Two cases of aortic pseudocoarctation were presented, the first one associated to rheumatic heart disease and the other one with an added obstruction of the left subclavian artery due to an angulation. The alterations of the subclavian arteries due to obstruction have been described very few occasions because they are usually asymptomatic. In our case there was only tiredness of the affected limb. The association of pseudocoarctation with rheumatic heart disease has been never reported, but we consider it to be just a mere coincidence. After reviewing what has been reported, we conclude that s

Hislop, A., Sanderson, M., and Reid, L. (1973).Thorax, 28, 435-441. Unilateral congenital dysplasia of lung associated with vascular anomalies. In three cases of unilateral lung dysplasia the structural changes have been analysed quantitatively to throw light on the nature of the anomaly and stage of its appearance. In one case the pulmonary artery was hypoplastic and the systemic supply normal but increased, suggesting onset in late intrauterine life or early childhood: in the other two cases the blood supply was only systemic, from both normal and abnormal arteries, suggesting onset in the e

Regional lung function has been studied with xenon-133 in two patients with unilateral transradiancy of the lung and coincidental cardiac disease. One patient had severe mitral stenosis with pulmonary venous and pulmonary arterial hypertension, the other had a secundum type of atrial septal defect with a pulmonary blood flow which was increased to 25 litres per minute. In both instances the overall perfusion of the transradiant lung was diminished but the regional distribution of blood flow was the same within both the `normal' and the transradiant lung. In the patient with mitral stenosis the

The position of surgery in the management of coarctation of the aorta in infants under 6 weeks of age is still not established, primarily because of the high mortality associated with resection of the coarctation. Five patients with coarctation in this age group who have undergone resection at St. Thomas' Hospital are described. One patient died at operation and one other has required a second operation to relieve restenosis of the anastomosis. The advisability of banding the pulmonary artery at the time of resection of coarctation complicated by other left-to-right shunts is suggested.

Twenty cases of corrected transposition of the great vessels of the bulbo-ventricular inversion type, either lone or combined with other intracardiac anomalies, were analysed. Rhythm and/or atrio-ventricular conduction disturbances were common to all groups of cases. QRS pattern changes were found to be related both to ventricular inversion and to ventricular hypertrophy. Isolated corrected transposition and corrected transposition with systemic ventriculo-atrial regurgitation give rise to tracings suggestive of systemic ventricular hypertrophy.Corrected transposition of the great vessels with

Extralobar sequestration of pulmonary tissues is relatively rare and seldom produces symptoms. An incidental finding is described of extralobar sequestration of the lung in a Nigerian child, presenting as an anterior mediastinal polycystic mass. The mass was attached to the right pulmonary artery by a fibrovascular pedicle which contained a small elastic artery, veins, and nerve bundles. Sequestrated lobes are usually situated in the posterior basal lobes of the lungs and may be associated with other congenital abnormalities, but the case reported here differs in these respects. Death was due

A case of multiple pulmonary arteriovenous fistulas is reported. Hereditary hemorrhagic telangiectasia is a characteristic associated finding, and in this instance affected 10 members of the patient's family over four generations. This association suggests that the pulmonary condition in its congenital form is part of a generalized vascular dysplasia. Clinically, the patient experienced increased dyspnea and fatigue but cyanosis and polycythemia were not noted. After surgical excision of the fistula with conservation of as much pulmonary tissue as possible, prompt relief of symptoms was obtain

The case records, cardiac catheterization and angiographic findings in 32 patients were reviewed to illustrate the high incidence of pulmonary artery stenosis in congenital rubella syndrome. These interesting lesions often are of equivocal or unknown clinical significance compared with the other malformations that beset these patients, but to know of them explains certain physical findings and to study their incidence is an unusual opportunity to pinpoint a definite cause for a congenital cardiac lesion.
