Modified arterial switch operation by spiral reconstruction of the great arteries in transposition.

BACKGROUND: Spiral relationship of the normally related great arteries (SRGA) has never been reconstructed in an arterial switch operation. METHODS: From March 1998 to April 1999, 9 consecutive cases of transposition of the great arteries (TGA) family (from 2 days to 1.6 years old) underwent arterial switch operations with SRGA at our hospital. Two had a congenitally corrected TGA (plus atrial redirection). Lecompte maneuver was not used in all. The posterior wall of pulmonary trunk was not divided but three were reattached, two of whom had had previous pulmonary trunk banding. Thus the wall

Preduodenal portal vein in the adult.

We present three cases of preduodenal portal vein in adult people, which were diagnosed in our department. All of them were identified during elective operation for cholelithiasis, caused some technical difficulties to the performance of the operation, but led to no major intraoperative or postoperative complications. None of them had any preoperative symptoms, which could be related to this anomaly. The preduodenal portal vein is a rare congenital anomaly, which is usually discovered in infants or children due to the obstruction of the duodenum. In adults, it is often asymptomatic, and is usu

Cyanotic congenital heart disease with decreased pulmonary blood flow in children.

Of the "five T's" of cyanotic congenital heart disease--tetralogy of Fallot, TGA, TAPVC, truncus, and tricuspid valve abnormalities (tricuspid atresia, stenosis, and displacement)--the first and last are commonly associated with diminished PBF. The four features that comprise tetralogy of Fallot--right ventricular hypertrophy, VSD, overriding aorta, and subpulmonary stenosis--are all secondary to a single morphogenetic defect: failure of expansion of the subpulmonary conus. This also explains the variability in clinical presentation. When neonates need intervention, shunts are usually performe

The use of 'pericardial hoods' for maintaining exact coronary artery geometry in the arterial switch operation with complex coronary anatomy.

OBJECTIVE: Complex coronary artery anatomy is the major risk factor for the arterial switch operation. Of the many approaches described the 'trap door' technique for coronary reimplantation is most flexible and allows safer transfer in complex arterial configurations. However, we have occasionally been concerned regarding torsion of the vessels with this approach. We therefore explored the role of trap-door augmentation with pericardial hoods to maintain exact coronary geometry during coronary transfer. METHODS: Between February 1992 and December 1997, 80 patients underwent an arterial switch

[MRI angiography of the thoracic vessels including coronary arteries: techniques and indications].

Because it is a non-invasive method, Magnetic Resonance Angiography has more and more indications in the thorax, particularly with the help of use of contrast media. Different types of sequences used (Cine-MR, Gradient-echo 2D and Gadolinium enhanced breath-hold and non breath-hold 3D) are described as well as acquisition parameters and reconstructions from natives images. Post-processing techniques (MIP, MPR, 3D Surface Rendering) and their limitations depending upon the region explored are explained. Major indications (congenital anomalies of the great vessels, aneurysms, dissections, post-o

Bidirectional Glenn shunt in association with congenital heart repairs: the 1(1/2) ventricular repair.

BACKGROUND: The bidirectional Glenn shunt has been used to incorporate a smaller tripartite ventricle into the circulation and create pulsatile pulmonary artery flow. We reviewed our operative experience and assessed hemodynamics of the bidirectional Glenn shunt in 1(1/2) ventricular repair or in conjunction with other repairs of congenital heart defects. METHODS: Between 1992 and 1998, 15 patients (mean age, 8.1+/-7.9 years) had bidirectional Glenn shunt in association with repair of congenital heart defects. Eighty-seven percent had at least one previous operation. All patients had simultan

Toward designing the optimal total cavopulmonary connection: an in vitro study.

BACKGROUND: Understanding the total cavopulmonary connection (TCPC) hemodynamics may lead to improved surgical procedures which result in a more efficient modified circulation. Reduced energy loss will translate to less work for the single ventricle and although univentricular physiology is complex, this improvement could contribute to improved postoperative outcomes. Therefore to conserve energy, one surgical goal is optimization of the TCPC geometry. In line with this goal, this study investigated whether addition of caval curvature or flaring at the connection conserves energy. METHODS: TC

Identification of an arteriovenous fistula during free tissue transfer despite negative preoperative angiography: a potentially correctable cause of venous thrombosis.

Although refinements in microvascular technique have resulted in great success with free tissue transfer, long-term anastomotic patency, particularly venous, is less predictable in patients with a history of intravenous drug abuse or other causes of intimal damage and/or venous hypertension. We report an unusual case in which an arteriovenous fistula was identified intraoperatively as the cause of pulsatile venous backflow despite a normal preoperative angiogram. A flow disturbance at the site of venous anastomosis and subsequent development of a venous thrombosis prompted more proximal venous

Myocardial blood flow and coronary flow reserve late after anatomical correction of transposition of the great arteries.

OBJECTIVES: Myocardial blood flow (MBF) in children late after arterial switch operation (ASO) was investigated quantitatively by positron emission tomography (PET). BACKGROUND: In children with transposition of the great arteries (TGA), ASO is widely accepted as the management of choice. The long-term patency of coronary arteries after surgical transfer to the neo-aorta, however, remains a concern. METHODS: Twenty-two normally developed, symptom-free children were investigated by PET with nitrogen-13 ammonia at rest and during adenosine vasodilation 10+/-1 years after ASO. A subgroup of 15

[Magnetic resonance of congenital cardiopathies].

OBJECTIVE: The objective of this study was to determine the usefulness of magnetic resonance in the diagnosis and follow-up in congenital heart disease. PATIENTS AND METHODS: Between 1992 and 1994, we have evaluated 250 patients (between 1 day-22 years of age) with congenital heart disease. We used a Philips-Gyroscan 0.5 Tesla set-up and the studies were made with ECG-gated multislice sequences with the time of repetition adapted to the cardiac frequency. The slice thicknesses were 3-10 mm and the field of view was 100-400 mm. Three axes were used: axial, oblique coronal and 30-60 degrees obl

Left-sided inferior vena cava. Report of a case occasionally encountered while performing an aorto bifemoral bypass and review of the literature.

We present a case of left-sided inferior vena cava unexpectedly observed during an operation of aorto bifemoral bypass in a patient with severe Leriche syndrome and almost complete obstruction of the infrarenal aorta. This very rare congenital malformation (0.2-0.5%) was not recognized by the duplex scanner performed preoperatively, probably because of the low level of suspicion carried on by an experienced operator. AngioCT or angioMR, which would have surely shown us the anomaly, were not done because, in the lack of an aneurysmal disease or other abdominal pathological situations, these inv

The switch to balloon atrial septostomy with echocardiographic guidance--a severe complication.

To warn about a severe complication of balloon atrial septostomy (BAS) guided by echocardiography, a retrospective review was undertaken of all BASs performed at Red Cross War Memorial Children's Hospital in Cape Town from 1984 to the end of 1992. BAS with fluoroscopy was carried out in 41 infants (group I). Since April 1991, 17 infants had BAS with two-dimensional echocardiography guidance in the intensive care unit (group II). The majority of patients (72%) had simple transposition of the great arteries. Only 3 patients in group I were catheterised via the umbilical vein (the remaining 38 vi

[Influence of coronary anatomy on the anatomic repair of transposition of great arteries].

INTRODUCTION: The anatomic diversity of the coronary artery and the alignment of the aortic and pulmonary valves have special importance on the surgical anatomic correction of the transposition of the great arteries and double outlet of the right ventricle, because of their repercussions on surgical technique and the effect on the operative results. OBJECTIVE: To determine the coronary artery pattern; the aortic and pulmonary valve alignment, and their effects on the surgical results in 57 patients submitted to anatomic correction in our hospital. METHOD: We used the Yacoub and Radley-Smith

[Perioperative management of a patient with bilateral renal cell cancer and large vena caval tumor thrombosis].

Von Hippel-Lindau disease is a kind of rare autosomal dominant hereditary disease characterized with many kinds of tumor or cystic lesion. In this 30 year old woman, vena caval tumor thrombi from retroperitoneal malignancies caused by renal cell cancer extended into the right atrium. She was scheduled to undergo bilateral radical nephrectomy and removal of vena caval thrombi under continuous hemodiafiltration (CHDF) and extracorporeal circulation. CHDF and Biomedics Pump were on stand-by during the operation. Radical left nephrectomy was performed. In the right kidney, however only the tumor c

Mixed total pulmonary venous drainage: still a surgical challenge.

OBJECTIVE: The aim of this report is to review the surgical experience of a single institution with a relatively large series of patients with mixed total pulmonary venous drainage. PATIENT POPULATION: Between January 1, 1971, and December 31, 1994, 232 patients with total pulmonary venous drainage underwent surgical correction. Twenty of these patients (8.6%) had mixed type total pulmonary venous drainage. Ages at operation ranged from 1 day to 46 months, with a median of 2.3 months. RESULTS: Both cardiac catheterization and echocardiography were performed before operation in 12 patients. F

[A successful modified Aubert procedure for transposition of the great arteries with a special coronary artery pattern--a case report].

Transplantation of the coronary arteries is an essential and critical part of the arterial switch operation, which is considered to be the treatment for most neonate with transposition of the great arteries. The presence of an intramural segment of coronary arteries, close coronary ostia adjacent to each other increase the risk of coronary twisting and kinking after transplantation. This case has an unusual coronary anatomy characterized by two coronary ostia arising from the same aortic cusp, intramural segment in a proximal part of the left anterior descending coronary artery (LAD) and slit-

Three-dimensional display of cardiac structures using reconstructed magnetic resonance imaging.

It is sometimes difficult to understand the three-dimensional (3D) relationship of cardiac and mediastinal structures despite advances in magnetic resonance (MR) imaging techniques. We present a low-cost system for 3D reconstruction of the major mediastinal structures by processing the MR imaging data on a NeXT workstation. MR images of multisection, multiphase, spin-echo techniques stored in a picture archiving and communication system (PACS) data base were used for the reconstruction. The computer program obtained the contours of the multiple components of the mediastinal structures by the c

[Surgical treatment of deep venous reflux. Techniques, indications and results].

INTRODUCTION: complications of deep venous insufficiency can lead to surgery if one fails with medical treatment. The etiology can be primitive or secondary, the symptoms are identical. They go from heaviness of the legs to chronic leg ulcers. Indication and technic of surgery depend on the etiology and the preoperative evaluation. PREOPERATIVE EVALUATION: it has to be anatomical and functional. Invasive investigations as venous blood pressure, ascending and descending venography, and non invasive investigations as duplex or triplex Doppler and plethysmography are used. TECHNICS: one can use

Coronary artery anatomy in complete transposition of the great arteries.

Knowledge of the variations in coronary artery pattern is important in the arterial switch operation for complete transposition of the great arteries (TGA). As autopsy specimens provide the most definitive means of identifying the coronary anatomy, 255 hearts with complete TGA were reviewed by a single pathologist. The age of the patients ranged from 1 day to 34 years (mean, 2.9 years). The origin of the coronary arteries was defined as seen by an observer looking from the pulmonary artery toward the aorta. The usual pattern with the right coronary artery originating from the right hand sinus

Differential diagnosis and repair of femoral artery pseudoaneurysms: report of clinical experience using color Doppler imaging.

Sixty-two sonograms of the groin in 58 patients referred after cardiac catheterization (n = 45) or local surgical by-pass (n = 13) were reviewed retrospectively to determine the accuracy of color Doppler imaging in distinguishing femoral artery pseudoaneurysms from other causes of groin masses (e.g. hematomas) and to evaluate the therapeutical impact of US-guided compression. Thirty-seven cases of pseudoaneurysm, three arteriovenous fistulae (one isolated and two associated with a pseudoaneurysm), sixteen hematomas and eight other conditions were studied; US-guided compression repair was consi
