Circumferential venous sinus: an innovative separation method for total vertical type II craniopagus twins.

Craniopagus twins, particularly the total vertical type II variant, represent one of the rarest and most complex congenital malformations, with significant surgical challenges. This technical note presents the successful staged separation of craniopagus twins at 3 years 9 months of age, the oldest known to be separated to date. The twins shared a prominent circumferential venous sinus, requiring a novel surgical strategy. Advanced imaging, 3D modeling, and virtual reality planning in the metaverse-between Brazil and the United Kingdom-guided the 7 staged surgeries. Initially, partial separatio

Closed-Chest Robotic Repair of Partial Anomalous Pulmonary Venous Connection and Sinus Venosus Atrial Septal Defect Using the Warden Technique.

An 11-year-old patient with partial anomalous venous connection, atrial septal defect (ASD), and persistent left superior vena cava underwent a closed-chest, totally thoracoscopic robotic repair using the Warden technique. The patient was extubated in the operating room and stayed in the intensive care unit for 24 h. He was discharged home on day 5 without complications. At 1-year follow-up, the right ventricular size had improved, and tricuspid regurgitation was reduced. He remains asymptomatic and tolerates physical activity adequately. Minimally invasive robotic surgery for pediatric and co

Proposed Standardized Nomenclature for Anomalous Aortic Origin of a Coronary Artery: An International Multidisciplinary Initiative.

Increasing use of advanced noninvasive imaging has provided great anatomic and morphologic details to characterize congenital coronary artery anomalies. Anomalous aortic origin of a coronary artery (AAOCA) is a subtype that may lead to myocardial ischemia with variable clinical manifestations across the lifespan. Management decision making in AAOCA is hindered by anatomic variants of differing physiologic consequences. Several systems of nomenclature have been used that lack uniformity in potentially high-risk morphologic features. The ICAAC (International Coronary Artery Anomalies Collaborati

Oral food challenge unmasks porto-systemic shunting: a case report of a novel provocation test for congenital porto-systemic shunts.

BACKGROUND: Congenital porto-systemic shunts (CPSS) are rare vascular malformations that may present with borderline haemodynamic significance under resting conditions, complicating decisions regarding intervention. Static, fasting-state assessments may underestimate the true physiological impact of these shunts. CASE SUMMARY: A 9-year-old male with repaired total anomalous pulmonary venous connection (TAPVC) presented with progressive breathlessness. Cardiac computed tomography (CT) identified a residual anomalous venous connection between the left atrium and portal vein. Baseline phase-cont

Masked ABCA3-Related Interstitial Lung Disease in a Neonate With Prenatally Diagnosed Total Anomalous Pulmonary Venous Connection.

Obstructive total anomalous pulmonary venous connection (TAPVC) and ABCA3-related interstitial lung disease (ILD) rarely coexist. We report a neonate with TAPVC complicated by compound heterozygous ABCA3 variants (c.4180dup/p.V1394fs maternal, c.3446A>G/p.D1149G paternal). Overlapping neonatal respiratory symptoms mask the underlying surfactant disorder, easily causing misdiagnosis and delayed targeted management. This case highlights that the genetic screening for ABCA3 deficiency is mandatory for infants with persistent severe hypoxemia and ventilator dependence after complete TAPVC surgical

Closure of Sinus Venosus Atrial Septal Defects with Transcatheter Balloon-Expandable Stents: A Single-Center Case Series.

BACKGROUND: Sinus venosus atrial septal defect (SVASD) is a congenital heart defect involving an opening between the superior vena cava (SVC) and the right upper pulmonary vein (RUPV). Surgical closure is the standard treatment. Nonetheless, it is more complex than secundum ASD reconstruction and is associated with higher complication rates. Transcatheter closure is an emerging alternative. METHODS: We report 5 cases of transcatheter SVASD closure and 1 case of failure requiring surgical intervention. All patients underwent preprocedural computed tomography angiography to assess defect size a

Restrictive Superior Vena Cava to Left Atrium Communication With a Large Left-to-Right Shunt: A Previously Undescribed Sinus Venosus Defect Variant.

A 40-year-old woman was referred for cardiac assessment following a syncopal event. Echocardiography showed a dilated right ventricle and subsequent cross-sectional imaging suggested partial anomalous pulmonary venous connection (PAPVC) and a sinus venosus defect (SVD). She reported exertional dyspnoea, fatigue, paroxysmal nocturnal dyspnoea, longstanding palpitations, and documented runs of atrial tachycardia. Transoesophageal echocardiography demonstrated a dilated right ventricle and pulmonary arteries, mild tricuspid regurgitation, anomalous connection of the right upper and right middle p

Case Report: Post-Total anomalous pulmonary venous connection pulmonary hypertension - novel treatment using sirolimus and atrial flow regulator implantation.

OBJECTIVE: In patients who have undergone surgical repair of TAPVC and who develop treatment-resistant pulmonary vein stenosis and postcapillary pulmonary hypertension, pre-capillary pulmonary hypertension may also develop as a consequence of arterial remodelling. We present a 3-year-old boy with surgically corrected TAPVC in whom AFR 8   ×   5 was implanted using a 9F delivery system due to end-stage precapillary PH. CASE REPORT: In a male term newborn, obstructive supracardiac TAPVR was diagnosed, and he underwent surgical repair on the third day of life. At three months of age, he was hosp

Conservative management of iatrogenic pulmonary vein obstruction following transcatheter sinus venosus ASD repair: a case insight and literature review.

BACKGROUND: Iatrogenic pulmonary vein obstruction (PVO) is a rare but serious complication that may occur following transcatheter correction of sinus venosus atrial septal defect (SVASD) with partial anomalous pulmonary venous connection (PAPVC) using covered stents. This procedure has gained increasing acceptance in recent years. The optimal management strategy, particularly the role of conservative therapy, remains poorly defined. A 42-year-old man with a 1.5 × 1.0 cm superior vena cava (SVC)-type SVASD, PAPVC, and severe pulmonary arterial hypertension (PAH) underwent transcatheter repair a

Enhanced Recovery After Surgery (ERAS) in Neonatal Cardiac Care: A Case Report of Nursing-Led Management for Obstructed Mixed-Type Total Anomalous Pulmonary Venous Connection.

BACKGROUND: Obstructed mixed-type total anomalous pulmonary venous connection (TAPVC) is a rare life-threatening congenital heart defect requiring urgent, high-risk surgery. Postoperative care is complex and fraught with complications. Enhanced recovery after surgery (ERAS) is a multimodal, evidence-based approach to optimize perioperative care, yet its application in neonatal cardiac surgery is nascent. CLINICAL FINDINGS: A 12-day-old male infant presented with lethargy, poor feeding, and severe respiratory distress (SpO 2 80%-85%), requiring immediate intubation. Examination revealed dimini

Cardiovascular Defects as the Initial Presentation in Two Prenatal Cases of De Novo Heterozygous PBX1 Variants.

PBX1-associated congenital anomalies of the kidney and urinary tract syndrome with or without hearing loss, abnormal ears, or developmental delay (CAKUTHED) is a highly pleiotropic, autosomal dominant developmental disorder. The disease spectrum is broad, and only a limited number of prenatal cases have been reported to date. We report two well-documented prenatal cases of CAKUTHED presenting with cardiovascular defects as the initial manifestation in the second trimester. Trio exome sequencing identified two de novo variants in PBX1 (c.192-3C>A and c.869G>A), confirming the genetic diagnosis

Echocardiography-Assisted Stenting of the Ductus Venosus in Infracardiac TAPVC: Report of a New Technique and Review of the Literature.

Neonates with infracardiac total anomalous pulmonary venous connection (TAPVC) frequently require urgent treatment. If surgical repair is contraindicated due to extreme prematurity, interventional stenting of the ductus venosus (DV) has been introduced successfully to postpone surgery. We report a new technique of transjugular stenting in an 890-g premature infant who presented with infracardiac TAPVC and functional closure of the DV. The DV was successfully crossed with a wire and stented under echocardiographic control. The case underlines the importance of echocardiography in diagnosing and

Left Bundle Branch Area Pacing as a Rescue Strategy After Failed Cardiac Resynchronization Therapy in a Patient With Persistent Left Superior Vena Cava, Absent Coronary Sinus Branches, After Bilateral Maze Procedure: A Case Report.

BACKGROUND Cardiac resynchronization therapy can fail in the presence of anatomical variations, such as persistent left superior vena cava, which can preclude coronary sinus cannulation and left ventricular lead placement. Conduction system pacing, particularly left bundle branch area pacing, has emerged as a physiological alternative for achieving ventricular resynchronization in such anatomically challenging situations and may provide an effective solution when standard cardiac resynchronization therapy (CRT) is not feasible. CASE REPORT We report the case of a 78-year-old man with chronic h

Single Persistent Left Superior Vena Cava Draining Into the Left Atrium Causing Neonatal Cyanosis: A Case Report.

BACKGROUND: The persistence of the left superior vena cava (PLSVC) is the most common variant of systemic venous drainage. It is usually diagnosed through echocardiography, with the PLSVC typically draining into the right atrium via the coronary sinus without hemodynamic consequences. In cases of a single PLSVC (without right superior vena cava), it generally drains into the right atrium. However, when it drains directly into the left atrium, systemic venous return bypasses the right atrium, causing cyanosis and hypoxemia. CLINICAL FINDINGS: Term newborn was admitted to the neonatal intensive

Surgical repair of extremely late presentation of supracardiac total anomalous pulmonary vein connection: a case series.

BACKGROUND: A late-presenting total anomalous pulmonary venous connection is extremely rare, and surgical repair remains a significant challenge. CASES PRESENTATION: We present two cases of surgical repair for late-presenting supracardiac total anomalous pulmonary vein connection: a 14-year-old female and a 32-year-old female. Both were successfully operated on and were discharged on postoperative days 9 and 29. The second patient had a prolonged hospital stay due to complications of acute kidney injury after surgery and the need for hemodialysis. Both patients had a small left ventricle and

Echocardiography-Guided Repositioning of a Hemodialysis Catheter in a Patient With Persistent Left Superior Vena Cava: A Case Report.

Persistent left superior vena cava (PLSVC) is an uncommon thoracic venous anomaly that is often identified incidentally during central venous access or thoracic imaging. We describe a case in which transthoracic echocardiography (TTE) was used to safely and effectively reposition a hemodialysis catheter within a PLSVC to enable continuous hemodiafiltration (CHDF). A 71-year-old man developed septic shock complicated by acute kidney injury requiring CHDF. Because vascular access options were limited, a dialysis catheter was inserted via the left internal jugular vein. Imaging revealed that the

Discovery of Hyperelastic Constitutive Laws from Experimental Data with EUCLID.

Aseptic nonunion is prevalent in orthopedic practice, causing persistent pain and functional impairment. Humeral shaft fractures, accounting for 3-5% of all fractures, have nonunion rates of 2-33% in nonoperative and 5-10% in surgical management. This study, the largest case series on operative management of humeral shaft nonunion, examines surgical techniques and outcomes. This retrospective study, conducted at a referral center in eastern India, included 132 patients with aseptic humeral shaft nonunion from 2002 to 2020. Cases were from May 2002 to April 2012 and May 2015 to December 2020. P
