[Total cavopulmonary connection in complex heart defects with a single functional ventricle].

Nineteen children aged between 1.5 and 9.8 years (10 patients younger than 4 years) underwent total cavopulmonary connection (TCPC) for complex congenital heart defects other than tricuspid atresia. Diagnoses included double inlet left ventricle (n = 11), transposition of the great arteries with a large ventricular septal defect (n = 4), double outlet right ventricle (n = 2), congenitally corrected transposition of the great arteries with a large ventricular septal defect (n = 1) and criss-cross heart (n = 1). Two patients had situs ambiguous with polysplenia syndrome and four patients had ano

Influence of anatomic correction for transposition of the great arteries on myocardial perfusion: radionuclide imaging with technetium-99m 2-methoxy isobutyl isonitrile.

OBJECTIVES: We sought to determine the incidence of late perfusion defects attributable to coronary artery mobilization in patients undergoing anatomic correction for complete transposition of the great arteries. BACKGROUND: Anatomic correction (arterial switch procedure) is currently the surgical treatment of choice for complete transposition. From its conception, there has been concern about the impact on myocardial perfusion of the coronary artery mobilization and reimplantation involved in the correction. Previous studies have demonstrated myocardial perfusion defects in patients after co

Total cavopulmonary connection--early results.

Presently total cavopulmonary connection is used as an alternative in complex congenital disorders requiring atriopulmonary connection. From January 1989 till November, 1992, a total of 18 cases underwent total cavopulmonary connection. There were 8 males and 10 females. The mean age at operation was 92 months (range 5 to 504 months). The average weight was 19.6 kg (range 5.25 to 42 kg). Diagnoses included: tricuspid atresia (6); univentricular heart (7); pulmonary atresia with intact interventricular septum (1); double outlet right ventricle, pulmonary stenosis with uncommitted ventricular se

[Perioperative management of systemic pulmonary shunts in the neonatal period].

We have analyzed 36 newborns (19 males and 17 females), with cyanotic cardiopathies in whom a systemic-pulmonary shunt had been performed. These patients were admitted to the Neonatal Intensive Care Unite between January 1985 and June 1990. We studied the perioperative events with the aim of describing the general features of this palliative surgery in the neonatal period and to determine the factors which indicate a bad prognosis. The age at admission was 10.5 +/- 16.5 days and at surgery was 19.5 +/- 20 days. Cardiopathy types included: 13 pulmonary atresias, 9 pulmonary stenoses, 6 D-transp

[Anatomical correction of transposition of the great arteries in 7 newborn infants].

In the presented article, the course and results of anatomical correction of transposition of great arteries (TGA) in 7 neonates (2 females and 5 males) with mean body mass of 3250 g and 2 to 5 days old (mean 3 days) are reviewed. Surgery was performed in moderate hypothermia. St. Thomas cold cardioplegia was used. Mean aortic clamping time was 70 min (55-115), and the time of extracorporeal circulation was 165 min (117-210). Low cardiac output in all patients in the postoperative period required prolonged mechanical ventilation and positive inotropic drugs. Out of 7 patients operated, two die

[Congenital anomalies of coronary artery origin: a diagnostic challenge].

From a series of 4,313 consecutive patients who underwent a diagnostic coronary angiogram, 16 (0.37%) presented a congenital anomalous origin of the coronary arteries. None of these patients had other congenital cardiac anomalies associated. Age was 57 +/- 9 years and 13 (81%) were male. The diagnostic catheterization was performed for unstable angina in 8 patients (50%), for stable angina in five (32%), for dyspnea in two and for atypical chest pain in the remaining patient. A previous myocardial infarction was present in 6 patients (37%) whereas one patient had apical hypertrophic cardiomyop

Surgical treatment of coronary artery anomalies.

In this paper, we review literature from June 1990 through May 1991. We discuss normal anatomy and physiology of the proximal coronary arteries, anomalous origin of the coronary arteries from an inappropriate aortic sinus, and anomalous origin of the left coronary artery from the pulmonary artery. Other topics reviewed include coronary arteriovenous fistulas, coronary anomalies in left-sided heart syndrome and pulmonary atresia with intact interventricular septum, coronary arterial abnormalities in transposition of the great arteries, and iatrogenic coronary anomalies.

[Color doppler-echocardiography study of the triatrial heart].

Two-dimensional echocardiography has proved its utility in the diagnosis of cor triatriatum sinister by visualizing the defect and its relation to the other structures. Recently pulsed and continuous wave Doppler have been used to determine the degree of obstruction of the membrane. In this study we present three patients with ages six, eight and fourteen months respectively, all with a clinical history of growth retardation, respiratory infections and dyspnea. The patients were studied with two-dimensional and color flow Doppler echocardiography. In all, a membrane was visualized, which divid

[Clinical and paraclinical identification by various imaging technics of congenital anomalies of the inferior vena cava (apropos of 4 cases)].

Four cases of anomalies of the inferior vena cava (IVC) were studied by two noninvasive imaging techniques: real-time ultrasonography and computed tomography. These techniques were used alone in 1 case of left IVC to determine the diagnosis. The other 3 cases were studied initially by phlebography: left IVC with iliac phlebitis; double IVC, also with iliac phlebitis, and a special case in which it was not clear whether the diagnosis was agenesis of the IVC or postnatal caval thrombosis. The two patients with recent iliac vein thrombosis were treated by heparin, but the other two received no pa

Pulmonary vascularity in pediatric heart disease.

In children, analysis of pulmonary vascularity can provide useful information about cardiac and noncardiac abnormalities. The abnormal vascular patterns encountered in children with heart disease include (1) active engorgement, (2) passive congestion, (3) diminished pulmonary blood flow, (4) cephalad redistribution, and (5) unequal blood flow. Familiarity with these various patterns enables the clinician to produce relevant differential diagnoses for each. With additional study of the cardiac chambers, great vessels, and other radiographic findings, a specific diagnosis may often be suggested.

Magnetic resonance imaging of congenital heart disease: sensitivity and specificity using receiver operating characteristic curve analysis.

MRI has shown promise for the evaluation of various congenital heart lesions. The current study was designed to assess the sensitivity and specificity of the technique for the evaluation of all major anatomic elements of the heart affected in simple and complex congenital heart disease. MR images of 51 patients (31 males and 20 females, ages 3 to 69 years) with a total of 110 congenital heart lesions were reviewed by a panel of two cardiac radiologists and one pediatric cardiologist, who assessed the cardiac pathoanatomy without knowledge of clinical details and assigned a confidence level to

[Goldenhar syndrome. Report of 5 cases in association with cardiac malformations].

The paper presents five cases of the Syndrome described by Goldenhar, which consists in oculo-auriculo-vertebral alterations. Classic descriptions relate the presence of epibulbar dermoid, pre-auricular appendixes, mandible hypoplasia and vertebral anomalies. Excepting by some vertebral alterations, all the other anomalies were common to the five cases. This syndrome may be accompanied by congenital cardiopathy in a percentage varying from 15 to 50%, depending on the publications. The reason of hospitalization was the real presence of congenital cardiopathy of important clinical repercussion.

Results of the arterial switch operation for transposition of the great arteries with ventricular septal defect. Surgical considerations and midterm follow-up data.

Between January 1983 and December 1987, 62 patients underwent an arterial switch operation for transposition of the great arteries with ventricular septal defect or double outlet right or left ventricle. There were three hospital deaths (4.8%), and no deaths occurred in neonates (less than 1 month of age, n = 18). There were three late deaths, one due to coronary obstruction and two due to pulmonary vascular obstructive disease. One child has been lost to follow-up. We have prospectively evaluated the remaining 55 survivors by clinical evaluation, echocardiography, cardiac catheterization, amb

Noninvasive three-dimensional reconstruction of the heart and great vessels by ECG-gated magnetic resonance imaging: a new diagnostic modality.

We have developed a new method of computer image processing that allows true three-dimensional (3-D) images of the heart and great vessels to be reconstructed from standard ECG-gated two-dimensional magnetic resonance (MR) images. Contiguous 5-mm thick MR images of the thorax from the level of the cardiac apex to the aortic arch were obtained in 4 normal volunteers and 3 patients with congenital heart disease: 1 with pseudotruncus arteriosus and 1 with a ventricular septal defect, each with Eisenmenger's complex, and 1 with aortic coarctation. Each image could be obtained at up to seven differ

Anatomical repair of transposition of the great arteries with intact ventricular septum in the neonate: guidelines to avoid complications.

Anatomical repair of transposition of the great arteries (TGA) seems more attractive than the more conventional atrial baffle procedures because of resultant left ventricular-aortic continuity. The results of anatomical repair in 16 consecutive neonates with TGA and intact ventricular septum were reviewed; special consideration was given to technique and guidelines to avoid complications. Infants underwent repair within 6 days of life (average weight, 3.3 kg). Survival was 88% (14 of 16 patients). One death occurred from pulmonary hypertension and atrial shunt reversal causing cyanosis after a

Vascular malformations of the central nervous system: a morphological overview.

Vascular malformations of the central nervous system (C.N.S.) are classified by size, location, and morphologic type, distinguishing capillary telangiectasias, cavernous malformations, venous angiomas, arteriovenous malformations (AVMs) including varix of the great vein of Galen, and other vascular malformations (e.g. Sturge-Weber syndrome). The morphology and predominant location pattern of the different types of vascular malformations in the brain and spinal cord, and their embryology are reviewed. In the brain and its coverings, all types mainly AVMs and venous angiomas do occur, representi

Vascular magnetic resonance imaging.

Natural contrast from flow void potentiates the diagnosis of the intimal flap of aortic dissection, the dilated contour of aortic aneurysm, the protruding plaque of atherosclerotic disease, the recognition of deep vein thrombosis, and the identification of a number of acquired and congenital vascular abnormalities. Although MRI is costly at the present, the advantages of vascular MRI include noninvasive multiplanar imaging, no ionizing radiation, no contrast material requirements, and its use as an alternative to technically limited ultrasonography or computed tomography. MRI can define anatom

Echocardiography in the neonate and young infant.

The neonate and young child present unique problems in echocardiographic diagnosis because of the wide spectrum of possible abnormalities and the tendency toward multiple lesions, requiring clear visualization of all great veins and arteries as well as intracardiac structures. Performance and interpretation of echocardiograms in this age group require an extensive knowledge of the pathologic anatomy of congenital heart disease. Recognition of unusual lesions is facilitated by displaying the heart in an anatomically familiar (upright) format in order to draw from experience in angiography and p

Congenital heart disease in infancy--surgery for specific defects.

The evolution of surgery for congenital cardiac malformations in infancy in the past decade warrants an analysis of the indications, technical aspects, advantages, complications and results of the treatment of specific defects. Considerable variations in approach are found, and every surgeon should attempt to evaluate the most appropriate current management in each particular case. Primary closure is usually preferred to pulmonary artery banding in patients with large ventricular septal defects. But will the same approach produce a high late survival rate in patients with other left-to-right s
