Systematic diagnostic method of two-dimensional echocardiography in congenital heart disease.

A two-dimensional echocardiographic (2-DE) method of segmental approach for diagnosis of congenital heart disease is presented. Firstly, the visceroatrial situs is determined, i.e., whether it is solitus, inversus, or ambiguous, by identifying the inferior vena cava to right atrium junction. Secondly, it is ascertained whether the ventricular loop is D-loop or L-loop. The ventricular loop is determined by identifying the internal structure of the right and left ventricles by 2-DE. Thirdly, the spatial relationships of the great arteries are determined by identifying the pulmonary artery and ao

Therapeutic catheter procedures in congenital heart disease: current status and future prospects.

Advancing technology in catheterization techniques have allowed the development of several therapeutic procedures in patients with congenital heart disease. Some of these, such as atrial balloon septostomy, are well established. Others, such as pulmonary valve dilatation, patent ductus closure, and systemic-pulmonary collateral obliteration are currently being tried with success in many institutions. In the future, certain forms of coarctation, some intracardiac shunts and perhaps other anomalies may be suitable for therapy by catheter techniques.

Transposition of the great arteries with posterior aorta and subaortic conus: anatomical and surgical correlation.

Three hearts studied at autopsy with transposition of the great arteries and posterior aorta with isolated or predominantly subaortic conus are described. In all cases the aorta was posterior and to the right of the pulmonary trunk. In two cases it was significantly distant from the pulmonary trunk and in one it was very close. The caliber of the pulmonary trunk was larger than that of the aorta: greater than 2:1 in two cases and less than 2:1 in one case. A well developed subaortic conus was seen in all cases and a small subpulmonary conus in one case. In the other two cases, the pulmonary va

The role of cerebral arteriovenous malformations in psychiatric disturbances: case report.

A patient is described who had congenital vascular malformation of the vein of Galen, with several psychiatric, as well as neurologic, manifestations. Prior to this patient's death, there was an increase in some psychiatric manifestations in response to suggestions of surgical therapy for the cerebral venous anomaly. Other symptoms appeared to represent direct organic effects of the lesion. The medical literature on the anomalies of the great vein of Galen reveals a frequent but not specific relationship to concomitant psychiatric manifestations.

Transcutaneous Doppler ultrasound in paediatric cardiology.

Recent developments in transcutaneous ultrasound, encompassing M-mode and two-dimensional echocardiography as well as continuous wave and pulsed Doppler techniques, have provided substantial contributions in facilitating noninvasive cardiac assessment. With Doppler systems diagnostic information is obtained through analysis of changes in the velocity and direction of blood flow, pulsatile character and flow pattern (laminar or turbulent) in the arteries, veins and cardiac chambers. Using Doppler, measurements of peripheral blood pressure correlate well with invasive recordings; coarctation of

Congenital unilateral pulmonary vein stenosis complicating transposition of the great arteries.

Four patients with transposition of the great arteries and unilateral pulmonary vein (PV) stenosis, all left-sided, were studied. Two patients had an intact ventricular septum (1 with a patent ductus arteriosus), 1 patient had a ventricular septal defect and 1 had a ventricular septal defect with pulmonary stenosis. Clinical signs, such as recurrent pneumonia, unilateral pulmonary edema and pleural effusion, were present preoperatively in 2 patients. Diagnosis was made at cardiac catheterization by cineangiography in 2 patients and at Mustard operation when the PV orifices were inspected in th

Anatomic correction of transposition of the great arteries associated with ventricular septal defect: midterm results in 50 patients.

From May 1977 to August 1982 50 patients who were 1.5 to 44 months old underwent anatomic correction of transposition of the great arteries (TGA) and closure of ventricular septal defect (VSD) at our institution. Thirty-nine patients underwent preliminary pulmonary arterial banding. Hospital mortality was 32%: four patients died as a result of technical problems, seven as a result of associated lesions, three of pulmonary hypertension, and two of left ventricular failure. Three other patients died after the first postoperative month (one of mediastinitis, one at reoperation for a residual VSD,

Management of transposition after the baffes procedure: a case report and review of our experience.

A case report details the operative technique used in the repair of complete transposition of the great arteries (TGA), ventricular septal defect (VSD), and pulmonary stenosis in a patient who had had Baffes procedure previously. Experience in managing 10 patients with complete TGA who had undergone a Baffes procedure is discussed. Four patients with a large VSD had pulmonary vascular disease. The condition of 1 of them was improved by a palliative Mustard operation. Four children with isolated TGA underwent a successful modified Mustard repair. The 2 remaining patients had a VSD and pulmonary

Transposition of the great arteries and ventricular septal defect: results with the Senning operation and closure of the ventricular septal defect in infants.

From May, 1978, to July, 1982, 46 infants ranging in age from 12 days to 12 months and in weight from 2.1 to 8.4 kg underwent repair of dextrotransposition of the great arteries (D-TGA) and ventricular septal defect (VSD) using a Senning repair and closure of the VSD. Ventricular septal defects were classified as membranous (47.8%), malaligned (28.3%), atrioventricular (AV) canal type (13.0%), subarterial (2.2%), muscular (2.2%), and multiple (6.5%). Hospital mortality was 15.2% and late mortality, 5.1%. Postoperative complications included tricuspid regurgitation (mild in 3 and severe, requir

Successful modified Senning operation for the repair of Taussig-Bing malformation.

Surgical correction was carried out successfully in a severely cyanotic 3-year-old boy with Taussig-Bing malformation. The cardiac anomaly was associated with semilunar valves side by side, and at the same height, a bilateral conus without pulmonary-mitral continuity and subpulmonary ventricular septal defect, on which pulmonary valve overode. The surgical correction consisted of the closure of the VSD by a right atrial approach and modified Senning procedure under profound hypothermic cardiac arrest with surface cooling and limited cardiopulmonary bypass. The patient survived operation and wa

Type IV total anomalous pulmonary venous connection. Unusual survival and review of the literature.

Total anomalous pulmonary venous connection (TAPVC) is a condition most frequently seen in the pediatric age group. Of those that survive uncorrected into adulthood, type I and type II constitute the great majority. It is most unusual to see a patient with a type IV lesion survive. We report a 34 year old female patient with TAPVC of the right upper lobe and left lung veins into the vertical vein and the right lower lobe vein into the coronary sinus. This patient has the second longest survival of those with type IV lesion of TAPVC, and the second oldest to have successful surgical correction.

Complex cyanotic congenital heart disease correctable with an intra-atrial baffle.

The clinical, angiocardiographic, and surgical findings in a cyanotic child with dextrocardia, L-loop, laterally inverted but otherwise normally related great arteries, and situs ambiguus are described and compared with other similar cases. Electrocardiographic, echocardiographic, and angiographic data are reviewed, and the segmental approach to diagnosis is stressed. Both systemic and pulmonary veins returned to the right-sided atrium. Blood then passed either through the mitral valve to the morphologically left ventricle and to the aorta or through an atrial septal defect to a small left-sid

Two dimensional echocardiographic diagnosis of situs.

At present there is no reliable method of recognising atrial isomerism by two dimensional echocardiography. We therefore used two dimensional echocardiography to examine 158 patients including 25 with atrial isomerism and four with situs inversus. Particular attention was paid to the short and long axis subcostal scans of the abdomen. Using the position of the inferior vena cava and the aorta with respect to the spine it was possible to separate those with situs solitus from the others. Two false positives for abnormal situs had exomphalos. In situs solitus the aorta lay to the left of the spi

[CT diagnosis of the carotid cavernous fistula and differential diagnosis (author's transl)].

One of the many urgent cases requiring immediate attention is the possibility of continuous bleeding into the sphenoidal sinus via a carotid cavernous fistula (CCF). Computed tomography offers a necessary and quick diagnostic method for such soft-tissue structures. Difficulty in CT diagnosis is enhanced by the limitations inherent in certain diagnoses. The article shows that safe diagnosis can be achieved in acute postoperative CCF with great shunt volumes. When diagnosing spontaneous cases we must consider the clinical data to differentiate CCF from other causes.

Raghib's syndrome: a report of two cases.

We treated two patients with a rare developmental complex. The persistent left superior vena cava draining into the left atrium (PLSVC into LA) was associated with an absent coronary sinus and an atrial septal defect. Ligation of PLSVC and patch-repair of the atrial septal defect were successfully performed in one stage. The atrial septal defect was located in the upper and posterior aspect of the interatrial septum and appeared to be an unique type of atrial septal defect. In the other patient, additional multiple cardiac defects were associated with this syndrome, including ventricular septa

Repair of combined total anomalous pulmonary venous connection and anomalous systemic connection in early infancy.

A simple intraatrial reconstruction can be utilized in infants to simultaneously correct both the systemic and pulmonary venous returns. Two infants with polysplenia syndrome treated in the first months of life are presented. One had levocardia and combined total anomalous pulmonary venous connection (type II-Cardiac) and anomalous systemic venous connection. The other had dextrocardia L-loop, L-normal great arteries, and combined total anomalous pulmonary venous connection (type II-Cardiac) and anomalous systemic venous connection. The posterior position of the systemic atrioventricular valve

Cardiovascular abnormalities in thoracopagus twins.

A detailed anatomopathologic study of the various developmental malformations encountered in a set of female thoracopagus twins is presented. Special emphasis has been devoted to the analysis and description of the complex cardiovascular abnormalities associated with the conjoined heart and liver of this set of twins. The heart was conjoined with two sets of great arteries. The great arteries of one twin were d-malposed and originated from a single right ventricle; a normally placed aorta originated from a single left ventricle and the hypoplastic pulmonary trunk originated from a rudimentary

Two dimensional echocardiographic evaluation of Mustard operation for d- transposition of the great arteries.

Two dimensional sector scan echocardiography was used to evaluate the morphologic characteristics of the surgically revised atria in 17 patients with d-transposition of the great arteries who had undergone the Mustard operation. Echocardiographic imaging of the atria was obtained from various planar projections. Dimensional measurements of various segments of the systemic and pulmonary venous atria were obtained in each patient. Correlative hemodynamic, angiographic, postmortem and echocardiographic data showed that seven patients (Group I) had no structural abnormalities of the atria. These 7

Glenn shunt: long-term results and current role in congenital heart operations.

Fifty cyanotic patients (aged 2 days to 22 years) underwent Glenn shunts for tricuspid atresia and other cyanotic heart defects. Thirteen of 15 operative deaths occurred in infants less than 4 months old, and only 1 death has occurred in the last 9 years. Results were poor in patients with Ebstein's anomaly, truncus arteriosus, transposition of the great vessels, and complex defects other than tricuspid atresia and univentricular heart. Of the 35 patients followed from 0.9 to 14.8 years, 12 were followed for more than 10 years. None of the 11 late deaths could be attributed to complications of

Surgical anatomy of double-outlet right ventricle with situs solitus and atrioventricular concordance.

Sixty-three hearts with situs solitus, concordant atrioventricular connection, and more than half of both great arteries connected to the right ventricle have been studied to delineate their surgically important features. Specifically noted were the relationship of the arterial valves to each other and the type and position of the ventricular septal defect with special reference to the conduction tissue location. Infundibular morphology was ascertained and other congenital abnormalities were recorded. An estimate was made of the surgical options available given the various anatomic abnormaliti
