The hemodynamic impact of congenital heart diseases during fetal-to-neonatal transition: an in-silico investigation.

BACKGROUND: Congenital heart disease (CHD) is a significant congenital anomaly, with ventricular septal defect (VSD) and transposition of the great arteries (TGA) being commonly encountered. Despite advancements in prenatal diagnosis and postnatal care, predicting neonatal outcomes remains difficult due to fetal compensatory mechanisms. This study utilizes and adapts existing mathematical models, without patient-specific parameters, to simulate the hemodynamic effects of VSD and TGA during the fetal-to-neonatal transition, validated against literature data. METHOD: The model uses a closed-loo

Accuracy of fetal echocardiography in detecting lesions associated with, and predicting surgical plan for, dextro-transposition of the great arteries and double outlet right ventricle with subpulmonary ventricular septal defect and predicted transposition physiology.

OBJECTIVES: In dextro-transposition of the great arteries (D-TGA) and double outlet right ventricle with subpulmonary ventricular septal defect (DORV-SPV) and predicted transposition physiology, the presence of associated lesions, such as ventricular septal defect (VSD), coarctation of the aorta (CoA) or pulmonary stenosis (PS), impacts prenatal counseling and complicates neonatal surgery. This study aimed to evaluate the diagnostic accuracy of fetal echocardiography (FE) and its ability to predict the postnatal surgical plan in fetuses with D-TGA or DORV-SPV. METHODS: This was a single-cente

Long-Term Results After Senning and Mustard Operations for d-Transposition of the Great Arteries: Atrial Switch Should Remain in Armamentarium.

OBJECTIVES: The Senning and Mustard atrial switch procedures, once the standard surgical treatment for transposition of the great arteries, carry the long-term risks linked to a systemic right ventricle. METHODS: A retrospective follow-up study was conducted to characterize long-term outcomes focusing on survival, baffle-related, and non-baffle-related reoperations. RESULTS: N = 417 patients with d-transposition of the great arteries (d-TGA) (70% male) were treated with an atrial switch operation in one centre (n = 88 Mustard, n = 329 Senning, treated in 1974-2001). The mean follow-up time w

Obesity prevalence and risk factors in surgically repaired CHD.

Children with CHD have demonstrated a rise in obesity, and have unique risks related to comorbidities of obesity, including feeding dysfunction and exercise limitations. The incidence and cause of obesity among patients with surgically corrected CHD are not fully understood. This single-centre, longitudinal, retrospective cohort study identified patients between 2004 and 2020 with surgical correction. Diagnoses were restricted to d-transposition of the great arteries, coarctation of the aorta, or tetralogy of Fallot with surgical repair by 6 months of life without long-term post-operative comp

The Rapid Long-axis Strain-based Prognostic Models Evaluate Composite Adverse Events for Asymptomatic Dextro-Transposition of the Great Arteries Patients after the Switch Operation.

RATIONALE AND OBJECTIVES: Dextro-Transposition of the great arteries (d-TGA), the second most common cyanotic congenital heart defect, requires early risk stratification to identify high-risk arterial switch operation (ASO) patients for long-term prognosis. The aim of this study was to 1) determine biventricular strain and rapid long-axis strain (RLAS) could detect impaired biventricular mechanical in D-TGA patients and 2) establish and validate strain-based prognostic models for improved risk stratification and prediction of composite adverse events. MATERIALS AND METHODS: All patients with

Proposed Standardized Nomenclature for Anomalous Aortic Origin of a Coronary Artery: An International Multidisciplinary Initiative.

Increasing use of advanced noninvasive imaging has provided great anatomic and morphologic details to characterize congenital coronary artery anomalies. Anomalous aortic origin of a coronary artery (AAOCA) is a subtype that may lead to myocardial ischemia with variable clinical manifestations across the lifespan. Management decision making in AAOCA is hindered by anatomic variants of differing physiologic consequences. Several systems of nomenclature have been used that lack uniformity in potentially high-risk morphologic features. The ICAAC (International Coronary Artery Anomalies Collaborati

Computational Model for Predicting Optimal Clinical Intervention in Pre-Operative Neonates with Transposition of the Great Arteries.

PURPOSE: Transposition of the Great Arteries (TGA) is a congenital heart defect characterized by the abnormal positioning of the pulmonary artery over the left ventricle and aorta over the right ventricle. Systemic oxygen delivery, therefore, is dependent on the ability for blood to flow (shunt) between the "parallel" (systemic and pulmonary) circulations. Prior to the standard procedure for treatment of TGA (the arterial switch operation (ASO)), shunting is commonly inadequate, resulting in systemic hypoxemia and hypoxia. In this case the standard intervention is the balloon atrial septostomy

Prenatal Diagnosis of Isolated Congenitally Corrected Transposition of the Great Arteries.

Congenitally corrected transposition of the great arteries (ccTGA) is an uncommon congenital heart defect characterized by atrioventricular and ventriculo-arterial discordance resulting from abnormal leftward looping of the primitive cardiac tube. Although this double discordance produces a physiologically corrected circulation, the morphologic right ventricle functions as the systemic ventricle, and the tricuspid valve serves as the systemic atrioventricular valve, predisposing affected individuals to late complications. Prenatal diagnosis of ccTGA is often challenging, particularly in isolat

Loeys-Dietz syndrome: 2026 updated care management primer.

Loeys-Dietz syndrome (LDS) represents a clinically and genetically heterogeneous group of connective tissue disorders that share features similar to Marfan syndrome, first identified in 2005. Characterized by significant manifestations, such as aortic aneurysms, arterial tortuosity, craniofacial and skeletal anomalies, LDS results from pathogenic variants in key genes of the transforming growth factor-beta signaling pathway. Given its variable expressivity, a multidisciplinary approach to management is critical. The article provides an updated overview of effective management practices since t

Complex Genomic Rearrangement Involving the TBX4 Promoter Manifesting With Variable Expressivity in a Five-Generation Family.

Variants involving TBX4 are associated with ischiocoxopodopatellar syndrome with or without pulmonary arterial hypertension (ICPPS; also known as small patella syndrome), pulmonary arterial hypertension (PAH), and lethal lung developmental disorders. The variability of penetrance and expressivity of TBX4 variants remains a prominent challenge in understanding their genotype-phenotype correlations. We investigated a five-generation family with 12 affected individuals presenting with isolated ICPPS, lung-related manifestations with features of ICPPS, and other milder abnormalities. Whole-genome

Embryological and morphological insights into diagnosing cardiac anatomy in heterotaxy syndrome.

AIM: The aim of this study was to highlight the hypothesis that embryological development and morphological aspects necessitate a comprehensive analysis of all situses and a detailed description of each anomaly, rather than attempting to force the malformation into the rigid categories of heterotaxy syndrome (HS) - left or right isomerism. This hypothesis is supported by describing the anatomy of a complex patient with multiple laterality defects resulting in multiple cardiac and organ malformations. CASE PRESENTATION: The unique aspect of the presented patient's morphological diagnosis lies

Integrated pregnancy and perinatal management of complex fetal congenital heart disease: a preliminary retrospective study.

BACKGROUND: Congenital heart disease (CHD) is the most prevalent congenital abnormality worldwide and is the primary contributor to neonatal mortality. Obstetricians in China continue to face uncertainties about optimal delivery timing, appropriate delivery mode, perinatal interventions, and postnatal surgical outcomes for fetuses with complex CHD. OBJECTIVE: To investigate integrated pregnancy and perinatal management strategies for fetuses diagnosed with complex CHD by prenatal ultrasound in China. METHODS: A retrospective analysis was performed on 123 fetuses with complex CHD diagnosed by

Is There a Unified Etiology of Hypoplastic Left Heart Syndrome? Evaluating Genetic, Structural, and Hemodynamic Models of Disease Initiation.

Background: Hypoplastic left heart syndrome (HLHS) is defined as "a spectrum of congenital cardiovascular malformations with normally aligned great arteries without a common atrioventricular junction, characterized by underdevelopment of the left heart with significant hypoplasia of the left ventricle including atresia, stenosis, or hypoplasia of the aortic or mitral valve, or both valves, and hypoplasia of the ascending aorta and aortic arch". Without treatment, HLHS is usually lethal in the neonate. Many hypotheses have been advanced to explain the etiology of HLHS; however, no single theory

Neighborhood Economic Deprivation, Hispanic Ethnic Enclaves, and Congenital Anomalies in Texas.

BACKGROUND: Congenital anomalies are common, costly, and potentially life-threatening. The impacts of structural determinants of health on risk for these conditions have scarcely been studied. We evaluated associations between residence in economically disadvantaged neighborhoods or Hispanic/Latino enclaves and congenital anomalies. METHODS: We performed a registry linkage study of 7.25 million births and 332,935 cases with anomalies in Texas, 1999-2018. We defined exposure groups using maternal residential address at delivery or termination and tract-level Yost socioeconomic index and Hispan

Right Ventricular Electrophysiology and Arrhythmias in Adults With Congenital Heart Disease: Scientific Basis for Management.

Right ventricular (RV) dysfunction and arrhythmias are major concerns in adults with congenital heart disease (CHD). The relationship between RV dysfunction and arrhythmogenesis is bidirectional, with structural and electrical remodeling contributing to arrhythmia development and, conversely, arrhythmias exacerbating RV failure. In addition to an RV in the standard subpulmonary position failing because of pressure and/or volume overload, other phenotypes associated with RV dysfunction in CHD include transposition of the great arteries with a systemic (subaortic) RV and univentricular hearts wi
