BACKGROUND: Patients with Fontan physiology have increased pulmonary perfusion heterogeneity compared with patients with normal hearts.
CASE SUMMARY: A 9-year-old girl with hypoplastic left heart syndrome, Scimitar syndrome, and unilateral right pulmonary vein stenosis underwent stent placement to relieve compression of the reimplanted right pulmonary vein after Fontan completion. Serial upper extremity lung perfusion scans demonstrated decreased right lung pulmonary blood flow despite cardiac catheterization demonstrating no in-stent stenosis and normal Fontan pressures. Pulmonary blood flow

BACKGROUND: Antithrombin resistance (ATR) is a congenital thrombotic predisposition caused by abnormal prothrombin, FII (Factor II). Abnormal FII with ATR characteristics, when converted to thrombin, FIIa (Factor IIa), shows a reduced affinity of binding to AT (antithrombin), resulting in reduced anti-FIIa and a thrombotic tendency. Data on the efficacy of anticoagulants in ATR are currently insufficient.
METHODS: Mouse models of ATR were generated to investigate the efficacy of anticoagulants. Because FII R596L, which causes ATR in humans, was considered to correspond to R593L in mice based

Postoperative chylothorax remains a significant complication after congenital heart surgery; however, little is known about temporal changes in pleural fluid characteristics during prolonged disease. We retrospectively reviewed 82 children with postoperative chylothorax treated between 2007 and 2023 to evaluate the outcomes of a stepwise treatment strategy, identify factors associated with refractory disease, and investigate serial pleural fluid findings. Patients were managed with dietary modification, total parenteral nutrition, octreotide, and additional intervention when necessary. Conserv

PBX1-associated congenital anomalies of the kidney and urinary tract syndrome with or without hearing loss, abnormal ears, or developmental delay (CAKUTHED) is a highly pleiotropic, autosomal dominant developmental disorder. The disease spectrum is broad, and only a limited number of prenatal cases have been reported to date. We report two well-documented prenatal cases of CAKUTHED presenting with cardiovascular defects as the initial manifestation in the second trimester. Trio exome sequencing identified two de novo variants in PBX1 (c.192-3C>A and c.869G>A), confirming the genetic diagnosis

Inferior vena cava (IVC) agenesis is a rare congenital vascular anomaly that may predispose young individuals to venous thromboembolism in the absence of classical risk factors. We report the case of a 35-year-old man, an active smoker with no significant medical history, who presented with a 10-day history of bilateral lower limb swelling and severe back pain radiating to the lower limbs. Doppler ultrasonography demonstrated extensive bilateral deep vein thrombosis (DVT), involving the right external iliac, common femoral, and femoral veins, with complete thrombosis of the left lower limb ven

BACKGROUND: Accessory tricuspid valve (TV) is a rare congenital anomaly usually associated with other cardiac anomalies. It is typically classified as mobile or fixed type; both are previously described as arising exclusively from the right ventricular septum. We report the first known case of a straddling accessory TV anchoring to the left ventricular outflow tract (LVOT) wall as well, causing severe LVOT obstruction in a neonate with coarctation complex.
CASE SUMMARY: A female neonate was diagnosed with coarctation of the aorta, ventricular septal defect (VSD), persistent left superior vena

Unroofed coronary sinus (CS) with CS orifice atresia and persistent left superior vena cava (PLSVC) is an exceptionally rare congenital anomaly that may coexist with acquired valvular disease, as illustrated in this case of severe rheumatic mitral stenosis. Multimodality imaging including echocardiography and cardiac CT was essential for precise anatomical delineation, guiding safe surgical intervention. Recognition of this complex anatomy is critical to avoid catastrophic complications such as inadvertent PLSVC ligation without ensuring adequate CS drainage. This case highlights the importanc

Coronary arteriovenous fistulas (CAFs) are abnormal communications between coronary arteries and cardiac chambers or great vessels.1 They are usually congenital but may be acquired after trauma, surgery, or percutaneous procedures. Although often incidental in childhood, hemodynamically significant CAFs in adults can cause myocardial ischemia, arrhythmia, heart failure, or endarteritis.2 Evidence to guide management is limited, and multidisciplinary evaluation at centers experienced in both percutaneous and surgical approaches is recommended.3 A 60-year-old man with hypertension and prior left

BACKGROUND: Coexisting coronary artery disease (CAD) and congenital heart disease (CHD) is rare in adults and poses complex management challenges requiring multidisciplinary coordination.
CASE SUMMARY: A 79-year-old man with a history of CAD and stenting was referred for evaluation given worsening dyspnea. Coronary angiography revealed severe ostial left circumflex stenosis and moderate left main disease. In addition, computed tomography demonstrated a previously undiagnosed sinus venosus atrial septal defect with partial anomalous pulmonary venous return. Multidisciplinary evaluation at a pa

BACKGROUND AND AIMS: Surgical correction of sinus venosus atrial septal defects (SVASD), often associated with partial anomalous pulmonary venous return (PAPVR), is characterized by low mortality but carries a risk of postoperative rhythm disturbances due to the proximity of the sinus node. This study aimed to compare the clinical outcomes and incidence of postoperative arrhythmias between two surgical techniques: the conventional patch repair (baffle) and the rotational "Z" flap technique.
METHODS: A retrospective, single-center non-inferiority analysis was conducted on 110 patients (adult a

This case report describes a rare clinical scenario involving a 40-year-old Ethiopian female patient with very severe rheumatic mitral stenosis, found to have an anomalous transposition of the inferior vena cava (IVC). She successfully underwent a technically challenging percutaneous mitral balloon commissurotomy. To the best of our knowledge, this is the first documented case in an African patient where such a procedure has been successfully performed in the context of a congenital IVC anomaly.

The heart functions as a coordinated four-chambered pump, with four valves ensuring unidirectional blood flow. The tricuspid valve, located between the right atrium and right ventricle, consists of anterior, posterior, and septal leaflets that coapt during systole to prevent retrograde flow. Valvular disorders may result in stenosis or regurgitation, thereby increasing cardiac workload and predisposing to chamber dilation and heart failure. Congenital abnormalities, including partial or complete leaflet absence, are rare but can significantly disrupt normal hemodynamics. Reports of absent tric

INTRODUCTION: Knee osteoarthritis (OA) is a degenerative joint disease characterized by pain, stiffness, and functional impairment. As many patients seek to avoid surgery, a synthesis of conservative treatment options is clinically relevant for evidence-based management.
METHODS: A structured PubMed literature search was conducted through December 15, 2025. Eligible studies included randomized controlled trials, cohort and case-control studies, systematic reviews, and meta-analyses evaluating nonsurgical interventions for knee OA. Extracted outcomes included pain, function, safety, and compar

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

OBJECTIVES: This report discusses the challenges and strategies involved in managing venous stenosis secondary to pacing or defibrillator leads, focusing on endovascular treatment options and procedural considerations.
KEY PROCEDURAL STEPS: Crucial steps of the procedure include thorough preprocedural planning, obtaining multisite venous access for optimal angiographic visualization, wiring through the stent using a percutaneous coronary intervention wire supported by a microcatheter and guiding catheter, confirming true lumen and intraluminal wire passage, and progressive ballooning using hi

BACKGROUND: Renal allograft dysfunction warrants prompt investigation as the differential diagnosis is often broad. Here, we present a pediatric kidney transplant recipient who experienced suboptimal allograft function due to previously undiagnosed inferior vena cava stenosis with resultant chronic congestive nephropathy.
METHODS: We retrospectively reviewed a pediatric kidney transplant case at our own institution, and a literature review of congestive nephropathy was performed.
RESULTS: The patient, a 13-year-old male with end-stage renal disease secondary to congenital renal dysplasia, un

Superior vena cava (SVC) type of sinus venosus defect (SVD) associated with anomalous right upper pulmonary vein (RUPV) drainage is often corrected by open-heart surgery. Transcatheter-covered stent exclusion is an attractive emerging alternative using a covered stent in the lower end of SVC to close the SVD and redirect the RUPV to the left atrium. The location of SVD posterosuperior to oval fossa challenges its transthoracic echocardiographic identification and delays its diagnosis to adult age. Lifestyle adult diseases, including atherosclerosis, elevate the left ventricular end-diastolic p
