Under Pressure: Increased Intracranial Pressure in Infants and Children. Presented at the 2024 AOC/AACO/AAO Sunday Symposium.

Intracranial hypertension (ICH) in children refers to increased pressure within the skull. The skull normally contains the brain, blood vessels, meninges, and cerebrospinal fluid (CSF). Abnormalities in any of these intracranial structures may cause ICH. The differential diagnosis for pediatric ICH includes brain tumors; intracranial hemorrhages; intracranial infections; craniosynostosis; disorders of CSF production, clearance, or abnormal CSF contents; and pseudotumor cerebri syndrome (PTCS), which may be primary (also known as idiopathic intracranial hypertension [IIH]) or secondary. Intracerebral Hemorrhage in a Middle-Aged Male With Classical Ehlers-Danlos Syndrome in the Absence of Macrovascular Lesions: A Case Report.

Ehlers-Danlos syndrome (EDS) is a heterogeneous connective tissue disorder characterized by tissue fragility, joint hypermobility, and variable vascular involvement. Although intracerebral hemorrhage (ICH) is classically associated with vascular EDS, emerging evidence suggests that cerebrovascular complications may occur across multiple EDS subtypes, even in the absence of overt vascular malformations. We report a 39-year-old Saudi male with genetically confirmed classical Ehlers-Danlos syndrome (cEDS) associated with COL5A1 variants who presented with acute decreased level of consciousness, e

A Rare Case of Gould Syndrome Presenting With Gastrointestinal Bleeding in a Pediatric Patient.

Gould syndrome, caused by COL4A1/COL4A2 mutations, is a rare multisystem disorder characterized by basement membrane defects leading to vascular fragility, cerebral small vessel disease, seizures, and renal involvement. While vascular complications are well-described, gastrointestinal bleeding remains exceedingly rare. We describe a four-year-old female with a known diagnosis of Gould syndrome who presented with hemodynamically significant hematochezia. Colonoscopy demonstrated tortuous submucosal vascular lesions consistent with colonic varices in the sigmoid colon, which were treated with bi

Binswanger's Disease: A Diagnostic Challenge in a Middle-aged Male.

Binswanger's disease, a rare form of subcortical vascular dementia, presents with a progressive decline in cognitive and motor functions due to chronic small vessel ischemic changes in the brain. Its clinical presentation often overlaps with other neurological conditions, particularly Wernicke's encephalopathy, leading to potential diagnostic delays. We report the case of a 45-year-old male who presented with altered mental status, gait abnormalities, memory impairment and ocular pain. Given his significant history of alcohol consumption, an imperial diagnosis of Wernicke's encephalopathy was

Pampiniform venous plexus thrombosis in a 37 years-old male patient with severe thrombocytosis: a multifactorial aetiology.

Atypical thrombosis occurs in unconventional anatomical sites such as splanchnic, renal, gonadal, and cerebral venous vessels [1] . Genetic factors, as inherited thrombophilia could increase the risk of atypical thrombosis. Likewise, acquired disease, as cancer, autoimmune diseases (e.g. systemic lupus erythematosus and APS) or infections [e.g. Cytomegalovirus (CMV) and Epstein-Barr virus (EBV) infections], can trigger such an inflammatory response which lead to thrombocytosis and endothelial injury, thus promoting a hypercoagulability state [2,3] . Among cancers, myeloproliferative disorders

Diffuse cerebral microhemorrhages in West Nile virus neuroinvasive disease: a case report.

West Nile virus neuroinvasive disease (WNND) most commonly presents as meningitis, encephalitis, or acute flaccid paralysis, with variable neuroimaging findings. While MRI abnormalities classically involve deep gray matter and brainstem structures, hemorrhagic manifestations are rarely reported. We report a 77-year-old woman who presented with rapidly progressive altered mental status following a brief febrile prodrome. Cerebrospinal fluid studies demonstrated a lymphocytic pleocytosis consistent with aseptic meningitis. Brain MRI revealed an acute lacunar infarct and diffuse cerebral microhem

Diffusion MRI Identifies a Prolonged Pre-WMH Phase in Cerebral Small Vessel Disease.

BACKGROUND: White matter hyperintensities (WMH) are widely used to assess cerebral small vessel disease but reflect late-stage injury. Diffusion magnetic resonance imaging (MRI) biomarkers have been proposed to capture earlier small vessel disease-related microstructural damage but their temporal progression relative to WMH and risk factors associated with progression remain unexplored. METHODS: We identified 2077 participants from the population-based cohort study of the Mayo Clinic Study of Aging in Olmsted County, Minnesota (aged 50-101 years) collected between 05/2005 and 09/2024 with lon

Towards credible digital twins for basic and preclinical research.

Atomoxetine (Strattera(R)) is a selective norepinephrine (noradrenaline) reuptake inhibitor that is not classified as a stimulant, and is indicated for use in patients with attention-deficit hyperactivity disorder (ADHD). Atomoxetine is effective and generally well tolerated. It is significantly more effective than placebo and standard current therapy and does not differ significantly from or is noninferior to immediate-release methylphenidate; however, it is significantly less effective than the extended-release methylphenidate formulation OROS(R) methylphenidate (hereafter referred to as osm

Optical coherence tomography angiography reveals insights into complementary vascular and neurodegenerative mechanisms in multiple sclerosis.

Optical coherence tomography angiography quantifies retinal microvasculature biomarkers, offering insights into neurovascular mechanisms underlying brain damage in multiple sclerosis. This study evaluated these potential mechanisms of neurodegeneration by examining associations between optical coherence tomography and optical coherence tomography angiography metrics, brain volumes and clinical outcomes in people with multiple sclerosis. This cross-sectional study included multiple sclerosis patients from a prospective cohort. Participants underwent optical coherence tomography/optical coherenc

Deep cervical lymph node analysis in central nervous system inflammatory disease.

A previously espoused notion that the brain is an immune-privileged organ has been challenged by evidence of bidirectional communication between the central nervous system and the periphery. A well-described "glymphatic" system in the brain and the meningeal lymphatic system serve as conduits through which antigens, immune cells, and metabolic waste travel from the brain to the deep cervical lymph nodes. These nodes, which are more than passive drainage points, serve as locales where dendritic cells, T cells, and B cells interact with central nervous system-derived signals and modulate immune

Two unique systemic venous connections of levoatriocardinal vein without intracardiac anomalies.

Levoatriocardinal vein is a rare venous anomaly occasionally presenting as a bidirectional shunt in structurally normal hearts. We describe two unique systemic connections of levoatriocardinal vein without intracardiac defects, one causing paradoxical embolism via the hemiazygos vein, and the other draining from the left jugular vein to the pulmonary vein, requiring surgical correction to preserve cerebral venous return.

Intracranial dural arteriovenous fistulas with deep venous drainage: a single-center retrospective cohort study.

OBJECTIVE: Dural arteriovenous fistulas (DAVFs) with deep venous drainage (DVD) (DAVFs-DVD) are characteristically associated with non-hemorrhagic neurological deficits, most notably cognitive impairment. Large studies have yet to thoroughly characterize these DAVFs. We conducted an analysis of the largest cohort of DAVFs-DVD to provide a comprehensive characterization of this specific subset. METHODS: This study was a retrospective cohort analysis of 207 patients with DAVFs-DVD who were hospitalized at a single medical center between 2001 and 2022. Logistic regression was employed to identif

Frontal Paraventricular Cysts: Refined Definitions and Outcomes.

BACKGROUND AND PURPOSE: Frontal paraventricular cystic changes have a varied etiology that includes connatal cysts, subependymal pseudocysts, necrosis, and enlarged perivascular spaces. These may be difficult to distinguish by neuroimaging and have a variety of associated prognoses. We aim to refine the neuroimaging definition of frontal horn cysts and correlate it with adverse clinical conditions. MATERIALS AND METHODS: In this cross-sectional study, the pre- and postnatal neuroimaging database at a quaternary referral children's hospital was searched for all reports containing "frontal horn

Cerebral vasculitis related to neurosarcoidosis: a case series and systematic literature review.

Cerebral vasculitis is a rare but severe manifestation of neurosarcoidosis (NS) that has received little attention. The aim of the present study was to characterize clinical and diagnostic features as well as potential treatment strategies of cerebral vasculitis related to NS. We assessed 29 patients with cerebral vasculitis related to NS (15 female, mean age at time of diagnosis 45 years, SD = 11.85) among these were four new cases from our hospital records and 25 previously published cases from a systematic literature review. The demographic, clinical, and diagnostic features of those 29 pat

Geometrical determinants of cerebral artery fenestration for cerebral infarction.

PURPOSE: Few data are available on the causality of cerebral artery fenestration (CAF) triggering cerebral infarction (CI) and this study aims to identify representative morphological features that can indicate risks. METHODS: A cohort comprising 89 patients diagnosed with CAF were enrolled from a total of 9,986 cranial MR angiographies. These patients were categorized into Infarction Group (n = 55) and Control Group (n = 34) according to infarction events. These two groups are divided into two subgroups depending on fenestration location (basilar artery or other cerebravascular location), re

Use of lecanemab and donanemab in the Canadian healthcare system: Evidence, challenges, and areas for future research.

Lecanemab and donanemab are monoclonal antibody therapies that remove amyloid-beta from the brain. They are the first therapies that alter a fundamental mechanism, amyloid-beta deposition, in Alzheimer disease (AD). To inform Canadian decisions on approval and use of these drugs, the Canadian Consortium on Neurodegeneration in Aging commissioned Work Groups to review evidence on the efficacy and safety of these new therapies, as well as their projected impacts on Canadian dementia systems of care. We included persons with lived experience with Alzheimer disease in the discussion about the bene

Endovascular Management of Arteriovenous Malformations in the Thalamic and Basal Ganglia: A Systematic Review.

Arteriovenous malformations (AVMs) in the thalamic and basal ganglia present significant challenges due to their deep-seated location and complex vessel architecture. This systematic review outlines the efficacy and outcomes of endovascular management of these lesions. A comprehensive analysis of seven studies including 53 patients revealed high technical success rates, with complete obliteration in 46.7-100% of cases, using advanced embolization agents, such as ethylene vinyl-alcohol copolymer, and precipitating hydrophobic injectable liquid. Adjunctive therapies, mainly stereotactic radiosur

Assessing the Feasibility and Utility of Patient-Specific 3D Advanced Visualization Modeling in Cerebrovascular Disease: Retrospective Analysis and Prospective Survey Pilot Study.

BACKGROUND: The prevalence, clinical burden, and health care costs (>US $100 billion) associated with cerebrovascular disease (CVD) will increase significantly as the US population grows and ages over the next 25 years. Existing 2D imaging modalities have inherent limitations in visualizing complex CVD, which may be mitigated with the use of patient-specific 3D advanced visualization (AV) technologies. There remain gaps in knowledge, however, regarding how and with what impact these technologies are being used in CVD. OBJECTIVE: The aim of this study was to characterize the clinical attribute
