Is routine preoperative cardiac catheterization necessary before repair of secundum and sinus venosus atrial septal defects?

Between January 1976 and July 1983, 217 patients with atrial septal defect underwent surgical repair at Children's Hospital. Thirty with a primum atrial septal defect and 26 who underwent cardiac catheterization elsewhere before being seen were excluded from analysis. Of the 161 remaining patients, 52 (31%) underwent preoperative cardiac catheterization, 38 because the physical examination was considered atypical for a secundum atrial septal defect and 14 because of a preexisting routine indication. One hundred nine (69%) underwent surgery without catheterization, with the attending cardiologi

Extracardiac atrial pedicle conduit repair of partial anomalous pulmonary venous connection to the superior vena cava in children.

Between June, 1982, and July, 1983, 6 children with partial anomalous pulmonary venous connection to the middle or high segment of the superior vena cava (SVC) underwent repair of the anomaly by division of the SVC proximal to the site of entry of the anomalous pulmonary veins. Continuity between the cephalad end of the SVC and the right atrium was established by direct anastomosis to the right atrial (RA) appendage or by creation of a pedicle conduit of RA appendage, RA free wall, and pericardium. The anomalous pulmonary veins remained in situ on the lower segment of SVC, blood being directed

An alternative method for repair of partial anomalous pulmonary venous connection to the superior vena cava.

The surgical management of 15 patients with partial anomalous pulmonary venous connection (PAPVC) to the high superior vena cava (SVC) is described. This new technique redirects the anomalous pulmonary venous flow into the left atrium through the cardiac end of the SVC, transected and oversewn above the anomalous pulmonary vein or veins, by coaptation of the atrial septal defect (or of the surgically created septal defect in patients with an intact atrial septum) to the intracardiac orifice of the SVC. Normal SVC-right atrial flow is reconstituted by atriocavoplasty to the cephalad portion of

Echocardiographic definition of right pulmonary venous connection at catheterization.

Traditional methods of identifying partial anomalous pulmonary venous connection to the right atrium in the presence of an atrial defect are not always reliable. Twenty patients were studied with a new technique in which the catheter is introduced into the right superior pulmonary vein followed by echocardiographic assessment of the catheter position in relation to the left atrium and atrial septum. The insertion site of the right pulmonary veins was detected in every patient and in ten patients has been verified at operation. This approach can be performed rapidly and appears to be accurate a

Radionuclide ventriculography: a noninvasive method for the detection and quantification of left-to-right shunts in atrial septal defect.

The present investigation was undertaken to assess a new scintigraphic method for the diagnosis of left-to-right shunts due to atrial septal defect based on the differing stroke volumes of left and right ventricles and to compare it with oxymetric data. Radionuclide ventriculography was carried out after injection of 20 mCi 99mtechnetium-labeled red blood cells. Time-activity curves were obtained from the left and right ventricular regions, and the ratio (A) of end-diastolic-end-systolic count rate differences for the left and right ventricles was calculated. The left-to-right shunt (in percen

Repair of atrioventricular canal malformation in the first year of life.

Disappointing results with pulmonary artery banding and subsequent correction led to the decision in 1977 that all infants presenting to our hospital with atrioventricular (AV) canal and evidence of severe heart failure, lack of growth, or pulmonary hypertension should have early operative correction. Since that time 24 consecutive infants have undergone repair. All had refractory heart failure. Average age at operation was 18 weeks (3 to 38) and average weight was 4.3 kg (2.3 to 6.4). Only four patients were older than 6 months of age at operation. Preoperative peak pulmonary artery pressure

Left-to-right shunt at atrial level due to anomalous venous connection of left lung: report of seven cases.

Our experience with seven patients with partial anomalous pulmonary venous connection (PAPVC) of the left lung, treated between 1955 and 1980, is reviewed. The developmental aspects and the hemodynamic effects of the anomaly are discussed, and the patterns of the abnormal venous connection in our patients are described. All the patients except one had an additional atrial septal defect (ASD). Treatment consisted in closure of the ASD and creation of a large anastomosis between the detached or ligated anomalous connecting vein and the left atrium, preferably the left auricular appendage. No ope

Cor triatriatum (subdivided left atrium).

Twenty-one patients (age 1 day to 156 months) with cor triatriatum sinister (subdivided left atrium) were treated over a 23 year period at two institutions. Anatomic classification of the hearts revealed 20 (95%) with classical cor triatriatum and normal pulmonary venous connections. One other patient had partial anomalous pulmonary venous connection. Three of these patients (14%) had other severe associated cardiac anomalies. Surgical correction of isolated cor triatriatum was performed in 13 patients of whom eight (62%) survived. Long-term results were excellent in seven (88%) patients; one

Echocardiographic assessment of Qp/Qs in children with atrial septal defect or partial anomalous pulmonary venous connection.

In 24 children with atrial septal defect of the secundum type and/or partial anomalous pulmonary venous connection, the pulmonary to systemic blood flow ratio (Qp/Qs) was estimated by echocardiography. Using M-mode echocardiography, right ventricular diameter (RVD), left ventricular diameter (LVD), total cardiac diameter (TCD) and aortic root (AOR) were measured, and the following 5 indices were obtained: RVD/LVD, RVD/TCD, RVD/AOR, RVD/BSA (body surface area) and TCD/BSA. The single linear correlation coefficients between these 5 indices and Qp/Qs, obtained by the Fick method during cardiac ca

[Atrial septal defect with pulmonary hypertension in infancy (author's transl)].

Four cases of atrial septal defect, "ostium secundum", associated with pulmonary hypertension and congestive heart failure in children under the age of two years are reported. A correct diagnosis could be made before surgery in only two of the case. In one case partial anomalous pulmonary venous connection was incorrectly diagnosed, while ventricular septal defect could not be excluded in the fourth case. The systolic pressure in the pulmonary artery was higher than 50 mm. Hg in all cases. One patient died after catheterization, and total surgical correction was performed in the other three ca

Use of contrast echocardiography in the diagnosis of partial anomalous pulmonary venous connection.

Contrast echocardiography is useful in diagnosing the presence of partial anomalous pulmonary venous connection, particularly when it occurs in association with an atrial septal defect. Injections of saline solution, Cardiogreen or the patient's own blood were made in the inferior vena cava, left atrium and pulmonary veins of 27 patients. In all patients, the inferior vena caval injections showed only right heart clouding with no evidence of a right to left shunt. In 21 patients, the left atrial and pulmonary venous injections showed clouding of both the right and left heart structures, indica

Unusual vascular anomalies causing persistent pulmonary hypertension in a newborn.

A unique case of pulmonary vascular anomalies causing persistent pulmonary hypertension in a newborn is described. The child died 3 days after birth. Necropsy revealed marked hypoplasia of the right and left pulmonary arteries with a normal main pulmonary artery, patent ductus arteriosus, bilateral systemic arteries to the lungs from the abdominal aorta, and partial anomalous pulmonary venous connection. Quantitative morphometric techniques demonstrated slight abnormalities of alveolar development and severe arterial medial hypertrophy with abnormal extension of muscle into small peripheral ar

Partial anomalous pulmonary venous connection to the azygous vein with intact atrial septum.

A five-year-old girl with partial anomalous pulmonary venous connection to the azygous vein with intact atrial septum is reported. The clinical and roentgenographic features suggested the correct diagnosis. Surgical correction of this previously unreported defect was accomplished by creating an atrial septal defect and constructing a baffle to direct the blood flow from the azygous vein to the left atrium.
