indices are Doppler mean gradient, systolic blood pressure (SBP), upper-to-lower-extremity SBP (ULE-SBP), aortic isthmus ratio, presence of collateral

Congenital heart defects (CHD) are among the most frequent congenital anomalies and represent a significant source of morbidity and mortality in infants. Both genetic and environmental factors are involved, and variants in the MYH6 are potential major genetic factors. The objective of the present study was to describe cardiac phenotypes in individuals with CHD and MYH6 variants. We included individuals with CHD and a MYH6 variant identified in four French genetics laboratories. Clinical and family data were collected. Of the 29 individuals included, 20 (68.9%) had a principal phenotype of left

OBJECTIVE: To characterize the fetoplacental response to acute maternal hyperoxygenation (AMH) in fetuses with suspected coarctation of the aorta (CoA) using fetal cardiac magnetic resonance imaging (MRI).
METHODS: Pregnant women carrying a fetus with suspected CoA were recruited prospectively from a tertiary fetal cardiology center as part of the Fetal Imaging with Maternal Oxygen (FIMOx) study between February 2019 and April 2022. MRI included phase-contrast flow measurements and oxygen-sensitive T2* sequences to assess placental and fetal brain oxygenation. Results were analyzed on a group

BACKGROUND: Monosomy 45,X is commonly associated with congenital heart defects, particularly coarctation of the aorta (CoA). In this case, the patient developed respiratory distress due to hemodynamic instability from a large bidirectional patent ductus arteriosus (PDA) shunt and systemic hypoperfusion secondary to CoA, which complicated diagnosis and management.
CASE PRESENTATION: We report a 34-week premature female neonate weighing 1.94 kg, delivered via lower segment cesarean section (LSCS) due to oligohydramnios and intrauterine growth restriction. She exhibited characteristic features o

Due to the shortage of pediatric hospital beds in general and due to the large annual burden of hospital admissions for common respiratory tract infections, such as influenza or RSV [respiratory syncytial virus] in particular, it can be expected that hospitalization of children with suspected or confirmed SARS-CoV‑2 infections will face hospitals with an even more tense situation than usual in the winter 2020/2021. This tenuous shortage may touch various aspects but in the first place it will affect isolation and cohorting. In addition, this shortage will not only apply to acutely ill children
