Bilateral duplicated middle cerebral arteries.

Bilateral identical duplications of the middle cerebral artery are extremely rare. We report this anatomical variation in a 56-year-old woman. The anatomical variation was identified using magnetic resonance angiography, which was performed to evaluate cerebrovascular disease. Bilaterally, one middle cerebral artery originated from the internal carotid artery bifurcation and the other originated from the internal carotid artery terminal segment. The anatomical variations were incidental findings, and the patient was asymptomatic.

Mediolateral Diameter of the Falcine Sinus as a Predictor of Clinical Outcomes in Fetal Vein of Galen Malformations.

BACKGROUND: The mediolateral diameter of the falcine sinus (FS) on fetal magnetic resonance imaging is a robust predictor of aggressive neonatal presentation requiring early treatment in vein of Galen malformation. Given the need for better overall prognostication for fetal vein of Galen malformation, we aimed to assess whether FS provides a more comprehensive prediction of outcomes. METHODS: Patients were identified retrospectively from a single referral center cerebrovascular database. Overall, 59 patients (55% male and 45% female) diagnosed with vein of Galen malformation via fetal magneti

Transcriptomic and morphologic vascular aberrations underlying FCDIIb etiology.

Focal cortical dysplasia type II (FCDII) is a major cause of drug-resistant epilepsy, but genetic factors explain only some cases, suggesting other mechanisms. In this study, we conduct a molecular analysis of brain lesions and adjacent areas in FCDIIb patients. By analyzing over 217,506 single-nucleus transcriptional profiles from 15 individuals, we find significant changes in smooth muscle cells (SMCs) and astrocytes. We identify abnormal vascular malformations and a unique type of SMC that we call "Firework cells", which migrate from blood vessels into the brain parenchyma and associate wit

Cerebral Amyloid Angiopathy Is Associated With Higher R2 Relaxation Rate: An MRI and Pathology Study.

OBJECTIVE: Cerebral amyloid angiopathy (CAA) involves β-amyloid deposition in the walls of cortical and leptomeningeal small vessels. Transverse relaxation rate (R2) is a major source of contrast in MRI. This study tested the hypothesis that CAA is associated with R2, extracted the spatial pattern of CAA-related R2 abnormalities, and evaluated R2 at different CAA severity levels in a large number of community-based older adults. METHODS: Cerebral hemispheres from 804 older adults who came to autopsy were included. All hemispheres underwent ex vivo MRI and detailed neuropathologic examination.

A 15-year follow-up of permanent intraoperative internal carotid artery occlusion for hemostasis in a giant cavernous sinus hemangioma: a case report.

BACKGROUND: Cavernous sinus hemangiomas are rare vascular tumors characterized by significant intraoperative bleeding. Their preoperative diagnosis is challenging and often leads to initial misdiagnosis as other middle fossa lesions, such as tumors or aneurysms. CASE PRESENTATION: We report the case of a 16-year-old Persian male presenting with progressive left-sided visual loss, cranial nerve palsy, with an intracranial lesion originating from the left cavernous sinus, extending into the sella, and occupying half of the middle fossa. He had previously undergone surgery with a preliminary dia

Outcomes of arteriovenous malformations with single versus multiple draining veins: A multicenter study.

BACKGROUND: Cerebral arteriovenous malformations (AVMs) are complex vascular lesions that pose a risk for hemorrhagic stroke. The number of draining veins has recently emerged as a significant predictor of rupture risk. This multicenter study aimed to evaluate the outcomes in adult AVM patients with single versus multiple draining veins. METHODS: We conducted a retrospective analysis of 735 AVM patients from the Multicenter International Study for Treatment of Brain AVMs (MISTA) database. Patients were categorized into single draining vein (n = 430) and multiple draining veins (n = 305) group

Microstructural and microvascular features of white matter hyperintensities and their association with small vessel disease markers.

White matter hyperintensities (WMH) are the most prominent imaging feature of small vessel disease (SVD). WMH and other imaging features of SVD are likely the result of ongoing insults associated with cognitive decline and cerebrovascular events. Emerging evidence suggests degradation of the neurovascular unit may underlie the pathogenesis of SVD. This prospective pilot study employed MRI in 19 subjects (68.2 ± 11.5 years of age) for diffusion-weighted imaging, applied to intravoxel incoherent motion (IVIM) modeling, to characterize microstructural and microvascular properties throughout the b

Safety, Tolerability, and Pharmacokinetics of NRL-1049, a Rho-Associated Kinase Inhibitor, in Healthy Volunteers: A Phase 1, First-in-Human, Single-Ascending Dose, Randomized, Placebo-Controlled Trial.

BACKGROUND AND OBJECTIVES: Cerebral cavernous malformations (CCMs) are vascular lesions of the brain that can lead to hemorrhage, focal neurologic deficits, and seizures. Rho-associated kinase (ROCK) overactivation plays a critical role in the development of CCMs, and a novel, selective ROCK2 inhibitor, NRL-1049, mitigated lesion burden and bleeding in mouse models of CCM. This study examined the safety, tolerability, and pharmacokinetics of NRL-1049 in healthy volunteers. METHODS: In this first-in-human, randomized, double-blind, single-ascending dose study, participants received a single, o

Multiple extra-axial calcified lesions and tortuous vessels in a patient with meningioangiomatosis and executive dysfunction.

Meningioangiomatosis (MA) is a rare benign hamartomatous lesion characterized by leptomeningeal and cortical vascular proliferation, and the predominant imaging feature is cerebral calcification. Although the radiological findings of this entity are well known, angiographic characteristics are incompletely understood. This report describes a case of a 54-year-old woman with sporadic MA and lifelong executive dysfunction with novel phase-specific angiographic features. Computed tomography revealed diffuse calcified lesions along the right cerebral convexity. Cerebral angiography showed previous

Midlife Retinal Microvascular Signs and Late-Life Neuroimaging Features of Cerebral Small Vessel Disease in the ARIC Study.

BACKGROUND AND OBJECTIVES: Cerebral small vessel disease (CSVD) is a leading cause of cognitive and functional deficits. Retinal vasculature abnormalities may be an early indicator of CSVD. The aim of this study was to investigate the associations between retinal signs in midlife and imaging markers of CSVD 18 years later. METHODS: This study included participants from the Atherosclerosis Risk in Communities Study, a prospective community-based cohort, who had retinal imaging at visit 3 (1993-1995) and 3T brain MRI at visit 5 (2011-2013). MRI scans were reviewed centrally and rated for white

A Rare Case of Intracranial Hemorrhage Mimicking a Neoplasm in a Child With Hereditary Hemorrhagic Telangiectasia.

Hereditary hemorrhagic telangiectasia (HHT) is an autosomal dominant disorder characterized by abnormal blood vessel formation, resulting in the development of arteriovenous malformations and telangiectasias throughout the body. We present a rare case of an intracranial hemorrhage mimicking a neoplasm in a medically complex pediatric patient with HHT, a diagnosis that was unbeknownst to both the clinical team and the interpreting radiologist early in the patient's clinical course. This case not only demonstrates the importance of obtaining a thorough medical history but having a clear understa

Absence of Deep and Basal Veins Is Common and Clinically Relevant in Sturge-Weber Syndrome.

BACKGROUND: Common intracranial vascular abnormalities in Sturge-Weber syndrome (SWS) include leptomeningeal venous malformations (LVMs) and enlarged deep veins. A few small studies have reported absent deep veins in some patients. We used susceptibility-weighted imaging (SWI), a magnetic resonance imaging (MRI) sequence sensitive to detecting small veins, to evaluate deep cerebral veins and the basal vein of Rosenthal (BVR) and assess the radiological correlates and clinical impact of their absence. METHODS: Fifty young subjects, including 30 patients with unilateral SWS and 20 healthy contr

5-year follow up after successful craniopagus separation: Review on hydrocephalus and venous system re-arrangement.

INTRODUCTION: Craniopagus is one of the rarest congenital abnormalities. Separation of craniopagus twin is associated with high morbidity and mortality, especially in total type, where the twin had shared dural venous sinuses. One of the complications after separation surgery is hydrocephalus. While detailed cerebral vasculature imaging is needed pre-operatively to ensure most optimal surgical approach, post-operative vasculature imaging is no less important to assess changes in cerebral venous system after separation surgery. CASE PRESENTATION: Separation surgery was successfully accomplishe
