Coarctation of aorta with right aortic arch. Report of surgical correction in 2 cases: one with associated anomalous origin of left circumflex coronary artery from the right pulmonary artery.

Two patients with right aortic arch, anomalous left subclavian artery, and coarctation of theaorta, have been operations for the relief of coarctation, successful in one but only partially successful in the other: in one case, a 'dacron' patch was inserted into the narrowed segment, but a mild recurrence of coarctation was found a year later; in the other, the proximal segment of the anomalous left subclavian artery was used for the repair, with complete relief of the coarctation, both immediately and at late follow-up. The first case had multiple associated congenital abnormalities, including

Echocardiography of the tricuspid valve in congenital left ventricular-riht atrial communication.

Two patients, ages 14 and 18, with congenital left ventricular-right atrial communication through a defect in the tricuspid valve were studied by echocardiography. Proof was obtained by angiocardiography and surgery in one and ultrasonic contrast injection and angiocardiography in the other. Both presented clinically as uncomplicated ventricular septal defects. Echocardiography consistently demonstrated a high frequency, low amplitude flutter of the tricuspid valve in systole and none in diastole. Following surgical correction of the defect in one patient, there was complete disappearance of t

[Classification of congenital aortic stenosis].

Recent epidemiological data relating congenital stenosis of the aorta are cited. An anatomical criterion is used in the formation of three classification groups, each containing a number of different types. There are no clear-cut clinical pictures corresponding to these anatomical varieties, however, so that differential diagnosis is dependent on the haemodynamic and contrastographic data. There are, on the other hand, clinical signs that serve as pointers to the site of the stenosis.

Persistent fifth aortic arch in man.

Persistence of the fifth aortic arch is a rare vascular anomaly of man, and in all cases reported thus far there has been an associated patent ductus Botalli (arteriosus). The present case is unique in at least three respects: lack of associated intracardiac malformation, the survival of the patient, and prenatal exposure to the possible teratogenic action of the anticonvulsant drug trimethadione (Tridione). Other noncardiovascular congenital defects were present in our patient. The dislocation of the hip and the patent ductus arteriosus have been corrected surgically.

Conduit reconstruction of right ventricular outflow tract. Experience with 17 patients.

Evaluation was made of 17 patients who underwent conduit reconstruction of the right ventricular outflow tract (for anomalies other than truncus arteriosus) at the Texas Heart Institute between December, 1965, and June, 1974. Fifteen patients survived the operation and have shown substantial clinical improvement. Several different conduits were used, principally an allograft aorta with the aortic valve and mitral leaflet attached, a woven Dacron prosthesis containing a xenograft (porcine) valve, and a valveless Dacron tube graft. Allograft conduits are sometimes difficult to procure and keep a

Congenital heart malformations associated with disproportionate ventricular septal thickening.

Asymmetric septal hypertrophy, or ASH, is a genetically determined myocardial disorder that is transmitted as an autosomal dominant trait. ASH is characterized by a disproportionately thickened ventricular septum that contains numerous hypertrophied, bizarrely-shaped and disorganized cardiac muscle cells. Disproportionate hypertrophy of the ventricular septum has also been observed in association with certain congenital cardiac malformations. To determine whether such congenital cardiac malformations are part of the disease spectrum of genetically determined ASH, cardiac pathologic observation

[Double aortic arch in infancy and childhood].

The diagnosis of double aortic arch was suspected by oesophagography and subsequently proven by angiography in six children. The patients' age ranged from 14 days to 13 years. The diagnostic value of angiography in establishing the existence of two aortic arches is emphasized, which can easily be performed by right heart catheterization. In five patients the aorta descended to the right of the spine; the left arch was hypoplastic and became stenotic or atretic distal to the subclavian artery. In only one case the aorta descended on the left side. In this case both arches had approximately the

[Congenital aortic arch anomalies and vascular rings (author's transl)].

The anomalies of the aortic arch are consequences of malformations in the first twelve weeks of the intrauterine life. Embryologically it is possible to explain and classify all of them. Most remain asymptomatic and do not need treatment. About one third of all anomalies are causing tracheo-esophageal compression with consequent complaints. In most cases the symptomatology starts in the first days of life. Some of them can be temporarily treated conservatively. The prognosis of patients, who need surgery is good, even in small infants. Surgery consists mainly in section of the atretic or hypop

[Anastomoses of the hepatic arteries angiographic appearances (author's transl)].

On the basis of 3,000 selective arteriograms of the coeliac trunk, the authors described the radiological appearance of anastomoses existing between the hepatic arteries. The anatomical study of COUINAUD had shown their existence. The increasingly frequent use of hepatic arteriography makes the demonstration easier. Such anastomoses exist under normal conditions but are more obvious, since they are increased in size, in various pathological states, in particular when there is obstruction of a hepatic artery. The anastomoses are of two types: of small calibre and usually situated in the porta h

Technical advances in resection and graft replacement of thoracic, abdominal, peripheral aneurysms.

Condition we now term aneurysm, deriving from the Greek word for "a sidening," has been recognized since the earliest days of medical history. The predilection of such lesions to rupture, with resultant hemorrhage, thrombosis, and distal ischemia, has led to constant attempts at surgical management, including ligation and incision, wrapping, wiring, plasticizing, packing, obliterative and reconstructive endoaneurysmorrhaphy, and a wide variety of procedures both ingenious and ingenuous. Modern treatment depends on minimal dissection, to obtain proximal and distal control, incision of only the

Nonspecific aorto-arteries.

In the USSR, like in other countries, aorto-arteritis is no rarity. Onehundred and twentysix patients with aorto-arteritis have been studied at the A.N. Bakulev Institute for Cardio-Vascular surgery during the past 13 years (81 women and 45 men from 8 to 49 years of age). Morphologic changes consisted in inflammatory infiltration of the adventitia and, to a lesser degree, of the media as well as in reactive hyperplasia of the intima. The predominant features in the chronic stage were those of sclerosis and fibrosis of the adventitia and the media which in combination with the intimal hyperplas

[Atypical suprarenal aortic stenosis as a cause of hypertension in young people (author's transl)].

In nine of ten hypertensive patients with "atypical" suprarenal aortic stenosis (aged 20 to 51 years, mean 28.5 years), operative treatment of the stenosis reduced blood pressure to normal. In the tenth patient, combined vascular and reno-parenchymal hypertension was effectively treated by drugs, the dosage of which could be reduced after resection. Aorto-aortic bypass was the operative procedure of choice because of its lower risk and better results than other methods. In a review of 346 cases of atypical coarcation reported in the literature, two-thirds were found to be located above or at t

Arterial hypertension and neurofibromatosis: renal artery stenosis and coarctation of abdominal aorta.

A 10-year-old girl had arterial hypertension, generalized neurofibromatosis, coarctation of the abdominal aorta and multiple stenoses at the origin of each renal artery. After resection of the stenotic areas and reimplantation of the renal arteries in the aorta, her arterial pressure decreased substantially. However, hypertension recurred and radiologic follow-up 4 1/2 years later showed distinct progression of the coarctation and renewed stenosis of all renal arteries at their origin. The stenotic areas showed eccentric intimal proliferation, frequently bulging into the lumen, with small nodu

Penetrating wounds of the heart and great vessels. Experience with 24 cases including 3 with intracardiac defects.

Twenty-four cases of penetrating cardiac injuries due to stab wounds, which were seen over a 3-year period, are reviewed. Ten patients were treated by aspiration alone because tamponade was mild, or because they were seen more than 12 hours after injury. One patient with multiple stab wounds in the ventricles and left atrium died during emergency thoracotomy in the admission room. The mortality rate of 4,5% is considerably lower than rates reported in other series. Two cases of traumatic ventricular septal defects, one case of traumatic aortic incompetence and sinus of Valsalva fistulae with r

Systemic-pulmonary artery anastomoses in infancy.

Over the last 10 years, 142 systemic-pulmonary artery anastomoses were performed in 134 infants under 1 year of age. These included shunts from the subclavian artery (46 per cent), ascending aorta (41 per cent), descending aorta (9 per cent), and superior vena cava (1 per cent) to the pulmonary artery. Ninety-six (72 per cent) of the infants were less than 4 months of age at the time of the initial procedure, and one half were less than 1 month old. When groups of patients were compared according to diagnosis and age at the time of operation, the immediate and long-term results of the subclavi

[Unusual combination of absence of the left pulmonary artery and homolateral malformation of the kidney and ureter (author's transl)].

In an one year old asymptomatic girl with a slight levoposition of the heart on chest X-ray and mild right ventricular hypertrophy in the ECG further cardiological investigation disclosed several anomalies: Absence of the left pulmonary artery, collateral vessels to the left lung both from the abdominal aorta and from a left brachiocephalic trunc, hypoplasia of the left lung, right sided aortic arch. There was no malformation of the heart itself. This is supposed to be the first reported case, in which the aforementioned anomalies were associated with hypoplasia and hydronephrosis of the left

[Congenital mitral stenosis. Report on 33 patients].

Thirty-three patients with congenital mitral stenosis are presented. Congenital mitral stenosis occurs as an isolated malformation and in combination with additional cardiac lesions. A new classification into 5 groups is suggested. It is possible to classify congenital mitral stenosis with and without additional lesions according to these 5 groups. This is of particular clinical advantage. Isolated congenital mitral stenosis is a rare malformation. In combination with additional obstructive left-sided cardiac malformations mitral stenosis occurs more frequently. In infancy and early childhood

Tricuspid atresia. An anatomical study of 17 cases.

This is a report on the anatomical characteristics of 17 cases of tricuspid atresia. Three of these cases had a discrepancy between the type of bulboventricular loop (dextro) and the position of the great arteries (the aorta being to the left of the pulmonary artery). In these a characteristic type of ventricular septal defect, located just beneath the tissue of the semilunar valves was found. In 3 cases with cardiac dextroversion and juxtaposition of the atrial appendages the great arteries were on the same frontal plane and there was a separation between the semilunar and the anterior mitral

The pathological anatomy of surgically reconstructable or prosthetically correctable congenital valvular malformation of the mitral region.

The special pathology of reconstructable or only prosthetically correctable congenital malformations of the mitral valve is described on the basis of the following examples taken from our own operative and autopsy material of the last 5 years: 1. Congenital isolated mitral stenosis in female twins (7 month old infant and 33 month old child). 2. Congenital isolated mitral insufficiency in a 7 1/2 year old boy. 3. Combined mixed mitral valve malformations with a parachute valve-like mitral valve anomaly, combined with hypoplasia of the ascending and descending aortas, in a 6 1/2 year old girl. 4
