Invasive Mucinous Adenocarcinoma in a Newborn With Antenatally Diagnosed Congenital Pulmonary Airway Malformation: A Case Report.

Congenital pulmonary airway malformations (CPAMs) are rare multicystic lung lesions typically diagnosed antenatally. We present a case of a term female neonate with antenatally diagnosed CPAM who required pleuro-amniotic shunting at 22 weeks of gestation. The patient was born with a right-sided pneumothorax and severe cardiorespiratory distress, necessitating extracorporeal membrane oxygenation (ECMO). Chest CT confirmed CPAM, revealing multiple cystic lesions in the right middle lobe and a significant contralateral mediastinal shift. On the second day of life, while on ECMO, the patient under

Clinical Care for Cardiovascular Disease in Patients With Williams-Beuren Syndrome.

Williams-Beuren syndrome (WBS) is a congenital multisystem disorder affecting the cardiovascular, central nervous, and musculoskeletal systems. Cardiovascular abnormalities, which consist principally of vascular stenoses, occur in approximately 80% of people with WBS and are the predominant cause of early morbidity and mortality. Supravalvar aortic stenosis and peripheral pulmonary artery stenosis are the most common stenotic lesions in WBS, though other stenoses often occur, including stenoses of the coronary arteries. Approximately one-third of people with WBS undergo cardiovascular interven

Optimal Shunt Type for Norwood Procedure: Predicted Adverse Impact of Discordant Surgical Approach.

BACKGROUND: The Single Ventricle Reconstruction (SVR) trial compared survival after Norwood procedure with either modified Blalock Taussig shunt (MBTS) or right ventricle pulmonary artery shunt (RVPAS). METHODS: Data from all 549 participants in the SVR trial were used to develop the MBTS TFSA algorithms, which predict the transplantation-free survival advantage (TFSA) after MBTS vs RVPAS at 1 and 6 years after Norwood procedure. Linear regression analysis of the MBTS TFSA values was performed to identify factors related to more optimal outcomes with MBTS at each timepoint. The impact of disc

Impact of aortopulmonary and venovenous collaterals on the onset of plastic bronchitis after the Fontan procedure.

OBJECTIVE: To clarify the association of the aortopulmonary collaterals (APCs)/venovenous collateral (VVCs) in patients with plastic bronchitis (PB) after the Fontan procedure. METHODS: All patients who underwent total cavopulmonary connection from 1994 to 2022 were reviewed. APCs/VVCs were detected using angiography. The impact of APC/VVCs, as well as other variables on the onset of plastic bronchitis, was evaluated. RESULTS: A total of 635 patients were included. Plastic bronchitis was observed in 15 (2.4%) patients, and the median duration between total cavopulmonary connection and the on

Fontan Circulation and Aortic Stiffness: Insights into Vascular Dynamics and Haemodynamic Interplay.

UNLABELLED: Increased aortic stiffness predisposes cardiac afterload and influences cardiac function. Congenital heart diseases involving aortic arch malformation and extended cardiovascular surgery, i.e. univentricular heart diseases, can lead to increased aortic stiffness. This study aimed to investigate whether Fontan patients (FO) have increased aortic stiffness within distinct aortic segments, and whether these parameters relate to Fontan-specific haemodynamics. In a prospective case-control study, 20 FO and 49 heart-transplanted control subjects with biventricular circulation underwent i

Cardiovascular magnetic resonance semi-automated threshold-based post-processing of right ventricular volumes in repaired tetralogy of Fallot.

BACKGROUND: Cardiovascular magnetic resonance (CMR) is the gold-standard to estimate right ventricular (RV) volumes, which are key for clinical management of patients with repaired tetralogy of Fallot (rTOF). Semi-automated threshold-based methods (SAT) have been proposed for CMR post-processing as alternatives to fully manual standard tracing. We investigated the impact of SAT on RV analysis using different thresholds in rTOF patients. METHODS: RV volumes and mass were estimated using SAT and standard fully manual tracing methods in rTOF patients. Two threshold levels were set for SAT, i.e.,

Non-compaction of the ventricular myocardium associated with large patent ductus arteriosus: primary or secondary?

BACKGROUND: The absence of other structural heart disease is a prerequisite for the diagnosis of non-compaction of the ventricular myocardium (NVM). We also observed that the phenomenon of non-compaction in ventricular muscle in some large patent ductus arteriosus (PDA) patients in children. This study was aimed to explore the prognosis of NVM associated with large PDA in children and provide a better understanding of the interplay between genetic and hemodynamic factors that lead to the phenotype of NVM. METHODS: We retrospectively analyzed the clinical data of the patients with the diagnosi

Short and Mid-term Outcomes of Ductal Graft with Bilateral Pulmonary Banding in Hypoplastic Left Heart Syndrome and Variants.

This study evaluates a novel surgical technique inspired by the hybrid Norwood procedure, which involves ductal graft placement and bilateral pulmonary artery banding. This technique is designed to avoid the high-risk classical Norwood stage 1 operation and to avoid the need for first stage invasive catheter interventions and interstage catheter interventions when ductal stent placement is not feasible in developing countries with limited resources. Between June 2015 and December 2022, 14 patients with hypoplastic left heart syndrome and variants were treated using this procedure at our center

A Young Woman with Hemoptysis as the main Manifestation of Isolated UAPA with Multiple Vascular Malformations.

BACKGROUND: The unilateral absence of a pulmonary artery (UAPA) is a very rare congenital pulmonary vascular malformation. Since the first case was reported in 1868, the overall incidence of UAPA has been at a level of 1/300,000 - 1,200,000. Isolated UAPA without other cardiac anomalies is rare. Here we report a young woman who came to the clinic with hemoptysis as the main manifestation. Isolated UAPA with multiple vascular malformations was finally diagnosed by DSA angiography. The diagnosis and treatment experience and gains of UAPA are discussed in combination with the treatment process of

Neonatal and Pediatric Pulmonary Vascular Disease.

Pediatric patients are affected by a wide variety of pulmonary vascular diseases ranging from congenital anomalies diagnosed at birth to acquired diseases that present later in childhood and into adolescence. While some pulmonary vascular diseases present similarly to those seen in adults, other forms are unique to children. Knowledge of the characteristic imaging features of these diseases is essential to facilitate prompt diagnosis and guide clinical management.

[Incidental discovery of a patent ductus arteriosus in adults].

The patent ductus arteriosus is a vascular structure that connects the proximal descending aorta to the main pulmonary artery near the origin of the left pulmonary artery. The persistence of ductal patency after the first weeks of life is abnormal. In adults, it is most often an isolated finding but it may be associated with other congenital anomalies. Its repercussions depend on the size of the duct, varying from the absence of signs and symptoms to Eisenmenger's syndrome. Echocardiography is the keystone of diagnosis. Multimodal imaging allows precise anatomical characterization. Treatment w

Fluid restriction for treatment of symptomatic patent ductus arteriosus in preterm infants.

BACKGROUND: The ductus arteriosus is a blood vessel connecting the main pulmonary artery and the proximal descending aorta. After birth, the ductus arteriosus usually closes completely. However, sometimes it remains patent in the neonate, which is called patent ductus arteriosus (PDA). If a PDA is clinically symptomatic, it can lead to overcirculation in the lungs, and reduced perfusion to the gut and kidneys. Most clinical practice guidelines suggest fluid restriction as a conservative management strategy for the treatment of symptomatic PDA. Restricting fluid has been shown to reduce the inc

Surgical Management of Norwood Procedure for Atypical Aortic Arch.

Background: Postoperative restenosis of the aortic arch after the Norwood procedure is still an important complication that significantly affects surgical outcomes. The rarity of the Norwood procedure for atypical aortic morphology means appropriate arch reconstruction methods and postoperative complications are still unknown. This study aimed to assess the rate of arch reintervention and clinical outcomes after the Norwood procedure for atypical aortic arch. Methods: This retrospective single-center study was conducted between 2001 and 2022. Sixteen patients were identified, eight with a righ

Management of a Patient With an Anomalous Right Coronary Artery: A Case Report.

Coronary artery anomalies are rare congenital defects that involve abnormalities in the origin, course, or termination of the three main epicardial coronary arteries. Due to the variety of aberrant coronary artery defects, the clinical presentation can differ. Anomalous origins of the right coronary artery include the pulmonary trunk, ascending aorta, left sinus of Valsalva, and a course that traverses between the great vessels. Diagnosis is made using multidetector computed tomography coronary angiography or coronary computed tomography angiography. Management may include lifestyle modificati

Biomechanical modeling combined with pressure-volume loop analysis to aid surgical planning in patients with complex congenital heart disease.

Patients with congenitally corrected transposition of the great arteries (ccTGA) can be treated with a double switch operation (DSO) to restore the normal anatomical connection of the left ventricle (LV) to the systemic circulation and the right ventricle (RV) to the pulmonary circulation. The subpulmonary LV progressively deconditions over time due to its connection to the low pressure pulmonary circulation and needs to be retrained using a surgical pulmonary artery band (PAB) for 6-12 months prior to the DSO. The subsequent clinical follow-up, consisting of invasive cardiac pressure and non-

Genetic etiology of truncus arteriosus excluding 22q11.2 deletion syndrome and identification of c.1617del, a prevalent variant in TMEM260, in the Japanese population.

Truncus Arteriosus (TA) is a congenital heart disease characterized by a single common blood vessel emerging from the right and left ventricles instead of the main pulmonary artery and aorta. TA accounts for 4% of all critical congenital heart diseases. The most common cause of TA is 22q11.2 deletion syndrome, accounting for 12-35% of all TA cases. However, no major causes of TA other than 22q11.2 deletion have been reported. We performed whole-genome sequencing of 11 Japanese patients having TA without 22q11.2 deletion. Among five patients, we identified pathogenic variants in TMEM260; the bi

Successful ductal stent implantation for initial palliation in two patients with aortic atresia and interrupted aortic arch.

The coexistence of aortic valve atresia and interrupted aortic arch are an extremely rare condition. In this pathology, blood flow to the ascending aorta and coronary arteries should be provided through the ductus arteriosus or collaterals originating from the descending aorta. In rare cases where bilateral ductus arteriosus is present, they can provide circulation. Here, we report two cases in which coronary arteries and ascending aorta were supplied by one ductus arteriosus and distal systemic circulation is supplied by a second ductus arteriosus in one patient and a collateral artery in the

Clinical Presentation and Therapy of Truncus Arteriosus.

Truncus arteriosus (TA, also known as common arterial trunk) consists of only one great artery ("the truncus") with a semilunar valve (truncus valve) arising from the heart and an additional ventricular septal defect and (Fig. 50.1). This great artery is positioned above the ventricular septal defect and gives rise to the coronary arteries, the pulmonary arteries, and the aortic arch. Historically, TA has been classified by Collet and Edwards in three types, where in type I there was a common pulmonary artery truncus, in type II the left and right PA arise separately but close to each other, i
