Infracardiac and mixed total anomalous pulmonary venous connection: Surgical challenges and practical solutions.

BackgroundTotal anomalous pulmonary venous connection (TAPVC) remains a complex congenital cardiac malformation. Despite improved survival, infracardiac and mixed variants continue to carry disproportionate risks of early mortality, postoperative pulmonary venous obstruction (PVO), and reintervention.MethodsThis narrative review examines the anatomic and technical challenges of infracardiac and mixed TAPVC, mechanisms and consequences of postoperative PVO, and contemporary strategies to reduce residual or recurrent obstruction, with particular emphasis on primary sutureless repair and anatomy-

Incremental Anatomical Value of CT Angiography Beyond Echocardiography in the Preoperative Assessment of Pediatric Conotruncal Anomalies: A Surgical Correlation Study.

Background: Conotruncal anomalies frequently involve extracardiac vascular structures that are critical for surgical planning yet incompletely assessed by transthoracic echocardiography (TTE). We evaluated the incremental diagnostic value of CT angiography (CTA) beyond TTE using intraoperative findings as the reference standard. Methods: In this single-center retrospective study, 239 children who underwent surgical repair of a conotruncal anomaly and had both preoperative TTE and CTA were analyzed. The incremental contribution of CTA was graded as none, minor, or major by two independent revie

Late-Detected Sinus Venosus Atrial Septal Defect and Subsequent Enteroviral Central Nervous System Infection: A Case Report.

Sinus venosus atrial septal defect (SVASD) is frequently associated with partial anomalous pulmonary venous return and may be missed on routine transthoracic echocardiography because of its superior and posterior location. We report a 68-year-old woman with atrial fibrillation who presented with dyspnea and marked right-sided chamber enlargement. Cardiac magnetic resonance imaging identified a 33 × 21-mm superior SVASD, anomalous drainage of the right upper and middle pulmonary veins, and a persistent left superior vena cava. The Qp/Qs was 3.7 by cardiac magnetic resonance and 3.2 by right hea

Single-Ventricle Physiology as a Dominant Determinant of Mortality, Resource Utilization, and Pulmonary Vein Reintervention in Total Anomalous Pulmonary Venous Connection: A Large Multicenter Study.

BackgroundChildren with single ventricle (SV) physiology and total anomalous pulmonary venous connection (TAPVC) face high surgical risk. Despite advances in single ventricle care, survival in SV/TAPVC patients has plateaued. We investigated multicenter outcomes, reintervention rates, and healthcare burden in this complex population.MethodsThe Pediatric Health Information System was queried for TAPVC repairs (2015-2024). Infants were stratified by single-ventricle versus biventricular (BiV) anatomy.ResultsAmong 1258 infants undergoing TAPVC repair, 10% (N = 120) had SV and 90% (N = 1138) had B

Inadvertent Coronary Sinus Cannulation During Central Venous Catheter Placement in a Liver Transplant Recipient with Persistent Left Superior Vena Cava: A Case Report.

Persistent left superior vena cava (PLSVC) is the most common thoracic venous anomaly and is usually asymptomatic. Central venous catheterization through a PLSVC can be challenging, and inadvertent advancement of catheters into the coronary sinus may result in arrhythmias, perforation, thrombosis, or inaccurate hemodynamic measurements. We report a case of coronary sinus cannulation by a left internal jugular dialysis catheter in a liver transplant recipient with previously unrecognized PLSVC. Initial chest radiography suggested catheter termination within a duplicated left superior vena cava;

Dual-Drainage in Mixed Total Anomalous Pulmonary Venous Connection - A Rare Variant.

Total anomalous pulmonary venous connection (TAPVC) is a rare congenital anomaly and mixed TAPVC with dual drainage is even rarer. We report the case of a 1-year-old male child presenting with progressive dyspnea, in whom computed tomography (CT) angiography revealed a complex mixed-type TAPVC with dual drainage into the coronary sinus and innominate vein. This case highlights the critical role of CT angiography in delineating complex pulmonary venous anatomy and guiding surgical planning to prevent residual shunting.

Surgical Ligation for Partial Anomalous Pulmonary Venous Connection With Dual Drainage in an Elderly Patient.

BACKGROUND: Partial anomalous pulmonary venous connection with dual drainage into both the superior vena cava (SVC) and left atrium (LA) is a rare congenital anomaly, with few reported cases. CASE SUMMARY: A 78-year-old woman presented with exertional dyspnea. Cardiac computed tomography revealed that the right pulmonary vein drained into both the SVC and LA, resulting in a significant left-to-right shunt. We ligated the anomalous connection with a median sternotomy. This approach allowed avoidance of complex reconstructions and cardioplegic arrest. DISCUSSION: Simple ligation may represent

Prenatal Diagnosis of an Isolated Persistent Left Superior Vena Cava With Absent Right Superior Vena Cava: A Case Report.

Persistent left superior vena cava (PLSVC) with absent right superior vena cava (RSVC) is a rare congenital venous anomaly resulting from the failure of the left anterior cardinal vein to regress. Although often asymptomatic in isolated cases, prenatal diagnosis has clinical benefits, including the exclusion of coexisting congenital heart disease (CHD) and awareness of possible complications during central venous access or pacemaker implantation. We report the case of a 34-year-old G2P1 woman who underwent a fetal echocardiography at 34 weeks of gestation. Examination of the four-chamber view

Transcatheter resolution of a post-surgical "Iatrogenic" circular shunt in a child.

Circular shunt is a very rare, albeit critical, condition. We present a case of a post-surgical circular shunt that resulted after correction of an anomalous total pulmonary venous connection to the coronary sinus with a persistent left superior vena cava. A left cavo-pulmonary anastomosis was performed; however, she developed heart failure during follow-up. The anastomosis was closed by interventional cardiac catheterisation without complications that led to resolution of heart failure symptoms.

Incidental Detection of Persistent Left Superior Vena Cava During Lung Perfusion Scan.

Lung perfusion scintigraphy is a cornerstone for evaluating pulmonary embolism but may reveal unexpected vascular anomalies. Persistent left superior vena cava, a congenital anomaly, can cause a right-to-left shunt when draining into the left atrium, bypassing pulmonary circulation. We report the incidental detection of persistent left superior vena cava during lung perfusion scintigraphy in a patient with post-COVID-19 hypoxia. Dual-site radiotracer injections and SPECT/CT confirmed the diagnosis, highlighting the need for adaptive imaging techniques to accurately interpret atypical findings

Prenatal diagnosis and genetic counselling of a rare de-novo 12p13.33p13.32 deletion and 15q26.2q26.3 duplication in a Chinese family.

BACKGROUND: Copy number variants are an important source of normal and pathogenic genome variations. Constitutional deletions involving the distal part of the short arm of chromosome 12(12p13.33p13.32) are very rare. These deletions are associated with an emerging condition associated with variable phenotype, including a specific speech delay sound disorder, labeled childhood apraxia of speech, intellectual disability, and neurobehavioral problems. Trisomy of distal chromosome 15q has rarely been reported. It is generally believed that the terminal duplication of 15q26.2q26.3 is related to ove

Unroofed coronary sinus and persistent left superior vena cava: a rare neonatal venous anomaly.

Approach to abnormal cardiovascular signs in the early neonatal period is challenging due to delayed fetal adaptation, closure of the patent ductus arteriosus and physiological immaturity. We present a rare case of unroofed coronary sinus (UCS) with persistent left superior vena cava (PLSVC) in a term neonate who presented with recurrent desaturation episodes and persistent oxygen requirements. Transthoracic echocardiography (TTE) revealed this rare venous anomaly, which is mainly reported in the older population and primarily as an incidental finding. TTE with an agitated saline study was the

Rebuttal: aortic coarctation and concomitant anomalies - left circumflex fistula, persistent left SVC and ventricular diverticulum.

Coarctation of the aorta (COA) is a relatively common cardiovascular congenital anomaly. It can be seen in up to 6 per 1000 live births. Rare cardiovascular anomalies have been found to be associated with COA, which may complicate management. We present a case of an adolescent male, incidentally found to be hypertensive. COA was confirmed on imaging, however, with multiple anomalies with an incidence in the general population as common as 0.4 per cent to as rare as 7 cases ever reported in the literature. The association of COA with left circumflex to coronary sinus fistula, ventricular divert
