Risk factors for early right ventricular-to-pulmonary artery conduit failure in congenital heart disease.

OBJECTIVES: Right ventricle-to-pulmonary artery conduits are a mainstay of treatment for patients with congenital heart defects. We investigated the association of underlying genetic abnormalities with conduit failure and hypothesized that 22q11.2 deletion syndrome was associated with shorter time to failure. METHODS: We conducted a single-center retrospective cohort study at the Children's Hospital of Philadelphia. Patients who underwent right ventricle-to-pulmonary artery conduit placement as part of a biventricular repair at 0 to 24 months of age between January 2010 and June 2020 were inc

Complete transposition of the great arteries with a patent ductus arteriosus, atrial septal defect, and juxta-arterial ventricular septal defect in a cat.

An 11-week-old male intact Siamese cat was presented for evaluation of a grade V/VI parasternal systolic murmur. Echocardiography revealed multiple congenital cardiac abnormalities, including parallel orientation of the aorta and main pulmonary artery, a patent ductus arteriosus, and a juxta-arterial ventricular septal defect. Cardiac remodeling secondary to these congenital defects was also noted. The patient was clinically stable at the time of presentation, but the owner ultimately elected for humane euthanasia before the onset of cardiac decompensation. Postmortem necropsy was performed to

Navigating the clinical complexity of acute heart failure in an untreated adult with dextrotransposition of the great arteries (D-TGA) and multiple cardiac anomalies: a case report and review of literature.

BACKGROUND: Transposition of the great arteries is a critical congenital heart defect characterized by the reversal of the aorta and pulmonary arteries, resulting in two parallel circulatory pathways. This condition is typically incompatible with life unless a shunt, such as an atrial or ventricular septal defect, facilitates the adequate mixing of oxygenated and deoxygenated blood in the heart. Survival into adulthood without surgical correction is exceptionally rare. While a limited number of cases have been reported globally, none, to the best of our extensive search, have been associated w

End-to-side anastomosis and autologous pulmonary artery patch for aortic coarctation and hypoplastic aortic arch.

BACKGROUND: Aortic coarctation (CoA) is a congenital heart disease that usually combines various cardiovascular malformations, including hypoplastic aortic arch (HAA). We explore end-to-side anastomosis (ESA) and autologous pulmonary artery patch (APAP) in CoA with HAA, to provide more clinical experience. METHODS: This is a single-center retrospective study. The target population is patients with aortic stenosis and HAA who underwent surgery between 2015 and 2021. All cases included were consecutive, and no instances were omitted due to absent data, lack of consent, or other factors. Collect

The Living Ross Procedure: A New Paradigm in Partial Heart Transplantation.

In the Ross procedure, homograft stenosis and regurgitation have limited the longevity of the right ventricle-to-pulmonary artery (RV-PA) conduit. Partial heart transplantation (PHT) facilitates conduit growth throughout the life of the child. We report the application of PHT of the RV-PA conduit during a Ross procedure, "living Ross," in a 3-year old patient with a congenital unicuspid aortic valve. Pulmonary autograft was used in the aortic position, and an aortic root allograft from the cardiectomy of an adult heart transplant recipient served as the RV-PA conduit. We hypothesize that this

Long-Term Outcomes in Infants After Tetralogy of Fallot Repair: Does Repair Strategy Matter?

BACKGROUND: Many studies of tetralogy of Fallot (TOF) with pulmonary stenosis repair focus on early outcomes. This study aimed to assess associations between TOF repair types and midterm or late outcomes. METHODS: This was a single-institution, retrospective cohort study of infant primary TOF repair from 1997 to 2020. Valve-sparing repair was compared with transannular patch repair. Outcomes were postdischarge unplanned reintervention on the pulmonary valve, main pulmonary artery, branch pulmonary artery, other reintervention and a composite of 1 or more of these reinterventions (composite).

Optimizing outcomes for patients undergoing superior cavopulmonary connection procedure: what we know.

The Superior cavopulmonary connection (SCPC) is typically the second stage of the single ventricle palliation. There are modifiable factors that have a complex interplay in determining outcomes such as mortality or hospital length of stay. There is no clear consensus on the timing of SCPC as this involves a critical balance between the timing of operation and the length of the high-risk interstage period. Younger age has not been associated with reduced transplant-free survival but has been associated with longer hospital length of stay. Another critical decision is for children with viral res

Complex developmental cardiovascular anomalies in a Sahiwal calf: a case study on patent ductus arteriosus, defective interatrial septum and arterial malformations.

We document complex developmental anomalies of the cardiovascular system, including patent ductus arteriosus (PDA), additional pulmonary arteries, malformed interatrial septum (IAS), unconventional origin of the common brachiocephalic trunk (CBCT) and subclavian arteries, in a 1-week-old calf of the Indian Sahiwal breed. The PDA was comprised of a cylindrical tube of regular calibre. Additional pulmonary arteries originated from the pulmonary trunk immediately after the bifurcation into the pulmonary arteries. The IAS had the form of an inverted cone frustrum, which projected towards the left

Aortic atresia, interrupted hypoplastic right aortic arch, single ventricle and bilateral arterial duct - a unique anatomy with surgical palliation.

BACKGROUND: Aortic atresia with interrupted aortic arch is usually incompatible with life. The survival of a newborn is only possible with a coexistence of the other cardiovascular defects. CASE PRESENTATION: We report a newborn with aortic atresia, interrupted hypoplastic right aortic arch, bilateral arterial duct, (S, L,L) transposition of the great arteries and double inlet left ventricle. The right aortic arch was interrupted between the right and left subclavian artery. Blood supply to the very hypoplastic ascending aorta and coronary arteries was provided by right arterial duct, connect

Total anomalous pulmonary venous connection - prenatal echocardiography and neonatal follow-up.

Total anomalous pulmonary venous connection is a rare congenital heart defect that can be diagnosed prenatally and might be very important for perinatal management. In addition to other cardiac abnormalities (levocardia, situs inversus, small left ventricle, double outlet right ventricle, parallel great vessels, and hypoplastic aortic arch), total anomalous pulmonary venous connection of a subdiaphragmatic type was diagnosed during a prenatal echocardiography examination in the second half of pregnancy. Fetal echocardiography monitoring showed no signs of congestive heart failure. The neonate

Arterial Tortuosity Syndrome: A Longitudinal Assessment of Cardiovascular Features and Interventions From the Collaborative for Longitudinal Aortic Research in the Young (CLARITY).

Arterial tortuosity syndrome (ATS) is an ultra-rare arteriopathy with a cardiovascular phenotype of vascular tortuosity, elongation, dilation, and stenosis. In 14 patients with ATS, aortic and pulmonary artery longitudinal growth, phenotypes, and interventions were evaluated. Medical records and images of 14 patients with ATS (defined as biallelic SLC2A10 variants with consistent phenotypes) were reviewed. Vascular measurements were collected from imaging reports. Z-scores were calculated. Dilation and hypoplasia were defined as z-scores ≥ +2.0 and ≤ -2.0, respectively. Slopes were calculated

Myocardial calcification revealed by postmortem computed tomography in a case of infantile myocardial ischemia caused by anomalous origin of the left coronary artery from the pulmonary artery.

Bland-Garland-White syndrome, or an anomalous origin of the left coronary artery from the pulmonary artery (ALCAPA), is a rare congenital coronary anomaly that can cause myocardial ischemia, congestive heart failure, and sudden death. In this study, we aimed to present a case of sudden infant death caused by ALCAPA. Postmortem computed tomography (PMCT) showed a collapsed left lung, enlarged heart volume, and severe calcification of the left ventricular papillary muscles and posterior left ventricular wall. Macroscopic examination during the autopsy revealed marked cardiac hypertrophy and an a

Left Pulmonary Artery Sling: Anatomy, Associations, and Outcome of Surgical Repair as Demonstrated by Autopsies.

Left pulmonary artery (LPA) sling is a rare congenital anomaly in which the LPA abnormally originates from the right pulmonary artery (RPA) and courses between the trachea and esophagus to reach the left pulmonary hilum. This anomaly is frequently associated with tracheobronchial and other cardiovascular anomalies and patients may manifest with varying airway and cardiovascular symptoms. Surgical repair is often required for symptomatic patients. Clinical outcomes largely depend on the extent and severity of coexisting anomalies, particularly tracheobronchial abnormalities. We report 2 autopsy

Nuclear blebs are associated with destabilized chromatin-packing domains.

BACKGROUND: Chronic mesenteric ischemia (CMI) due to either atherosclerosis of the mesenteric arteries or median arcuate ligament syndrome (MALS) is an underdiagnosed entity. The etiology of MALS and its existence have been debated and questioned. We aimed to identify plasma biomarkers indicating mesenteric ischemia in patients with CMI and MALS. METHODS: Plasma α-glutathione S-transferase (α-GST), intestinal fatty acid-binding protein (I-FABP), citrulline, and ischemia modified albumin (IMA) were analyzed in fifty-eight patients with CMI (Group A, n=44) and MALS (Group B, n=14) before and af

The impact of congenital heart disease on somatic growth: a monozygotic twin study.

Children with congenital heart disease (CHD) often experience growth faltering. However, somatic growth is significantly influenced by genetic and environmental factors. Given the unique genetic and environmental similarities of monozygotic twins, this study aims to evaluate the relationship between CHD and growth faltering, while minimizing the influence of these factors. We retrospectively reviewed data from 41 pairs of twins, where one had CHD and the other did not. Anthropometric measures were collected from birth to 3 years of age. Growth differences within the CHD group were further anal

May-Thurner syndrome: a forethought for asymptomatic pulmonary embolism.

May-Thurner syndrome (MTS) is a clinicopathological condition where the left common iliac vein is compressed by the overlying right common iliac artery, potentially leading to deep vein thrombosis (DVT) or obstruction to venous outflow. Unlike in DVT due to other causes, pulmonary embolism is not a frequent association of MTS. We report a man in his 30s who presented with left lower limb pain and swelling, without chest pain or shortness of breath. A contrast CT angiography of the abdomen and pelvis revealed a left common iliac, external iliac and femoral extensive DVT due to MTS. Incidental b

Risk of Recurrent Stroke Among Patients With Patent Foramen Ovale vs Pulmonary Arteriovenous Malformation: A Nationwide Study.

OBJECTIVES: Pulmonary arteriovenous malformation (pAVM) is an anatomical intrapulmonary left-to-right vascular shunt, and it may be a cause of paradoxical stroke. However, the risk of stroke associated with pAVMs is unknown. In this study, we seek to quantify the risk of stroke recurrence among cryptogenic stroke (CS) patients with pAVM compared with those with patent foramen ovale (PFO). METHODS: This was a retrospective analysis of the 2016-2022 Nationwide Readmissions Database. Patients with acute ischemic stroke were identified, and those with embolic, thrombotic, lacunar, or other specif
