Basement membrane components are integral to the physiologic function of cerebral microvessels. Immunotherapy (including by the use of lecanemab) in Alzheimer's disease patients may result in vascular complications identified by neuroimaging (ARIA-E and ARIA-H). This pilot study (on a relatively small number of human brain specimens) suggests one mechanism might be through the effects of lecanemab on a collagen component of said microvessel walls. It utilizes microvessels isolated from human brains (of Alzheimer's disease patients) and maintained in a viable state, to examine this mechanism us

BACKGROUND: Internal carotid artery fenestration is a rare vascular anomaly that may present with transient ischemic attack symptoms due to localized flow disturbances. It is important to report such cases, as they present unique diagnostic challenges and clinical implications, particularly when associated with systemic vascular risk factors such as diabetes and hypertension. Differentiating internal carotid artery fenestration from other conditions, such as internal carotid artery dissection, is crucial for appropriate management and patient outcomes.

Severe spinal muscular atrophy (SMA) is a life-limiting neurodegenerative disease of infancy and early childhood, caused by reduced expression of the ubiquitous survival motor neuron protein (SMN). While current therapies aim to increase SMN levels and preserve motor neurons, significant deficits remain in treated patients and non-neuronal manifestations of SMN deficiency are underexplored. Vascular abnormalities including intrinsic endothelial cell dysfunction, altered vessel morphology, and altered vascular distribution have been reported in preclinical SMA models. Here, we characterised vas

Cerebral amyloid angiopathy (CAA) is a common brain pathology in older people and has been recently recognized as a major risk factor for amyloid-related imaging abnormalities during anti-amyloid antibody therapy. CAA pathophysiology may involve iron released from ruptured vessels, but the association between postmortem CAA and brain iron is unclear. This study investigates the association between CAA and brain iron and whether elevated iron modifies the association between CAA and cognitive decline. We studied 626 Rush Memory and Aging Project decedents (mean age at death = 90 [SD = 6.1] year

An early adolescent male from a consanguineous family presented with progressive neurological deterioration, including developmental delay, seizures, left hemiparesis and cognitive decline from early childhood. He subsequently developed gastrointestinal bleeding, chronic liver disease with oesophageal varices, severe anaemia and thrombocytopenia. Initial investigations suggested TORCH (Toxoplasmosis, Other, Rubella, Cytomegalovirus, Herpes simplex) infection due to intracranial calcifications and multisystem involvement. However, detailed neuroimaging revealed extensive periventricular calcifi

BACKGROUND: The dural venous sinus system (DVSS) plays a central role in cerebral venous drainage and cerebrospinal fluid (CSF) reabsorption. Compromise of the DVSS is traditionally thought to predispose patients to elevated intracranial pressure and hydrocephalus. However, research evaluating this relationship is often scarce.
OBJECTIVE: To determine whether compromise of the DVSS system, especially the superior sagittal sinus, is associated with an increased risk of hydrocephalus in pediatric and young adult patients.
METHODS: We performed a retrospective cohort study (n=62) of pediatric a

Cystic lymphangioma is a rare benign congenital malformation of the lymphatic system resulting from abnormal embryologic development of lymphatic vessels. The cervicofacial region represents the most common localization. Prenatal ultrasound plays a key role in early diagnosis and follow-up. We report the case of a 30-year-old gravida 2 para 1 woman with a pregnancy complicated by primary cytomegalovirus (CMV) infection during the first trimester and multiple fetal anomalies identified on antenatal ultrasound, including ventriculomegaly, cerebellar hypoplasia, abnormal cerebral gyration, cardio

INTRODUCTION: Brain arteriovenous malformation (bAVM) is present in 0.01% of the population and has devastating neurologic consequences when ruptured. This study reports and evaluates a novel two-step genetic risk stratification model for bAVM, taking into account both rare Mendelian and common genetic variants, to identify patients with high genetic risk who can then receive screening MRI.
METHODS: Participants in the Penn Medicine Biobank (PMBB) with both exome sequencing and imputed (TOPMED) genotyping array data were included in this study. Participants were stratified into a "Mendelian c

Anomalous origin of the vertebral artery from the common carotid artery is a rare vascular variation, often associated with other aortic arch anomalies. We report the case of a 39-year-old woman who presented with acute dizziness and was diagnosed with a cerebellar infarction. Initial Computed tomography angiography revealed a right vertebral artery arising from the right common carotid artery, in conjunction with an aberrant right subclavian artery originating as the last branch of the aortic arch. A follow-up examination identified a de novo aneurysm in the V1 segment. The patient underwent

The persistent primitive hypoglossal artery (PPHA) is a rare and anatomically complex cerebrovascular variant. Although typically asymptomatic, its presence is associated with various cerebrovascular pathologies, including intracranial aneurysms, carotid artery stenosis, acute large vessel occlusion, moyamoya disease, brain arteriovenous malformations, and other vascular anomalies. When such lesions involve the PPHA, therapeutic intervention may be necessitated. Surgical management is particularly challenging due to the artery's deep anatomical location and intricate surrounding vasculature. C

BACKGROUND AND PURPOSE: Amyloid-related imaging abnormalities (ARIA) are radiographic findings observed in the natural course of Alzheimer's disease and have been reported at higher rates in patients receiving anti-amyloid monoclonal antibody therapy. Identifying novel radiographic factors predicting ARIA risk may help prevent its occurrence, improve patient stratification, and provide insight on the underlying biological mechanisms. It remains unclear whether cerebral perivascular spaces (PVS) along with other quantitative radiographic markers of cerebral small vessel disease may help predict

BACKGROUND AND PURPOSE: Despite high rates of macrovascular recanalization, approximately half of patients with large vessel occlusion stroke fail to achieve functional independence after endovascular thrombectomy (EVT). Residual tissue-level perfusion abnormalities - detectable on post-procedural CT perfusion (CTP) - have been proposed as a biomarker of futile recanalization. We aimed to establish post-EVT perfusion imaging (by CT or MR perfusion) as a surrogate marker predictive of functional independence on the modified Rankin Scale - by synthesizing evidence-based perfusion thresholds and
