Families of 100 patients with coarctation of the aorta and 50 controls for age, sex, and social status were studied to assess the influence of genetic and environmental variables in the aetiology. A tendency to familial aggregation of the condition and other congenital heart defects compatible with multifactorial inheritance was discerned. Recurrence risk for sibs is approximately 1 in 200 for coarctation of the aorta, and 1% for any form of congenital heart defect. The heritability of coarctation is estimated at 58%. The tendency for other non-cardiac defects to occur in the patients with coa

In the Surgical Department of the Städtisches Krankenhaus München-Harlaching, 7 malformations of the lung in the shape of intralobar or extralobar sequestration were found among 1150 lung resections. Preoperative diagnosis caused no difficulty, even when angiography was dispensed with. In two cases a coincidence with other deformities was demonstrated. Deviations from the usual differentiation between intralobar and extralobar sequestration were found repeatedly with regard to location and vascular supply. All the 7 patients affected were cured by resection therapy.

The development of myocardial fibrosis as a result of cardiac hypertrophy was studied in 11 cats in which the pulmonary artery was banded, six rabbits in which the ascending aorta was banded, and eight cats with various congenital cardiac anomalies. Histological examination of the myocardium revealed multifocal areas of degeneration and necrosis with healing by fibrosis in the right ventricle of cats in which the pulmonary artery was banded and in the left ventricle of rabbits in which the aorta was banded. In five of eight cats with congenital anomalies, myocardial necrosis and fibrosis were

Efforts to close left-to-right shunts at Ochsner Medical Institutions have been directed toward atrial septal defects (ASD) and patent ductus arteriosus (PDA). PDA's were constructed in dogs by interposing a segment of jugular vein between the aorta and main pulmonary artery. Five dogs in which the PDA was closed by a plug device inserted through the femoral vessels were put to death at 6 to 12 months. Histologic sections showed good fibrous ingrowth with endothelial covering on the aorta and pulmonary artery sides. There were no migrations redidual shunts. At cardiac catherterization, 18 pati

Ten infants under 6 months of age with right ventricular-pulmonary artery discontinuity have undergone reparative operations. Five infants had either Type I or Type II truncus and 5 had pulmonary atersia with some discontinuity between right and left pulmonary arteries. There have been 4 survivors in each group. The one death in each category was due to pulmonary hypertension. The remaining 8 patients have low pulmonary vascular resistance. The survival rate after the reparative technique in infancy appears better than that with other forms of palliative treatment, such as banding of shunts, i

Plug closure of patent ductus arteriosus without thoracotomy is reliable and can be a good alternative to surgical closure for selected patients. We report our experiences with plug closure in 87 consecutive patients (age range, 3 to 38 years). Closure was successful in 83 patients (95 percent). There has been no mortality, and the failure in four patients (5 percent) was mainly due to an oversized ductus. A major complication during the procedure was dislodgment of the plug into either the aorta or the pulmonary artery, which occurred in five patients (6 percent). The late results were quite

The radiographic, angiographic, hemodynamic, and surgical features of 2 cases of cor triatriatum complicated by other cardiac anomalies are presented. In the first case, the patient had tetralogy of Fallot, the scimitar syndrome, and left ventricular obstruction. In the second case the patient had truncus arteriosus. The first case was revealed only after augmentation of pulmonary flow by an aorta-pulmonary artery anastomosis. In the second case, the diagnosis was retrospectively suggested by certain angiographic and hemodynamic clues. The prospective diagnosis was made only after a modified R

Two adult cases of corrected transposition of the great arteries are presented. One of the two males had associated ventricular septal defect, the other one presented as an isolated anomaly. The characteristic clinical features are: loud second heart sound with audible pulmonic component of the left sternal border in the absence of pulmonary hypertension. Abnormal precordial impulse upon palpation. Chest X-ray shows absence of pulmonic segment and of the aortic knob with the contour of the ascending aorta to the left of the mediastinum. Ecg anomalies may be multiple; typically anomalous septal

The relative size of the main pulmonary artery was determined from cineangiograms of 117 patients with d-transposition of the great arteries by calculating the ration between the diameters of the main pulmonary artery and aorta. The pulmonary artery was largest in patients with ventricular septal defect or patent ductus arteriosus, or both, because of increased pulmonary arterial pressure and flow. In patients with an intact ventricular septum or with left ventricular outflow tract obstruction, or both, the main pulmonary artery was approximately the size of the aorta. Two cases of d-transposi

Serum zinc and serum copper concentrations during early pregnancy in 84 consecutive primigravidae were correlated to other haematological factors and were also correlated to complications of labour and/or complications affecting the infant. In women with complications such as abnormal labour or atonic bleeding, serum zinc concentrations were significantly reduced (p less than 0.001) during early pregnancy. Women who gave birth to immature infants also showed significantly lower serum zinc in early pregnancy (p less than 0.01). Women delivered in the 37th week or earlier or in the 43rd week or

A ten-year-old girl suddenly died on a playground, after repeated syncopal attacks during the preceding two years. Post-mortem examination revealed the left coronary artery to have arisen by a separate ostium from the right sinus of Valsalva, then coursing to the left between the route of the aorta and the pulmonary trunk where it divided as usual. There was no narrowing of the coronary-artery lumen and the wall was normal. The sudden death is explained by a rise in pressure in the root of the aorta with valve-like closure of the coronary artery. This anomaly should be thought of in other wise

Reconstruction of the right ventricular outflow tract with an aortic homograft conduit was performed in 75 patients from 1966 to 1974. The types of congenital heart disease were as follows: pulmonary atresia, 35 cases; severe tetralogy of Fallot, 22 cases; truncus arteriosus, 6 cases; transposition of the great arteries (TGA), 3 cases; single ventricle, 2 cases; and tricuspid atresia, 7 cases. Ninety per cent of the patients had one or more previous shunts, and this was a factor affecting the mortality rate. Other factors included age, pulmonary vascular resistance, surgical anatomy, and techn

The study has been carried out on eight cases of pulmonary atresia with ventricular septal defect, this abnormality involves both valvular atresia and the stenosis of the outflow tract of the right ventricle; that means a pulmonary atresia both valvular and infundibular. The atresia is always combined with a dextroposition of the aorta, overriding the ventricular septum; often with an auricular septal defect or a patency of the foramen ovale. In two cases, other associative caridac malformations exist : either a single ventricle or an atresia of the tricuspid valve. The arterial supply to the

Selective aortic root angiography was performed in 18 patients with the clinical diagnosis of hypoplastic left heart syndrome in order to allow more precise correlation with echocardiography. Aortic valve atresia and severe hypoplasia of the left heart chambers subsequently were substaniated at necropsy in each instance. Wide variation in the size and shape of the aortic root and ascending aorta was demonstrated angiographically in these patients. In some patients, despite aortic valve atresia, the dimensions of the aortic root and sinuses of Valsalva at angiography were almost normal. In addi

Aortoventriculoplasty is a new method of treatment for left ventricular outflow tract obstructions. The concept is based on creating a surgical defect which is patched in such a way as to provide the largest possible outflow to the left ventricle. The incision of the aorta continues down as far as necessary, with the right ventricular wall, the aortic ring, and the septum being cut. Reconstruction with an inner Dacron patch on the septum is completed by replacing the aortic valve with an adequate prosthesis, covering the aortic incision with the same patch, and patching the right venticular op

1. The dissection of 223 subjects has revealed 3 cases of A. lusoria (1.06%). In all these cases, the A. lusoria was the last branch of the aortic arch. 2. X-ray pictures were taken after injection of radio-opaque substance to elicit the topographical relations of A. lusoria with esophagus. 3. There were only small variations in the calibre and in the way of its branches. Right common carotid artery directly arose from the aortic arch. 4. The three Aa. lusoriae crossed the dorsal face of esophagus between the 4th and 2nd cervical vertebrae where they provoked at neat natch. 5. In one case, the

Two patients are described who had the body habitus of the Marfan syndrome, cutaneous hyperextensibility, and atrophic "cigarette paper" scars. One had mild joint hypermobility and the other had generalized ligamentous laxity. Both individuals are considered to be examples of the Marfanoid hypermobility syndrome, a generalized heritable connective tissue disease with features of, but genetically distinct from, the Marfan and Ehlers-Danlos syndromes. One patient had a "floppy" mitral valve and the other had an aortic aneurysm due to cystic medionecrosis of the aorta; both findings provide evide

Two cases of diaphragmatic extralobar pulmonary sequestrations are described. One of them simulated a suprarenal tumour and the other was associated with a diaphragmatic hernia. Both of them were firmly attached to the left hemidiaphragm and were partly located within it. They had an arterial supply from the abdominal aorta. Both sequestrations were treated surgically by sequesterectomy. The patients had an uneventful recovery.

Patients with isolated total interruption of the arch of the aorta not accompanied by other cardiovascular anomalies may survive to advanced age. Their symptoms and course resemble those of patients with postductal coarctation of the aorta. However, there are basic differences in the pathologic findings in these two types of anomalies that may be of aid in the radiographic diagnosis.

Bicuspid aortic valves are much more common than monocuspid valves and diagnosed by A. P. aortography and, above all, lateral views. One may distinguish the true bicuspid valves which include two Valsalva sinuses, often asymmetrical, and bicuspid valves whith a supplementary raphe, the commonest type, in which on angiography, one may distinguish three Valsalva sinuses, one of which is larger than the two others. These cases of bicuspid aortic valve may be associated with other malformations of the aorta or may occur alone. In the latter case, the main risks are aortic valvular stenosis or aort
