Foetal Persistent Left Superior Vena Cava-Not Always a Benign Finding?

OBJECTIVE: To determine the proportion of patients with a persistent left superior vena cava (PLSVC) and structurally normal heart who have associated extra cardiac anomalies and/or genetic variations and to review their clinical outcomes. METHODS: A retrospective cohort study of foetuses with a prenatal diagnosis of PLSVC and structurally normal heart seen at Saint Mary's Hospital Foetal Medicine Unit Manchester, UK between January 2017 and December 2024. Outcome of the pregnancy and for the infants up to one year follow-up data was collected from Electronic Patient Records (EPR). RESULTS:

Implantable venous access port placement in the upper arm of breast cancer patients with persistent left superior vena cava: a case series and literature review.

Persistent left superior vena cava (PLSVC) is a rare congenital venous anomaly. Although implantable venous access port placement in PLSVC has been reported, the optimal technique, catheter positioning, and safety considerations remain uncertain. This study describes our experience in managing three breast cancer patients with PLSVC and proposes a safe and efficient approach for port placement. All three cases of PLSVC were identified among breast cancer patients undergoing implantable venous access port placement guided by intracavitary electrocardiogram (IC-ECG). A negative P wave appeared o

Radiofrequency ablation of persistent left superior vena cava with left double accessory pathways: a case report and systematic review.

PURPOSE: Ablation of the persistent left superior vena cava (PLSVC) with a left accessory pathway is more difficult than ablation in the case of an ordinary left accessory pathway. We report a case of successful ablation of the PLSVC with a left double accessory pathway. We also conducted a systematic review of the literature to summarize the procedural precautions and technical points for patients with PLSVC with a left accessory pathway. OBSERVATION: A 48-year-old man was admitted with recurrent episodes of tachycardia. Electrocardiography revealed preexcitation. Preoperative echocardiograp

Persistent left superior vena cava is associated with complex atrial tachyarrhythmias in repaired tetralogy of Fallot: evidence for a right-sided arrhythmogenic substrate.

BACKGROUND: Atrial fibrillation (AF) is an important late complication in patients with repaired tetralogy of Fallot (TOF); however, its clinical determinants and arrhythmogenic substrates remain uncertain. Identifying high-risk anatomical profiles is essential for optimal rhythm management and tailored ablation strategies. METHODS: We retrospectively analyzed 137 consecutive patients with repaired TOF followed at the University of Tsukuba Hospital (2013-2024). Clinical characteristics, echocardiographic parameters, and procedural findings were compared between patients with and without AF. L

Persistent Left Superior Vena Cava in an Adult Presenting With Atrial Fibrillation.

Persistent left-sided superior vena cava (PLSVC) is usually an asymptomatic, rare congenital anomaly, but has been associated with atrial arrhythmias due to remnant pacemaker tissue within the vessel. This report describes a 62-year-old male who presented with new-onset atrial fibrillation (AF) and was incidentally found to have a PLSVC draining into a dilated coronary sinus, along with additional thoracic and vascular abnormalities, including a bovine aortic arch and tracheal diverticulum. Transthoracic echocardiography with agitated saline injection and computed tomography confirmed the diag

Anomalous Coronary Venous Drainage to the Right Atrium: A Case Report.

Anomalies of the coronary venous system are rare and may have important implications for cardiac imaging and invasive procedures. We report a case of anomalous coronary venous drainage identified on chest computed tomography angiography (CTA) in a 53-year-old man undergoing routine follow-up after surgical repair of a type A aortic dissection. Imaging demonstrated a dilated coronary sinus, secondary to distal narrowing, with dilation of the great cardiac vein suggestive of compensatory redistribution of venous return. Additionally, the left marginal vein was observed to drain directly into the

Pulsed Field Ablation of Persistent Left Superior Vena Cava in an Athlete With Atrial Fibrillation.

BACKGROUND: Masters endurance athletes have an elevated risk of atrial fibrillation (AF) due to atrial remodeling, autonomic shifts, and long-term training loads. Management is challenging when persistent AF limits performance despite guideline-directed care. CASE SUMMARY: A 60-year-old lifelong swimmer and cyclist with persistent AF experienced a 20% to 25% decline in exercise performance. AF recurred shortly after pulmonary vein isolation, and antiarrhythmic drugs were ineffective or poorly tolerated. A known persistent left superior vena cava (PLSVC) suggested a non-pulmonary vein trigger.

Raghib Syndrome Physiology Revealed by Multimodality Cardiac Imaging.

BACKGROUND: Raghib syndrome is a congenital anomaly that is caused by a persistent left superior vena cava draining into the left atrium, unroofed coronary sinus, and atrial septal defect. It can lead to cyanosis, cryptogenic strokes, pulmonary hypertension, and arrythmias. CASE SUMMARY: A 60-year-old woman presented with palpitations due to atrial flutter. Cardiac imaging, including computed tomography, transesophageal echocardiography, and magnetic resonance imaging, confirmed bilateral superior vena cavae, with the left superior vena cava draining into the left atrium and minimal right-to-

Coronary sinus and cardiac venous anatomy for cardiac resynchronization therapy: A Clinician's view.

The coronary sinus and its tributaries constitute the anatomical foundation for successful cardiac resynchronization therapy. Despite the emergence of conduction system pacing as a physiological alternative, conventional transvenous CRT retains its important role-not all clinical scenarios can be addressed by left bundle branch area or His bundle pacing alone. Understanding coronary venous anatomy therefore remains essential for every implanting physician. This review provides a clinically oriented analysis of coronary venous anatomy as it pertains to left ventricular lead implantation. We exa

Covered Stent Correction of Sinus Venosus ASD With Anomalous Left Sinus Interarterial Right Coronary Artery.

BACKGROUND: Late presentation worsens the morbidity of adult congenital heart disease. Concurrent defects and associated anomalous coronary artery from the opposite sinus (ACAOS) complicates management. CASE SUMMARY: A 62-year-old man was referred by his urologist because of a murmur. Multimodality imaging unraveled concurrent defects: 1) interarterial coursing right coronary artery originating from the left sinus; 2) anomalous left circumflex artery from the first diagonal branch of the left anterior descending artery; 3) sinus venosus atrial septal defect (SVASD); 4) anomalous right upper p

Karyotype-specific cardiovascular and metabolic profiles in Turner syndrome: a retrospective echocardiographic study.

BACKGROUND: Turner syndrome (TS) is associated with cardiovascular abnormalities and metabolic risk, but karyotype-specific phenotypes remain incompletely defined, particularly in pediatric and adolescent cohorts assessed by transthoracic echocardiography (TTE). METHODS: We conducted a retrospective cross-sectional study of 289 patients with TS evaluated between October 2016 and October 2023. Patients were grouped as 45,X monosomy (n = 149) or mosaic/structural karyotypes (n = 140). Baseline or preoperative TTE data were used. The primary endpoint was a clinically relevant structural cardiova

Mitral Isthmus Conduction Delay After Pulsed Field Ablation of a Persistent Left Superior Vena Cava.

A persistent left superior vena cava (PLSVC) can contribute to atrial fibrillation (AF) and it often requires complex ablation because of multiple electrical connections with the left atrium. An 86-year-old woman underwent pulsed field ablation (PFA) for pulmonary vein and PLSVC isolation. After successful isolation without causing either phrenic nerve injury or coronary spasm, postprocedural remapping revealed a newly formed low-voltage area with conduction delay in the adjacent mitral isthmus. No atrial tachyarrhythmia recurrence was observed during the one-year follow-up. This case suggests

Balance-shaped sternal elevation-assisted uniportal subxiphoid video-assisted thoracoscopic surgery for thymoma with persistent left superior vena cava.

The subxiphoid approach for video-assisted thoracoscopic surgery (VATS) thymectomy offers a midline view but is constrained by the limited retrosternal space. This limitation is accentuated in cases with complex vascular anatomy, such as a persistent left superior vena cava (PLSVC). We describe and evaluate a novel balance-shaped sternal elevation technique designed to overcome this spatial challenge. A 52-year-old male with a 6.3 cm anterior mediastinal mass and a concomitant PLSVC underwent uniportal subxiphoid VATS thymectomy. A percutaneous suspension system utilizing a balance-shaped retr

Anomalous connection of inferior vena cava to left atrium together with cardiac total anomalous pulmonary venous connection and persistent left superior vena cava.

A term neonate presented on day 1 with severe cyanosis and metabolic acidosis. Echocardiography and computed tomography angiography revealed a cardiac-type total anomalous pulmonary venous connection to the coronary sinus with anomalous inferior vena cava drainage to the left atrium. Despite intensive management, the infant died. This exceptionally rare dual venous anomaly illustrates abnormal incorporation of a venous pole during cardiac embryogenesis.

Anaesthetic Management of a Case of Minimally Invasive Surgical Repair of Sinus Venosus Atrial Septal Defect (SVASD) and Persistent Left Superior Vena Cava (LSVC), Assisted by Unique Bilateral Percutaneous Superior Vena Cava Cannulation.

The most common venous abnormality of the thorax is persistent left superior vena cava (LSVC), incidence being less than 0.5%. Of all the congenital heart anomalies, the sinus venosus type of atrial septal defect (SVASD) is most commonly associated with LSVC and accounts for 4-10% of all ASD. We report a 52-year-old male patient with a SVASD with LSVC and PAPVC, who underwent minimally invasive cardiac surgical (MICS) repair of the same, assisted by unique bilateral percutaneous SVC cannulation and had an uneventful course and recovery. Presence of LSVC usually requires sternotomy and its drai
