Deep learning-based automated assessment of pulmonary artery indices and surgical approach triage for tetralogy of Fallot from multicenter cardiac computed tomography (CT).

AIM: To develop an automated framework for deep learning (DL)-based segmentation, pulmonary artery indices (PAIs) computation, and rule-based surgical triage from cardiac CT (CCT) in pediatric Tetralogy of Fallot (TOF). MATERIALS AND METHODS: Preoperative CCT scans of pediatric TOF patients were collected from the primary internal centers and two external centers. A modified 3D nnU-Net architecture was used for automated segmentation of the pulmonary arteries (PA) and descending aorta (DAO). Following segmentation, PAIs were automatically computed using predefined geometric algorithms. Segmen

Current Status and Priorities of Valved Conduits for Right Ventricle-to-Pulmonary Artery Reconstruction in Japan: A Nationwide Survey.

Right ventricle-to-pulmonary artery reconstruction is essential in congenital heart surgery. In Japan, cryopreserved homograft availability is limited, necessitating alternative materials. This study systematically elucidated the status of right ventricle-to-pulmonary artery conduit use and surgeon priorities in Japan. In 2024, a web-based nationwide survey was conducted by the Japanese Society of Paediatric Cardiology and Cardiac Surgery, targeting 126 institutions. We analysed data from 2021 to 2023 regarding primary diagnoses, conduit types, and selection criteria. Responses were obtained f

Retreatment of persistent pulmonary arteriovenous malformations: factors associated with technical success and durable occlusion.

AIM: To evaluate factors that contribute to successful retreatment of persistent Pulmonary Arteriovenous Malformations (PAVMs) after initial embolization, focusing on long-term occlusion outcomes. MATERIALS AND METHODS: This study reviewed consecutive treatments for persistent PAVMs re-treated at a single institution. The success of repeat treatment was assessed on follow-up imaging. RESULTS: Twenty-four retreatment procedures in 19 patients were included (median age 59, range 9-71). Most PAVMs were simple (71%) rather than complex (29%), and saccular (71%) rather than non-saccular (29%). PA

A New Treatment Paradigm for Tetralogy of Fallot/Absent Pulmonary Valve with Significant Airway Compromise: Addressing the Airway First.

PURPOSE: Tetralogy of Fallot with absent pulmonary valve (TOF-APV) can manifest neonatally with severe respiratory insufficiency secondary to tracheobronchomalacia (TBM). Typical treatment of patients with TOF-APV with significant respiratory compromise is directed at the underlying heart defect rather than the airway. Complete repair often does not ameliorate airway issues and subsequently requires a tracheostomy or ventilator. DESCRIPTION: Given this lack of effective treatment, a bioresorbable, patient-specific, 3-dimensionally printed splint was developed that provides external support to

Intra-Diaphragmatic Extralobar Pulmonary Sequestration in an Adult With Laparoscopic Resection.

Intra-diaphragmatic extralobar pulmonary sequestration (IDEPS) is a rare congenital anomaly that is exceedingly uncommon in adults. Herein, we report an adult case of incidentally detected IDEPS. Computed tomography revealed a 49-mm mass beneath the left diaphragm with internal calcifications; however, preoperative diagnosis and precise localization were challenging. The differential diagnoses included extralobar pulmonary sequestration, bronchogenic cyst, and other tumors. Diagnostic surgical resection was planned with a laparoscopic approach based on imaging findings. Intra-operatively, the

Pulmonary Sequestration in the Guise of Pneumonia: Clues to a Hidden Entity.

Pulmonary sequestration is an important yet often overlooked congenital lung malformation in adolescent medicine characterized by dysplastic, non-functioning pulmonary tissue that lacks communication with the tracheobronchial tree and derives its blood supply from an anomalous systemic artery. We report the case of a 12-year-old female patient referred to our institute with a six-month history of intermittent fever, cough, and chest pain, initially managed as a case of right-sided pneumonia elsewhere. Contrast-enhanced computed tomography of the thorax revealed a cavitary soft tissue lesion in

Phenotypic Characterization of Unclassified Pulmonary Hypertension.

BACKGROUND: Some symptomatic patients manifest pulmonary hypertension (PH), despite normal pulmonary vascular resistance and pulmonary artery wedge pressure-a condition termed unclassified PH. Although hypothesized to reflect increased flow as seen in congenital heart disease, broader clinical implications remain unknown. METHODS: The current analysis included pulmonary vascular disease phenomics participants with either no PH or unclassified PH who underwent dynamic right heart catheterization and transpulmonary metabolomics. In a validation cohort, patients with no PH or unclassified PH und

Malignant melanoma identified within a pulmonary sequestration- a case report.

BACKGROUND: Pulmonary sequestration (PS) is a congenital lung malformation, which is defined by an anomalous arterial supply from systemic circulation and an absence of communication with the tracheobronchial tree. It is further divided into intralobar and extralobar PS. Pulmonary sequestrations are usually treated surgically, especially if they are symptomatic. Malignant tumors are rarely found within such malformations, mainly adenocarcinomas. We report a case of a malignant melanoma found in a patient operated for intralobar pulmonary sequestration, which is, to the best of our knowledge, t

[Deep vein thrombosis resulting in arterial occlusion].

A patent foramen ovale (PFO) is present in approximately 25-30% of adults and is usually asymptomatic. However, under conditions of elevated right heart pressure, a PFO can enable right-to-left shunting, allowing venous thrombi to enter the arterial circulation: a mechanism known as paradoxical embolism. We present a case of a 51-year-old woman who developed dyspnea, abdominal pain and, a painful swollen leg. Imaging revealed a saddle pulmonary embolism, renal and splenic infarctions and occlusion of the right brachial artery. Echocardiography confirmed the presence of a PFO. Other common site

Staged embolisation strategy for spinal epidural arteriovenous fistula with concomitant pulmonary arteriovenous malformation.

A woman in her 80s was referred for evaluation and treatment of progressive paraparesis due to a suspected spinal arteriovenous shunt. During hospitalisation at the referring institution, she developed multiterritory embolic stroke, prompting investigation.Bilateral deep venous thrombosis and a large pulmonary arteriovenous malformation (PAVM) with a high-grade extracardiac right-to-left shunt were identified, supporting paradoxical embolism as the most likely mechanism. Spinal angiography confirmed a spinal epidural arteriovenous fistula (SEDAVF) with venous drainage into intradural spinal ve

[Bilateral Thoracoscopic Surgery for Bilateral Pulmonary Arteriovenous Malformations:Report of a Case].

Pulmonary arteriovenous malformation (PAVM) is a congenital vascular anomaly that forms a shunt between the pulmonary artery and vein. It is frequently detected incidentally in asymptomatic patients. We encountered a 69-year-old female patient with bilateral PAVMs that were incidentally detected on abdominal computed tomography( CT) performed for a detailed examination of appendicitis. Abdominal CT revealed nodular opacities with contrast enhancement in the middle lobe of the right lung and the lingular segment of the left lung. They were further examined, and a diagnosis of PAVMs measuring 14

Unusual radiological presentation of extralobar pulmonary sequestration: a preoperative diagnostic challenge.

BACKGROUND: Extralobar pulmonary sequestration is a rare disease and is frequently associated with congenital malformations. Herein, we report an adult case of an unusual extralobar sequestration accompanied by a bronchogenic cyst and the concurrent absence of the left pericardium. CASE PRESENTATION: A 57-year-old female patient presented with an abnormal shadow on a chest radiograph. Contrast-enhanced computed tomography revealed a partially enhanced cystic mass in the left anterior mediastinum. Another cystic lesion was detected on the left side of the trachea in the upper mediastinum. The a

Embryological and morphological insights into diagnosing cardiac anatomy in heterotaxy syndrome.

AIM: The aim of this study was to highlight the hypothesis that embryological development and morphological aspects necessitate a comprehensive analysis of all situses and a detailed description of each anomaly, rather than attempting to force the malformation into the rigid categories of heterotaxy syndrome (HS) - left or right isomerism. This hypothesis is supported by describing the anatomy of a complex patient with multiple laterality defects resulting in multiple cardiac and organ malformations. CASE PRESENTATION: The unique aspect of the presented patient's morphological diagnosis lies

Transcatheter Pulmonary Artery Intervention in Tetralogy of Fallot With Pulmonary Atresia During Pregnancy.

BACKGROUND: While long-term survival for tetralogy of Fallot continues to improve, residual hemodynamic abnormalities are common and increase risk for cardiovascular events during pregnancy. CASE SUMMARY: A young woman with tetralogy of Fallot with pulmonary atresia (TOF/PA) having undergone biventricular repair presented with heart failure symptoms and near-systemic right ventricular (RV) pressure in the setting of an unplanned pregnancy. Given her desire to continue the pregnancy coupled with the significant maternal and neonatal risk, percutaneous intervention was performed, improving RV p

Hybrid Congenital Pulmonary Malformation Revealed by Chronic Hemoptysis in a Six-Year-Old Girl: A Case Report.

Congenital lung malformations (CLMs) are developmental conditions characterized by overlapping clinical and imaging features, with hybrid lesions that combine congenital pulmonary airway malformation (CPAM) and bronchopulmonary sequestration (BPS) becoming increasingly recognized by multidetector CT (MDCT). We present the case of a six-year-old girl with a one-year history of recurrent low-volume hemoptysis. Her chest radiograph indicated a subtle opacity in the right basal area. Thoracic CT angiography revealed parenchymal consolidation in the right lower lobe, characterized by an air broncho

Early-Diastolic Right Ventricle-Pulmonary Artery Pressure Gradient by Doppler Echocardiography to Guide Cardiac Magnetic Resonance Assessment of Pulmonary Regurgitation after Right Ventricular Outflow Tract Reconstruction.

BACKGROUND: Pulmonary regurgitation (PR) after right ventricular outflow tract (RVOT) reconstruction is a major driver of right ventricular (RV) remodeling. Cardiac magnetic resonance (CMR) is the reference standard for quantifying PR but is not routinely feasible in many pediatric follow-up settings. We evaluated whether an echocardiographic early-diastolic RV-pulmonary artery (PA) pressure gradient (Echo-EaDPG), derived from early-diastolic peak PR velocity (ΔP = 4v2), can serve as a practical complementary hemodynamic marker to guide CMR assessment. METHODS: We retrospectively analyzed pat

Quantification of Tricuspid Valve Regurgitation With 2D and 4D-Flow MRI: Comparison of Different Methods in Adult Patients With Ebstein's Anomaly.

BACKGROUND: Ebstein's anomaly (EA) is a congenital heart disorder involving tricuspid valve dysplasia and right heart abnormalities resulting in severe tricuspid regurgitation (TR). Multiple techniques assess regurgitation severity, but their correlation with EA severity markers remains unclear. PURPOSE: To compare MRI techniques for quantifying TR and regurgitant volume (RVol) in adults with EA. STUDY TYPE: Retrospective. SUBJECTS: Thirty-three adult patients with EA (15 males/18 females). Risk of congenital anomalies among infants of patients with endometriosis: a population-based cohort study.

BACKGROUND: Maternal chronic conditions are implicated as risk factors for congenital anomalies, but few studies have assessed reproductive conditions such as endometriosis. Given the association between endometriosis and infertility requiring fertility treatments, we sought to estimate risks of congenital anomalies in infants of patients with endometriosis and mediation by subfertility and fertility treatments. METHODS: We conducted a population-based study of births in Ontario (Apr. 7, 2006, to Mar. 31, 2021) to identify infants born at 20 weeks' gestation or later to patients aged 15 to 50

An Unusual Case of Acquired Dilated Cardiomyopathy Due to Long-Standing Patent Ductus Arteriosus.

Patent ductus arteriosus (PDA) is a common congenital cardiac condition in neonates. Its frequency is substantially higher in preterm infants, and it is frequently associated with other congenital heart defects. Its persistence into adulthood has become increasingly uncommon due to advances in neonatal screening, echocardiographic diagnosis, and timely therapeutic intervention. The hemodynamic burden imposed by PDA depends on the magnitude of the left-to-right shunt and may result in both short- and long-term complications. While small shunts may remain asymptomatic throughout life, moderate a
