Efforts to close left-to-right shunts at Ochsner Medical Institutions have been directed toward atrial septal defects (ASD) and patent ductus arteriosus (PDA). PDA's were constructed in dogs by interposing a segment of jugular vein between the aorta and main pulmonary artery. Five dogs in which the PDA was closed by a plug device inserted through the femoral vessels were put to death at 6 to 12 months. Histologic sections showed good fibrous ingrowth with endothelial covering on the aorta and pulmonary artery sides. There were no migrations redidual shunts. At cardiac catherterization, 18 pati

Three patients with interruption and seven with hypoplasia of the aortic arch were treated surgically. The subclavian artery and the aortic isthmus were employed for reconstructing the aortic arch in five, and a Dacron prosthesis was used to restore the aortic continuity in five. A ductus arteriosus coexisted in all patients and a ventricular or atrial septal defect in nine. Congestive heart failure and pulmonary hypertension were prominent clinical features, and the role of the ductus and other intracardiac anomalies on their pathogenesis in discussed. Six patients, one with an interrupted an

Ten infants under 6 months of age with right ventricular-pulmonary artery discontinuity have undergone reparative operations. Five infants had either Type I or Type II truncus and 5 had pulmonary atersia with some discontinuity between right and left pulmonary arteries. There have been 4 survivors in each group. The one death in each category was due to pulmonary hypertension. The remaining 8 patients have low pulmonary vascular resistance. The survival rate after the reparative technique in infancy appears better than that with other forms of palliative treatment, such as banding of shunts, i

Among 843 patients with Tetralogy of Fallot, 10 showed unilateral absence of a pulmonary artery. In 7 cases the cause was congenital aplasia, whereas thrombosis of one of the pulmonary arteries following systemic-pulmonary anastomosis was the cause in the remaining 3 patients. According to the reports of other authors and to our own experiences, this rare malformation is treated best with palliative operations, the technique depending on the patient's age and on the individual anatomical condition. Only in very few cases with an acceptable pathologic anatomy there will be a reasonable chance f

Plug closure of patent ductus arteriosus without thoracotomy is reliable and can be a good alternative to surgical closure for selected patients. We report our experiences with plug closure in 87 consecutive patients (age range, 3 to 38 years). Closure was successful in 83 patients (95 percent). There has been no mortality, and the failure in four patients (5 percent) was mainly due to an oversized ductus. A major complication during the procedure was dislodgment of the plug into either the aorta or the pulmonary artery, which occurred in five patients (6 percent). The late results were quite

The hospital mortality rate was 9.1 per cent (three deaths) in 33 patients less than 2 years old undergoing primary repair of single large ventricular septal defects (VSD's) since January 1972, and was not related to age; it was zero in 50 patients older than 2 years (Group A). The mortality rate with multiple VSD's was 36.4 per cent, not related to age. Age at operation, preoperative pulmonary vascular resistance, and pulmonary artery pressure were directly related to mean pulmonary artery pressure 5 or more years later in 74 patients (Group B) operated upon between 1955 and 1968. Three patients with congenital absence of the left pericardium and one patient with a partial left pericardial defect are described. Congenital absence of the left pericardium is characterized roentgenologically by levoposition of the heart, a prominent pulmonary artery segment and cardiac apex, interposition of lung between the left hemidiaphragm and the base of the heart, and unusual mobility of the heart. The electrocardiogram frequently shows right axis deviation, an incomplete right bundle branch block and leftward displacement of the transition zone in the precordial leeds. The anomaly h

A Muller-Dammann banding operation was performed in 64 infants and small children within 5 years. The diagnosis and the surgical risk (between 5.9% and 65% depending on the degree of intracardial malformation) were presented. Complications and cause of death were described and compared with data presented in the literature. Twenty-three patients were given a follow-up examination with special cardiac diagnostic methods. The hemodynamic results were compared with those from 9 children who were operated on late. As other authors, were also found a pronounced reduction in pulmonary flow in those

The radiographic, angiographic, hemodynamic, and surgical features of 2 cases of cor triatriatum complicated by other cardiac anomalies are presented. In the first case, the patient had tetralogy of Fallot, the scimitar syndrome, and left ventricular obstruction. In the second case the patient had truncus arteriosus. The first case was revealed only after augmentation of pulmonary flow by an aorta-pulmonary artery anastomosis. In the second case, the diagnosis was retrospectively suggested by certain angiographic and hemodynamic clues. The prospective diagnosis was made only after a modified R

Complete and unselected data concerning the postoperative pathology of congenital heart disease are presented for the first time. This study was based on 2,365 autopsies performed at the Children's Hospital Medical Center, Boston, in the 9 years from 1966 through 1974. Of these, 586 autopsies (25 percent) revealed congenital heart disease--238 performed in medically treated patients (41 percent) and 348 in surgically treated patients (59 percent). Tetralogy of Fallot, including cases with pulmonary outflow tract atresia and other associated malformations, was the congenital heart disease most

In this series of 13 patients undergoing repair of transposition of the great arteries with ventricular septal defect and pulmonary atresia, the operative risk and postoperative complications were greater than for repair of either transposition of the great arteries with ventricular septal defect and pulmonary stenosis or pulmonary arterial atresia with ventricular defect. Nevertheless, 6 of the 8 survivors improved clinically. Because operative and late mortality and morbidity rates are related to persistent right ventricular hypertension caused primarily by restricted pulmonary arterial outf

Arterial oxygen tension was measured from radial artery samples in 276 infants referred for cardiological investigation. Values obtained during air breathing in infants with congenital heart disease showed considerable overlap between 'cyanotic' and 'acyanotic' groups, and are of limited diagnostic use. By contrast, values obtained while breathing oxygen in concentrations of over 80%, measured in 182 infants, allowed clear differentiation between these groups. All infants with acyanotic, but only 2 of 109 with cyanotic lesions, achieved an arterial oxygen tension of more than 150 mmHg. In the

The relative size of the main pulmonary artery was determined from cineangiograms of 117 patients with d-transposition of the great arteries by calculating the ration between the diameters of the main pulmonary artery and aorta. The pulmonary artery was largest in patients with ventricular septal defect or patent ductus arteriosus, or both, because of increased pulmonary arterial pressure and flow. In patients with an intact ventricular septum or with left ventricular outflow tract obstruction, or both, the main pulmonary artery was approximately the size of the aorta. Two cases of d-transposi

Pulmonary outflow tract obstruction may occur at subvalve, valve, or pulmonary artery level. Selective ventriculography will usually define the intracardiac obstruction (whether valve, subvalve, or both) and may define pulmonary arterial abnormalities. Increasing experience in the surgical management of patients with tetralogy of Fallot and other conotruncal abnormalities characterized by significant pulmonary outflow tract obstruction suggests that preoperative recognition of pulmonary arterial stenosis is essential to the successful operative management of these patients (1-4). Because of th

We have studied the disposition of the cardiac conducting tissues in four hearts from situs solitus individuals possessing primitive ventricles without outlet chambers. These hearts correspond to the type of univentricular heart defined as common ventricle by Lev. All the hearts studied possessed normally positioned great arteries. Two groups, each consisting of two hearts, could be distinguished. The first type possessed a small posterior ridge which divided the posterior portion of the primitive ventricle into right and left ventricular sinuses. The papillary muscles to the atrioventricular

The combination of absent pulmonary valve with ventricular septal defect, subvalvular pulmonic stenosis and aneurysm of the pulmonary artery presents a characteristic heamodynamic and clinical profile. It should be differentiated from other morphologically related cardiovascular malformations. According to the degree of right ventricular outflow tract obstruction, the circulatory pattern and the clinical picture is varying widely. We are differentiating an obstructive and a congestive form of the disease. An own case is demonstrated. After a short review of the literature the clinical and haem

A ten-year-old girl suddenly died on a playground, after repeated syncopal attacks during the preceding two years. Post-mortem examination revealed the left coronary artery to have arisen by a separate ostium from the right sinus of Valsalva, then coursing to the left between the route of the aorta and the pulmonary trunk where it divided as usual. There was no narrowing of the coronary-artery lumen and the wall was normal. The sudden death is explained by a rise in pressure in the root of the aorta with valve-like closure of the coronary artery. This anomaly should be thought of in other wise

Surgical considerations and results of repair of 103 patients with pulmonary atresia are reviewed. The operative mortality was 10%, related primarily to inability to relieve right ventricular hypertension and to low cardiac output after complex repairs involving significant associated anomalies. In a follow-up study ranging from 6 months to 8 years, there were four late deaths, each being a result of persistent right ventricular hypertension. All but one of the surviving patients are in Class I or Class II. Since operative and late mortality and morbidity are related to unrelieved right ventri

Seven patients with pulmonary regurgitation (PR), normal pulmonary artery (PA) pressures and large left-to-right atrial shunts are reported. Six had secundum atrial septal defects (ASD) and one had anomalous pulmonary venous drainage. These comprised 4% of 180 patients with atrial shunts and normal PA pressures. Pulmonary regurgitation was diagnosed clinically by mid-frequency diastolic decrescendo murmurs beginning after the pulmonic component of the second heart sound, and diagnoses were confirmed by catheterization. In two patients who had serial preoperative catheterizations over 8 and 16
