Extracardiac predictors of functional capacity in a group of Fontan patients.

BACKGROUND: The Fontan procedure, a palliative surgical approach for single-ventricle physiology, has significantly improved long-term survival. However, it is frequently associated with reduced exercise capacity and multiorgan dysfunction. In this context, peripheral muscle function and body composition may influence functional outcomes. OBJECTIVE: The aim of the study was to evaluate the relationships among body composition, handgrip strength, and functional capacity in Fontan patients, as well as to investigate the impact of Fontan-related variables on these parameters. METHODS: This sing

Evaluation of high intensity interval training in patients with a right ventricle to pulmonary artery conduit in a randomized controlled trial: Rationale and design of the "Right HIIT" study.

BACKGROUND: Exercise training is a promising, relatively low-cost strategy to optimize exercise capacity, primarily studied in patients with tetralogy of Fallot and Fontan physiology. However, rare and complex biventricular disorders are understudied. High intensity interval training (HIIT) is a time-efficient alternative to the more commonly studied aerobic training. Despite group-level improvements in exercise capacity, individual variation suggests the existence of responders and non-responders to exercise training. We therefore designed a HIIT intervention trial for patients with complex o

Pulmonary Artery Diameters in Children, Teenagers and Young Adults Derived from Quiescent Interval Slice Selective (QISS) Magnetic Resonance Angiography.

The radial Quiescent Interval Slice Selective (QISS) technique, which is used in cardiovascular magnetic resonance (CMR), is a reliable, contrast-free technique for obtaining high-quality vascular images. It is distinguished by its safety, insensitivity to motion and arrhythmias, and its overall simplicity and adaptability. However, previously published studies assessing pulmonary artery (PA) dimensions in children have not yet incorporated this technique. CMR scans of 139 patients (95 males), aged 6 weeks - 20 years, with no evidence of PA disorders were analyzed. Main PA (MPA), right PA (RPA

Associated Coronary and Cardiac Anomalies among One to Eighteen Years Old Tetralogy of Fallot Patients.

Precise pre-operative diagnosis of any coronary or cardiac anomalies in Tetralogy of fallot (TOF) patients can minimize intra-operative and postoperative complications. In such cases time appropriate alternative surgical approaches may be required to consider, thus the consequent morbidity and mortality can be reduced. The study attempted to observe the TOF patients to assess the pattern of associated coronary and cardiac anomalies among them. This cross sectional study was carried out in the departments of Paediatric Cardiology in three tertiary level hospitals in Dhaka, Bangladesh, from Janu

Relief of Proximal Left Pulmonary Artery Branch Stenosis Using a Self-Expandable Stent following Tetralogy of Fallot Repair.

Proximal pulmonary artery (PA) stenosis is frequently observed following surgery for congenital heart disease, especially in cases of tetralogy of Fallot (TOF), and is one of the most common indications for reoperation. However, limited information is available regarding the efficacy of self-expandable stent implantation for proximal branch PA stenosis after total corrective surgery of TOF.Self-expandable stent placement was performed in 8 patients with left proximal PA stenosis from August, 2016 to August, 2020. The patients were mostly women (75%), with a median weight of 55.8 kg (range, 44-

ELN-Associated Supravalvular Aortic Stenosis With a Williams Syndrome-Like Cardiovascular Phenotype in a Neonate: A Case Report.

Williams syndrome (WS) is a rare microdeletion disorder affecting chromosome 7q11.23, including the ELN gene, which encodes elastin. Haploinsufficiency of ELN leads to vascular abnormalities, such as supravalvular aortic stenosis (SVAS), pulmonary stenosis, and coronary artery disease. SVAS can also occur in isolation due to heterozygous loss-of-function variants in ELN, independent of the broader WS deletion. We present a case of a term neonate with a Grade 4/6 systolic ejection murmur who underwent echocardiography, revealing severe supravalvular pulmonary stenosis, branch pulmonary artery s

Diagnostic Challenges in a Hybrid Congenital Pulmonary Airway Malformation: A Case Report.

Congenital pulmonary airway malformation (CPAM) and bronchopulmonary sequestration are rare congenital lung anomalies that can occasionally coexist as a "hybrid" lesion. We report a case of a 15-year-old female with a two-week history of fever with evening-predominant spikes, cough, and right hypochondrial pain. The pain was aggravated by deep inspiration and coughing. Imaging revealed a right lower lobe consolidation containing air-filled cystic spaces on chest X-ray and computed tomography (CT). An anomalous arterial supply from the descending thoracic aorta on CT angiography was also noted.

One Ostium, Three Coronary Arteries: A Rare Coronary Anomaly in a Patient With Secundum Atrial Septal Defect.

Atrial septal defects (ASDs) are among the most common congenital heart defects diagnosed in adulthood, with percutaneous device closure being the standard treatment for suitable secundum ASDs. However, the presence of anomalous coronary artery anatomy, particularly a high-risk interarterial course, may complicate procedural planning due to potential myocardial ischemia and sudden cardiac death. Single-ostium coronary artery anomalies are exceptionally rare, and their coexistence with a secundum ASD is even more uncommon, providing distinct diagnostic and treatment issues. We report the case o

Pulmonary sequestration with a rare renal artery subtype: a comprehensive clinicopathological analysis of 20 pediatric cases.

OBJECTIVE: This comprehensive study of 20 pediatric pulmonary sequestration (PS) cases primarily aims to demonstrate the pivotal role of histopathological confirmation in achieving a definitive diagnosis, particularly in atypical, extrathoracic, or diagnostically inconclusive presentations where imaging alone is often insufficient. METHODS: We conducted a retrospective analysis of clinical data from 20 children with pathologically confirmed PS who underwent surgical resection at our hospital between October 2022 and March 2025. We conducted a comprehensive assessment of clinical, laboratory, i

Computational Model for Predicting Optimal Clinical Intervention in Pre-Operative Neonates with Transposition of the Great Arteries.

PURPOSE: Transposition of the Great Arteries (TGA) is a congenital heart defect characterized by the abnormal positioning of the pulmonary artery over the left ventricle and aorta over the right ventricle. Systemic oxygen delivery, therefore, is dependent on the ability for blood to flow (shunt) between the "parallel" (systemic and pulmonary) circulations. Prior to the standard procedure for treatment of TGA (the arterial switch operation (ASO)), shunting is commonly inadequate, resulting in systemic hypoxemia and hypoxia. In this case the standard intervention is the balloon atrial septostomy
