Twin Reversed Arterial Perfusion (TRAP) Sequence Phenotypes: A Comprehensive Visual Analysis.

Twin reversed arterial perfusion (TRAP) sequence is a rare and severe complication of monochorionic twins, occurring in approximately 1 in 35 000 pregnancies. It is characterized by the presence of a severely abnormal, nonviable acardiac twin and a structurally normal pump twin, which sustains the circulation of both fetuses through abnormal vascular placental anastomoses. This pictorial essay aims to illustrate the sonographic and Doppler findings associated with TRAP sequence, emphasizing the diagnostic prenatal features. Typical findings include the absence of cardiac activity in the acardi

Ischiopagus Conjoint Twins: A Case Report.

BACKGROUND: The birth of conjoined twins is an extremely rare neonatal condition, occurring in approximately 1 in 100,000 live births. Ischiopagus, a form of conjoined twinning where the twins are joined at the pelvis, accounts for 6-11% of all conjoined twins. Ischiopagus conjoined twinning is a congenital anomaly. The surgical separation of conjoined twins is challenging due to the intricate anatomy and physiology involved. Thorough preoperative evaluation, meticulous planning, and a skilled surgical team are essential for ensuring a successful outcome. This condition significantly requires

Extracoronary Arterial Pathologies in Patients With Spontaneous Coronary Artery Dissection.

Spontaneous coronary artery dissection (SCAD) is increasingly recognized as a cause of acute coronary syndrome and has been associated with extracoronary arteriopathies, such as fibromuscular dysplasia (FMD), aneurysms, and dissections across other vascular beds. However, these associations remain understudied in the literature. This study aims to characterize the prevalence and distribution of extracoronary arteriopathies in a large cohort of SCAD patients. Patients diagnosed with SCAD were extracted from 2018 to 2024. Baseline characteristics and comorbidities were collected. Available vascu

Screening for fibromuscular dysplasia after spontaneous coronary artery dissection unmasks a massive right renal artery aneurysm requiring ex vivo surgical resection and autotransplantation.

Spontaneous coronary artery dissection (SCAD) is an increasingly recognised cause of acute coronary syndrome predominantly affecting women (>90% of cases) that is frequently associated with other arteriopathies, such as fibromuscular dysplasia (FMD) and migraine. We present a case of multi-vessel SCAD in a woman in her 40s presenting with myocardial infarction in whom incidental widespread FMD was found, including a massive right renal artery aneurysm requiring ex vivo resection, repair and autotransplantation. The case underscores the need for routine angiographic screening for FMD, which has

Selective embolisation for renal arteriovenous fistula-induced pulmonary hypertension.

We report an alternative to nephrectomy for managing renal arteriovenous fistula (R-AVF) in patients with severe pulmonary hypertension and high-risk anaesthetic profiles. A man in his 70s, with right heart dysfunction and severe pulmonary hypertension, secondary to a left R-AVF, was deemed unsuitable for nephrectomy due to high anaesthetic risk. Instead, he underwent percutaneous transarterial embolisation, leading to significant renal function improvement and complete resolution of pulmonary hypertension on radiological imaging and clinical assessment. This is the first clinically and radiol

Genetic etiology of truncus arteriosus excluding 22q11.2 deletion syndrome and identification of c.1617del, a prevalent variant in TMEM260, in the Japanese population.

Truncus Arteriosus (TA) is a congenital heart disease characterized by a single common blood vessel emerging from the right and left ventricles instead of the main pulmonary artery and aorta. TA accounts for 4% of all critical congenital heart diseases. The most common cause of TA is 22q11.2 deletion syndrome, accounting for 12-35% of all TA cases. However, no major causes of TA other than 22q11.2 deletion have been reported. We performed whole-genome sequencing of 11 Japanese patients having TA without 22q11.2 deletion. Among five patients, we identified pathogenic variants in TMEM260; the bi

Socioeconomic deprivation and illness trajectory in the Scottish population after COVID-19 hospitalization.

BACKGROUND: The associations between deprivation and illness trajectory after hospitalisation for coronavirus disease-19 (COVID-19) are uncertain. METHODS: A prospective, multicentre cohort study was conducted on post-COVID-19 patients, enrolled either in-hospital or shortly post-discharge. Two evaluations were carried out: an initial assessment and a follow-up at 28-60 days post-discharge. The study encompassed research blood tests, patient-reported outcome measures, and multisystem imaging (including chest computed tomography (CT) with pulmonary and coronary angiography, cardiovascular and

Kidney Stones Account for Increased Imaging Studies in Autosomal Dominant Polycystic Kidney Disease.

KEY POINTS: Kidney stones are common in patients with autosomal dominant polycystic kidney disease compared with matched controls with CKD. The increase in imaging seen in patients with autosomal dominant polycystic kidney disease correlates with the kidney stone burden in these patients. BACKGROUND: Patients with autosomal dominant polycystic kidney disease (ADPKD) may have more imaging studies than patients with other forms of CKD. We characterized the imaging burden of patients with ADPKD relative to a CKD population to determine which factors lead to increased imaging in patients with ADP

Diagnostic Pitfalls of the Bleeding Origin after a Percutaneous Renal Biopsy.

A percutaneous renal biopsy (PRB) is a standard procedure for diagnosing renal disease, but can cause bleeding complications. Bleeding after a PRB can be classified as early- or late-onset, depending on the timing of the onset of the bleeding symptoms (<24 h or ≥24 h). We herein report two patients who experienced bleeding complications: one experienced early-onset bleeding from the 12th subcostal artery, and the other experienced late-onset bleeding from an arteriovenous fistula between a branch of the renal artery and renal vein. In both cases, the origin of the bleeding vessel was misjudged

Smooth muscle-specific deletion of cellular communication network factor 2 causes severe aorta malformation and atherosclerosis.

AIMS: Cellular communication network factor 2 (CCN2) is a matricellular protein implicated in fibrotic diseases, with ongoing clinical trials evaluating anti-CCN2-based therapies. By uncovering CCN2 as abundantly expressed in non-diseased artery tissue, this study aimed to investigate the hypothesis that CCN2 plays a pivotal role in maintaining smooth muscle cell (SMC) phenotype and protection against atherosclerosis. METHODS AND RESULTS: Global- and SMC-specific Ccn2 knockout mouse models were employed to demonstrate that Ccn2 deficiency leads to SMC de-differentiation, medial thickening, an

Fetal Autopsy: Insights Into the Spectrum of Dysraphisms With Associated Anomalies.

Introduction and aim Malformations of the central nervous system (CNS) are one of the most devastating, yet most common congenital anomalies. Congenital CNS anomalies are structural or functional abnormalities of the brain and spinal cord that occur during the intrauterine developmental period and have a high intrauterine mortality. This study aimed to determine the frequency and distribution of neural tube defects (NTDs) in a tertiary care center for a period of 4.5 years. The neural tube defect was analyzed along with the other associated anomalies in the fetus. Its association with maternal

The effects of standardized intravenous treprostinil in pulmonary arterial hypertension patients after total cavo-pulmonary connection procedure.

OBJECTIVE: Total cavo-pulmonary connection (TCPC) is a palliative treatment for single ventricular malformations. For high-risk patients (preoperative mean pulmonary arterial pressure, mPAP > 15 mmHg), between the inhaled and oral targeted medications, the application of intravenous treprostinil as a bridge therapy to achieve "seamless" management is core postoperative treatment. This study intends to explore the effect of different administration regimens on early postoperative recovery. METHODS: This was a retrospective cohort study. High-risk pediatric patients (age ≤ 14 years) who underwe

Kidney and vascular involvement in Alagille syndrome.

Alagille syndrome (ALGS) is an autosomal dominant, multisystemic disease with a high interindividual variability. The two causative genes JAG1 and NOTCH2 are expressed during kidney development, can be reactivated during adulthood kidney disease, and Notch signalling is essential for vascular morphogenesis and remodelling in mice. Liver disease is the most frequent and severe involvement; neonatal cholestasis occurs in 85% of cases, pruritus in 74%, xanthomas in 24% of cases, and the cumulative incidences of portal hypertension and liver transplantation are 66% and 50% respectively at 18 years

A Precious Puzzle: Unveiling a Suspected VACTERL (Vertebral, Anal, Cardiovascular Malformations, Tracheo-esophageal Fistula, Renal Anomalies, and Limb Defects) Association in a 28-Day-Old Neonate.

VACTERL is an acronym that stands for a wide range of congenital anomalies, including vertebral anomalies, tracheoesophageal fistula, anorectal anomalies (anal atresia), cardiac anomalies, renal anomalies, and limb malformations. While not a true syndrome, the co-occurrence of these findings suggests a common underlying cause. Here, we present a case of a 28-day-old neonate with suspected VACTERL association. The baby was delivered preterm to non-consanguineous parents. The baby had vertebral abnormalities, renal defects, an imperforate anus, a posterior urethral valve, and an absent right pul

European association of perinatal medicine (EAPM) position statement: Screening, diagnosis and management of congenital anomalies of the umbilical cord.

Congenital anomalies of the umbilical cord are associated with an increased risk of pregnancy and perinatal complications. Some anomalies of the cord have a higher prevalence than other fetal structural anomalies. The most common anomalies are the absence of an umbilical artery and velamentous insertion of the cord (with or without vasa previa). These anomalies, even when not associated with fetal structural defects, increase the risk of adverse perinatal outcome including, fetal growth restriction and stillbirth. In the absence of prenatal diagnosis, vasa previa is associated with the highest

Diagnosis of single umbilical artery and risk of foetal congenital malformations by prenatal ultrasound: a retrospective study.

BACKGROUND: Single umbilical artery (SUA) is strongly associated with foetal structural abnormalities; however, the exact pattern of this association has not been described. We aimed to investigate the occurrence of malformations in singleton pregnancies with SUA in China and to study the association between the absent side of the umbilical artery and foetal malformations. METHODS: This was a retrospective study of singleton pregnancies for which routine first-trimester anatomical screening was performed at 11+ 0-13+ 6 gestational weeks and, if the pregnancy continued, a second-trimester scan

Kidney Transplant Loss Due to May-Thurner Syndrome: Case Report and Review of the Literature.

BACKGROUND: May-Thurner syndrome (MTS) is an extrinsic venous compression by the arterial system against bony structures in the iliocaval territory. The most common variant of MTS is due to compression of the left iliac vein between the overlying right common iliac artery and the fifth lumbar vertebrae. The prevalence of MTS is unknown; therefore, there are only a few publications about MTS in kidney transplant recipients. Risk factors that may progress from usually asymptomatic to symptomatic MTS are female sex, scoliosis, dehydration, coagulation disorders, and radiation. Clinical presentati
