CBCT-Based 3D Assessment of Maxillary Sinus and Pharyngeal Airway in Unilateral Cleft Lip and Palate.

BACKGROUND: Cleft lip and palate (CLP) is one of the most common congenital craniofacial anomalies, often associated with structural variations in the maxillary sinus and pharyngeal airway. These alterations may predispose individuals to complications such as sinusitis and obstructive sleep apnea. While previous studies have investigated these structures, most relied on two-dimensional imaging, which has inherent limitations for evaluating three-dimensional structures. Moreover, the maxillary sinus and airway have rarely been evaluated together in a unified three-dimensional framework, despite

A Comparative Assessment of Surgeon Appraisal of Presurgical Infant Orthopedics Outcomes Using PLANA and NAM.

Presurgical lip, alveolus, and nose approximation (PLANA) is a novel form of presurgical infant orthopedics (PSIO) involving a prefabricated silicone intranasal device combined with hydrocolloid adhesive lip taping. Early studies have demonstrated favorable nasolabial morphologic outcomes using PLANA and a reduction in the burden of care on infants and their caregivers relative to nasoalveolar molding (NAM). However, the efficacy of PLANA in reducing cleft severity to facilitate surgical repair has not been comprehensively assessed from the perspective of the cleft surgeon. This study, therefo

Impact of Nasal Stents on Three-Dimensional Morphologic Outcomes Following Primary Cleft Rhinoplasty in Unilateral Cleft Lip.

BACKGROUND: Objective assessment of nasal stents following primary cleft rhinoplasty is limited. Using three-dimensional (3D) photogrammetry, this 10-year study quantifies how postoperative nasal stents affect nasolabial morphology after unilateral cleft rhinoplasty. METHODS: A retrospective review included patients with unilateral cleft lip (uCL) repair from 2013 to 2023. Patient age, surgical technique, nasal stent use, and 1-year postoperative 3D photos were collected. A total of 29 facial landmarks were manually annotated to obtain 15 facial measurements. Fisher's exact test and Mann-Whit

Choanal Atresia Repair Using Mucoperiosteal Flaps Without Stenting: 10-Year Experience.

OBJECTIVES: Congenital choanal atresia (CCA) is a rare craniofacial anomaly that may present as a neonatal emergency or with delayed symptoms in childhood. Although endoscopic endonasal repair is the current standard of care, data on long-term functional outcomes and factors associated with revision surgery remain limited. This study evaluates clinical characteristics, surgical outcomes, and predictors of relook surgery following endoscopic endonasal CCA repair using mucosal flaps without stenting. METHODS: A retrospective single-center study was conducted including children (≤ 18 years) who

Transnasal Endoscopic Repair of Unilateral Choanal Atresia in a Young Adult Using a Cross-Over Nasoseptal Flap Technique and a Bioabsorbable Mometasone-Furoate-Eluting Stent: A Case Report.

Background and Clinical Significance: Choanal atresia is a rare congenital obstruction of the posterior nasal aperture, with an estimated incidence of one in 5000 to one in 8000 live births. Bilateral disease typically presents as a neonatal emergency, whereas unilateral disease is more frequent and may remain undiagnosed for years or decades, presenting in adolescence or adulthood with chronic unilateral nasal obstruction and ipsilateral mucopurulent rhinorrhoea. Optimal surgical management remains debated, particularly with regard to mucosal-flap reconstruction and the choice of postoperativ

Selective interaction of SMCHD1 with chromatin is governed by LRIF1 and SMCHD1 ATPase activity.

The chromosomal protein SMCHD1 is a GHKL ATPase with important roles in epigenetic silencing, including on the inactive X chromosome (Xi). Mutations of SMCHD1 have been linked to facioscapulohumeral muscular dystrophy (FSHD) and Bosma arrhinia microphthalmia syndrome (BAMS). Here, we use live-cell and single-molecule imaging to investigate SMCHD1 interactions with chromatin and its function in epigenetic silencing. We show that chromatin binding of SMCHD1 genome-wide, including on the Xi, is critically dependent on the protein LRIF1 that mediates interaction with H3K9me2/3-modified nucleosomes

Clinical Diversity in Currarino Syndrome: Insights From Monozygotic Twins and a Single-Center Retrospective Study.

Currarino syndrome (CS) is a congenital disorder caused by MNX1 gene mutation, resulting in abnormal caudal cell mass development. Clinical presentation varies from severe to mild forms. Classic triad of CS includes sacral agenesis, presacral mass, and anorectal malformation (ARM). We report a pair of monozygotic twins, both with the MNX1: c.780C>G mutation, presenting with different clinical phenotypes of Currarino syndrome (CS). Both were suspected of anorectal atresia in utero, and postnatal diagnosis confirmed CS with tethered cord syndrome (TCS). They underwent staged surgeries, with norm

Delayed diagnosis and successful surgical management of bilateral choanal atresia in a 10-year-old male: a case report.

BACKGROUND: Choanal atresia is a rare congenital anomaly characterized by the blockage or narrowing of one or both posterior nasal passages (choanae), either due to membranous or bony obstruction. It occurs in approximately 1 in 7,000 to 1 in 10,000 live births. This condition can be unilateral or bilateral, with bilateral choanal atresia being more severe and typically presenting with significant symptoms early in life. Choanal atresia may lead to nasal obstruction, nasal discharge, snoring, difficulty feeding, cyanosis, and mouth breathing. It is often associated with other congenital anomal

Synergistic Role of Retinoid and Gata3 Signaling in Olfactory Epithelium Development.

Craniofacial development requires precise coordination of epithelial patterning and morphogenesis. However, the molecular mechanisms governing olfactory epithelium development remain incompletely understood. Retinoic acid (RA) signaling and Gata3 have each been independently implicated in craniofacial morphogenesis, particularly in the formation of the primitive choanae, which constitute the opening between the nasal cavity and the oral cavity. Here, we generated a tamoxifen-inducible, genetically controlled compound mutant mouse model to simultaneously disrupt Rdh10, a rate-limiting enzyme fo

Endoscopic endonasal management of sellar/suprasellar arachnoid cysts: tailored surgical strategies and systematic review.

INTRODUCTION: Sellar and suprasellar arachnoid cysts(SACs)are rare lesions that may cause headaches, visual impairment, and endocrine dysfunction. While endoscopic endonasal approaches(EEA)are increasingly preferred, optimal surgical strategy-simple fenestration versus wide cyst-subarachnoid space(SAS)communication-remains debated. This study reports one of the largest single-center series of SACs treated via EEA for SACs and integrates these findings with an updated systematic review of the literature. METHODS: A retrospective review was conducted on 6178 endoscopic endonasal surgeries perfo
