Total anomalous pulmonary venous connection - prenatal echocardiography and neonatal follow-up.

Total anomalous pulmonary venous connection is a rare congenital heart defect that can be diagnosed prenatally and might be very important for perinatal management. In addition to other cardiac abnormalities (levocardia, situs inversus, small left ventricle, double outlet right ventricle, parallel great vessels, and hypoplastic aortic arch), total anomalous pulmonary venous connection of a subdiaphragmatic type was diagnosed during a prenatal echocardiography examination in the second half of pregnancy. Fetal echocardiography monitoring showed no signs of congestive heart failure. The neonate

The "Heart-and-Brain Interaction" in Newborns with Complex Congenital Heart Disease.

OBJECTIVES: Neonates with complex congenital heart disease (CHD), in particular newborns with hypoplastic left heart syndrome (HLHS) or transposition of the great arteries (TGA), may show brain pathologies and altered brain growth after birth. Our prospective study investigates brain volumes and immature brain structures in these patient groups compared to healthy controls. METHODS: Neonatal cerebral magnetic resonance imaging (MRI) scans were analysed by semi-automated segmentation (dHCP pipeline) in 51 children: 31 HLHS/hypoplastic left heart complex (HLHC)/UVH (61%) at stage I, in 18 neona

A Rare Case of Dichorionic Twins Concordant for Arterial Tortuosity Syndrome: Case Report and Review of the Literature.

INTRODUCTION: Arterial tortuosity syndrome (ATS) is a rare congenital disorder characterized by elongation and tortuosity of the aorta and mid-sized arteries. Additional features typical of connective tissue disorders are usually present, but the clinical presentation of the syndrome can extensively change. The cardiovascular implications are the major source of morbidity and mortality and can be present even during the neonatal period; therefore, a correct neonatal management is extremely important. However, only few cases of ATS have been suspected or diagnosed prenatally. CASE PRESENTATION

Complex Reimplantation of Aneurysmal Left Main Coronary Artery During Reoperative Aortic Root Replacement.

Coronary artery fistula (CAF), an aberrant connection between a coronary artery and a chamber of the heart or great vessel, is a rare congenital abnormality for which management has been previously described. However, there remains a paucity of literature on the treatment of sequelae, such as coronary artery aneurysm, in adults with acquired cardiac disease. We present an innovative surgical approach for the repair of an aneurysmal aortic root in a patient with a history of a coronary artery fistula repair and aortic valve replacement. The described operation integrates techniques of advanced

Self-expandable Venus P-valve in Patient with Transposition of Great Arteries and Arterial Switch Operation.

We report the successfull transcatheter pulmonary implantation of a self-expandable Venus P-valve in a patient with transposition of the great arteries, status post arterial switch operation in infancy. She had free pulmonary regurgitation, severely dilated right ventricle, and was in NYHA class II. After confirmation with the sizing balloon of the distensibility of the "choke point" at the main pulmonary anastomosis, the valve was implanted and became wedged, without compromising the final result. Follow-up confirmed the excellent result. This suggests that a self-expandable valve is an inter

A Rare Case of D-transposition of the Great Arteries (TGA) With Ventricular Septal Defect (VSD), Dysplastic Pulmonary Valve (Absent Pulmonary Valve Physiology), and Aortic Valve Stenosis in a Term Neonate.

Absent pulmonary valve (APV) syndrome is an uncommon congenital cardiac anomaly most frequently associated with tetralogy of Fallot, representing only 3% of such cases. Its occurrence with transposition of the great arteries (TGA) is exceptionally rare, and the presence of coexisting aortic valve stenosis adds significant hemodynamic complexity. We describe a term neonate who presented immediately after birth with severe cyanosis and desaturation, requiring intubation and prostaglandin E₁ infusion. Chest radiography demonstrated marked cardiomegaly, while echocardiography confirmed TGA with a

En Bloc Rotation of the Outflow Tracts - Double Root Translocation.

En bloc rotation of the outflow tracts or double root translocation offers an anatomic repair of transposition of the great arteries, ventricular septal defect, and left ventricular outflow tract obstruction and closely related forms of double outlet right ventricle. The technical principle is to excise aortic and pulmonary root en bloc, rotate them as a whole by 180°, and reimplant them. The left ventricular outflow tract is enlarged with the patch closing the ventricular septal defect. In our experience, two thirds of the pulmonary valves could be preserved. Growth of the aortic and pulmonar

NODAL variants are associated with a continuum of laterality defects from simple D-transposition of the great arteries to heterotaxy.

BACKGROUND: NODAL signaling plays a critical role in embryonic patterning and heart development in vertebrates. Genetic variants resulting in perturbations of the TGF-β/NODAL signaling pathway have reproducibly been shown to cause laterality defects in humans. To further explore this association and improve genetic diagnosis, the study aims to identify and characterize a broader range of NODAL variants in a large number of individuals with laterality defects. METHODS: We re-analyzed a cohort of 321 proband-only exomes of individuals with clinically diagnosed laterality congenital heart diseas

Human Genetics of d-Transposition of Great Arteries.

Although several genes underlying occurrence of transposition of the great arteries have been found in the mouse, human genetics of the most frequent cyanotic congenital heart defect diagnosed in neonates is still largely unknown. Development of the outflow tract is a complex process which involves the major genes of cardiac development, acting on myocardial cells from the anterior second heart field, and on mesenchymal cells from endocardial cushions. These genes, coding for transcription factors, interact with each other, and their differential expression conditions the severity of the pheno

Molecular Pathways and Animal Models of d-Transposition of the Great Arteries.

During normal cardiovascular development, the outflow tract becomes septated and rotates so that the separate aorta and pulmonary trunk are correctly aligned with the left and right ventricles, respectively. However, when this process goes wrong, the aorta and pulmonary trunk are incorrectly positioned, resulting in oxygenated blood being directly returned to the lungs, with deoxygenated blood being delivered to the systemic circulation. This is termed transposition of the great arteries (TGA). The precise etiology of TGA is not known, but the use of animal models has elucidated that genes inv

Human Genetics of Defects of Situs.

Defects of situs are associated with complex sets of congenital heart defects in which the normal concordance of asymmetric thoracic and abdominal organs is disturbed. The cellular and molecular mechanisms underlying the formation of the embryonic left-right axis have been investigated extensively in the past decade. This has led to the identification of mutations in at least 33 different genes in humans with heterotaxy and situs defects. Those mutations affect a broad range of molecular components, from transcription factors, signaling molecules, and chromatin modifiers to ciliary proteins. A

Are aortic biomechanical properties early markers of dilatation in patients with Marfan syndrome? A systematic review and meta-analysis.

Although tissue stiffness is known to play an important role in aortic dilatation, the current guidelines for offering preventative surgery in patients with Marfan syndrome rely solely on the aortic diameter. In this systematic review and meta-analysis, we analyze and compare literature on in vivo aortic stiffness measures in Marfan patients. Our aim is to assess the potential of these measurements as early indicators of aortic dilatation. Following the PRISMA guidelines, we collected literature on diameter and three in vivo stiffness measures: Pulse wave velocity (PWV), β -stiffness index (SI

High-resolution three-dimensional atlas of congenital heart defects based on micro-CT images of human postmortem wax-infiltrated heart specimens.

INTRODUCTION: Postmortem heart specimens are essential for education and research on the anatomy, morphology, and pathology of congenital heart defects. However, such specimens are rarely obtained these days, and the specimens stored in formalin are inexorably deteriorating. This study aimed to develop methods to archive three-dimensional data of rare human heart specimens and to publish the data. METHODS: All wax-infiltrated human postmortem heart specimens stored in the Cardiac Registry, Boston Children's Hospital were scanned using microfocus computed tomography (X-Tek HMXST225, Nikon Metr

[Analysis of Prenatal Ultrasound Manifestations in 15 Cases of Cantrell Syndrome].

OBJECTIVE: Cantrell syndrome, a rare congenital disorder, is characterized by a unique collection of defects on the midline abdominal wall, the lower sternum, the anterior diaphragm, and the diaphragmatic pericardium in addition to some form of intracardiac defect. So far, most of the reports on fetuses with Cantrell syndrome worldwide are either case reports or literature reviews, and few comprehensive studies on fetuses with Cantrell syndrome have been reported, especially in domestic literature. This study aims to provide a detailed analysis of 15 cases of Cantrell syndrome fetuses, focusin
