The hazard of malarial nephropathy.

Renal damage in malaria occurs as a result of two distinct mechanisms: microcirculatory disturbance, which is confined to falciparum malaria, and immunological damage through the deposition of soluble immune complexes. Quartan malarial nephropathy, due to Plasmodium malariae, is a progressive disease ending in chronic, hypertensive, renal failure. The disease seems most prevalent in adolescents, placing a heavy burden on the health services of developing countries where falciparum malaria is endemic.

[Glomerulopathies associated with parasitoses].

Glomerular diseases linked to parasitic infections are frequent in tropical areas: the nephrotic syndrome of African children is often related to malarial infections and hepato-splenic schistosomiasis may be associated with glomerulonephritis. Histological studies indicate that the same parasite can induce a spectrum of glomerular lesions. Immune complexes play a major role in the pathogenesis of these nephropathies but their precise composition is largely unknown. Experimental studies suggest that some of these complexes could be formed by immunoglobulin interactions related to the perturbati

Patterns of glomerulonephritis in Zimbabwe: survey of disease characterised by nephrotic proteinuria.

Ninety-eight Zimbabweans with glomerulonephritis characterised by nephrotic proteinuria were studied. There was no evidence to implicate Schistosoma mansoni or S. haemotobium in the aetiology, although schistosomiasis was diagnosed in 54 patients in the series. Similarly, Plasmodium malariae proved unimportant as a cause of the nephrotic syndrome, only one patient showing focal segmental glomerulosclerosis which was associated with subclinical quartan malarial infection. Nevertheless, infections were shown to play a major role in the genesis of glomerulonephritis which was associated with beta

The role of infectious agents in the aetiology and pathogenesis of childhood nephrotic syndrome in Africa.

Childhood nephrotic syndrome is common in Africa where infectious agents are prevalent. This paper reviews the possible aetiological role of infectious agents in childhood nephrotic syndrome in Africa. There is a strong association, possibly causal, between childhood nephrotic syndrome on the one hand and Plasmodium malariae, Schistosoma mansoni and hepatitis B antigens on the other. Beta-haemolytic streptococci are less strongly associated with nephrotic syndrome, and a few other organisms are suspect. The many ubiquitous infectious agents and the prevalence of multiple infections make it dif

Glomerular disease in adults in the Savannah region of Nigeria--clinical and pathological features.

In a consecutive series of 134 adult patients with glomerular disease, 82.7% showed proliferative glomerulonephritis on renal biopsy; 45% of the cases were asymptomatic. Nephrotic syndrome in adults in northern Nigeria should respond to immunosuppressive therapy only rarely. There was no evidence to implicate Plasmodium malariae as the cause of glomerular disease in adults in the savannah belt in Nigeria. The roles of hepatitis B virus and Yersinia enterocolitica were discussed and require further serological investigations. O-serotype 8 yersiniosis is reported outside the USA for the first ti

Quartan malaria following splenectomy 36 years after infection.

Quartan malaria developed following splenectomy 36 years after infection in a 63-year-old hypertensive man. The patient underwent nephrectomy because of left renal calculus, increasing proteinuria and hypertension. Splenectomy was done additionally because metastasis of renal tumor to the spleen was suspected at the operation. Attention is drawn to the long silent infection with Plasmodium malariae and to the importance of the spleen in malaria.

The nephrotic syndrome in Ghana: clinical and pathological aspects.

Clinical and pathological features of the nephrotic syndrome were studied in 36 adults and 25 children in Ghana. No evidence was found to implicate Plasmodium malariae as a cause and in the majority of patients the aetiology was not identified. Minimal change glomerulonephritis responsive to steroids was demonstrated in 14/25 children and 5/36 adults which was surprising as this lesion has been reported only rarely from tropical Africa. The other major histological lesions were focal segmental glomerulosclerosis (12/61), diffuse proliferative glomerulonephritis (11/61) and membranous glomerulo

Immunological aspects of nephrotic syndrome in northern Nigeria.

Immunological aspects of 40 northern Nigerian children with nephrotic syndrome of recent onset are reported. Eight our of 30 had hepatitis-associated antigen in their sera. Hypocomplementaemia was rare. Measurement of serum C3, C4, and ASOT was not of diagnostic value. Proteinuria selectivity index was poor in half of the patients, and appeared t o depend on the severity of the kidney lesion. Abnormal immunofluorescence of kidney glomeruli to immunoglobulins, complement, Plasmodium malariae, and Plasmodium falciparum was found in 26 of the 29 children. The pattern of immunofluorescence was chi

Studies on the immunopathology of the nephrotic syndrome associated with Plasmodium malariae. 1. Serum levels of an immune adherence inhibitor.

A comparative study of the serum levels of an immune-adherence inhibiting factor was carried out on serum samples from eighty-seven nephrotic syndrome children, twenty-eight nephrotic adults, 202 normal school children, 116 blood donors, twenty-five falciparum malaria children and 172 patients with miscellaneous diseases. Low titres (1/32 and below) of the factor were present in sera from 63.2% of the nephrotic children 60.7% of nephrotic adults and 60.0% of children malaria, as compared with 30.7% of the normal children, 25.5% of the patients with miscellaneous diseases and 41.4% of the blood

Quartan malarial nephrotic syndrome in children.

Quartan malarial infection causes an immune complex nephritis in some individuals, which, once established, is sustained by mechanisms not yet fully explained, but which probably involve autoimmune processes. The presenting clinical and biochemical findings of the quartan malarial nephrotic syndrome are similar to those classically described for the nephrotic syndrome in childhood, but the renal pathology seen on light, electron, and immunofluorescent microscopy show striking differences and distinctive features. The disease tends to pursue a chronic course and in most patients is nonresponsiv

The quartan malarial nephrotic syndrome.

A specific relationship between P. malariae and the nephrotic syndrome, originally postulated on epidemiologic evidence, has been substantiated by clinical, pathologic and immunologic observations. It would appear that quartan malaria causes an immune complex nephritis in some individuals that, once established, is sustained by mechanisms not yet fully explained but which may involve an autoimmune process. Evidence to support an immunologic pathogenesis of the renal lesions is provided by the presence of immunoglobulin, complement (C3) and quartan malarial antigen in biopsy specimens studied b

[Renal involvement in malaria (author's transl)].

Malaria infection leads to renal involvement. Reversible proteinuria accompanies many plasmodial infections. Chronic malarial nephrotic syndrome is specifically associated with quartan malaria. Acute renal failure is restricted to infections with Plasmodium falciparum. The pathogenesis of renal involvement during malarial infections includes immunological mechanisms. It is now realized that there exist at least two types of immunological processes: acute transient immune-complex glomerulonephritis with reversible proteinuria and chronic immune-complex glomerulonephritis with irreversible nephr

Differential protein clearances and response to treatment in Nigerian nephrotic children.

Remission followed prednisolone therapy in 9 out of 21 Nigerian children with the nephrotic syndrome who had highly selective proteinuria (CG/CA less than 15%). Of these, 5 patients have remained well off all treatment during a follow-up of nearly 5 years, 4 have relapsed more than once but have responded to further courses of prednisolone. 3 of 21 with less selective proteinuria also remitted but all relapsed and only one of these has responded again. The other two have relapsed and further courses of prednisolone have not totally abolished their proteinuria though they are asymptomatic and i
