A neonate who underwent corrective surgery for cardiac-type total anomalous pulmonary venous connection (TAPVC) was suspected of having a pseudoaneurysm of the left ventricular (LV) free wall on transthoracic echocardiography (TTE) on postoperative day 11. Emergency surgery was performed the following day, revealing LV rupture due to a congenital partial defect of the LV free wall. The defect was successfully repaired using double-patch closure reinforced with BioGlue. The postoperative course was uneventful. This case highlights that left ventricular rupture may occur due to an unrecognized c

BACKGROUND: Raghib syndrome is a congenital anomaly that is caused by a persistent left superior vena cava draining into the left atrium, unroofed coronary sinus, and atrial septal defect. It can lead to cyanosis, cryptogenic strokes, pulmonary hypertension, and arrythmias.
CASE SUMMARY: A 60-year-old woman presented with palpitations due to atrial flutter. Cardiac imaging, including computed tomography, transesophageal echocardiography, and magnetic resonance imaging, confirmed bilateral superior vena cavae, with the left superior vena cava draining into the left atrium and minimal right-to-

To develop a predictive model for optimal anastomosis sizing in TAPVC surgery, focusing on the role of pulmonary venous confluence (PVC) size. A patient-specific fluid-structure interaction (FSI) model simulated blood flow through various anastomosis sizes. Key variables included body weight, anastomosis length, and PVC size. The model's predictions were validated against postoperative echocardiographic measurements from nine TAPVC cases. A strong positive correlation was found between flow velocity and the ratio of body weight to anastomosis length and PVC circumference. Including PVC size si

The coronary sinus and its tributaries constitute the anatomical foundation for successful cardiac resynchronization therapy. Despite the emergence of conduction system pacing as a physiological alternative, conventional transvenous CRT retains its important role-not all clinical scenarios can be addressed by left bundle branch area or His bundle pacing alone. Understanding coronary venous anatomy therefore remains essential for every implanting physician. This review provides a clinically oriented analysis of coronary venous anatomy as it pertains to left ventricular lead implantation. We exa

BACKGROUND: Late presentation worsens the morbidity of adult congenital heart disease. Concurrent defects and associated anomalous coronary artery from the opposite sinus (ACAOS) complicates management.
CASE SUMMARY: A 62-year-old man was referred by his urologist because of a murmur. Multimodality imaging unraveled concurrent defects: 1) interarterial coursing right coronary artery originating from the left sinus; 2) anomalous left circumflex artery from the first diagonal branch of the left anterior descending artery; 3) sinus venosus atrial septal defect (SVASD); 4) anomalous right upper p

BACKGROUND: Turner syndrome (TS) is associated with cardiovascular abnormalities and metabolic risk, but karyotype-specific phenotypes remain incompletely defined, particularly in pediatric and adolescent cohorts assessed by transthoracic echocardiography (TTE).
METHODS: We conducted a retrospective cross-sectional study of 289 patients with TS evaluated between October 2016 and October 2023. Patients were grouped as 45,X monosomy (n = 149) or mosaic/structural karyotypes (n = 140). Baseline or preoperative TTE data were used. The primary endpoint was a clinically relevant structural cardiova

BACKGROUND: Congenital heart diseases (CHD) require an in-depth anatomopathological understanding. 3D printing is a promising educational tool for creating physical prototypes from imaging data, but costs are a major issue.
OBJECTIVE: This study aimed to describe a feasible low-cost workflow for the development of 3D-printed (3DP) CHD models and create a shareable educational set.
METHODS: Data from cardiac tomography images were used, following the steps of image acquisition, segmentation, digital design, slicing, 3D printing, and post-printing. Valvar structures were created from echocardi

A persistent left superior vena cava (PLSVC) can contribute to atrial fibrillation (AF) and it often requires complex ablation because of multiple electrical connections with the left atrium. An 86-year-old woman underwent pulsed field ablation (PFA) for pulmonary vein and PLSVC isolation. After successful isolation without causing either phrenic nerve injury or coronary spasm, postprocedural remapping revealed a newly formed low-voltage area with conduction delay in the adjacent mitral isthmus. No atrial tachyarrhythmia recurrence was observed during the one-year follow-up. This case suggests

Mixed-type total anomalous pulmonary venous connection (TAPVC) is the least common variant in Darling's classification and is further subdivided into three distinct subtypes. We report an exceptionally rare mixed TAPVC with a 2 + 2 drainage pattern, associated with an unusual venous anomaly in which the hemiazygos vein drains into the innominate vein, forming a characteristic "garland" configuration.

The subxiphoid approach for video-assisted thoracoscopic surgery (VATS) thymectomy offers a midline view but is constrained by the limited retrosternal space. This limitation is accentuated in cases with complex vascular anatomy, such as a persistent left superior vena cava (PLSVC). We describe and evaluate a novel balance-shaped sternal elevation technique designed to overcome this spatial challenge. A 52-year-old male with a 6.3 cm anterior mediastinal mass and a concomitant PLSVC underwent uniportal subxiphoid VATS thymectomy. A percutaneous suspension system utilizing a balance-shaped retr

A term neonate presented on day 1 with severe cyanosis and metabolic acidosis. Echocardiography and computed tomography angiography revealed a cardiac-type total anomalous pulmonary venous connection to the coronary sinus with anomalous inferior vena cava drainage to the left atrium. Despite intensive management, the infant died. This exceptionally rare dual venous anomaly illustrates abnormal incorporation of a venous pole during cardiac embryogenesis.

The most common venous abnormality of the thorax is persistent left superior vena cava (LSVC), incidence being less than 0.5%. Of all the congenital heart anomalies, the sinus venosus type of atrial septal defect (SVASD) is most commonly associated with LSVC and accounts for 4-10% of all ASD. We report a 52-year-old male patient with a SVASD with LSVC and PAPVC, who underwent minimally invasive cardiac surgical (MICS) repair of the same, assisted by unique bilateral percutaneous SVC cannulation and had an uneventful course and recovery. Presence of LSVC usually requires sternotomy and its drai

This report details a rare cause of early postoperative pulmonary venous obstruction following supracardiac total anomalous pulmonary venous connection repair in a 4-week-old infant. Despite a successful sutureless anastomosis, intraoperative transesophageal echocardiography revealed significant flow acceleration (1.69 m/s and a mean gradient of 5 mm Hg). Surgical inspection identified that the left atrial appendage (LAA) was sagging over the common chamber-to-left atrial anastomosis, causing an obstruction. The mechanical obstruction was relieved by suturing the LAA to the undersurface of the

Total anomalous pulmonary venous connection (TAPVC), a congenital cardiac disorder, usually manifests in early infancy. Its presentation may be catastrophic if the drainage site into the systemic venous circulation is obstructed. Conversely, in the absence of such obstruction, its presentation may be delayed. Very few sporadic case reports or small series describing its presentation in adult patients are available. In this report, we present an adult patient with this diagnosis, in whom all four pulmonary veins had joined to form a common pulmonary venous confluence (CPVC) that drained into a

BACKGROUND: Congenital anomalies of the coronary sinus (CS) are rare and often overlooked on standard imaging. Among these, the unroofed coronary sinus (URCS) is the most recognized, typically involving the proximal CS and frequently associated with a persistent left superior vena cava. In contrast, fistulous communications between the distal CS and the left atrium (LA) are exceptionally uncommon. Their recognition is important during atrial fibrillation (AF) ablation, where unanticipated channels may complicate catheter manipulation and lesion delivery.
CASE SUMMARY: A 76-year-old woman with
