Pulmonary vein stenosis after repair of total anomalous pulmonary venous connection (TAPVC) is a life-threatening complication that frequently recurs, particularly in patients with asplenia. Although intrinsic mechanisms such as ostial narrowing and progressive neointimal proliferation are well recognized, extrinsic mechanical factors are less commonly emphasized in congenital heart disease. We report a case of recurrent pulmonary vein stenosis in a patient with asplenia in whom intrinsic susceptibility and possible posterior extrinsic compression may have acted synergistically. UNLABELLED: Lamotrigine (Lamictal), a phenyltriazine derivative, is a well established anticonvulsant agent that has shown efficacy in the prevention of mood episodes in adult patients with bipolar I disorder. The mechanism of action of the drug in patients with bipolar disorder may be related to the inhibition of sodium and calcium channels in presynaptic neurons and subsequent stabilisation of the neuronal membrane. Lamotrigine monotherapy significantly delayed time to intervention with additional pharmacotherapy or electroconvulsive therapy for any new mood episode (mania, hypomania, depres

Coexisting transposition of the great arteries (d-TGA) and total anomalous pulmonary venous connection (TAPVC) is a rare combination, with the infracardiac variety being more rare. So far, only a few cases are described in the literature with combined infracardiac TAPVC and d-TGA. We present a 4-month-old girl having TGA and obstructed infracardiac TAPVC with early left ventricle mass regression. She underwent successful complete surgical anatomical correction of this rare entity without postoperative mechanical circulatory support uneventfully. Simultaneous complete anatomical correction of T

BackgroundSurvival beyond one month of age is rare in children born with obstructed infracardiac total anomalous pulmonary venous connection (TAPVC). There are limited data available on surgical outcomes of the same subset. We conducted this retrospective study to identify risk factors associated with surgical outcomes in this patient population.MethodWe present a retrospective study of patients presenting at our institute with obstructed infracardiac TAPVC surviving beyond one month of age during the period from January 2015 to August 2019 (n = 14). Data reviewed included demographic paramete

BACKGROUND: Recurrent pulmonary vein stenosis (PVS) following surgical repair of total anomalous pulmonary venous connection is associated with poor prognosis. Preclinical studies have shown that use of an angiotensin receptor blocker can attenuate intimal hyperplasia; notwithstanding, its clinical utility is of uncertain benefit.
METHODS: This single-center study included patients undergoing total anomalous pulmonary venous connection repair in 2020 to 2021. Since August 2020, patients have been considered for valsartan therapy early after operation. Contemporaneous participants were subcate

BACKGROUND: Patients with obstructed infra-cardiac total anomalous pulmonary venous connection (TAPVC) require urgent intervention to relieve the obstruction, with or without restoration of anatomical continuity between the pulmonary veins and the left atrium. In cases of infra-cardiac TAPVC draining into the inferior vena cava (IVC) or hepatic vein, the obstructed channel can be accessed via the systemic venous approach for endovascular palliation. However, in cases of infra-cardiac TAPVC draining into the portal venous system, an endovascular approach to the obstructed channel is not possibl

PURPOSE: Development of postoperative obstruction in total anomalous pulmonary venous connection (TAPVC) is a major cause of morbidity and mortality. Although preoperative echocardiography has often been cited as prognostic of postoperative outcome, its predictive value has not been fully evaluated. Pulmonary venous variability index (PVVI) is an echocardiographic metric developed at our center and previously shown to correlate with preoperative clinical markers and catheterization findings of obstruction. We hypothesized that preoperative PVVI would be superior to maximum and mean velocity fo

Total anomalous pulmonary venous connection (TAPVC) is a rare congenital heart defect where the pulmonary veins abnormally connect to the systemic venous circulation, rather than the left atrium (LA). This study retrospectively analyzed 48 children diagnosed with TAPVC from January 2014 to January 2024 to investigate how specific echocardiographic and morphological characteristics impact prognosis. Perioperative records, including echocardiography, computed tomography angiography (CTA) images, surgical details, and patient outcomes, were reviewed. Data were statistically analyzed using tools s

Total anomalous pulmonary venous connection is a rare congenital anomaly that requires prompt surgical repair. Associated anomalies are common; however, its coexistence with coarctation of the aorta is extremely rare. Herein, we present a rare case of a neonate who underwent successful one-stage repair of infracardiac total anomalous pulmonary venous connection associated with coarctation of the aorta.

Surgical results for correction of total anomalous pulmonary venous connection (TAPVC) have improved in the current era. Postrepair pulmonary vein stenosis (PR-PVS) remains a major complication following TAPVC repair, often necessitating multiple reinterventions, and impacting patient outcomes. Considerations for patient and anatomical risk factors, surgical techniques for primary TAPVC repair, as well PR-PVS postoperative surveillance and management will be reviewed. We will also touch on emerging areas of research related to PR-PVS pathogenesis and incorporation of patient-specific simulatio

BACKGROUND: Pulmonary vein anatomy varies widely in total anomalous pulmonary venous connection (TAPVC), with diagnostic accuracy affecting surgical planning and prognosis. Echocardiography is the preferred screening modality. Cross-sectional imaging (cardiac computed tomography [CT] or magnetic resonance [MR]) may offer added value.
METHODS: As the first step in a quality improvement initiative, we sought to describe practice patterns in presurgical imaging in TAPVC with attention to diagnostic error. We also evaluated the available literature on diagnostic accuracy of imaging in TAPVC. Pati

A newborn with right atrial isomerism, single-ventricle physiology, supracardiac total anomalous pulmonary venous connection, and pulmonary atresia underwent vertical vein stenting, resulting in ductus arteriosus compression. Subsequent ductal stenting restored patency, preventing haemodynamic compromise and ensuring survival. This case highlights the risk of secondary ductal obstruction in vertical vein stenting and the importance of early recognition and intervention.

Obstructive total anomalous pulmonary venous connection (TAPVC) is a life-threatening congenital anomaly requiring urgent intervention. A one-month-old, 2.5 kg neonate presented with tachypnea, respiratory distress, lethargy, and 70% oxygen saturation. Echocardiography revealed obstructed supracardiac TAPVC with a 20 mmHg gradient at the pulmonary venous confluence and a 3.5 mm atrial septal defect with right-to-left shunting. Due to the patient's critical condition, palliative stenting of the vertical vein (VV) was performed. Despite significant tortuosity, a Progreat microcatheter successful

Lung agenesis associated with congenital heart disease is a rare and serious malformation that requires immediate medical attention. If not addressed with preplanned surgical and medical support, it can often lead to mortality. This report describes an unusual case of a 2-month-old male infant with left lung agenesis and obstructed infracardiac total anomalous pulmonary venous connection. The infant presented with congestive heart failure, dominated by symptoms of obstruction and pulmonary hypertension. We detail the successful surgical intervention and postoperative outcomes for this rare ass

Total anomalous pulmonary venous connection (TAPVC) is a rare and life-threatening condition, and long-term patient outcomes remain to be fully explored. This registry-based national cohort study including all Danish children born with TAPVC between 2000 and 2018 aimed to evaluate the survival rates of Danish children receiving a TAPVC repair procedure. Additionally, the prevalence of early morbidity, severe chronic diseases, and mental and/or behavioral disorders in children diagnosed with TAPVC was investigated using Danish health registries. A total of 89 children with TAPVC were identified
